PROTHESE POUR LE REMPLACEMENT D'UNE ARTICULATION DE LA MAIN. PROSTHESIS FOR THE REPLACEMENT OF A HAND JOINT.
La présente invention concerne les prothèses pour le remplacement d'une articulation de la main entre un os carpien et un os métacarpien, qui trouvent une application particulièrement avantageuse comme prothèses de remplacement de l'articulation entre l'os trapèze et le premier métacarpien. Il est connu de telles prothèses, notamment des prothèses trapézo- métacarpiennes qui sont d'une seule pièce souple, par exemple en élastomère de silicone, comme celles qui sont décrites dans les US-A 3 875 594, 4 158 893, 4 198 713, 4 313 232, 4 367 562, et 4 871 367. Il en existe même une qui a été mise au point par l'un des Inventeurs cités dans la présente Demande de Brevet, qui est une prothèse dite à "cardan souple" en un matériau comme un élastomère de silicone, qui a pour caractéristique essentielle et originale de tenir compte de la physiologie d'une articulation comme celle qui existe entre un os carpien, le trapèze, et le premier métacarpien, c'est-à-dire une articulation à deux axes. Cette prothèse donne objectivement d'après une étude statistique portant sur un certain nombre de cas sur une longue période des résultats satisfaisants, par disparition de la douleur dans 96% des cas et amélioration significative de la fonction, en particulier de l'opposition et de la force de la pince pouce-index. Sa probabilité de survie à 15 ans est de 98,17%. La présente invention a cependant pour but de réaliser un perfectionnement de cette prothèse dite « à cardan », et ainsi une prothèse « à cardan » ayant en elle-même une structure encore plus fiable lui conférant une durée de survie grandement augmentée, tout en facilitant le travail du Praticien qui réalise cette implantation. Plus précisément, la présente invention a pour objet une prothèse pour le remplacement d'une articulation de la main entre un os le premier métacarpien et le trapèze, ladite prothèse étant constituée d'un bloc en une seule pièce en un premier matériau souple ayant une rigidité donnée, ledit bloc comportant : • un premier corps oblong définissant un premier axe longitudinal, • une première patte d'ancrage définissant un deuxième axe longitudinal, ladite première patte étant agencée pour être apte à s'enficher dans le canal médullaire de l'os métacarpien, cette dite première patte étant solidaire du premier corps oblong sensiblement en son milieu et de façon que les premier et deuxième axes soient sensiblement perpendiculaires entre eux, • un second corps oblong définissant un troisième axe longitudinal, • une seconde patte d'ancrage définissant un quatrième axe longitudinal, ladite seconde patte étant agencée pour être apte à s'enficher dans un orifice réalisé dans l'os carpien, cette dite seconde patte étant solidaire du second corps oblong sensiblement en son milieu et de façon que les troisième et quatrième axes soient sensiblement perpendiculaires entre eux, et • des moyens de liaison élastique reliant entre eux les premier et second corps oblongs, ces dits moyens de liaison étant agencés de façon que les premier et troisième axes soient sensiblement orthogonaux et les deuxième et quatrième axes soient sensiblement colinéaires, caractérisée par le fait qu'au moins l'un des deux éléments suivants, les premier et second corps oblongs, comporte une âme centrale réalisée en un second matériau ayant une rigidité supérieure à celle du premier matériau.
