IMPLANT INTRA-ARTICULAIRE INTRA-ARTICULAR IMPLANT

The present invention relates to so-called "intra-articular" implants for the reconstruction of joints between the ends of two of bone, which find a particularly advantageous application for the reconstruction of the joints between the bones of the hand and the bones of the foot when these joints are, which is generally the case, surrounded by a capsule, that is to say a fibrous structure which delimits and connects the bones connected to these joints. Metallic intra-articular implants known as resurfacing already exist for the treatment of osteonecrosis or aseptic osteoporosis in an attempt to maintain the healthy cartilage capital of a joint. Such an implant schematically comprises a wide central tail with lugs which provide anchoring on the bones. It only replaces the affected cartilage zone and its bone sequestrum, and is used only in the most adverse cases. To solve less extreme cases, has been developed an intra-articular implant consisting essentially of a biocompatible fabric interlayer capable of being interposed between the ends of the bones of the joint and means for securing this insert with these ends of 'bone. Such an intra-articular implant is for example described in WO 02/054992. This implant is of a rather complicated configuration and its realization is therefore relatively expensive. In addition, the means for securing the interlayer with the ends of the bones are complex and traumatic for these bones because they include screws or the like which must penetrate. Also, the present invention aims to provide an implant called "intra-articular" for the reconstruction of a joint between the ends of two bones, more particularly in the hand and foot, when in addition the articulation is surrounded by a capsule, which is of a very simple structure inducing a relatively low cost, and which is also easy to implement, thus facilitating the work of the Practitioner while safeguarding the interests of the patient on which the implant must be placed. More specifically, the subject of the present invention is an implant called "intra-articular" for the reconstruction of a joint between the extremities of two bones, more particularly of the hand and the foot, said joint being furthermore surrounded by a connecting capsule. said bone ends, comprising: an interlayer made of biocompatible fabric capable of being interposed between the ends of the two bones of the joint, and means for securing said interlayer with said ends of the two bones, characterized in that said means for securing said interlayer with the ends of the two bones are constituted by at least one connecting wire for connecting at least one point of said spacer with at least one point of said capsule. Other features and advantages of the present invention will become apparent from the following description given with reference to the accompanying drawings by way of illustration, but in no way limiting, in which: FIG. 1 is a side view of a first embodiment of FIG. the intra-articular implant according to the invention, in an application to the restoration of the articulation between the metacarpal bone and the trapezoidal bone of the hand, FIG. 2 is a schematic view from above of the first embodiment of the implant according to Figure 1, that is to say without the bones of the joint, but with a partial representation of the capsule with which the implant is adapted to cooperate, Figure 3 is a schematic view side of a second embodiment of the intra-articular implant according to the invention, and Figure 4 is a schematic side view of a third improved embodiment of the intra-articular implant according to the invention. . It is first of all specified that, in the figures, the same references designate the same elements, whatever the figure on which they appear and whatever the form of representation of these elements.

Similarly, if elements are not specifically referenced in one of the figures, their references can be easily found by referring to another figure. It is also stated that the figures essentially represent three embodiments of the object according to the invention, but that there may be other embodiments that meet the definition of this invention. It is furthermore specified that, when, according to the definition of the invention, the object of the invention comprises "at least one" element having a given function, the embodiment described may comprise several of these elements. Conversely, if the embodiment of the object according to the invention as illustrated has several elements of identical function and if, in the description, it is not specified that the object according to this invention must obligatorily comprise a number particular of these elements, the object of the invention may be defined as comprising "at least one" of these elements. Lastly, it is specified that when, in the present description, an expression defines, by itself, without a specific particular mention concerning it, a set of structural characteristics, these characteristics may be taken, for the definition of the object of the protection requested, when technically possible, either separately, or in full and / or partial combination. With reference to the appended figures, the present invention relates to an implant called "intra-articular" 1 for the reconstruction of a joint 2 between the ends of two bones 3, 4 more particularly of the hand and the foot, when this joint is in in addition surrounded by a capsule 5 connecting the ends of bone 3, 4, which is almost always the case. This implant comprises an element called "interlayer" 10 made of a biocompatible fabric, this insert being able to be interposed between the ends of the two bones 3, 4 of the joint 2, more particularly between the two cartilages ensuring the joint which terminate. both ends of bone and that have been damaged for different reasons, but usually because of the well-known phenomenon of osteoarthritis.

It further comprises means 12 for securing the insert 10 with the ends of the two bones 3, 4. According to an important feature of the invention, these means 12 are arranged to indirectly fasten the insert 10 with the ends of the two two bones 3, 4. More specifically, they are constituted by at least one connecting wire 13 for connecting at least one point of the insert 10 with at least one point of the capsule 5. In this way, as the capsule 5 is secured to both ends of the two bones 3, 4 and as the insert 10 is secured to the capsule 5, 10 the interlayer is also secured to both ends of bone. However, it is conceivable that a single connecting wire can prevent the interlayer from pivoting around this link, even if it is interposed at the contact between the two ends of bone 3, 4. Also, so that Advantageously, the means 12 for securing the interlayer 10 with the ends of the two bones 3, 4 are constituted by at least two connecting son 13 for connecting two distinct points of this insert 10 with two distinct points of the capsule 5. Figure 2 shows such an embodiment where, for even greater security, the connecting son 13 are four in number. They are preferably distributed at the four corners of the biocompatible cloth interlayer 10, when it is substantially in the form of a flat rectangular parallelepiped, as illustrated in this FIG. 2. It is specified that these connecting wires can to be resorbable son or not. If they are not resorbable, to remove them, it will then be necessary to perform another procedure, for example after two to three weeks after the implantation of the implant 1. It is also specified that the implant 1 can be offered for sale, already having the connection son pre-positioned on the intermediate element 10. But it is also possible that it does not include connection son 30 initially pre-positioned. In this case, the Practitioner will put these connection wires just before the implantation of the implant, which will allow him to choose, according to his will, the nature of the son he wants to use, that is to say Absorbable or not, and their location.

Advantageously, the interlayer 10 made of biocompatible fabric is made with PLLA (poly-L-lactic acid) which may have a beneficial effect on at least partial reconstruction of the damaged cartilage of the joint, because the The lactic acid it contains is of the same nature as that which the human body naturally produces. According to a possible embodiment, such as that illustrated in FIG. 3, the interlayer 10 made of biocompatible fabric may consist of a plurality of layers 10-1, 2, 3, ... superimposed one on the other, of preferably by forming a plurality of pleats as illustrated in Figure 3 or a winding. This embodiment makes it possible to obtain, with a single product offered for sale, an implant of a desired thickness, for example by eliminating the number of layers necessary to bring the thickness of the implant to that desired by the practitioner. In this case, the connecting son will be associated as described above to the tab 10 thus produced. For certain therapeutic applications, the interlayer 10 may be constituted, FIG. 4, by at least two layers 21, 22 which define between them a receiving space 24 and at least one strip 28 of a non-resorbable material disposed in this reception space . This band 28 is provided to promote the sliding of the two layers 21, 22 relative to each other and to avoid that there is a "weld" between the two cartilages of the two ends of the bone, so to restore and / or maintain the original mobility of the joint. According to a particularly advantageous embodiment, the material in which this strip 28 is made is silicone. It is further preferable that such an implant according to Figure 4 further comprises means of links 29 for holding and enclosing the band 28 in the receiving space 24, the link means 29 interconnecting the edges of the two layers 21, 22 to close the reception space and to prevent the band 28 from sliding out of this reception space. These connecting means 29 can connect the edges of the two layers 21, 22 in at least one of the following ways: stitches, that is to say very close points made with a single or discrete points, c ' that is, points relatively distant from each other with multiple wires. It may also be advantageous to provide that the connecting wire 13 (or the connecting son 13) constitutes at least part of the link means 29 defined above. The establishment of such an implant is strictly identical to that of the intra-articular implants of the prior art, except that, after the insert 10 has been interposed between the ends of the two bones 3, 4 ( FIG. 1), the Practitioner surrounds part of the capsule 5 with the two portions of connecting threads 13 emerging from the interlayer and knots these two portions of connecting threads on this portion of capsule 5 (FIGS. 1 and 2). ).
