Abstract

Système de fixation d'un plateau tibial pour prothèse de genou, dans lequel le plateau tibial comporte une embase (3) présentant des ouvertures (7) pour le passage de vis (18) destinées à être vissées dans le genou pour assurer la fixation du plateau. Chaque ouverture (7) est prolongée, du côté inférieur du plateau par une cheminée (9) dans laquelle est engagée et vient en butée la tête (13) d'une vis (8) en position de fixation du plateau, la paroi délimitant chaque ouverture (7) étant taraudée (10) et chaque ouverture (7) servant à l'engagement d'une vis (19) destinée à assurer le blocage de la vis de fixation (8).

Description

La présente invention a pour objet un système de fixation d'un plateau
tibial pour prothèse de genou.
L'implantation d'une prothèse de genou nécessite la mise en place, du côté du tibia, d'un plateau tibial.
Pour la mise en place de ce plateau tibial, il est procédé à la résection de la partie supérieure du tibia puis à la mise en place, à la surface ainsi obtenue, d'un plateau tibial. Ce plateau tibial est stabilisé par une tige qui fait saillie de sa partie inférieure et qui pénètre longitudinalement dans le tibia. Le plateau tibial est fixé par plusieurs vis, qui sont vissées dans le tibia, et dont la tête affleure la face supérieure du plateau tibial. La partie métallique du plateau tibial, qui vient d'être décrit, reçoit un plateau en polyéthylène.
II arrive, qu'au cours de la durée de vie de la prothèse, il existe un mouvement relatif entre une vis et l'embase du plateau, se traduisant par un frottement entre la tête de cette vis et le plateau en polyéthylène, assurant la formation de particules d'usure du polyéthylène. Ces particules de polyéthylène peuvent migrer le long des vis, et générer un phénomène d'ostéolyse, c'est-à-dire de dégénérescence osseuse. Dans un tel cas, les vis peuvent prendre du jeu et le plateau peut se désolidariser du tibia.
Un mouvement de jeu de la vis, relativement au plateau, peut provenir d'un tassement de l'appui osseux, la tête de vis remontant dans son logement, et venant en contact avec la face inférieure du plateau en polyéthylène. Outre les particules d'usure qui ont été évoquées précédemment, et qui peuvent provenir de ce contact, la pénétration de la tête d'une vis dans le plateau en polyéthylène peut former une amorce de rupture de celui-ci, ou exercer une force entraînant le décoinçage du plateau en polyéthylène, hors de l'embase métallique du plateau tibial.
Le but de l'invention est de fournir un système assurant une parfaite fixation de l'embase du plateau tibial, et évitant les risques de migration d'éventuelles particules de polyéthylène le long des vis de fixation du plateau.
A cet effet, le système de fixation qu'elle concerne, dans lequel le plateau tibial comporte une embase métallique, présentant des ouvertures pour le passage de vis destinées à être vissées dans le tibia pour assurer la fixation du plateau, est caractérisé en ce que chaque ouverture est prolongée, du côté inférieur du plateau par une cheminée dans laquelle est engagée et vient en butée la tête d'une vis en position de fixation du plateau, en ce que la paroi 2869216 2 délimitant chaque ouverture est taraudée et en ce que chaque ouverture sert à l'engagement d'une vis destinée à assurer le blocage de la vis de fixation.
Chaque vis de blocage assure le blocage d'une vis de fixation en position de fixation du plateau. II n'y a donc pas de risque que les vis de fixation se désolidarisent du tibia et remontent à l'intérieur de leur logement, pour atteindre le plateau en polyéthylène qui recouvre l'embase métallique. En outre, chaque vis de blocage obturant l'ouverture par laquelle a été introduite la vis de fixation correspondante, assure une étanchéité vis-à-vis des particules, et empêche le passage de particules de polyéthylène qui pourraient éventuellement se détacher du plateau en polyéthylène pour des raisons autres que le frottement avec la tête d'une vis de fixation, puisque ce contact est impossible compte tenu de la présence de la vis de blocage.
Avantageusement, en position de fixation du plateau, la tête de chaque vis de blocage affleure la surface supérieure du plateau.
Suivant une autre caractéristique de l'invention, chaque vis de blocage possède un organe de poussée profilé, destiné à prendre appui dans le fond d'une cavité de profil complémentaire ménagée dans la tête d'une vis de fixation.
De préférence, les surfaces d'appui, respectivement de l'organe de 20 poussée et de la cavité de la tête de vis, sont en forme de tronçons de surfaces sphériques.
Il est ainsi possible d'assurer un excellent blocage des vis de fixation, même si une vis de fixation et la vis de blocage qui lui est associée ne sont pas parfaitement alignées.
Suivant une autre caractéristique de l'invention, les surfaces d'appui, respectivement d'une vis de fixation et de la face intérieure d'une cheminée, sont en forme de tronçons de surface sphériques. Cette caractéristique permet de disposer d'une certaine latitude dans l'orientation des vis de fixation, assurant un appui mécanique parfait entre la tête d'une vis de fixation et l'embase du plateau, même si la vis n'est pas parfaitement perpendiculaire au plateau.
De toute façon, l'invention sera bien comprise, à l'aide de la description qui suit, en référence au dessin schématique annexé, représentant, à titre d'exemple non limitatif, une forme d'exécution de ce plateau tibial: - la figure 1 est une vue en perspective partiellement éclatée d'un plateau tibial avec une vis de fixation en position montée, une vis de fixation et une vis de blocage avant leur mise en place, le plateau en polyéthylène étant partiellement arraché pour montrer ces vis.
- la figure 2 est une vue en coupe longitudinale de ce plateau tibial: les figures 3 et 4 sont deux vues partielles, en coupe longitudinale et à échelle agrandie, représentant une vis de fixation et une vis de blocage en position montée, et une ouverture équipée d'un simple obturateur.
Le plateau tibial représenté au dessin, comporte une embase 2 métallique, possédant une partie 3 en forme de plateau, de la face inférieure de la laquelle fait saillie une tige 4 destinée à être engagée dans le tibia. La paroi supérieure 3 est délimitée par un rebord 5, et constitue un logement destiné à recevoir un plateau 6 en polyéthylène. Dans la forme d'exécution représentée au dessin, le plateau 3 comporte quatre ouvertures 7 pour le passage d'autant de vis de fixation 8. II est à noter que le chirurgien peut utiliser les quatre ouvertures 7 pour faire passer quatre vis de fixation 8, ou n'utiliser que certaines de ces ouvertures pour le passage de vis de fixation. Chaque ouverture 7 est prolongée du côté inférieur du plateau par une partie 9 en forme de cheminée. La partie supérieure (10) de l'ouverture, c'est-à-dire celle débouchant dans la face supérieure du plateau est taraudée.
Chaque vis de fixation comprend une partie filetée 12 se terminant par une tête 13. II doit être noté que les surfaces en contact, respectivement 14 de la tête 13 et 15 de la cheminée 9, sont constituées par des tronçons de surface sphérique, permettant de compenser un léger désalignement d'une vis par rapport à l'axe de l'ouverture, sans créer de contrainte mécanique anormale sur le plateau tibial. La tête de chaque vis 8 comprend une cavité 16 dont la partie extérieure présente une empreinte 17 à six pans pour permettre son entraînement par un tournevis de forme complémentaire, et dont le fond 18 est en forme de tronçons de surface sphérique.
La partie supérieure de l'ouverture 7, sert au montage d'une vis 19. Cette vis 19 comporte une tête 20 filetée, de diamètre correspondant au diamètre de l'ouverture 7, destinée à coopérer avec le filetage 10 de cette ouverture. Comme montré notamment à la figure 3, la tête de la vis affleure la face supérieure de la partie métallique du plateau. Cette vis peut être actionnée par l'intermédiaire d'une empreinte 22, formant une empreinte dite "à six pans creux", actionnable par un outil équipé d'une tige de forme complémentaire.
La vis comporte un organe de poussée 23, dont l'extrémité, destinée à venir en appui dans le fond de la cavité 16 de la vis de fixation, possède une surface 24 constituée par un tronçon de surface sphérique, de même forme que la surface 18 formant le fond de l'empreinte 16.
La figure 4 représente une forme d'exécution dans laquelle l'une des ouvertures 7 n'est pas utilisée pour le passage d'une vis. Dans ce cas, un obturateur 25 est mis en place dans la cheminée 9, cet obturateur étant retenu par une vis de blocage 19, analogue à celle qui vient d'être décrite précédemment.
D'un point de vue pratique, le plateau polyéthylène 6 étant retiré, le praticien réalise la mise en place du plateau tibial, en positionnant celui-ci sur le tibia qui a préalablement été préparé, il fixe ensuite le plateau tibial à l'aide des vis 8 qui doivent être utilisées, après quoi les ouvertures non utilisées sont fermées par les obturateurs 25. Le praticien procède ensuite à la mise en place des vis de blocage 19 qui, d'une part, assurent le blocage des vis de fixation dans leur cavité respective et, d'autre part, assurent une étanchéité évitant la migration de particules de polyéthylène à partir du plateau 6, en direction des vis 8.
Comme il ressort de ce qui précède, l'invention apporte une grande amélioration à la technique existante, en fournissant un système de fixation d'un plateau tibial, de structure simple, garantissant une excellente fixation de ce plateau, et évitant la migration de particules de polyéthylène le long des vis de fixation.
Comme il va de soi, l'invention ne se limite pas à la seule forme d'exécution de ce système, décrite ci-dessus à titre d'exemples, elle en embrase au contraire toutes les variantes demeurant dans le cadre des
revendications qui suivent.

Claims (5)

REVENDICATIONS
1. Système de fixation d'un plateau tibial pour prothèse de genou, dans lequel le plateau tibial comporte une embase (3) présentant des ouvertures (7) pour le passage de vis (18) destinées à être vissées dans le genou pour assurer la fixation du plateau, caractérisé en ce que chaque ouverture (7) est prolongée, du côté inférieur du plateau par une cheminée (9) dans laquelle est engagée et vient en butée la tête (13) d'une vis (8) en position de fixation du plateau, en ce que la paroi délimitant chaque ouverture (7) est taraudée (10) et en ce que chaque ouverture (7) sert à l'engagement d'une vis (19) destinée à assurer le blocage de la vis de fixation (8).
2. Système selon la revendication 1, caractérisé en ce qu'en 15 position de fixation du plateau, la tête (20) de chaque vis de blocage (8) affleure la surface supérieure du plateau.
3. Système selon l'une des revendications 1 et 2, caractérisé en ce que chaque vis de blocage (8) possède un organe de poussée profilé (23), destiné à prendre appui dans le fond d'une cavité (16) de profil complémentaire ménagée dans la tête (13) d'une vis de fixation (8).
4. Système selon la revendication 3, caractérisé en ce que les surfaces d'appui (24, 18) respectivement de l'organe de poussée (23) et de la cavité (16) de la tête de vis (8) sont en forme de tronçons de surfaces sphériques.
5. Système selon l'une des revendications 1 à 4, caractérisé en ce que les surface d'appui (14, 15) respectivement d'une vis de fixation (8) et de la face intérieure d'une cheminée (9) sont en forme de tronçons de surface sphériques.
