The targeting of rays during a specific localized treatment of a patient or waves, such as in particular shock waves during an intervention to destroy lithiasis, is particularly important and must be all the more precise as the Target location defects can cause serious lesions on surrounding tissues, cause painful reactions in patients and affect the effectiveness of treatment. It is therefore very important that this targeting can be carried out adequately with great precision and reliability.
The use of ultrasound visualization equipment is known, particularly with the lithotreators for which it is a question of proceeding precisely to the location of the lithiasis and to make coincide the receiving hearth of the reflector with this last one to carry out the shots , the emitter hearth having previously been positioned on the generator of the shock waves. In particular for this application, the precise positioning of the lithiasis is an essential condition to guarantee the efficiency of the apparatus. Some known devices include means for moving the patient relative to the apparatus to bring the lithiasis and the receiving home into coincidence. Other devices are arranged to bring the receiving focus mechanically linked to the device in coincidence with the lithiasis. In yet another known embodiment, the apparatus comprises a mechanical locating arm on which the ultrasound probe is attached, this arm being provided with sensors for locating the probe in space. In all cases, the constructions are heavy and cumbersome. In addition, all the components, and in particular the display means, are dedicated, which makes the devices considerably more expensive and does not make them easy to use. The method according to the invention makes it possible to overcome the drawbacks of the devices of the prior art and to effectively identify an internal anatomical anomaly so that it can then be processed. This procedure, although particularly suitable for the treatment of lithiasis, is also effective for all treatments that require focusing of ray waves on an internal anatomical anomaly and, for this purpose, precise determination of the coordinates of this anomaly relative to the generator of waves or rays is needed. This object is achieved by the method according to the invention, characterized in that one initially proceeds to a calibration of said cameras by pointing with the ultrasound probe fixed predetermined points of a work area corresponding to the zone in which the internal anatomical anomaly is located, in that this internal anatomical anomaly is subsequently identified by means of the ultrasound probe and records the spatial position of said probe using said cameras, what is determined calculates the distance between the ultrasound probe and the internal anatomical anomaly to determine the spatial coordinates of the latter, in that one automatically positions the treatment device in such a way that the rays or the emitted waves are focused on said internal anatomical anomaly. According to a preferred embodiment, in order to proceed with the calibration of said cameras, the four angles of a work plane corresponding to the working area are plotted and the images of these four angles are fixed by means of the two cameras.
Particularly advantageously, all shots are taken by means of a first camera which is arranged substantially perpendicularly to the working plane and by means of a second camera which is angularly offset by approximately 90 relative to the first camera. camera.
The present invention will be better understood with reference to the description below of a preferred embodiment of the method according to the invention and with reference to drawings, given by way of non-limiting examples, in which FIG. schematic view illustrating the camera position for implementing the method according to the invention, and Figure 2 is a schematic view of the ultrasound image illustrating an essential phase of the method.
FIG. 1 diagrammatically represents a display device 10, comprising an ultrasound probe 11, preferably a cylindrical probe, and two video cameras 12 and 13. The first camera 12 is arranged substantially perpendicular to a plane 14 which defines a working area in which is located said internal anatomical anomaly and the second camera 13 is angularly offset from the first camera. In the example shown, the first camera is substantially vertical and the second camera is shifted by 90. As a result, the first camera 12 reads the XOY coordinates of the positions pointed by the ultrasound probe 11 and the second camera records the YOZ coordinates of these positions. The cameras 12 and 13 fix the captured image and record the position of the probe 11 in the space of the processing apparatus to which the display device 10 is associated. A calculator then makes it possible to determine the spatial coordinates of the probe from these coordinates and the ultrasound image of the internal anatomical anomaly obtained, to determine the spatial coordinates of this anomaly in the space of the treatment apparatus. During an initial phase of calibration, the operator points the four corners of the work plane, using the ultrasound probe, these points being viewed by means of a pointer. This approach aims to define the spatial coordinates of the cameras in relation to the work plan. In the operative phase, the operator points, using the ultrasound probe, the internal anatomical anomaly to be treated, in particular a lithiasis, and acquires a fan image 20, as illustrated in FIG. this anomaly. When the spatial coordinates of the probe have been established by the cameras, the computer is able to determine the distance d which separates the probe from the anomaly, by measuring directly on the display screen the ultrasound image of the probe. 11, the distance d between the substantially central point 21 of the anomaly and the upper edge 22 of the ultrasound image 20. The computer is arranged to calculate the relative position of the anomaly to be treated and the treatment source which is then positioned appropriately. In the case of a lithotripter, the emitting focus is positioned relative to the patient so that the receiving focus coincides with the lithiasis to be treated. In the case of a radiation treatment, the source generating the rays is positioned in such a way that they are focused on the anomaly to be irradiated.
