Abstract
Description
The present invention relates to the field of orthopedic surgery.
The present invention relates to a device consisting of an instrument for extracting the medullary rods, final or test, from the endoprostheses of the medullary canal in which they are implanted, in particular the femoral hip rods.
L'arthroplastie articulaire consiste à remplacer les parties de l'articulation endommagées ou pathologiques par des pièces prothétiques constituées notamment de tiges métalliques scellées ou impactées à force dans le canal médullaire de l'os.Joint arthroplasty consists in replacing the damaged or pathological parts of the joint with prosthetic parts made up in particular of metal rods sealed or impacted by force in the medullary canal of the bone.
When placing the implants, the surgeon is led to use test rods and trial rasps which, due to their wedge shape, can become trapped in the medullary canal. The surgeon must remove them quickly and without damage.
In use, the stents get damaged: they wear out, loosen, or even break. they may also not be supported by the body which reacts to the presence of a foreign body.
Their lifespan is less than the lifespan of the patient.
In all these cases the surgeon must practice the ablation of the material to, in general, replace it. It is therefore necessary to extract the medullary rods, which is all the more difficult since they are designed to be fixed as firmly as possible in the medullary canal, for example: by using cement, by using self-locking forms, by promoting bone bonding by surface effect.
Currently, the simplest extraction instruments are of the punch type: they consist of an axis terminated by an anvil and the other end of which is positioned in a blind bore located under the neck of the spinal cord. To extract the medullary rod, the surgeon places the end of the punch in the hole and strikes with the hammer on the anvil. These instruments have the following disadvantages:
- they can only be used on medullary stems which have the appropriate piercing,
- taking into account the eccentric position of the bore relative to the axis of the medullary rod, the
extraction stresses induce a moment of rotation which produces bracing at the level
of the stem, unfavorable for its extraction.
accessible.- in certain ways initially practiced by the surgeon, the piercing under the collar is not
accessible.
According to other devices, the instrument consists of a rigid force transmission element, curved or not, of a force generating means, fixed or adaptable, and of a gripping element of the neck of the medullary rod of open form such as hook, or of closed form such as flared ring, manually positioned by the surgeon so that it takes support under the prosthetic head, the latter acting as a stop during extraction. These instruments have the following disadvantages:
- they only fit on certain monobloc rods (the prosthetic head and the medullary rod do not
form a single element) of defined geometry and not on the modular rods (the prosthetic head
and the spinal cord are in two removable parts)
- they are not locked to the spinal cord, which leads to poor control of the good
positioning of the instrument during extraction.
cours des manipulations.- when the diameter of the prosthetic head is not much larger than the diameter of the neck of the
spinal cord, the support under the prosthetic head is insufficient and the instrument slides
during the manipulations.
nécessite des efforts d'extraction importants.- the open-shaped gripping elements deform when the extraction of the medullary rod
requires significant extraction efforts.
According to other devices, the instrument consists of a rigid force transmission element, curved or not, of a force generator means, fixed or adaptable, and a locking system which makes it possible to block the instrument with the spinal cord to be extracted. Locking systems, such as: jaws, elastic cylindrical forceps, vice, hang on the neck or on the end cone of the medullary stems. These instruments have the following disadvantages
- given the shapes, dimensions and mechanisms of the locking systems, they do not
only fit on certain modular rods of defined geometry and not on monoblock rods.
l'utilisation d'autres instruments.- they require the disassembly of the prosthetic head to allow their installation on the neck or
on the rod cone. Disassembly of the prosthetic head is often difficult and requires
the use of other instruments.
- they are unsuitable for the extraction of the humeral medullary stems.
In many cases, the surgeon is not aware of the brand, type and model of the stent to be explanted, even after preoperative radiography. It therefore does not have specific adapted equipment (if it exists). n then uses common instruments: pliers, punches, scissors, which are very often ineffective when the implant is properly sealed. He then practices:
- either the cutting of a diaphyseal window and the extraction from below,
- either the tapping of the rod and the extraction by using a threaded rod.
These operating procedures are long and fraught with consequences for patient safety because:
- the duration of anesthesia is increased,
- the period of care and postoperative recovery are increased,
- the anatomical environment risks being damaged.
The device according to the invention is an instrument which makes it possible to remedy these drawbacks.
The instrument is of the type comprising a rigid force transmission element, curved or not, a force generator means, fixed or adaptable, and a locking system which makes it possible to block the instrument to the medullary rod to be extracted, characterized in that it consists of a threadlike, deformable and resistant body, with one or more strands, such as: cable, chain, textile or composite braid, which is manipulated so that the gripping orifice d end, constituted by a loop of said body, expands and then closes on the extra-osseous part of the medullary rod to secure the instrument and the endoprosthesis to be extracted. The end loop can be sliding, such as: slip knot or U-shaped.
This characteristic allows the device to be universal; it allows to grip any spinal cord, of any brand, any type:
- such as: monobloc, modular, femoral, humeral, of any shape and any morphology: - such as: spherical, hemispherical head; ; cylindrical, conical, prismatic neck, whatever its
position, of any dimensions in diameter and in length:
- from a few millimeters to several centimeters, which only depends on the elongation of the body
filiform, deformable and resistant.
This feature also eliminates the need to disassemble the modular prosthetic heads to grip the modular medullary rods.
This characteristic also makes it possible to grasp one-piece medullary rods the head of which is much larger than the neck.
According to the other characteristics described below, the instrument is efficient, easy to use and allows the surgeon to quickly extract the medullary rod with security.
According to particular embodiments: - the lower attachment end of the instrument, removable or not, which makes it possible to grip the extra-osseous part of the medullary rod, can consist of the end loop of the body filiform, deformable and resistant and an open geometry anchoring jaw in the shape of a V or curvilinear or self-locking, adapted to the shapes of the extra-osseous parts to be gripped. By tension, the end loop of the filiform, deformable and resistant body immobilizes the extra-osseous part gripped against the anchoring jaw.
- le mors d'ancrage, démontable ou non, peut être de géométrie fermée en forme d'anneau ou d'oblong, à l'intérieur de laquelle passe la partie extra-osseuse à agripper.- The anchoring jaw, removable or not, can be of closed geometry in the form of a ring or oblong, inside which passes the extra-osseous part to be gripped.
The surface of the anchoring jaw may include roughness or protruding parts which promote a tenacious connection between the instrument and the medullary rod to be extracted.
The threadlike, deformable and resistant body may include a protective eyelet which may be non-slip and of a shape adapted to the shapes of extra-osseous parts to be gripped.
- la manipulation du corps filiforme, déformable et résistant peut se faire par l'intermédiaire de poignées d'entraînement comportant ou non des éléments démultiplicateurs d'efforts, ou par des éléments d'entraînement motorisés.- The manipulation of the filiform, deformable and resistant body can be done by means of drive handles with or without force reduction elements, or by motorized drive elements.
The filiform, deformable and resistant body can be guided in its movements by orifices or obstacles such as: bores, grooves, hoods, sheaths, which impose a defined trajectory on it.
La coaxialité peut être obtenue par ajustement.- thanks to its curved shape, the axis of the instrument can be coaxial with the axis of the medullary rod allowing it to exert only axial tensile extraction forces on it. The moments induced on the stem, which hamper by bracing its extraction, are canceled. Coaxiality can be obtained by adjustment.
- The upper end of the instrument can end with an adapter which allows the attachment of a pneumatic, ultrasonic, electric or electromagnetic vibrating motor which makes it possible to exert repeated stresses.
The upper end of the instrument can end with a sliding counterweight, removable or not, which allows inertia to exert axial tensile forces.
The upper end of the instrument can end with an anvil, removable or not, which allows the user to strike on the instrument.
The accompanying drawings illustrate the invention:
- Figure I shows the sectional instrument anchored on the neck of a femoral stem,
FIG. 2 represents a variant of FIG. 1 with an adjustable and removable anchoring jaw,
- Figures 3, 4, 5, 6, 7 and 8 show the upper and lower ends of the instrument according to
variants with the end views 3A, 4A and 5A corresponding to Figures 3, 4 and 5.
- Figures 9 and 10 show two types of manipulation handles.
Referring to Figure 1, the instrument has at its hooking end a cable (1) whose U-shaped end loop (2), once extended by loosening, allows to enclose the femoral neck (3) then, by tightening, blocking the neck (3) against an anchoring jaw (4). The cable is guided in its movements by two holes (5) drilled in a curved axis (6). The cable (1) is welded to a support washer (7) immobilized in rotation by a pin (8) positioned in a vertical groove (9) of the curved axis (6). The support washer (7) is driven in translation by a threaded handle (10) manipulated by the surgeon by screwing-unscrewing. The handle (10) is screwed on a threaded axis (1 1) which ends in a grooved adapter (12) which allows the attachment to a vibrating motor. Thanks to the curved shape of the curved axis (6), the axis of the adapter is coaxial with the axis of the femoral stem, which induces thereon only axial tensile forces.
In the embodiment according to Figure 2, the attachment end of the instrument consists of a threaded jaw (13) which allows disassembly and which allows to obtain by adjustment the alignment between the axis of the adapter and the axis of the femoral stem.
Selon la figure 5, le câble comporte un oeillet de protection antidérapant (14).The surfaces of the jaw have aspects favoring attachment: knurling (FIG. 3A), raking (FIG. 4A), beveled (FIG. 5A). According to Figure 5, the cable comprises a non-slip protective eyelet (14).
According to Figure 6, the anchoring jaw is oblong, flared towards the rear, inside which is engaged the femoral head.
(16),
- d'un axe fileté (11) terminé par une large enclume (17).According to the variants illustrated in FIGS. 7 and 8, the upper end of the instrument is made up respectively:
- a threaded axis (11) terminated by an anvil (15) and on which slides an inertia counterweight
(16),
- a threaded axis (11) terminated by a large anvil (17).
According to FIGS. 9 and 10, the handle for handling the cable is respectively:
- cylindrical and grooved to facilitate handling,
- foldable with non-slip undulations.
By way of nonlimiting example, all the elements of the instrument are made of stainless steel to, in particular, allow sterilization.
By way of nonlimiting example, the instrument will have dimensions of the order of:
- length: 250 mm,
- diameter of the handling handle: 40 mm,
- opening diameter of the cable loop: 50 mm, - closing diameter of the cable loop: 10 mm.
