Abstract

Le dispositif d'assemblage entre deux parties d'un élément prothétique notamment, comprend au moins une vis (3) dont la tête (3a) est montée dans un logement (1b) de l'une des parties (1) avec capacité de déplacement en translation limité et dont la tige (3b) est vissée dans l'autre partie, pour permettre, en fonction du sens d'entrainement de la vis, (3) le rapprochement ou l'éloignement de deux portées coniques mâle (2b) - femelle (1a) que présentent, d'une manière complémentaire, l'une et l'autre des parties.

Description

Dispositif d'assemblage entre deux Darties d'un élément prothétiaue notamment.
L'invention concerne les implants orthopédiques. Plus particulièrement, I'invention concerne tout élément prothétique, comprenant deux parties devant être assemblées l'une part rapport à l'autre.
Avantageusement, le dispositif d'assemblage s'applique pour une prothèse de l'articulation de l'épaule, comprenant un implant huméral coopérant avec un implant glénoïdien, adapté sur un support fixé dans la glène de l'épaule. Notamment, le problème que se propose de résoudre l'invention, est d'améliorer l'assemblage entre l'implant glénoïdien et le support.
Généralement, le support est de forme générale circulaire, et présente une portée externe filetée pour être vissée dans une portée taraudée que présente de manière complémentaire, L'implant glénoïdien.
Une telle solution n'est pas totalement satisfaisante car on ne peut exclure des risques de dévissage.
On a proposé également des systèmes de clipage complémentaires entre le support et l'organe glénoïdien. Dans ces conditions, le démontage de l'élément glénoïdien par rapport au support est très difficile, voir impossible. Or, un tel démontage est nécessaire.
Pour cette raison, un assemblage du type cône morse est également difficilement envisageable, étant donné que des problèmes vont apparaître pour le démontage.
L'invention s'est fixée pour but de remédier à ces inconvénients.
Le problème que se propose de résoudre l'invention est d'assurer un assemblage sûr et efficace entre les deux parties de l'élément prothétique considéré, tout en ayant la possibilité de les désolidariser facilement.
Pour résoudre un tel problème, il a été conçu et mis au point un dispositif d'assemblage qui comprend au moins une vis dont la tête est montée dans un logement de l'une des parties avec capacité de déplacement en rotation et en translation limitée et dont la tige est vissée dans l'autre partie, pour permettre, en fonction du sens d'entrainement de la vis, le rapprochement ou l'éloignement des deux portées coniques mâle - femelle que présentent, d'une manière complémentaire, I'une et l'autre des parties.
Pour résoudre le problème posé de rapprocher ou d'éloigner les deux portées coniques, la vis est disposée coaxialement aux deux portées et est vissée dans un trou taraudé de la portée mâle.
Pour résoudre le problème posé d'emprisonner la tête de vis dans son logement, ce dernier est obturé d'un côté, par un organe rapporté fixé d'une manière démontable dans un agencement de la partie correspondante.
L'organe rapporté est, soit fixé à partir de l'ouverture de la portée femelle de la partie correspondante, soit fixé à l'opposé de la portée femelle.
Avantageusement l'organe rapporté est une rondelle qui présente une portée externe circulaire filetée.
Pour résoudre le problème posé d'éviter de desserrer la rondelle lors de l'assemblage des deux parties, le pas de vis de la rondelle est inverse de celui de la vis d'assemblage.
Dans le cas d'une application du dispositif à une prothèse d'articulation de l'épaule, la partie recevant la tête de vis est constituée par une calotte sphérique male, d'un implant glénoidien huméral présentant la portée conique femelle, L'autre partie étant constituée par un support de section circulaire dont le bord périphérique constitue la portée conique male, ledit support présentant un plot central dans lequel est vissée la vis.

Comme indiqué, le dispositif d'assemblage selon l'invention, s'applique à tous types d'éléments prothétiques présentant deux parties (A) et (B) devant être assemblées l'une part rapport à l'autre, avec capacité de démontage. Par exemple, la partie (A) constitue un implant glénoîdien huméral (1), tandis que la partie (B) constitue un support (2) destiné à recevoir ledit implant (1). Le support (2) présente, de manière connue, un plot central coaxial (2a) destiné à être logé dans la glène.
Le support (2) est de forme générale circulaire et présente une portée mâle conique (2b) conformée pour être engagée dans une portée femelle complémentaire conique (la) de l'implant glénoidien (1). A ce niveau, on obtient donc, entre le support (2) et implant (1) un assemblage du type cône morse.
Selon une caractéristique à la base de l'invention, cet assemblage est complété par une vis (3). La fonction de cette vis est, d'une part, de permettre la liaison par coincement des portées coniques mâle femelle (2b) (la) correspondant à la solidarisation des deux parties (A) et (B) et, d'autre part, de permettre le désaccouplement desdites portées (2b) (1 a), correspondant à la désolidarisation des deux parties (A) et (B).
Dans ce but, la tête de vis (3a) est montée dans un logement (1 b) de la partie (A) par exemple, avec capacité de rotation et de déplacement en translation limitée. La tige (3b) de la vis (3) est vissée dans l'autre partie (B), de sorte qu'en fonction du sens d'entrainement de la vis (3), compte-tenu de la possibilité de déplacement limité en translation de cette dernière dans le logement (1 b), on obtient le rapprochement ou l'éloignement relatif des deux portées coniques (2b1) (las). Dans l'exemple illustré, la vis est vissée dans un taraudage (2al) du plot central (2a).
Comme le montre la figure 2, par exemple, le logement (lob) est obturé par une rondelle (4) fixée dans la partie (A). Par exemple, la rondelle (4) est vissée dans un chambrage (1c) formé coaxialement à partir du fond de la portée femelle (1a).
Avantageusement, la rondelle présente une portée externe filetée (4a) dont le pas est inverse de celui de la vis (3).
La vis (3) est engagée dans le logement (1b) à partir de la portée femelle (la) en y étant maintenue par la rondelle (4). Une solution inverse peut être prévue, selon laquelle la vis est engagée à partir de l'extérieur de la partie (A), c'est-à-dire à l'opposé de la femelle. Dans ce cas, comme le montre la figure 4, le logement (1b) est débouchant et est obturé par un bouchon fileté (5) par exemple.
La tête de vis (3a) présente une empreinte interne polygonale (3a1), pour être manoeuvrée par une clé correspondante, engagée dans un trou débouchant (il), formé coaxialement au logement (lob) recevant ladite tête.
Le fonctionnement du dispositif d'assemblage, tel que décrit est particulièrement sûr et efficace.
La vis (3) est montée dans les conditions indiquées, dans le logement (lob) de la partie (A), par exemple l'implant glénoïdien (1). La vis (3) est engagée dans le trou taraudé du plot central (2a) du support (2), de sorte qu'une action de serrage exercée sur ladite vis a pour effet d'engager la portée conique mâle (2b) du support (2), dans la portée conique femelle complémentaire (1a), provoquant, de manière concomitante, le coincement desdites portées, constituant ainsi un assemblage du type cône morse.
Dans ce cas, la tête de vis est en appui dans le fond du logement (il), au niveau de sa partie de raccordement avec la tige filetée (3b) (figure 2).
Inversement, une action de desserrage exercée sur la vis provoque d'une manière concomitante, I'éloignement du support (2) par rapport à l'implant glénoïdien (1), la tête de vis étant dans ce cas, en appui dans le fond du logement, au niveau de sa face externe (figure 3). Dans ce cas, la vis crée une force d'expulsion permettant le désaccouplement de l'assemblage du type cône morse.
Les avantages ressortent bien de la description, en particulier on souligne et on rappelle:
- La combinaison d'un assemblage par vis et cône morse permettant d'obtenir, d'une part, un blocage sûr et efficace des deux parties de l'ensemble considéré et, d'autre part, un désaccouplement aisé.
- La facilité de manoeuvre.
- La simplicité de réalisation.
- L'application à tout type d'ensemble prothétique, présentant au moins deux parties à assembler.

Claims (9)

    REVENDICATIONS I'une et l'autre des parties.
  1. -1- Dispositif d'assemblage entre deux parties d'un élément prothétique notamment, caractérisé en ce qu'il comprend au moins une vis (3) dont la tête (3a) est montée dans un logement (1b) de l'une des parties (1) avec capacité de déplacement en translation limité et dont la tige (3b) est vissée dans l'autre partie, pour permettre, en fonction du sens d'entrainement de la vis, (3) le rapprochement ou l'éloignement de deux portées coniques mâle (2b) - femelle (la) que présentent, d'une manière complémentaire,
  2. -2- Dispositif selon la revendication 1, caractérisé en ce que la vis (3) est disposée coaxialement aux deux portées (2b) (la) et est vissée dans un trou taraudé de la porte mâle.
  3. -3- Dispositif selon la revendication 1, caractérisé en ce que le logement (1 b) de la tête de vis (3a) est obturé d'un côté, par un organe rapporté (4) fixé d'une manière démontable dans un agencement de la partie correspondante.
  4. -4- Dispositif selon la revendication 1, caractérisé en ce que l'organe rapporté (4) est fixé à partir de la portée femelle (la) de la partie correspondante.
  5. -5- Dispositif selon la revendication 1, caractérisé en ce que l'organe rapporté (4) est fixé à l'opposé de la portée femelle (1 a).
  6. -6- Dispositif selon la revendication 1, caractérisé en ce que l'organe rapporté (4) est une rondelle.
  7. -7- Dispositif selon la revendication 1, caractérisé en ce que la rondelle (4) présente une portée externe circulaire filetée (4a).
  8. -8- Dispositif selon la revendication 1, caractérisé en ce que le pas de vis de la rondelle (4) est inverse de celui de la vis d'assemblage (3).
    L'autre partie étant constituée par un support (2) de section circulaire dont le bord périphérique constitue la portée conique male (2b), ledit support (2) présentant un plot central (2a) dans lequel est vissée la vis (3).
  9. -9- Dispositif selon la revendication 1, caractérisé en ce que la partie recevant la tête de vis (3) est constituée par une calotte sphérique male (1), d'un implant glénoïdien huméral présentant la portée conique femelle (la),
