Abstract
Description
AGRAFE LIGAMENTAIRE PERFECTIONNEE
The present invention relates to a new ligament staple. This clip is more particularly intended for the fixation of artificial or natural ligaments during operations for restoring deficiencies or for therapy for injuries to the cruciate ligaments of the knee, shoulder injuries and the like.
There are known in the prior art a number of ligament staples formed from a more or less flat head bracing two generally rough legs or, at least, provided with non-return elements preventing the withdrawal of the staples after their implantation in the bone.
Document EP-A-O 127 994 describes staples provided with a head and legs, said head having a flattened and protruding upper section, extending outside the surface delimited by the legs. The legs are fitted with non-return scales.
Document GB-A-2 154 688 describes staples provided with a head and legs, of general triangular section, the two external faces being provided with non-return elements, that is to say retention bards.
GB-A-2 118 474 describes staples and ancillary equipment for extracting the staples. This extraction material grips the head of the staple, the shape of which approximates that described in EP-A-O 127 994.
Document FR-A-2 628 312 describes staples and ancillary impact material. The head has two opposite longitudinal grooves, arranged on its two large front and rear faces, used to receive the two jaws of the appropriate impact tool in the form of pliers.
These staples, the implantation of which is most often only temporary since the ligament in place is quickly rehabilitated by natural ligament and bone tissues, must normally be removed. It is however often preferred to leave it in place for reasons essentially due to the fact that the staple, once the tissues are rehabilitated, attaches particularly well to the surrounding tissues. The extraction of the staple would then require a fairly substantial new incision in the implantation area, with the disadvantage of operating suites, always more or less traumatic for the patient, and above all the weakening of the ligament at the level of the incision.
On the other hand, most of the staples currently available have a head having a shape such that, left in place, for example on the crest of a tibia in the case of a restoration of the posterior cruciate ligament of the knee (called LCP), it does not present a relief risking, at least, to surface under the skin in an unsightly manner, or to be accidentally hung, or even to create an inflammation of a tendon or subcutaneous tissue .
The staples for extraction do not have this profile and therefore are not suitable for final implantation.
We are therefore looking for a staple that can be implanted and extracted easily, while presenting a profile that allows final implantation.
The present invention aims to remedy these drawbacks by proposing a new ligament staple.
Thus, the present invention provides a ligament staple comprising a head from which two legs extend substantially perpendicularly, the head having an upper part whose section is flattened and convex and a lower part carrying at least one groove.
The new staple according to the present invention is therefore a ligament staple intended for fixing natural or artificial ligaments to the bones. More particularly, the present clip will be used with advantage during operations for restoring deficiencies or for repairing therapy for ligament trauma. The ligaments concerned are any ligament, but in particular the ligaments of the knee, for example anterior and posterior cruciate ligament, the ligaments of the shoulder, of the acromioclavicular joint and the like.
The head of the clip according to the present invention provides a double advantage. Indeed, the upper part has a flattened and convex section, and therefore does not have a sharp angle. This allows the staple to be left in place permanently, significantly reducing the risks resulting from the protrusion of the head of the staple under the skin. As an example of a head, mention may be made of a head also having chamfers.
Furthermore, the head also has, at its lower level, that is to say in contact with the bone, at least one groove. This recessed groove can constitute, if necessary, a fulcrum to an extractor tool with lever arm, having a distal end capable of engaging in said groove. The grooves also provide a larger and safer grip for the ancillary extraction tool.
Des extracteurs sont connus dans l'art; à titre d'exemple, on peut citer l'extracteur selon la demande FR-A-2 674 741.Extractors are known in the art; by way of example, there may be mentioned the extractor according to application FR-A-2 674 741.
In an alternative embodiment of the invention, the head has two grooves arranged symmetrically with respect to the median plane separating the clip along its greatest length; this arrangement makes it possible to extract the clip in all cases, without worrying about knowing the side where a single groove would be.
Preferably, each groove extends in the direction of the length of the head, which facilitates access to the extractor tool when it is necessary to remove the staple.
According to one embodiment, each groove has a width representing 10 to 30 of the width of the clip.
According to one embodiment, the lateral parts each carry at least two recesses. These recesses allow easier gripping of the clip. This grip can be useful during extraction, an extractor lever adapting in these recesses can also be used and thus avoid making an excessive incision in the implantation area. This second extractor can be used in a coordinated manner in the form of a lever with the first extractor. These recesses also allow the use of ancillary impaction material which ensures an effective grip. This material can, for example, include two jaws in the shape of a clamp, each jaw carrying a nipple corresponding to the recess. Once the jaws and nipples are positioned facing the recesses, the mutual approximation can be caused by displacement of a ring tightening until you firmly pinch the clip. It is then perfectly attached to the tool and can be manipulated, pressed in or removed easily and precisely.
Preferably, the lateral parts each carrying two recesses located substantially in the axis of the legs.
Thanks to the gripping parallelepiped thus formed according to this variant, the grip is perfectly ensured.
The legs of the staple can have any suitable shape. However, in the context of the present invention, staples are preferred in which the legs of the staple have a triangular section. According to this variant, preference will be given to staples in which the legs of the staple are mounted on the head so that a facet of one of the legs is vis-à-vis with a facet of the other leg.
These legs are advantageously provided with means allowing a solid anchoring. Thus, it is preferred that non-return elements are arranged on the legs.
In order to obtain reliable attachment of the ligaments, it is preferred, in the context of the present invention, that spikes are provided on the lower part of the head.
According to this embodiment, the tips are located between the two grooves.
- la figure 1 est une vue de côté de l'agrafe;
tête de l'agrafe;
levier;
Other characteristics and advantages of the present invention will emerge more clearly from the description which follows of a preferred embodiment of a ligament staple, given by way of nonlimiting example with reference to the appended drawing in which:
- Figure 1 is a side view of the clip;
- Figure 2 is a view of the lower part of the
staple head;

Figure 1 is a side view of a clip according to the present invention. This clip is conventionally made up of a head 1, two lateral tabs 2, the head forming a bridge between the tabs. The head is flattened in width and in length. In Figure I, there are also points 5. Indeed, it has proven useful to have points 5, for example conical, between the two grooves 3 of the lower part of the head 1 of the clip. These points 5, which can be arranged in a row or in staggered rows, ensure better fixation of the ligament or of the reimplanted tendon, avoiding any risk of slipping and crushing of the latter. These points 5 could be arranged elsewhere than between the two grooves 3. The legs 2 are further provided with non-return bards 2a, ensuring good adhesion, preventing the staple from rising and therefore offering a secure anchoring. These bards can more generally be any element having a non-return function, roughness or roughness, or the like.
In Figure 1, there are also two recesses 4 located in alignment with the legs. The recess shown has a generally cylindrical shape, but any other shape is suitable. Furthermore, the free end of the tabs 2 is bevelled inwards so that, in a manner known per se, the tabs 2 of the clip tend to approach each other when the clip is implanted in the bone.
According to Figure 2, the lower part of the head 1 has two grooves 3 extending longitudinally between the two legs 2. In the embodiment shown in the drawing, these two grooves 3 have a width substantially equal to 10% of the small width of the clip, which is sufficient for the engagement of the distal end of a lever arm extractor. In Figure 2, there are also legs with a triangular shape. Each leg is mounted so that a facet of one of the legs is opposite with a facet of the other leg. Furthermore, according to this embodiment in which the tabs are triangular, the non-return elements 2a are present on the two facets located on the outside of the clip. This choice of 2 triangular tabs makes it possible to penetrate the clip in an atraumatic way, that is to say injuring and damaging the ligament, tendon and bone tissues less than conventional staples with quadrangular legs.
Figure 3 shows how the staple can be extracted using an extractor forming a first order lever.
This type of extractor has an "S" shape with a distal end 6 in the shape of a spatula curved upwards to normally engage under the staple to be extracted, a bulged part located on the side opposite the spatula providing support. on the bone for the user to allow him to pry under the clip using the handle.
Figure 4 shows a preferred staple according to the present invention. Head 1 is rounded and its dimensions are: 12 mm x 5 mm x 1.5 mm. The legs 2 are triangular, their length is 20 mm from the base of the head, for a width of the triangular base of 3.5 mm. Non-return elements 2a are arranged along these legs. The legs are spaced 8 mm apart. The clip has two grooves 3, represented by the dotted line, and extending over 8 mm, i.e. the wheelbase. These grooves are circular, with a radius of 0.63 mm. Two recesses 4 are also present, their depth is 0.75 mm, which corresponds to the space not occupied by the legs. These recesses are located in the axis of the legs. The height of each recess is 1.15 mm, with a radius for the rounding of 0.65 mm. Conical points 5 are present. Their length is 2 mm, for a diameter at the base of 1 mm. The spacing is 2 mm between each tip. Furthermore, circular reinforcements 2b, with an internal radius of 1.5 mm and 1 mm in width, are present to increase the rigidity of the assembly.
Of course, the present invention is not limited to the embodiments described and shown but it is capable of numerous variants accessible to those skilled in the art without departing from the spirit of the invention.
