Abstract
Description
Tige fémorale pour Prothèse de hanche. Femoral stem for hip prosthesis.
The invention relates to the technical field of orthopedic implants and more particularly hip prostheses.
Many solutions have been proposed in an attempt to obtain the safest and most effective metaphyseal fixation possible, since it is at this level that the fixation of the femoral stem must be particularly well suited.
Solutions have also been proposed to take account of the fact that there are in reality two large families of femur, namely the substantially straight or straight femurs and the curved femurs, that is to say with large internal curvature.
Patent application EP 91420329.4, of which the applicant of the present also holds a hip prosthetic assembly, comprising two families of femoral rods, each corresponding to straight, substantially rectilinear femurs and to curved femurs.
The femoral stems of each of the families have, at their proximal part, on the one hand, in the sagittal plane, an anterior curvature and, on the other hand, in the frontal plane, an internal curvature.
Such characteristics prove to be particularly important and effective, for straight and curved femurs, generally corresponding respectively to elderly and young subjects. On the other hand, this solution does not tackle the problem of taking into account the extra-curved femurs which one very generally meets at very young subjects.
To solve such a problem, it was designed and developed, a femoral stem which has, at its proximal part, an anterior curvature in the sagittal plane and, in the frontal plane, an internal curvature corresponding to the family of extra-curved femurs called "champagne flute", according to the NOBLE classification.
To take account of this particular shape, which appears essentially at the level of the metaphysis, the femoral stem according to the invention has, on the external side, a bulged part capable of exerting a compressive force at the level of the external cortex of the femur, allowing thus balancing the flaring generated by the strong internal curvature.
Still having the objective of taking into account the morphology of the femur, the stem has a neck which is angularly offset in the sagittal plane relative to the axis of symmetry, to correspond to the anteversion angle of the head of the femur .
The neck is lateralized according to the internal radius of curvature, to correspond to the desired cervico-diaphyseal angle.
Note also that depending on the increase in size, the length of the neck is increased according to the direction of the cervico-diaphyseal angle.
The invention is set out below in more detail using the appended drawings, in which:
Figure 1 is a front view of a femoral stem according to the invention; I a femoral rod of the same size has been superimposed, corresponding to a curved femur (line drawn with mixed lines) and to a straight femur (line drawn with broken lines), corresponding to the teaching of patent application EP 91420329.4.
FIG. 2 is a side view of the femoral stem corresponding to FIG. 1.
The femoral stem generally designated by (1) comprises, at the level of its proximal part and in the frontal plane, an internal curvature (la) corresponding to the family of extra-curved femurs, the metaphyseal part of which has a flute shape of Champagne. Opposite the curvature (la), that is to say on the external side, the rod has directly during its manufacture, a swollen part (1 b). This part (ib) is intended to exert a compression force at the level of the external cortex of the femur after introduction of the rod into the medullary canal, making it possible to balance the flaring generated by the strong internal curvature (la) ~
We therefore see that the combination of the internal curvature (la) and the part (lob) ensures perfect metaphyseal filling in the case of an extra curved femur.
It appears that the internal curvature (la) is much more accentuated than in the case of a femoral stem adapted for curved femurs (tracing mixed lines) (figure 1) and even more compared to the right femur (tracing broken lines) ( figure 1). Likewise, the bulged part (lob) is in dimensional relationship with the internal flaring generated by the radius of curvature (la).
The femoral stem, as defined for the extra-curved femurs, presents in the sagittal plane, an anterior curvature (II). This anterior radius of curvature (1c) is the same, both for a femoral stem of the family of straight femurs as for a femoral stem of the family of curved femurs. Reference is made to patent application EP 91420329.4.
The stem has a neck (nest) lateralized in the frontal plane and which is offset in combination with the internal curvature (1a). This offset corresponds to the prosthetic diaphyseal cervico angle.
In addition, the neck has, in the sagittal plane, an offset with respect to the axis of symmetry which corresponds to the anteversion of the femoral head.
Obviously, the rod (1), as defined according to the characteristics of the invention, is executed in different sizes.
Depending on the increase in size, the length of the neck (Id) is increased in the direction (xx ') of the cervico-diaphyseal angle.
Les avantages ressortent bien de la description. The advantages are apparent from the description.
