Abstract
Description
"Wrist orthosis for the treatment of epicondylalgia".
The epicondylar muscles, extensors of the fingers and the wrist, have a common attachment on the external tuberosity, called the epicondyle, of the lower end of the humerus.
The intensive use of the epicondylar muscles, for example during the practice of tennis, by the repetitiveness of certain gestures by the housewife, but also following a shock on the epicondyle, causes their attachment to the epicondyle , an inflammatory syndrome called epicondylitis or epicondylalgia.
The invention relates more particularly, but not exclusively, to the treatment of epicondylalgia caused in tennis or when handling a jackhammer, by repeated impacts on the hand, while being tightened with locking, that is to say with the epicondylar muscles in tension.
In fact, under these conditions, and as indicated in European patent application 197 771, the shocks and vibrations perceived by the wrist are transmitted to the extensor muscles in tension and are brought by them and with amplification, to the attachment tendon. on the epicondyle.
To limit the transmission of vibrations and the inflammatory reaction, which is very painful at the level of the epicondyle, the state of the art provides numerous means available either at the elbow, to compress the epicondylar muscles or their attachment tendon, as in French patent applications 2,360,300, 2,596,980, 2,636,229 and in US patent 3,970,081, or on the forearm, as described in international patent application 86/04811 and in European patent applications 62234 and 86960.
Such means bring an improvement and allow to continue the movements generating the trauma, but for a short period, and in any case insufficient to allow an athlete to pursue a championship taking place over several days.
Wrist bandages are also known which, by enclosing this wrist, tend to reduce the transmission of vibrations to the epicondyle.
Their action is preventive and becomes totally ineffective as soon as an inflammatory syndrome appears on the epicondyle.
The present invention aims to provide a wrist orthosis overcoming the drawbacks of current means of treatment of epicondylalgia, having a therapeutic action, while allowing the continuation of activity over a period several times greater than that tolerated by current means.
maintien. This orthosis is of the type composed of a tablecloth, able to wrap the
wrist extending longitudinally on either side thereof,
stiffening ribs arranged in at least one longitudinal pocket
formed on the dorsal part of the envelope and transverse straps of
maintenance.
In the orthosis, according to the invention, the envelope extends over the hand to the back of the first phalanges to which it is linked by a strap,
passing against the internal face of these first phalanges and returning
hang on the end of the back part of the envelope, while the
front part of the stiffening means, extending between the end
anterior of the envelope and the fold of the wrist, forms, with respect to its posterior part, coming against the wrist, an angle of between 20 and 30 wedging the wrist in dorsal flexion.
When this orthosis is put in place, it wedges the wrist in a position putting the epicondylar muscles at rest and not hampering the tightening movements of the hand which can very easily be used for
grab a handle and for example that of a tennis racket. Thanks to this position, during the movements of the players, the epicondylar muscles do not work on the whole of their possibility, are therefore not very tense, and do not transmit to the epicondyle the vibrations perceived by the hand. Such a
orthosis therefore promotes the absorption of epicondylalgia, while allowing to
continue the activity at the origin of this epicondylalgia.
Other characteristics and advantages will emerge from the description which follows with reference to the appended diagrammatic drawing representing, by way of nonlimiting example, an embodiment of this orthosis.
Figure 3 est une vue de côté en élévation montrant l'orthèse lorsqu'elle est mise en place.FIG. 1 is a perspective view of the orthosis when it is placed flat,
FIG. 2 is a view in longitudinal section of the orthosis in its part comprising splints,
Figure 3 is a side elevation view showing the orthosis when it is in place.
In a known manner, this orthosis is composed of a sheet 2 comprising on its back, at least one and in this case two pockets 3 containing stiffening splints 10, and transverse straps 4 and 5 for holding on the wrist.
According to the invention, the front part 2a of the envelope 2 extends to the back of the first phalanges 7 of the fingers and, more precisely, up to the proximity of the articulation of these first phalanges with the second phalanges 8. Furthermore, as shown in more detail in FIG. 2, each of the splints 10 disposed in the pockets 3 has an anterior part 10a which is bent upwards and forms, with respect to its rear part 10b, an angle a of between 20 and 30 and around 25 ".
As shown in Figure 3, when the splints 10 are placed in the pockets 3, formed on the dorsal part of the envelope 2, their elbows 10c are positioned in the fold 1 1 of the wrist and their front part 10a s' extends over the hand and over the first phalanges 7.
The front end 2a of the envelope, stiffened by splints 10, is linked to the hand by a transverse strap 12 forming, with the envelope, a belt surrounding the first phalanges, while leaving the thumb of the hand free. In the embodiment shown, the various transverse straps 4-5 and 12 are fixed by one of their ends and in 4a-5a and 12a on the back of the casing 2 and comprise, at their other end, a section 13 of hook or pin tape capable of hanging on a section of loop tape 14 fixed to the dorsal part of the envelope 2.
As shown in FIG. 3, when the orthosis has passed, it is perfectly linked to the forearm and the wrist, respectively, by the straps 4 and 5. By the strap 12 and by virtue of the anterior parts 10a of the splints 10, it keeps the hand in a dorsal flexion position favoring the detention of the epicondylar muscles. This figure clearly shows that the orthosis leaves the fingers of the hand sufficient mobility so that they can grasp an organ such as a racket handle or one of the handles of a jackhammer.
This orthosis allows, while relieving the epicondylalgia, to continue the activity which is at the origin of this epicondylalgia, and this for a time much longer than that allowed by current treatment means.
