Abstract
Description
The subject of the invention is an optokinetic stimulator which can be used for vestibular rehabilitation.
The optokinetics acts on the vestibulo-ocular reflex, the vestibulo-spinal reflex and the muscles of posture.
This type of rehabilitation device makes it possible to treat certain crises of rotary vertigo or instability and to reinforce the function of equilibration at a well determined speed for each patient or with an optokinetic nystagmus.
Optokinetic nystagmus is a good clinical test for the diagnosis of oculomotor abnormality.
The optokinetic nystagmus is an ocular motor reflex response to a displacement of the entire visual field. The answer is a spring nystagmus whose slow phase is in the direction of the stimulus and the fast phase is opposed to displacement. It is a common stimulus produced by every natural movement of the head. Thus a movement of the head to the right causes a displacement of the visual environment on the left, and the component of these nystagmus follows the movement of the visual field, helps to stabilize the position of the eye relative to the environment. In clinical practice, it is easy to cause such nystagmus by the horizontal displacement of a surface of paper marked with vertical lines.
This stimulator uses the techniques of Professor BARANY, described in 1890 and used by others since.
The state of the art can be defined by patent FRA2.619.002 which, however, is not created for vestibular rehabilitation, but for the observation and recording of eye movements.
- FR-A-2.619.002: "Infrared / video electronystagmographic device for the observation and recording of eye movements. Device characterized in that it comprises: a first and a second hollow eyepiece each comprising a surface interior pierced with a first and a second opening, sealing means for completely eliminating any visible light liable to fall on the subject's eyes, reflection means intended to reflect and disperse energy in the infrared range spectrum; these infrared source means being coupled to each of the first and second eyepieces: video detection means intended to detect the reflected infrared images of the horizontal, vertical, oblique and rotary movements of the eye. "
The stimulation technique is based on the Barany drum principle. The panoramas and rotating chambers method is very little used in clinical practice, given the size of these devices. A simple apparatus was produced, derived from that used in the oculography laboratory in Tours. It is a Barany type drum, driven by an electric motor, the whole being secured by a frame that can be tilted by 90, to pass from horizontal stimulations to vertical stimulations. An inverter allows the rapid change of the direction of rotation of the drum and a rheostat makes it possible to modify the speed of rotation and to carry out tests at different speeds or at progressively accelerated or decelerated speeds. The drum is pierced over its entire surface with small squares distant from each other. These small squares are closed with alternating blue, yellow and red colored papers and are lit from the inside of the drum. The test is performed in the dark. The drum is located 60 cm from the subject, whose head is held fixed by an ophthalmic support. The rotational speeds chosen are: 120 "/ s, 180 / s and 210" / s, corresponding under the conditions described above to apparent speeds of 24 "/ s, 36" / s and 42 / s. Each small square represents an angular distance of 2.87.
Currently there is an optokinetic generator. This generator is composed of a ball having orifices on its surface and, inside said ball, a light source creates rays which, passing through said orifices, form light spots on the whole of the room in which finds the optokinetic generator arranged. The ball is mobile according to three axes, so as to allow optokinetic stimulation. Stimulation is possible along each axis or by combining all the axes. The speed of rotation along each axis is adjustable.
Ce générateur présente quelques inconvénients. This generator has some drawbacks.
The spots are not adjustable in their dimensions.
Les taches sont peu lumineuses. The spots are dim.
Due to the presence of the light source inside the ball, the power supply cables hinder the mobility of the ball.
The ball can only rotate in the plane of its axis of rotation by 1800.
The optokinetic stimulator according to the invention tends to solve all these problems.
The spots are very bright and adjustable in size. However, the more luminous and voluminous the spot, the more the retina is impressed, the more the subject is sensitized.
The light source is independent of the ball which projects the light spots. The ball is therefore movable around two axes of rotation.
With a simpler technique, the stimulator according to the invention is of lower cost, of greater reliability.
To this end, the optokinetic stimulator that can be used for vestibular rehabilitation is of the type using a movable ball emitting light rays, so as to illuminate the entire room, according to the three planes of space, characterized in that the ball is a ball with reflective facets, illuminated by a light source external to the ball, said source is adjustable in intensity and in focus, so as to allow the intensity and the surface of the reflected light spots to be varied, said ball is mobile around two axes of rotation.
Each of the axes is driven by an electric motor, said motors can operate in one direction or another, and it is possible to combine the movement of the two axes. The said motors are adjustable in speed.
The axis motors and the adjustment of the focus of the light source can be remote-controlled.
Programs for these different settings can be saved and programmed later.
According to a particular embodiment, the stimulator is composed of a base comprising a bracket on which is mounted a ball support. Said support is mobile on an axis perpendicular to the axis of rotation of the ball, the light source is mounted on a mobile rod which can, by an electric motor, approach or move away from the ball with reflective facets.
The light source can be a laser beam device.
The light source can also be an intermittent light source.
