Abstract

Sternal orthesis for use following sternotomy, capable of exerting a constant pressure on a sternal wound, characterised in that it consists of a corset (1) which has a dorsal closure and can be opened, which corset, made at least partly of an elastic fabric, has plunged areas (2) in order to bear under the armpits and is provided in its front central area (3) with a foam pad (11) reinforced by a suitable whalebone arrangement (12).

Description

We know that many surgical procedures at the level of the cardiac system involve a sternotomy which determines a relatively large wound whose scarring is always delicate.

To promote this healing, it is of course necessary to exert a regular pressure on the wound which affects the patient's sternum so as to keep the hares of the said wound in place, but the location of the latter, in man as in woman, lends itself very poorly to the application of such pressure
The means currently used for this purpose are most often rudimentary. If one intends to avoid the installation of staples, always traumatic, one generally uses self-adhesive tapes with elastic properties, but experience has shown that these properties are too weak to generate a regular and constant pressure during a lapse of time. sufficient time. Scarring therefore remains relatively long to take place.

It is to this drawback that the present invention mainly intends to remedy, and this using a specific orthosis constituted by an openable belt with dorsal closure, which belt, established in an elastic fabric, is notched to take place under armpits and is provided in its front central part with a foam cushion reinforced by an appropriate whalebone.

Other remarkable characteristics of this sternal belt will emerge from the description which follows with reference to the appended drawing, which drawing, given by way of example, will make it possible to better understand the invention, the characteristics which it presents and the advantages which '' it is likely to provide:
Fig. 1 is a schematic plan view illustrating the general arrangement of a sternal orthosis according to the invention, male model.

1. Fig. 2 is a detail section on a very enlarged scale, along the plane indicated in II-II in FIG. 1.

Fig. 3 and 4 show from the front, respectively from the rear, the orthosis once placed on the chest of a patient.

Fig. 5 is a plan view similar to that of FIG. 1, but corresponding to a female model.

The orthosis or sternal belt shown in fig. 1 is constituted by a strip 1 made entirely or in part of an elastic fabric, this strip being cut into 2 on either side of a central part 3. In the vicinity of each of its ends, the upper edge of this strip 1 is made integral, for example by sewing, with a bre of suspension 4 whose free end is cut out of openings 4a adapted to cooperate in an adjustable manner with a button 5 fixed to the upper edge of the central part 3.

The free ends 6 of the strip 1 are provided with one of the longitudinal straps 7, the other with corresponding return loops 8. The end which carries the straps 7 is provided with two vertical self-gripping strips 9 (of the type of those known under the Trademark "VELCRO", for example) capable of retaining the ends of said straps once they are returned, after tightening, in the loops 8.

The arrangement of the central part 3 is illustrated in FIG. 2. The elastic fabric 3 is assembled by sewing an internal lining 10 which encloses a foam cushion 11. The assembly of the fabric thicknesses 3 and 10 is carried out so as to define two sheaths oriented substantially vertically, inside each of which is housed a reinforcing whale 12, made of synthetic material. It will be observed that two other whales, referenced 13, are provided obliquely at the level of the indented areas 2.

Fig. 3 and 4 show the orthosis once placed on a patient. The straps 4 are passed over the shoulders of the latter so as to ensure the suspension of the belt, the indented areas 12 being received at the armpits. This belt is capable of being firmly tightened in place using straps 7, loops 8 and bands 9, these elements thus ensuring effective back closure. The central part 3 with its foam cushion 11 and its reinforcing ribs 12 is consequently pressed against the patient's sternum.

Clinical trials have made it possible to verify that the sternal orthosis thus arranged ensures perfectly regular pressure and remarkably promotes healing of the sternotomy wound, this healing occurring within a very short time, of the order of eight to ten days. only.

It will be observed that the production of reinforcing ribs 12 and 13 in synthetic material allows post-operative radiographic or radioscopic examinations, without the need to temporarily remove the orthosis.

It goes without saying that the sternal belt described above would not be suitable for a woman, given the female morphology. the adaptation is however very simple, as illustrated in fig. 5 which represents a female model. The reinforcing ribs 12 of the central part 3 are somewhat close to one another, while the lateral ribs 13 are, on the contrary, offset in the direction of the ends 6, so as to leave room for two clean openings 14 to allow breast passage.

It should moreover be understood that the above description has been given only by way of example and that it in no way limits the field of the invention from which one would not depart by replacing the execution details described by all other equivalents.
