Abstract

Le dispositif est une prothèse de type "rotule" comportant une cupule en polyéthylène (I) et deux pièces métalliques: - La pièce métacarpienne (II) est le pivot dont la base (1) est évasée ce qui permet d'y ménager dans son axe longitudinal, un orifice (2) de forme cylindro-conique. Il est assez profond pour être prolongé d'un trou fileté (3) destiné à y visser l'extrémité d'une tige destinée à manœuvrer la pièce en cours d'intervention chirurgicale. Cet orifice reçoit une des extrémités (4) de la pièce intermédiaire qui s'y emboite selon le mode "cone-morse". La pièce intermédiaire (III) est le dispositif original. Il possède à son extrémité proximale une sphère métallique (5) qui est l'un des deux composants de l'articulation type "rotule". A l'autre extrémité, c'est la partie mâle du cone morse, en forme de tronc de cone (4) emboitée dans la partie creuse (2) ménagée dans l'axe de la base de la pièce métacarpienne. Entre ces deux extrémités, c'est le col (6) présenté en trois longueurs. Pour mesurer l'écartement articulaire qui doit recevoir la taille adaptée de la pièce intermédiaire, on dispose d'une pince écartante étalonnée (IV) qui permet de faire le meilleur choix de longueur.

Description

PROTHESE TOTALE TRAPEZO-METACARPIENNE AVEC
PIECE INTERMEDIAIRE
I - La prothèse totale trapézo-métacarpienne est prévue pour le remplace
ment de l'articulation de la base du pouce. Cette prothèse est habituellement
une articulation de type "rotule" faite de deux pièces : l'une en métal composée
d'une queue ou pivot visant à être introduit et scellé dans le canal médullaire
du premier métacarpien. Cette pièce dispose aussi habituellement d'un col à
l'extrémité duquel se trouve une tête ou demi-sphère destinée à être articulée
avec la pièce trapézienne. Cette dernière est une cupule en polyéthylène dont
la cavité s'articule avec la sphère métallique. Ce dispositif est composé de
deux pièces dont seule la pièce métacarpienne, dans son ensemble, peut varier
de taille pour faire face aux variations de longueur.Le dispositif présenté
(figure 1) diffère en ce qu'il est composé de trois pièces dont une intermédi
aire qui assure par ses trois tailles les variations d'écartement anatomique
dans la zone articulaire à reconstruire.
Il - Le dispositif composé de trois pièces, présente une cupule (I) intro
duite et scellée dans le trapèze et la pièce métacarpienne (II) introduite et
scellée dans le canal médullaire du premier métacarpien. Cette pièce est compo
sée d'une tige cylindro-conique dont la partie la plus large (1) est dans sa
portion la plus proximale. Cet élargissement de la base de cette pièce permet
de recevoir dans son axe longitudinal, un orifice tronconique (2) destiné à re
cevoir une des extrémités de la pièce intermédiaire selon le mode#'emboitement
"cone-morse". Le fond de ce canal est lui-même prolongé d'un trou fileté (3) jFig iFig destiné à recevoir l'extrémité d'une tige utilisée pour porter et placer
la pièce au cours de l'opération chirurgicale (Fig.2).
III - La pièce intermédiaire (III) en métal, présente deux extrémités et un
col. L'extrémité distale (4) est un tronc de cône destiné à être introduit dans
son logement creux (2) situé dans la base de la pièce métacarpienne en place.
L'extrémité proximale (5) est munie d'une tête sphérique, un des deux composants
de l'articulation type "rotule". Entre ces deux extrémités, c'est le col (6)
dont la longueur varie suivant trois tailles.
IV - Le choix de la longueur idéale est assuré par une pince spécialement
conçue à cet effet (IV) (fig.3). Il s'agit d'une pince écartante dont les mors
(7) sont plats et disposent en opposition, chacun un relief, l'un sphérique (8) l'autre cylindrique (9) introduits respectivement et provisoirement d'un côté dans la cupule et de l'autre, dans le canal de la base de la pièce métacarpienne. Ainsi, la pince écartante dispose-t-elle de points d'appui permettant en toute sécurité, la mesure de l'écartement articulaire. La mesure est donnée par un étalonnage selon trois marques (lO)disposées sur la crémaillère. Ces trois marques indiquent les longueurs de col à choisir.

Claims (5)

REVENDICATIONS
1 - Prothèse totale trapézo-métacarpienne pour les lésions articulaires de la base du pouce composée de trois pièces dont deux métalliques et une cupule en polyéthylène (I).
2 - Dispositif selon la revendication 1 caractérisé en ce que la pièce métacarpienne (II) est composée d'une queue à base évasée (1) en tronc de cône creusée dans son axe d'un canal longitudinal (2) dans le fond duquel est ménagé un second canal avec trou fileté (3) destiné à recevoir l'extrémité d'un instrument pour porter la pièce et exécuter les manoeuvres chirurgicales.
3 - Dispositif selon la revendication 2 caractérisé en ce que le logement axial (2) ménagé dans la base de la pièce métacarpienne, est destinée à recevoir à titre définitif l'une des extrémités de la pièce intermédiaire (III) selon le mode d'emboitement type "cone-morse".
4 - Dispositif selon les revendications 1, 2, et 3 caractérisé en ce que la pièce intermédiaire est une pièce présentée en trois tailles munie de deux extrémités. L'une est une sphère (5), articulée avec la cupule trapézienne.
L'autre est en forme de tronc de cone (4) destinée à être emboitée dans la partie creuse (2) ménagée dans la base de la pièce métacarpienne.
5 - Dispositif selon la revendication 4 caractérisé en ce que le choix de la longueur adéquate de la pièce intermédiaire est assuré par une pince spéciale écartante (IV) munie en opposition sur ses mors (7) de reliefs (8) (9) adaptés à ceux en creux des pièces métacarpiennes et trapézienne afin d'assurer un bon appui à la manoeuvre d'écartement. La mesure est donnée par un étalonnage en trois marques (lO)situées sur la crémaillère. L'écartement idéal permet de choisir la taille adaptée de la pièce intermédiaire.
