Abstract
Description
The present invention relates generally to the dental implantology technique and in particular a metal implant in the form of a blade, positioned in a pre-grooved intraosseous medium, a solid barrel incorporating the means necessary to fix one or more artificial teeth therein, a bridge or any other: superstructure intended for chewing. These means are located in and on the barrel at its end directed towards the oral cavity.
Generally this type of implant is known and is the subject of commercialization in different forms, determined more by primary intuition than by basic research and strict systematic analysis of clinical results.
Despite the care taken in carrying out the intraosseous bleeding prior to the insertion of the blade, even a slight clearance exists between the walls of the bone cavity and the implanted blade. This game smoothes several degrees of freedom to the blade during chewing efforts or contractions of the tongue.
These movements are detrimental to the hard recalcification of the bone part which cannot exceed its first stage, that is to say the formation of a fibrous, cross-linked tissue, without reaching the stage of osteointegration.
On the other hand, the micro-movements of the blade, due to the indirect interfaces, cause, upon impact, settling in the bone body, increasing the space between blade and bone body, requiring ultimately the replacement of the implant. .
The width of the blades commonly used to date is reduced at the point of connection of the barrel. This weakening of the central part determines, under the compressive forces, a deformation in pseudo-arc of a circle of the blade, causing parasitic tiltings in the bony trench which are added to the beats mentioned above.
In addition, the barrel, intended to receive either bridge elements or one or more artificial teeth, requires a sufficiently large section to withstand the stresses generally transmitted by a locking screw in the internal thread of the barrel. Its diameter is usually greater than the thickness of the blade. This difference in section, in addition to complicating the surgical insertion operations, causes gingival irritation.
The width of the blades, known to date, equals the height of the intraosseous cavity. By isolating the two bony faces, it eliminates the possibility of reossification in a bridge, leaving the bone weakened by the groove and eliminating any possibility of locking the implant during suction.
The present invention generally deals with maxillary implantation techniques and more particularly with improved structures of intraosseous dental implants of the type generally designated: blade implant.
The invention therefore aims to remedy the damage caused by relative movements and deformations of all kinds of the implant in the bone cavity; therefore define the mechanical characteristics of shape and structure of the blade, object of the invention.
In order to simplify the descriptive description and improve understanding, we can compare the elevation view of the implant, object of the invention, to a house gable, the base of which constitutes the support face in bottom of groove, the sides represent the lateral supports, the chimney represents the barrel, the roof with two slopes image the shape of the sides connecting the barrel to the lateral supports.
According to the invention, an implant of the blade type, as described at the beginning and below, is characterized by its cross section of isosceles trapezoid shape. Thus, under pressure forces, the blade is in intimate contact with the bone structure. The clamping stress, and more particularly the transverse component of the pressure forces on the walls, is limited by the degree of angulation of the faces and by the predetermined clearance, during the execution of the intraosseous slot, between the bottom. of the cavity and the base of the blade.
The lateral supports of the blade are oblique and radiated to match the shape of the ends of the groove cut by the tool in the bone part. These ends have a semi-frustoconical shape.
The barrel of the structure, in accordance with the invention, is an integral part of the blade. it has no excess thickness with respect thereto. In general, trunk of pyramids with rounded edges, it has in its center a thread with a "sawtooth" profile and a centering intended to position and fix the intra-mucous element, after impacting the implant. In the final phase, the superstructure is joined to the whole. The connection of the barrel to the blade is reinforced by shelving.
According to another characteristic of the invention, the roof sides of the blade are straight, convex or concave in shape with a large radius, depending on the local requirements of the intervention. Thus, on the one hand, the blade iincluded in the bone cavity only partially masks the groove sides and allows a bone bridging overlap, blocking the implant during suction efforts, avoiding its entrainment by the superstructure engaged in mastication when opening the jaw. On the other hand, the pressure forces, occlusion, are transmitted rigidly to the lateral and lower ends of the blade without risk of tilting thereof.
According to the present invention, the body of the blade is perforated with through-holes. These orifices are specially arranged so as to cause structural weaknesses in certain parts of the blade in order to favor axes that transmit force towards the zones capable of supporting them without risk of parasitic deformations of the blade.
Another characteristic of the invention relates to the physico-chemical composition of the surface of the implant. The metal chosen for its compatibility with the bone medium and its stainlessness must have on the surface the intrinsic qualities which were decisive for its choice. Macro and micro-topological examination reveals that after machining and particularly after cutting or grinding, the surface layer of the metal consists of a compressed amorphous mantle, essentially consisting of grains of crushed crystal structure, abrasive waste and various materials melted, oxidized and burned. After the implant-blade is shaped by machining, its surfaces are subjected to cold abrasion in a vibrator, in the presence of solid and cutting abrasive such as cubic boron nitride for example , or any other suitable abrasive, intended to destroy the amorphous layer without harming the final surface state while dulling the sharp edges of the part. For cases of difficult reossification, defined during clinical examination, a layer of biologically neutral ceramic, such as hydroxyl apatite, for example, coats the walls of the blade implant.
The implant according to the invention thus satisfies the conditions indicated at the beginning: to be included directly in the bone cavity without the possibility of movement, thereby facilitating the transformation of the intermediate fibrous tissue into bone material. Transverse re-ossification, the only solution to blocking the suction implant is made possible. The favorable pressure zones in the cavity are predetermined.

The process of placing the blade implant, object of the invention, differs in certain points from the traditional method. The groove is primed with a dentist's cutter that generates the straight line of reference for the bone trench. The groove is shaped by a set of calibrated frustoconical burs, fitted with a rear stop delimiting penetration, while following the outline of the preliminary groove. A wedge caliber, undersized, aseptic, introduced into the bony trench, allows three-dimensional control. After a period of endosted stimulation, the implant-blade is impacted, then placed in a nurse for the osteointegration phase after which the superstructure will be fixed, determining the activation of the implant.
