Description

REPERFUSION MACHINE
The present invention relates to a machine for reperfusion of an organ that has been temporarily irrigated by blood.

This organ will often be the heart, but may also be a kidney or the brain, for example.

The description which follows relates to the frequent case, that is to say an insufficiency of the coronary circulation.

Such insufficiency may follow an infarction or result from the removal of the organ for transplant.

In all cases, after insufficient coronary circulation. it is necessary to restore said circulation gradually and very controlled.

In fact, the cells do not support brutal troD oxygenation after an ischemic state. It is therefore essential to ensure very strict control of irrigation according to parameters such as the quantity, temperature, or oxygenation of the blood injected.

It is moreover necessary to adapt the treatment according to the clinical condition of the patient and possibly to add drugs to the injected blood in order to further improve the quality of the treatment.

Document EP-A-192 575 describes a coronary perfusion pump, for temporary therapeutic assistance in the event of insufficient coronary circulation, comprising a probe fitted with an inflatable balloon intended to fix the end of the probe cans coronary artery, a pump capable of injecting or through the ring of oxygenated blood and possibly drugs, the pump being placed under the control of a synchronizer authorizing the injection of blood essentially during the diastolic period of the cycle cardiac.

The objective pursued in this document is to ensure the injection of oxygen blood when the signals of pressure in the aorta and of electrocardiography are in coincidence at a defined point of the cardiac cycle.

This notion of coincidence of signals has not proved essential, and the notion of oxygenated blood must be qualified.

It is easy to understand that it is difficult
Dossibie has the medical team to constantly monitor the evolution of all parameters and to act in order to modify in a precise manner each of said parameters during a reperfusion which can last for hours or a few days.

To this end, the present invention provides a machine for controlling a reserfusion characterized in that it communicates with a database in order to define a protocol to be implemented according to parameters which it receives either from the medical team, or cators to ensure real-time monitoring of the reoerfusicn.

According to an advantageous provision of the invention, the medical team can at any time intervene to modify the protocol established by the machine in the event of particular conditions.

The database allowing the establishment of the protocol to be implemented takes into account parameters relating for example to age and weight. certain history and condition of the patient to the medical team supplied to the machine using a termina; computer science. The terminal is linked to the microprocessor containing the data bank.

The microprocessor also receives measurements, coming from several sensors located at several points of the reperfusion device.

The reperfusion is a treatment which @ erme @ to irrigate in a progressive way of the @ones before su @@ an insufficiency of circulation for example following a narrowing of the coronary artery.

To this end, a catheter fitted with an inflatable ball @ n is introduced into the coronary artery downstream of the narrowing. The balloon is then inflated in order to cut off any relationship between the diseased part situated downstream and the part of the coronary artery situated upstream of said balloon. Then a mixture comprising blood and, if necessary, medicines in the form of perfusate, is brought in through the catheter.
Thanks to the presence of the inflated balloon to cut circulation in the artery, we are sure of the quantity and the quality of the mixture irrigating the diseased part.

Said mixture should not be too oxygenated so as not to destroy the diseased part. this is why we use a mixture of venous blood and arterial blood taken directly from the patient, or obtained by compatible blood transfusion @
This blood mixture is of course strictly dosed then it is brought to a suitable temperature before being introduced cons the catheter at a suitable pressure.

According to the invention. it is the machine cui, by the microprocessor. determine and control the content and flow of the mixture.

To this end, sensors supply the machine with the value of the blood flow after the venous blood-arterial blood mixture, the value of OH and the quantity of oxygen of said mixture.

Other sensors provide information on the blood and drug mix. as regards its temperature, the presence or absence of bubbles, the pressure of said mixture.

The machine also receives an indication of the value of the coronary pressure, that is to say that at the distal end of the catheter.

On the basis of this information, the ma @@ ine established the treatment protocol. Dis ilote these volumetric pumps ensuring the supply of venous blood and arterial blood, an e @@ pump or a syringe pump ensuring the supply. in medicaments, the temperature of the fluid ensuring by heat exchange in a heat exchanger a modification of the temperature of the blood / medicament mixture which will be sent into the catheter, the machine also controls a so-called main pump which ensures the circulation of these mixtures in the circuit and pressure.

Figure 1 shows the circuit in which circulates and is treated the mixture intended to be injected into the coronary artery towards the diseased area through the catheter 1.

In the example represents the catheter. is ur. three lumen catheter i.e. b three internal longitudinal channels.

One of said channels, the inlet of which is at 11, makes it possible to breathe air to inflate the balloon intended to close the circulation in the diseased artery,
Alternatively, the catheter 1 could have only two internal channels. the air supply channel to the balloon can be external.

The second channel, channel 12 ensures the circulation of the reperfusion mixture towards the interior of the coronary artery.

The third channel, channel 13 is a channel by which the coronary pressure is measured, that is to say the pressure prevailing at the distal end of said catheter, & inside the coronary artery.

Said pressure is measured by a pressure sensor 2. the measurement is sent after processing to: the data acquisition card cue includes The machine.

The patient's blood, or in some cases external blood. is introduced into the circuit by the pumps 31, 32.

The pump 31 ensures the arrival of venous blood, while the pump 32 ensures the arrival of arterial blood @ iel @ These two pumps are volumetric pumps.

The blood from the two pumps is mixed and sent to tubing 4.

Said tubing 4 first passes through a flow sensor 5 ensuring the measurement of the flow by DopDler effect and envcyant the measurement to the acquisition card.

The blood mixture is then subjected to sensors 6, 7 ensuring the measurement of the amount of oxygen it contains and its pH.

A pump 33, called the main pump, ensures the circulation of the blood mixture in the tubing 4 and therefore maintains a pressure in this tubing. Said pressure in the tubing is measured by a sensor 10 located near the entrance to the catheter.

The tube 4 also passes through a heat exchanger 8 in which its temperature is measured by a thermistor 81 and modified as a function of the temperature of a second fluid crossing the exchanger.

The circuit also includes a bubble detector 9.

Via a branch branch of the circuit, drugs for this purpose are supplied to the blood mixture, if necessary, a pump 34 for infusion of the drug and an electric syringe pump 20 @ are provided.
In FIG. 1, the information given by the various sensors to the microprocessor has been symoolized by a double arrow through the acquisition card, @ the simple arrows symbolize the orders given by said microprocessor to certain receivers. These orders are transmitted to the pumps in order to adjust their rotation speeds and therefore the volume they move; the heat exchanger 8 and the syringe pump 2 also receive orders.

The keyboard is represented in 51 by which the medical team can bring to the machine the clinical information concerning the patient which is necessary for the choice of the reperfusion protocol.

All the information is thus brought to the microcomputer 52 which has a database making it possible to define the reperfusion protocol to be applied.

Said microcomputer 52 then controls the pumps, the heat exchanger and the syringe pump in order to administer to the patient the quantity of blood which is necessary for him. at the right temperature. at the right dosage, with the right drugs and under the right pressure.

All the acquisitions being made continuously, the microcomputer can modify the protocol according to the evolution of the patient's condition.
