Abstract
Description
Slave nutrition pump
The present invention relates to a servo-controlled and remote-controlled nutrition pump.
Since the development, ten years ago, of nutrition pumps called "nutri-pumps", numerous scientific publications have shown the interest and the importance of these devices in cases where parenteral nutrition or mouth is faulty and unusable.
However, the devices currently marketed and used have a certain number of drawbacks which can be stated as follows
- poor agitation (therefore homogeneity) of the nutritive liquid,
- uncertain reproducibility and reliability of the flow of the nutritive liquid due to the use of a peristaltic type pump,
- total absence of a feeding program for the patient according to his physiological needs and his possibilities of assimilation,
- need for handling of the devices by highly specialized personnel.
The present invention aims to overcome these various drawbacks; this invention is essentially based on the following two findings
- it is possible thanks to a choice of simple measures to adapt the diet of the patient to the physiological needs and the possibilities of assimilation of this patient,
- It is desirable, depending on the measurements thus carried out, to have a device having, on the one hand, a slave to the results of the measurements and, on the other hand, a remote control allowing easy remote control.
The measures chosen are
- on the one hand, a measure of the resistivity of the gastric medium and the intestinal medium,
- on the other hand, a measurement of the pressure at the stomach level and at the intestinal level.
The resistivity measurement was chosen because on the one hand, the devices making it possible to carry out this measurement are known and usable in a nutrition pump and on the other hand, it allows to have a relatively precise idea of the osmolarity and the acidity or alkalinity of the medium. The latter notion has been retained as that making it possible to overcome as much as possible the drawbacks encountered in current systems and linked to a poor adaptation of artificial feeding to the needs and possibilities of assimilation of patients. The consequence of this choice is that, depending on the resistivity measurements carried out in the gastric and intestinal medium, it will be necessary to be able to adapt the patient's diet, hence the need for two food reserves, one containing a hyperosmolar nutritive liquid and the other containing a nutritive liquid hypoosmolar and two pumps which will be controlled by the measurement results.
The pressure measurement was chosen because it is the one that best illustrates the state, at all times, of the stomach and intestine (filling state and flow speed for these two organs ). On the other hand, the devices making it possible to carry out this measurement (the pressure differences to be measured are of the order of a few centimeters of water) are common and sufficiently miniaturizable to be placed at the desired level (stomach and intestinal).
This pressure measurement will condition the total flow of liquid food to each in ## stant; there will therefore be enslavement of the two pumps to the results of the pressure measurements carried out.
In addition, care should be taken to ensure that the flow rates of the pumps which transfer the food liquids are adjusted as exactly as possible. For this it was found on the one hand, that it was useful to use not peristaltic pumps but piston pumps which are moreover more reliable and on the other hand that there was in general interest to agitate nutritious liquids. Finally, in health centers, the results of the various measurements, the operation of the pumps and the instantaneous state of the food reserves available in the feeding devices can advantageously be grouped in a control room or the same supervisor can thus be responsible. the proper functioning of several nutrition pumps according to the invention.
The device, nutrition pump according to the invention therefore having liquid food reserve, and pump for distributing said reserve in a probe admitted-in the estômac # 'a patient is characterized in that it comprises
two reserves of nutritive liquids, one of these liquids being hyperosmolar and the other being hypoosmolar, said liquids being preferably constantly agitated,
- two piston pumps controlled independently and allowing the control of the flow rates of each nutrient liquid,
- devices for measuring the resistivity of the stomach and intestinal environment,
- devices for measuring the pressure in the stomach and intestine.
- And a servo mechanism making it possible to adjust, as a function of the results obtained by the devices for measuring the resistivity and the pressure; the flow rate of each of said pumps.
As indicated above, the device according to the invention can be supplemented by known means allowing remote control of the proper functioning of said device.
As a nonlimiting example, it is possible to diagram the device according to the invention in the single figure.
