Abstract

The invention relates to an instrument making it possible to excise a vein (carry out a saphenectomy) by perivenous dissection without counterincision or counterpuncture of the skin. It consists of a rod 5, terminated at its distal end by a semi-cutting ring 6, and at its proximal end by a handle 1. A steel wire 3, secured by a lug 2 situated on the handle 1, passes through a sheath 4. Pulling its free edge has the effect of constricting the vein at the ring. The instrument according to the invention avoids counteropening of the skin during perivenous excisions of veins.

Description

DESCRIPTION
DESCRIPTION
The present invention relates to a surgical instrument allowing, during the ablation of a vein or varicose vein, a vein by action external to the vessel without cutand counter-orifice.

Prior state of the art
The ablation of varicose veins, particularly of the long saphenous vein, is usually carried out by means of a cable introduced by an end inside the varicose vein at the other end of the varicose vein, this cable is necessarily taken out by a counter-incision or counter-puncture. S through this second opening, a fastener attached to the varicose vein is used for rolling it up. Rolling up does not have maximum effectiveness, because an external region of the wall of the varicose vein (adventitia and / or part of the media) can tear, remain in place, responsible for subsequent recermeabilization.

Varicose vein removal is sometimes, but less often, by means of a ring on a rod inserted from one end outside the vessel to the other end of the varicose vein, a counter-incision or counter-perforation skin is necessary to cut the varicose vein, hence the unsightly presence of a scar at the site of the skin counter-perforation
General principle
There is given, by way of nonlimiting example, an embodiment referring to the drawings below.
The instrument according to the invention makes it possible to resolve these drawbacks by combining maximum efficiency by external totoparietal veining or periveinous dissection associated with the absence of cutaneous perforation at the other end of the varicose vein. 7
The instrument consists of a solid rod (5) terminated at one of its ends by a semi-cutting ring (6) which ensures external stripping; a steel cord (3), maneuvered from the handle {1), is housed in a hollow cylinder (6) pressed along the rod, makes its loop in the thickness or around the ring (6). One of the two strands of the steel wire is stowed on a lug <2) secured to the handle t1) When pulling the free strand of the steel wire at the handle, the loop located in or around the ring tightens, strangles then cuts off vein avoiding any cutaneous perforation. The varicose vein is then removed through the single initial incision.

Another rod (7) or flexible or semi-rigid cable can serve as a guide. It has a swollen proximal end (8) and an angulated distal end and provided with a sphere <9) Introduced inside the varicose vein, it avoids path errors during the progression of the perisaphenian ring.

The steel wire can, by itself, achieve a loop back and forth or it can be unique from the handle, making only a closed loop at the level of the ring.

According to a preferred embodiment, the instrument is made either of stainless steel, or of plastic or other non-metallic bio compatible and sterilizable materials.

According to a preferred embodiment, the rigid rod (5) is long up to 60 cm, 2 to 6 mm thick, terminated by a semi-sharp oval ring (6) with a degree of angle of 150 and oriented at an angle of 1354 relative to the rod. The steel wire (3) is 0.2 to 0.5 mm in diameter. This rope activates by pulling at the level of the handle grips and cuts the vein at the end of the ring, avoiding cutaneous counter-perforation. The handle (~) can be either single or double. The rupture of the vein inside the ring is carried out either physically by hot or cold, or mechanically by a steel wire, a scissor effect or the traction of the instrument enclosing the vein, either by a radius in electromagnetic mode allowing varicose rupture at the level of the ring.

- la figure 3 représente le guide.
