Abstract
Description
The present invention relates to a new infant diet milk food, mainly for the feeding of normal infants and whose digestibility is comparable to that of women's milk.
L'importance des graisses dans l'alimentation de l'enfant, et plus particulièrement du nourrisson, est liée non seulement à leur caractère énergétique mais aussi à leur rôle dans le développement du système nerveux (GALLI, 1963;
SINCLAIR 1972 et 1973) et à leur interférence possible dans le développement ultérieur de l'obésité et de l'athéro- sclérose (FREDMAN, 1975).The importance of fats in the diet of the child, and more particularly the infant, is related not only to their energy character but also to their role in the development of the nervous system (GALLI, 1963;
SINCLAIR 1972 and 1973) and their possible interference in the subsequent development of obesity and atherosclerosis (FREDMAN, 1975).
It is therefore necessary that infantile milk foods provide sufficient intakes of essential fatty acids, while allowing optimal absorption of fats, which represent 40 to 60% of the total caloric intake of infants.
Essential fatty acids, including linoleic acid (C18: 2), play a role in the synthesis of biological membranes and prostaglandins, and their level is eight times lower in unmodified cow's milk than in the milk of blackbirds.
ne.The digestibility of fats and therefore their ability to be absorbed by the immature gut, depends on three factors - the length of fatty acid chains, - the degree of saturation of double bonds and - the structure of triglycerides, that is, to say the position
on the glycerol molecule of long-chained fatty acids
born.
TABLEAU I
Unmodified cow's milk, lower in unsaturated fatty acids than women's milk, is rich in triglycerides, of which palmitic acid (in C16) is mainly in positions 1 and 3, and is the subject in the first four months of the infant's life, a malabsorption that can reach a third of the ingested fats. This inadequacy of cow's milk fats in the diet of the newborn and the infant has resulted in their partial or total replacement, in the milk foods proposed in the prior art, by fats of vegetable origin which make it possible to to increase the intake of unsaturated long-chain fatty acids. This is why the dietetic foods currently proposed for sale in the market for infants of the first age have a composition in accordance with the French regulations currently in force [Order of 1 / 7/76], such as that given by way of example in Table I which will follow

<tb> * Mature female milk is full-lactation milk following the colostral phase of lactation, around the 15th day after birth.
Previous research carried out by the Inventor has led him to develop, to meet the specific needs of new born babies of low birth weight, hypotrophic and / or premature premature dysmature, whose nutritional functions are immature, especially at the point lipid, a high-fat diet milk diet rich in medium chain triglycerides rich in medium chain triglycerides (MCT C8 to C12). These MCTs are completely and very rapidly metabolized ; their absorption cycle is faster than that of long chain fatty acids; they do not need carnitine to enter the mitochondria and their oxidation is as fast as that of glucose for a calorie intake twice as high, and much faster than that of long-chain fatty acids. MCTs are present in diet milk foods for low birth weight newborns, more than 40% by weight and provide a readily available and easily digestible energy source.
N.Y.However, if diet milk foods for hypotrophic or premature infants make it possible, by virtue of their high MCT content (more than 40%), to considerably improve the metabolic balance of premature infants, it would be unthinkable to conceive dietary milk products for normal infants. containing such doses of
Medical MCT due to the eventual impact of such high doses of MTC on the child's metabolism, including the risk of induction of obesity that high-grade milk foods may cause in TCM. On the other hand WHYTE and Al. LPEDIATRIC
RESEARCH, VOL. 20, No. 2, 19863 have reported significant metabolic differences in preterm or low birth weight infants fed milk diet compositions containing MCTs of 46% and 4% by weight respectively: according to these authors, the comparison organic acids excreted in the urine of infants fed in this way shows that the succinate content of the urine is double in the first case of what it is in the second case; the contents of dicarboxylic acids, sebacate (C10), suberate (Cg) and adipate (C6), in particular, are very different in both cases, with, in the second case, a very low content of suberate and adipate and a total absence of sebacate and 5-hydroxyhexanoate. Such results have confirmed the thesis of TANTIBHEDYANKUL and HASHIM ULL. NY
ACAD. ACAD. MED., 46 (1971) p. 17-333 that the fat digestibility coefficient is increased from 76% to 97% in low birthweight infants fed MCT at high doses, and this improved digestibility coefficient is related to the occurrence of urinary excretions. organic acids, especially C6, C8 and C10 dicarboxylic acids.
As a result, the use of low doses of TCM appeared of no interest to normal infants. Indeed, as is known, the medium chain dicarboxylic acids (c6-C10) are metabolites derived from the hepatic oxidation of circulating fatty acids and the fact that there is no urinary excretion of acid. sebacic (in
C10) in premature infants treated with low-grade (4%) milk-based TCM compositions, is detrimental to the adoption of diet compositions containing such low TCM levels.
However, the inventor has been able to establish that the feeding of normal infants into a milk dietetic diet containing a relatively low content of MCT has the effect of providing them with a food whose digestibility is close to that of the woman's milk and of determine the excretion in the urine of these normal children, adipic, suberic and sebacic acids, which shows that, compared to the aforementioned work of WHYTE et al., a relatively low MCT content of the proposed milk diet according to the present invention, is sufficient to normalize the urinary excretion of sebacic acid with reference to the ideal case of maternal breast feeding.
The subject of the present invention is therefore an infant dietary milk product, essentially intended for the feeding of normal infants, which comprises a protein fraction, a carbohydrate fraction, a lipid fraction, and also mineral salts, which food is characterized in that it has a TCM content of between 2 and 14% by weight approximately.
According to an advantageous embodiment of the milk feed according to the invention, the TCMs present in said food are partially naturally contained in the lipid fraction and partly contributed by an additional addition of TCM.
According to an advantageous arrangement of this embodiment, the TCM provided in addition to those contained in the lipid fraction are added in a proportion of between 0.25 and 10% by weight approximately, of the milk feed.
According to an advantageous embodiment of the milk feed according to the invention, the lipid fraction comprises fats of vegetable origin and / or fatty tri-esters of animal origin.
According to an advantageous arrangement of this embodiment, the lipid fraction is constituted by a mixture of 20 to 60% fat of animal origin, 80 to 40% fat of vegetable origin and added TCM.
According to an advantageous modality of this arrangement, the milk feed has a total average proportion of TCM, naturally present and added, substantially between 11 and 14% by weight approximately, its content in C8 to C12 medium chain fatty acids whose ester bond with glycerol in triglycerides is more rapidly and more easily hydrolyzed by lipase than for long chain fatty acids C14 to c18, is increased, and its content of saturated fatty acids C14 + C16 + C18 is decreased, Similarly, its oleic and linoleic acid content is slightly reduced, such a composition of the lipid fraction conferring on the milk feed according to the present invention an excellent digestibility.

According to another advantageous arrangement of this embodiment, the content of saturated C14-C16-C18 fatty acids of the lipid fraction of the dietetic milk diet is of the order of 30-42% approximately.

According to another advantageous embodiment of the milk feed according to the invention, it contains taurine and / or carnitine in small proportions, of the order of about 0.025 to 0.04%.
According to yet another advantageous embodiment of the milk feed according to the invention, it contains approximately 15 to 20% by weight of malto-dextrin, the proportion of this latter in the total carbohydrate fraction amounting to About 25-35%.
According to an advantageous embodiment of the milk feed according to the invention, its lipid fraction represents 15 to 30% by weight of its total composition.
The milk feed according to the invention is either in powder form or in liquid form, concentrated or ready for use.
As mentioned above, it has been demonstrated in the context of the present invention that the feeding of normal newborns by a milk dietetic dietary composition whose lipid fraction is enriched in TCM according to the present invention, determines the urinary exertion of dicarboxylic acids whose profile is qualitatively and quantitatively comparable to that observed in breastfed infants.

According to the invention, the presence of adipic, suberic and sebacic acids excreted in the urine of normal infants fed with dietetic milk diet containing a relatively low to moderate
TCM, is detected by an appropriate assay method, and it has substantially the same value as the corresponding physiological urinary excretions in breastfed infants, either
10.6 t 2.8 mmol / mol creatinine, adipic acid (C6),
3.3 1 0.8 mmol / mole creatinine, suberic acid (in C8),
0.91 i 0.52 mmol / mole creatinine, sebacic acid (in clo) l values found in relation to a dietetic milk food whose lipid fraction is constituted for 35% by fats of plant origin, for 60% by fat of animal origin, preferably milk, and for 5% by added TCM.
The present invention also relates to a method for the determination of dicarboxylic acids excreted in body fluids and in particular in the urine, which is characterized by the coupling of gas chromatography with mass spectrometry and detection by selective search of electron impact ions of t-butyldimethylsilyl (TBA1S) esters, adipic acids (in
C6), suberic (in Cg) and sebacic (in C10) using phenylsuccinic acid as internal standard.
This method of assay, the sensitivity of which is better than the methods using trimethylsilyl esters (WHYTE, cited Loc.), Makes it possible to detect and dose very small physiological quantities of dicarboxylic acids present in the urine of newborns, despite a diuresis limited, and which would be badly or not detected by the methods of the prior art indicated above.
In addition to the preceding provisions, the invention also comprises other provisions, which will emerge from the description which follows.
A clinical study of a dietetic milk food according to the invention was carried out on 215 newborns during the stay of these in maternity.

In this clinical study, the milk-fortified food enriched in TCM according to the present invention shows a nutritional efficiency better than that of a dietary milk diet at the usual commercial age.
The first days of life were marked by - a weight loss reaching its maximum at D2, less im
than that noted with the milk control diet, - a faster recovery.
This trial, performed in open, brings very positive results for this new TeM-enriched milk diet.
Regarding tolerance, the milk feed according to the invention did not cause more regurgitation or transit disorders than the control milk diet.
The determination of the urinary dicarboxylic acids was carried out by gas chromatography / mass spectrometry (Nermag) coupling and detection by selective electron ion detection of t-butyldimethylsilyl (TBDMS) esters. This type of assay has made it possible to obtain a much better sensitivity than with trimethylsilyl esters and can therefore be applied to the determination of physiological quantities of dicarboxylic acids in the urine of newborns despite limited diuresis.
The urine of newborns participating in the experimentation of the milk feed according to the invention was collected at the 7th day of life. Their dicarboxylic acid content was measured by acidifying 1 ml of urine with HCl, and then adding the internal standard, phenylsuccinic acid, at 100 μmol / liter, and extracting the urine thus previously treated, with 3 ml of ethyl acetate. The organic phase was evaporated to dryness and the residue dissolved in 0.05 ml of a solution of
N-methyl-N- (t-butyldimethylsilyl) -trifluoroacetamide / pyridine (1: 1) and heated for 10 minutes at 60 ° C. 1 microliter was placed on a Ros injector of a gas chromatograph-spectrometry apparatus mass (Nermag, Rueil-Hameling, France) equipped with a silica column (CPSil, 25m x 0.2mm, Chrompack, The Netherlands) at 1800C, with a temperature programmed to rise 50C per minute.
The ion selective search of the t-butyldimethylsilyl (TBDMS) ester fragments of adipic, suberic, sebacic and phenylsuccinic acid used as internal standard was performed. The quantifications were made by comparison with pre-established calibration curves, relative to creatinine excretion, which were measured separately on a Beckman-Astra analyzer.

I, - group III was fed a dietetic milk meal
according to the invention, the composition of which is

These children were fed "ad libitum" and on the 7th day, a collection of urine was made between 9 am and 12 pm.
These urine were immediately frozen while waiting for the dosage.

té ler âge usuel (cf. Tableau I) entraîne une chute signi
"profil" d'acides dicarboxyliques urinaires comparable à
The assay method according to the invention has demonstrated that - adipic, suberic and sebacic acids are
normal health of the urine of normal newborns (group
pe I); - feeding normal newborns with a lake food
age (see Table I) leads to a fall in
fictitious excretion of sebacic acid by comparison
children who are breastfed; - feeding normal newborns with a lake food
according to the invention (see Table III) restores a
"profile" of urinary dicarboxylic acids comparable to
observed in breastfed children and demonstrates
therefore the digestibility of the milk feed in accordance with the
vention, comparable to that of women's milk.
