Description

PROTHESE ARTICULAIRE PERFECTIONNEE EN TITANE.
L'invention concerne une prothèse articulaire perfectionnée en titane, notamment une prothèse totale de hanche à fixation primaire.
Comme on le sait, une prothèse totale de hanche à fixation primaire, comprend essentiellement trois parties, à savoir respectivement
- une tige fémorale destinée à être inserrée dans le fémur
- une cupule de bassin dénommée également "cupule cotyloïdinne, destinée à s'engager dans la cavité du même nom de la hanche considérée
- un col prothétíque, coiffé d'une sphère, reliant la tige et la cupule, destiné à assurer l'articulation proprement dite.
La coiffe de l'extrémité du col prothétique est donc amenée à frotter en permanence contre la cupule.
Dans ce type de prothèse, comme dans des prothèses analogues, on a déjà proposé d'utiliser le titane qui présente de nombreux avantages du fait de son poids, de son élasticité et de sa tolérance. Dans de telles prothèses, la coiffe frottante est actuellement réalisée en céramique, car le couple de frottement entre le titane et le polyéthylène de la cupule est trop élevé. Cet arrangement donne d'excellents résultats. Toutefois, lorsque la tête fémorale a un petit diamètre, les têtes en céramique que lgon pourrait utiliser risquent de casser du fait de la fragilité de la céramique aux chocs.Pour pallier cet inconvénient, on a suggéré d'utiliser des têtes en acier inoxydable.Malheureusement, cette disposition semble provoquer des couples électrochimiques entre l'acier inoxydable et le titane, couples qui sont susceptibles d'engendrer des complications influant tant sur l'état de l'articulation que sur l'état général du patient.
L'invention pallie ces inconvénients. Elle concerne une prothèse articulaire en titane qui se caractérise en ce que les parties frottantes de cette prothèse sont revêtues d'une couche de nitrure de titane.
-En pratique, la couche de nitrure de titane caractéristique de l'invention a une épaisseur de l'ordre de quelques microns, avantageusement comprise entre trois et dix microns. Si l'épaisseur est inférieure à trois microns, l'effet obtenu risque de s'atténuer dans le temps. Au-delà de dix microns, on augmente inutilement le prix de ces matériaux sans en améliorer pour autant les avantages.
Cette couche de-nitrure de titane peut être déposée sur la coiffe prothétique par tous moyens connus, tels que notamment dépôt en phase gazeuse de nitrure de titane, notamment à basse température, c'est-à-dire à une température de l'ordre de 500 C.
Comme on le sait, le dépôt d'une couche de nitrure de titane en phase gazeuse est un procédé bien connu pour améliorer la dureté des matériaux. Il est donc surprenant que ce moyen connu depuis fort longtemps en métallurgie permette de supprimer avec succès des inconvénients inhérents aux prothèses articulaires en titane, alors que ces inconvénients n'étaient pas liés à des problèmes de dureté.
La figure unique annexée, montre une prothèse totale de hanche à fixation primaire, conforme à l'invention dans laquelle (1) représente la tige fémorale filetée é- vasée en titane, dont la tête (2) légèrement conique vient s'emboiter dans un cône femelle conique (4) complémentaire ménagé à cet effet dans le col prothétique (3), également en titane. L'ajustage de l'ensemble est assuré par une vis (5).
La tête mâle (6) du. col (3) est emboitée à son tour dans un cône femelle (7) correspondant de la coif fe (8) également en titane.
Selon la caractéristique de l'invention, la surface extérieure de cette coiffe (8), qui est destinée à venir frotter en permanence contre la cupule (10), est recouverte d'une couche (9) de nitrure de titane de six microns d'épaisseur, déposée par dépôt physique en phase gazeuse, technique plus connue sous la dénomination "Physicol Vapor Deposition - PVD" à basse température, c'est-à-dire de l'ordre de 530 C.
Si l'exemple décrit concerne une prothèse de hanche, l'invention peut être également adaptée à toute autre prothèse dans laquelle on trouve en présence une partie frottante articulée en contact avec une partie fixe, telle que par exemple une prothèse de genou.

Claims (4)

REVENDICATIONS r
1/ Prothèse articulaire perfectionnée en titane, du type comportant des parties fixes (3,10) et des parties frottantes (8) sur l'une de ces parties fixes (3), caractérisée en ce que la partie frottante (8) en titane est revêtue d'une couche (9) de nitrure de titane.
2/ Prothèse selon la revendication 1, caractérisée en ce que le revêtement en nitrure de titane a une épaisseur de trois à dix microns.
3/ Prothèse selon l'une des revendications 1 et 2, caractérisée en ce que la couche de nitrure de titane est déposée en phase gazeuse.
4/ Prothèse de hanche totale à fixation primaire en titane comportant une tige fémorale (1), une cupule cotyloidienne (10) et un col prothétique (3) dont la tête (6) présente une coiffe (8) articulée qui frotte contre la cupule (10), caractérisée en ce que la coiffe (8) du col prothétique en titane est revêtue d'une couche (9) de nitrure de titane.
