Abstract
Description
"Orthèse de la main". "Hand brace".
It is known in hand surgery to use orthotic support from the first post-operative days and for long-term rehabilitation of the hand. This support is provided by an orthosis generally composed of a gauntlet blocking the wrist, a hammock supporting the finger segment, and elastic means which, going from the rear part of the gauntlet to the hammock, passing over means of return, exert on the finger a traction contributing to the tendon repair.
Traditionally, these elastic means are constituted by an elastic cord, made of rubber or the like, extending from the rear part of the gauntlet to the hammock for hanging the finger and passing over the return means fixed to the gauntlet. If this technique has the advantages of being simple to implement and easily replaceable by an identical commercial element, it also has significant drawbacks in terms of the ignorance and the irregularity of the tension exerted by the elastic means and of their reliability. More precisely, the force developed by the elastic cord is difficult to measure, varies over time, cannot be calibrated and leads to a power / elongation ratio different from that of a muscle. As a result, the tension supplied is always the even whatever the subject, man, woman or child, or whatever the action sought.
One of the objects of the invention is to provide elastic means which do not have these drawbacks and which can be better adapted to post-operative or rehabilitation needs, as well as to the strength of the patient.
Furthermore, in current orthoses, the return bracket, determining the point of application of the tension of the elastic means on the finger segment to be mobilized, to a large bulk hindering the dressing and forcing its user to remove the orthosis with the risks involved. In addition, by extending beyond the hand, it constitutes a danger for the environment of the patient, but also facilitates its attachment with the projecting elements of this environment.
Another object of the invention is to provide a less bulky and more efficient stem in its function.
To this end, the orthosis according to the invention comprises, on the one hand, at least one support integral with the gauntlet and, on the other hand, a calibrated spring which, chosen from a series of calibrated springs providing different tensile forces for identical lengths, is hooked, by its rear end, to a means of hooking the gauntlet and by its front end, to an inextensible relay cable connected to the hammock and passing through a longitudinal tunnel formed in the upper part of the corresponding support.
Thanks to its structure, this orthosis allows the fitting of different hands by adapting to the multiple locations of hand lesions and the plurality of patients treated, differentiated by the shape of the hands and the length of the fingers. In addition, the use of calibrated springs makes it possible to adapt the forces applying to the extensor and flexor tendons depending on the age, sex, strength and morphology of the patient, but also on the progression sought in treatment or rehabilitation.
In one embodiment of the invention, the support body has, in plan from above, the shape of a regular trapezoid whose inclined lateral sides diverge in the direction of the fingers protruding from the gauntlet and, in vertical cross section, a triangular shape with rounded top, and is extended longitudinally by two fixing tabs on the gauntlet.
Due to its shape, the support can be juxtaposed with three identical supports on the same gauntlet to simultaneously pair four fingers while maintaining the divergence of the fingers.
Advantageously, each support is extended by lugs allowing it to be fixed to one of the elements of the gauntlet according to the morphology of the hand, the number of fingers concerned and the treatment.
In other words, the orthosis is made by a professional depending on the rehabilitation, and is therefore perfectly suited to the needs, which is not the case with current orthotics produced in series with the search for versatility.
Other characteristics and advantages will emerge from the description which follows with reference to the appended diagrammatic drawing, representing by way of nonlimiting examples, an embodiment of this orthosis.

Figure 8 is an abacus of the characteristics of the various springs indicating on the ordinate the forces in grams and on the abscissa, the elongations in millimeters.
In known manner, this orthosis is constituted by a gauntlet (A) carrying at its upper end at least return means (B) and elastic means (C) going from the rear end of the gauntlet to a hammock (D) suitable to receive the treated finger segment.
In the embodiment of Figures 1 to 5, the return means are composed of a support (2) and a lamellar bracket (3). As shown in more detail in Figures 4 and 5, the support (2), made of molded material, has, seen in plan from above, the shape of a regular trapezium whose lateral sides (2a) are inclined and delimit between them an angle (a) of the order of 100. When the support is fixed to the gauntlet (A), its lateral sides diverge towards the outside of the gauntlet in the direction of the underlying fingers. The support is extended by lugs (4) allowing its attachment to the gauntlet, by riveting or other similar means. As shown in FIG. 5, the support has, in vertical cross section, a generally rounded triangular shape limiting its size and its risks of catching during the patient's movements. Its lateral faces thus define an angle (b) having a value of on the order of 600. The support is traversed longitudinally, on the one hand, by a bore of rectangular section (5), corresponding substantially to the transverse dimensions of the lamellar bracket (3), and, on the other hand, by a tunnel (6).
Depending on the applications, the bore (5) receives either a short blade, as shown in FIG. 2 corresponding to the application of the prosthesis for the rehabilitation of lesions of the extensors in the verdan area 4-5-6-7-8 , or a long, bent blade, as shown in Figure 3 corresponding to the rehabilitation of extensor lesions in the -verdan 2-3 area. Whatever blade (3) is used, it can slide freely in the bore (5) to allow adjustment of the point of application of the tension force on the finger segment. - Once this adjustment has been made, the blade (3) is locked in position by means of screws (7) screwed transversely into the support (2) and bearing on this blade.
The hammock (D) is constituted by a band (8) capable of enveloping the finger segment and provided on its opposite faces with complementary fastening means. These means may consist of strips of synthetic velvet respectively with loops and hooks arranged one on the inside of the strip, the other on the outside. The strip (8) which is intended to form a ring, completely enveloping the finger segment, has on its dorsal face a flat hook (9) for fixing the ends of the elastic means ensuring the return and rehabilitation tension.
According to the invention, these elastic means consist of a calibrated spring (10) whose rear end is fixed to hooking means (12) of the gauntlet and whose front end is hooked to a relay cable (13 ). This cable crosses the tunnel (6) of the support (2), passes over a pulley (14), mounted for free rotation in an upper yoke (11) at the free end of the lamellar bracket (3), and s' hangs on the ring (9) of the hammock (D). The spring (10) is chosen from a series of tension springs providing, over the same elongation range of the order of 0 to 50 mm, different forces ranging from 50 grams to 2 kilograms. The tests carried out show that range of twelve springs makes it possible to satisfy all applications and to adapt the orthosis on the one hand, to the strength of the patient, a strength which is different for men, women and children, but also on the other hand, in need of rehabilitation.
FIG. 8 shows on the ordinate the force curves provided by these springs, as a function of their elongation. This calibration range adapts to different situations of hand surgery. This allows a customization of the orthosis, which can be adapted to subjects and fingers of different strengths as well as an adaptation of the orthosis over time taking into account the evolution of the post-operative situation.
As shown in Figure 8, the calibration of these springs is identified by different colors avoiding any confusion in their use and any therapeutic error.
Advantageously, the hooking means (12) of the spring (10) on the gauntlet (2) are adjustable, that is to say movable longitudinally so as to be able to communicate to the spring (10) an initial tension.
These adjustment means can be diverse and consist, for example, of complementary notches formed on the attachment means and on the gauntlet or in a succession of perforations produced in the gauntlet and each capable of receiving a rod of the attachment means.
It should be noted that thanks to the use of a calibrated spring and a relay cable (13), made of inextensible material at least under the forces provided by the spring, the force communicated to the finger segment is perfectly adapted and remains constant over time. , which was not the case with the elastic cords of traditional orthotics.
Figure 4 shows that each support can, thanks to the angulation of its side faces, be juxtaposed with other identical supports on the same gauntlet (A). Under these conditions, the angle (x) formed by the longitudinal axes of the two juxtaposed supports, has a value of 80 which corresponds to the divergence of two metacarpals. In other words, this support makes it possible to ensure the simultaneous fitting of two or more neighboring fingers while respecting the divergence of the fingers.
FIGS. 1 to 3 show that, thanks to the position of the bore (5) in the lower part of the support (2), the lamellar bracket (3) is in the immediate vicinity of the finger but without contact with it, which reduces the risk of snagging while perfectly ensuring the function of returning the relay cable.
Figure 3 shows that the blades (3), long and angled, are associated with a stop (15) adapted to match the dorsal face of the finger segment.
This stop, internally lined with dense foam, is mounted adjustable longitudinally with respect to the blade (3). To this end, it is linked to this blade by a screw (16) passing through a longitudinal lumen (17) formed over the greatest length of this blade.
It appears from the above that by its structure, and in particular the shape of its support, its lamellar stem and its elastic means, this orthosis provides versatility both in terms of the number of fingers involved and in terms of morphology of the hand, the areas of verdan concerned and the forces used to ensure post-operative treatment or rehabilitation of the finger concerned.
Figures 6 and 7 show the application of the orthosis to a kleinert, respectively for superficial common flexor and for deep common flexor. The gauntlet (A), comprises in the lower part a support (2a), with a yoke (17) in which a pulley (14) is used for returning the relay cable (13) going from the spring (10) to the end of the finger to which it is sewn. In this application, as in the previous one, the use of a pulley (14) improves the transmission of the dynamic assistance while reducing the coefficient of friction and ensures the precision of the movement of the mechanical assistance, while allowing, according to the different situations, bringing dynamic assistance to the digital end that we want to mobilize. As explained above, such pulleys are indifferently used for a dynamic extension orthosis or for dynamic palm flexion orthosis in the post-operative rehabilitation of the flexor tendons.
