Description
Sector técnico de la invención Surgical splint to guide the mandibular or maxillary bone drill during an implant surgery. Technical sector of the invention
Antecedentes de la invención The invention relates to a surgical splint to guide the mandibular or maxillary bone bore during an implantology intervention. Background of the invention
The drilling of the mandibular or maxillary bone for the subsequent insertion of a threaded rod for the support of a dental prosthesis is a delicate operation, which requires a high degree of precision. It is a known practice to use radiological templates or splints that reproduce the patient's dental arch equipped with means to guide the burr during the drilling operation of the implant bed. In order to determine the exact position and axial orientation of the drill to be carried out, said splints incorporate guide perforations in which cylindrical sheaths, generally metallic, are housed to guide the milling cutter. The position and orientation to be taken by each of said cylindrical sleeves are determined by the physician before each splint is manufactured, which are made to measure for each patient following various procedures, for example by obtaining prior molds or by computer models that partially or totally reproduce the dental arch including the settlements of the missing pieces, intended to be replaced by prosthetic pieces.
Among the important recommendations to ensure optimal implant integration, it is important to avoid excessive heating of the bone during the perforation of the bone bed that can cause protein denaturation and bone necrosis. For this purpose, physiological serum is conventionally provided to the milled bed during the drilling operation.
Although the internal irrigation strawberries, which emit the refrigerant serum through fine holes near its leading edge, are suitable for delivering an adequate volume of serum to the milled bed, sometimes these cannot be used when splints are used as previously described and a remarkable depth of drilling in the bone is required, or when the diameter of the cutter is small. The milling cutters suitable for drilling with these speci fi cations cannot be provided with any cavity for the transport of serum because they would be too fragile and could break. Consequently, when the outer diameter of the milling cutter is adjusted to the inner diameter of the cylindrical sheath and serum cannot be irrigated through the cylindrical sheath, it is necessary to remove the milling cutter from the cylindrical sheath to irrigate, through said cylindrical sheath, The implant bed. Consequently, it is necessary to stop the milling operation and make successive movements of extraction and introduction of the milling cutter through the cylindrical sheath each time it is required to irrigate the implant bed.
It is a main objective of the present invention to provide a solution to the aforementioned drawbacks. A secondary objective of the invention is a splint specially adapted to solve said drawback, applicable to known splints obtainable by any of the known methods to date.

Figs. 1 to 3 show a splint 1 according to the invention that reproduces the dental arch of a patient. In the splint of Figs. 1a3 there are several guide holes for a cutter from which the guide holes 4a and 5a have been pointed out, in each of which a corresponding cylindrical and metal tubular sheath 6 and 7 are arranged, intended to guide the cutter during the drilling operations of the mandibular bone
It is observed in Figs. 1 to 3 that in the vicinity of each of said guide perforations 4a and 5a the splint 1 is provided with irrigation perforations 4b and 5b, through which it is possible to irrigate the implant beds located immediately with physiological serum below the tubular sleeves 6 and 7, respectively, when the splint is placed in the operative position.
Así, la perforación de irrigación 4b está asociada Thus, irrigation drilling 4b is associated
a la perforación de guía 4a, y desemboca en el espacio que queda entre la embocadura de salida de la está colocada en posición operativa; y la perforación de irrigación 5b está a su vez asociada a la perforación de guía 5a, y desemboca en el espacio que queda entre la embocadura de salida de la funda tubular 7 y la mucosa del lecho del implante correspondiente. to the guide perforation 4a, and ends at the space between the outlet mouth of the is placed in operative position; and the irrigation perforation 5b is in turn associated with the guide perforation 5a, and flows into the space between the outlet opening of the tubular sleeve 7 and the mucosa of the bed of the corresponding implant.
Naturally, it is not only contemplated that the irrigation perforations 4b and 5b are located either in the mouth or in the vestibular, depending on the anatomy of the patient, but also the possibility that the splint 1 of two irrigation perforations can be provided associated to the same guide perforation, arranged one in buccal and one in vestibular or both, coincidentally, in buccal or vestibular. In any case, it is the doctor when designing the splint 1 that decides its number and arrangement depending on other factors of the position of the operator, the assistant and the complexity of each case, with the purpose of not obstructing the surgical field.
In the splint 1 of Figs. 1 to 3, the irrigation perforations 4b and 5b house corresponding smaller, cylindrical tubular sleeves 8 and 9, axially oriented in the direction of the axial extension of the tubular sleeves 6 and 7, also cylindrical in the embodiment, associated.
In these splints, the diameter of the smaller tubular sleeves 8 and 9 is adapted to receive the removable coupling of the end of an irrigation device 10, which can be introduced with some adjustment into any of the smaller tubular sleeves 8 or 9, of so that it can remain plugged into them without the need to manually hold the irrigation device, as shown in Fig. 3.
It should be said that tubular sleeves 6 and 7 can be adapted for guiding strawberries of different external diameter, using known solutions, and that the protection of the splint 1 object of the invention is not conditional on the method by which said splint has been obtained, being irrelevant to the problem here solves the means used for its design, being able to be computer
