DISPOSITIVO DE CIERRE Y SO PO RTE PARA ELEVACIÓN DE SENO S CLOSURE AND SUPPORT DEVICE FOR SINUS LIFT S
PARANASALESPARANASALS
The object of the present invention consists of a closure and support device in filling operations for paranasal sinus elevation, in which an opening is produced in the maxillary lateral wall to access the maxillary sinus and cause, in the lower part thereof through the introduction of a regenerative filling, the reinforcement of said lower wall so that it can be the carrier of a dental implant prosthesis. Its purpose is, on the one hand, to hold the cut piece that folds into the maxillary sinus along with the sinus membrane, and on the other, to close the opening in the maxillary lateral wall, combining a membrane for bone regeneration with a titanium support in the form of a square that is attached to the maxillary side itself.
ANTECEDENTES DE LA INVENCIÓNBACKGROUND OF THE INVENTION
Currently there is patent US5711315 that describes a way of accessing the maxillary lateral wall to the maxillary sinus, making several holes through which a tool for separating and elevating the sinus membrane is inserted on one side, and through another hole the light and camera to visualize the operation, to introduce the regenerative filling through one of them. In said patent, reference is made to the state of the art prior to said application, in which the technique of opening an entrance in the maxillary lateral wall and, through it, separating the cut piece towards its interior and removing the sinus membrane in the same way, proceed with the regenerative filling to finally close the opening made by the gingival tissue itself that covers the maxillary lateral wall. This had effectively been giving rise to a multitude of problems in said operation and closure, mainly due to the fact that the cut piece was left without support inside and this, together with a closure with very weak gingival tissue that was very easily divided, giving rise to serious regenerative fill problems. In the same way that proceeding through the holes proposed in the aforementioned patent are insufficient due to the difficulty in handling and separating the sinus membrane. These techniques referred to in said patent US5711315, are overcome by the solution offered by the closure and support device in stuffing operations in paranasal sinuses object of the present invention, which solves the problem in the art known prior to said patent, and whose purpose is the opening in the maxillary lateral wall.
Patent application US2019/0008615 has recently been filed in which it is proposed to carry out the regenerative filling by accessing the maxillary sinus through the gum itself, to reach the sinus membrane from said hole and proceed to separate it by pressure produced by the filling itself that is introduced in a controlled way. However, the risk of sinus membrane rupture remains very high, since there is no direct control over the membrane and using pressure alone does not prevent sinus membrane rupture or perforation. In addition, the entrance to the maxillary sinus through the lower wall that is to be regenerated is not recommended, since inevitably with the perforation it is directly weakened, and in any case the hole made must be regenerated in the same way, on whose regeneration will perform the manipulation again for the placement of the corresponding dental implant. In other words, said technique does not impair the advantages obtained with the closure and support device in paranasal sinus lift operations object of the present invention.
The true advantage and characteristic of the present invention is that the closure and support device is incorporated in filling operations for paranasal sinus lift, guaranteeing, on the one hand, the retention of the cut piece with the opening, and on the other, closing the opening made. in a stable and safe way to proceed with the regeneration of both the bottom of the maxillary sinus and the maxillary lateral wall itself. For this, using the combination of a membrane for bone regeneration with the titanium closure in the form of a square which supports the membrane for bone regeneration on its upper part and covering the lower part of the closure and titanium support, throughout its perimeter. , giving rise to the closure and support device object of the present invention.
EXPLICACIÓN DE LA INVENCIÓNEXPLANATION OF THE INVENTION
The object of the present invention is a closure and support device for filling operations in paranasal sinuses. These operations are known as "sinus lifting" which are caused by the existence of a cavity in the lower part of the maxillary sinus, which makes the bone too thin to place dental implants, which requires regeneration. of the same to be able to place said devices with the certainty that, on the one hand, the bone itself will not break and, on the other hand, that it will be a stable and safe implant, thus avoiding all kinds of complications and infections.
Among the many operating techniques to obtain said regenerative filling of said sinus cavity, it is known that it is carried out through an opening made in the maxillary lateral wall with the previous cutting of the gingival tissue that covers, on the outside, said maxillary lateral wall and creates direct access to it to proceed with the direct cut of said wall in a controlled manner, with a depth of cut up to the sinus membrane, respecting its integrity. Said sinus membrane covers the entire inner wall of the maxillary sinus, which will then be separated from said wall in a controlled manner so that it does not break and that allows: a) On the one hand, to hold and act as a hinge for the displacement of the cut piece of the maxillary lateral wall which is still attached to the sinus membrane, b) To establish a sinus cavity to be regenerated limited by the maxillary sinus cavity itself and the sinus membrane, a cavity that will be covered by the regenerative filling that is introduced by the cavity made in the maxillary lateral wall.
Que consiste en:After proceeding with the cut in the maxillary lateral wall, it is when the closure and support device object of the present invention is applied. Consisting of:
A) A cutting template, which will serve to accurately determine the size of the opening to be made.
La membrana se ubica encima del dispositivo de soporte y cierre.B) In combination, once the cut and opening have been made, as well as the regenerative filling, with a membrane for bone regeneration which is inserted in the upper part of the cavity to be regenerated in contact with the cut piece of the lateral wall maxilla, which is attached to the sinus membrane and positioned horizontally and perpendicular to the upper side of the opening made in the lateral wall maxillary. The membrane is located on top of the support and closure device.
Todo ello da lugar a un dispositivo de cierre y soporte objeto de la presente invención en operaciones de relleno en senos paranasales aquí descritas.C) A titanium closure, since said material has the characteristics of being an inert, insoluble, adherent and innocuous material, which is formed by two planes, one in its upper part constituted by two legs arranged in a 90° square /-with the other quadrangular part, in which its legs serve as a support for the membrane for bone regeneration, which in turn is in its upper part in contact with the cut piece of the maxillary lateral wall. As a result, all of this rests on said legs of the closure, which in its lower part has a rectangular shape, thus providing the sealing of the opening made by means of the membrane for bone regeneration reinforced by the titanium closure piece. All this gives rise to a closure and support device object of the present invention in filling operations in paranasal sinuses described here.
Regarding the membrane for bone regeneration, it should be pointed out that this is sometimes not necessary, being sufficient for the closure the closure piece itself and titanium support, which due to its shape and characteristics turns out to be sufficient to give full compliance. with its purpose, taking into account the characteristics of the titanium and the structure of the piece.
