Abstract
Die Erfindung schafft eine implantierbare Kiefergelenksteilprothese (90, 90', 90'') mit einem an einem Unterkiefer (40) befestigbaren ersten Implantatteil (10, 10', 10'') mit einem Gelenkkopf (42, 42', 42''), zu welchem eine an einem Cranium (50) angeordnete Gelenkfläche (52, 82) ein Widerlager bildet; wobei das erste Implantatteil (10, 10', 10'') eine aus einem metallischen Werkstoff gebildete erste Komponente (20, 20', 20'') aufweist, welche an dem Unterkiefer (40) befestigbar ist, und eine aus einem Kunststoff gebildete zweite Komponente (30, 30', 30'') aufweist, welche den Gelenkkopf (42, 42', 42'') aufweist; und wobei die erste Komponente (20, 20', 20'') mit einem ersten Verbindungsstück (22, 22', 22'') versehen ist und die zweite Komponente (30, 30', 30'') mit einem zweiten Verbindungsstück (32, 32', 32'') versehen ist, und das erste Verbindungsstück (22, 22', 22'') und das zweite Verbindungsstück (32, 32', 32'') ineinandergreifend zumindest bereichsweise formschlüssig lösbar miteinander verbindbar oder verbunden sind. Die Erfindung schafft ebenfalls ein entsprechendes Herstellungsverfahren und eine entsprechende Kiefergelenksprothese.
Description

Die vorliegende Erfindung betrifft eine implantierbare Kiefergelenksteilprothese mit einem an einem Unterkiefer befestigbaren ersten Implantatteil mit einem Gelenkkopf, zu welchem eine an einem Cranium angeordnete Gelenkfläche ein Widerlager bildet; wobei das erste Implantatteil eine aus einem metallischen Werkstoff gebildete erste Komponente aufweist, welche an dem Unterkiefer befestigbar ist, und eine aus einer Keramik oder einem Kunststoff gebildete zweite Komponente aufweist, welche den Gelenkkopf aufweist. Außerdem betrifft die Erfindung ein entsprechendes Verfahren zur Herstellung einer implantierbaren Kiefergelenksprothese sowie eine implantierbare Kiefergelenksprothese, die die Kiefergelenksteilprothese umfasst.
Das Kiefergelenk (im Folgenden auch als TMJ = temporomandibular joint bezeichnet) ist eines der meist genutzten und wichtigsten Gelenke im menschlichen Körper. Es spielt eine wesentliche Rolle in der Bewegungsführung der Mandibula (des Unterkiefers), beim Kauen, Sprechen, Schlucken und der Stressbewältigung. Dabei ist es sogar im Schlaf durch die Schluckbewegung ständig in Bewegung. Damit das Kiefergelenk seine Bewegungsabläufe durchführen kann, müssen die Form der Gelenkflächen, der Zustand des Gebisses, Zahnstellung, Zahnform, die Kaumuskulatur ein funktionales System bilden, welches durch die Vielzahl an Komponenten einem gewissen Maß an Störungsanfälligkeit unterliegt. Das linke und rechte Kiefergelenk arbeiten dabei stets zusammen und bilden insoweit eine funktionelle Einheit.
Treten im Bereich des Kiefergelenks jedoch Störungen, wie z.B. Funktionseinschränkungen oder Erkrankungen, auf, kann es zu namhaften Beeinträchtigungen der Lebensqualität im Alltag von Patienten kommen. Eine Wiederherstellung von korrekten Gelenkstrukturen ist hierbei meist nur im Wege einer chirurgischen Lösung, also durch Resektion des gestörten Gelenks und dessen Ersatz durch eine Kiefergelenksprothese, möglich.
Eine Kiefergelenksprothese ist beispielsweise aus der EP 3 003 225 B 1 bekannt, welche einen Mechanismus bereitstellt, der eine kombinierte Translations- und Rotationsbewegung zwischen Schädel und Unterkiefer gestattet, bei der eine gleitende Bewegung zwischen Oberflächen stattfindet. Allerdings kann dabei ein Verschleiß auftreten.
Es die Aufgabe der vorliegenden Erfindung, eine verbesserte implantierbare Kiefergelenksteilprothese, ein entsprechendes Herstellungsverfahren und eine entsprechende Kiefergelenksprothese zu schaffen, mittels der die gleitende Bewegung der Oberflächen möglichst zuverlässig und sicher und dabei verschleißarm ausgeführt wird, und bei welcher der Gelenkkopf an die individuellen Gegebenheiten des vorliegenden TMJ in geeigneter Weise aufwandsarm angepasst werden kann.
Die Aufgabe wird durch eine implantierbare Kiefergelenksteilprothese mit den Merkmalen des Anspruchs 1 gelöst. Die erfindungsgemäße Lösung besteht also insbesondere darin, dass bei einer Kiefergelenksteilprothese der eingangs genannten Art die erste Komponente mit einem ersten Verbindungsstück versehen ist und die zweite Komponente mit einem zweiten Verbindungsstück versehen ist, und das erste Verbindungsstück und das zweite Verbindungsstück ineinandergreifend zumindest bereichsweise formschlüssig miteinander verbindbar oder verbunden sind.
Mit der erfindungsgemäßen implantierbaren Kiefergelenksteilprothese wird also für die individuell vorliegende, das Widerlager bildende Gelenkfläche, eine zuverlässige Gleitpaarung dadurch bereitgestellt, dass an dem ersten Implantatteil mehrere Komponenten vorgesehen sind, die mit ihren Verbindungsstücken ineinandergreifen und mit diesen einen Formschluss bilden, wodurch sie zuverlässig miteinander verbindbar oder verbunden sind. An dem TMJ bildet die Teilprothese dabei zusammen mit der Gelenkfläche (Fossa) eine Gleitpaarung, die eine 5-achsige Bewegung des Kiefergelenks (TMJ) gestattet, wobei der Gelenkkopf (Kondylenkopf) als das abrollende Element individuell gestaltet werden kann.
Bevorzugte Weiterbildungen der erfindungsgemäßen implantierbaren Kiefergelenksprothese finden sich in den entsprechenden Unteransprüchen.
Um zwischen den Komponenten der erfindungsgemäßen Kiefergelenksteilprothese eine besonders haltbare Verbindung zu erreichen kann bei einer bevorzugten Weiterbildung eines der Verbindungsstücke mit wenigstens einem Vorsprung ausgebildet sein, welcher in wenigstens eine komplementäre Ausnehmung an dem anderen Verbindungsstück greift.
Dabei können der Vorsprung und die Ausnehmung an einander gegenüberliegenden Flächen den betreffenden Formschluss bilden. Es können hierbei an den Komponenten auch jeweils seine Mehrzahl von Vorsprüngen und Ausnehmungen ausgebildet sein, wobei jede der Komponenten sowohl mit Vorsprüngen, wie mit Ausnehmungen versehen sein kann.
Für eine besonders haltbare Kopplung der Verbindungsstücke, und damit der Komponenten aneinander können in einer weiteren Weiterbildung der wenigstens eine Vorsprung des einen Verbindungsstücks und die wenigstens eine komplementäre Ausnehmung des anderen Verbindungsstückes einen allseitigen Formschluss bilden. Hierbei können bei einer einzelnen, mehreren oder allen eine Verbindung mit einem zugeordneten Verbindungsstück der jeweils anderen Komponenten eingehenden Verbindungsstücken allseitige Formschlüsse gebildet werden.
In einer die Festlegung der Verbindungsstücke der jeweiligen Komponenten weiter verstärkenden Weiterbildung können die erste Komponente und die zweite Komponente durch wenigstens ein zwischen den jeweiligen Verbindungsstücken angeordnetes Verbindungselement als dritter Komponente aneinander festgelegt sein. Diese dritte Komponente dient mithin der Arretierung der Gelenkkopfposition. Durch das betreffende Verbindungselement sind die Verbindungsstücke zum einen sicher aneinander gehalten, zum anderen wir ein in einer Steckrichtung zum Verbinden der Verbindungsstücke miteinander gegebenenfalls noch existenter Freiheitsgrad festgelegt. Überdies kann das Verbindungselement auch mehrteilig, mit miteinander zu verbindenden Teileelementen ausgebildet sein, letztere können z.B. steckbar sein.
In einfach herzu stellender und gut handhabbarer Weise kann bei einer anderen Weiterbildung das wenigstens eine Verbindungselement als dritte Komponente die erste Komponente und die zweite Komponente zumindest in einem Abschnitt des jeweils zugeordneten Verbindung Stücks durchgreifen. Dies ist insbesondere dahingehend zweckmäßig, als die jeweiligen Durchgriffe an den Komponenten bzw. deren Verbindungsstücken jederzeit auch nachträglich in diese einbringbar sind. Dabei kann der Durchgriff an jeder der Komponenten auch so ausgebildet sein, dass das Verbindungselement in diese nur eingreift.
Bei einer bevorzugten Weiterbildung der erfindungsgemäßen Kiefergelenksteilprothese können an den Verbindungsstücken der ersten Komponente und der zweiten Komponente miteinander fluchtende Durchgriffe angeordnet sein, in welche das wenigstens eine Verbindungselement einführbar/einsteckbar ist. Dabei kann das Verbindungselement eines der Verbindungsstücke durchgreifen während es in das andere lediglich eingreift, oder aber auch beide Verbindungsstücke vollständig durchgreifen.
Eine gut handhabbare Weiterbildung kann darin bestehen, dass die Durchgriffe der Verbindungsstücke und das in diese eingeführte wenigstens eine Verbindungselement sich im Wesentlichen quer zu der Erstreckungsrichtung des Gelenkkopfendes des Implantatteils angeordnet sind. Bei einer solchen Konfiguration lässt sich das Verbindungselement besonders einfach anordnen, um mit den Verbindungsstücken gegebenenfalls nachträglich verbunden zu werden. Das Verbindungselement kann hier einfach durch die fluchtenden Durchgriffe gesteckt werden, im Falle von Teilelementen auch von mehreren Seiten. Es ist aber auch denkbar, dass das in die Durchgriffe eingeführte wenigstens eine Verbindungselement in einer durch die Bewegung des ersten Implantatteils aufgespannten Ebene, und damit im Wesentlichen quer zu der zuvor beschriebenen Konfiguration, angeordnet ist.
Bei einer anderen Weiterbildung der Kiefergelenksteilprothese kann an dem ersten und dem zweiten Verbindungsstück jeweils ein dem anderen Verbindungsstück gegenüberliegend angeordneter Stützbereich vorgesehen sein, der die Verbindungsstücke beim Zusammensetzen
z.B. gegen eine Steckrichtung abstützt, so dass diese in einer vorbestimmten, definierten Position zueinander angeordnet werden können.
In einer bevorzugten Weiterbildung, bei der auch nachträglich Veränderungen an der Kiefergelenksteilprothese vorgenommen werden können, können die Komponenten mit ihren Verbindungsstücken lösbar miteinander verbindbar sein, so dass Komponenten, insbesondere die zweite Komponente mit dem Kondylenkopf, in einfacher Weise ausgetauscht werden können.
Bei einer anderen Weiterbildung, die haltbar, belastbar und gut verträglich ist, kann die erste Komponente mit dem ersten Verbindungsstück aus einem Titanmaterial ausgebildet sein.
In geeigneter Weise durch Nachbearbeitung an die konkrete individuelle Situation anpassbar kann bei noch einer anderen Weiterbildung der erfindungsgemäßen Kiefergelenksteilprothese die zweite Komponente mit dem zweiten Verbindungsstück aus einer Keramik oder einem thermoplastischen Kunststoff ausgebildet sein. Im Falle einer keramischen zweiten Komponente kann diese vorteilhaft z.B. aus einem mit Yttrium verstärkten Zirkonoxid-Material ausgebildet sein.
Bevorzugt kann bei einer Weiterbildung der thermoplastische Kunststoff durch ein Polyolefin wie UHMWPE (ultrahochmolekulares PE) oder ein Polyaryletherketon wie PEEK (Polyetheretherketon) gebildet sein, und besonders bevorzugt ist das Verbindungselement hierbei aus Polyethylen ausgebildet.
Die erfindungsgemäße Kiefergelenksteilprothese kann also ein erstes Implantatteil umfassen, das mit zwei Komponenten versehen ist, von den eine lasttragend, und die andere die Kieferbewegung abrollen ausgebildet ist. Die erste, lasttragende Komponente kann beispielsweise mit einer schaftartigen, aus Titan ausgebildeten an der Mandibula angeordneten Platte bestehen, an deren einer Gelenkfläche zugewandtem Ende die abrollende zweite Komponente als patientenspezifischer Kondylenkopf aus Zirkonoxid verstärkt mit Yttirum oder UHMWPE oder PEEK bestehen angeordnet. Beide Komponenten sind dabei individuell gestaltbar bzw. anpassbar und werden über ein Verbindungselement z.B. aus Polyethylen miteinander verbunden bzw. aneinander fixiert. So kann die erfindungsgemäße Kiefergelenksteilprothese aus zwei oder drei fest ineinander verankerte Materialien bestehen, die nach der Fertigung nicht voneinander trennbar sind, wodurch ein Hybrid-Implantat gebildet wird. Die Verbindung der Komponenten sowie des Verbindungselements wird über eine ineinandergreifende formschlüssige Oberflächentopografie gewährleistet, wobei die Gleitpaarung mit einer ggf. natürlichen Gelenkfläche des Kiefergelenks eine fünfachsige Bewegung gestattet.
Die obige Aufgabe wird ebenfalls gelöst durch ein Verfahren zur Herstellung einer implantierbaren Kiefergelenksteilprothese nach Anspruch 14.
Bei einer bevorzugten Weiterbildung wird zwischen den Verbindungsstücken ein Verbindungselement angeordnet wird, so dass die Verbindungsstücke der Komponenten fest aneinander gehalten sind und zwischen den Verbindungsstücken eine annährend lastfreie Fixierung gelingt.
Bei einer bevorzugten Weiterbildung kann das Verbindungselement durch einen Sinterprozess zwischen den Verbindungsstücken angeordnet werden, wobei das Verbindungselement beispielsweise in Art eines Stiftes zwischen den Verbindungsstücken angeordnete Verbindungselement bei dem Sinterprozess in die zusammengefügten Verbindungsstücke eingebacken wird.
Bei einer bevorzugten Weiterbildung kann das Verbindungselement durch Verdichten eines Kunststoffpulvers hergestellt werden. Hierbei kann das Verbindungselement z.B. in Pulverform in eine die Verbindungs stückle gemeinsam durchgreifende Bohrung eingebracht werden, und in der Form, in der die Verbindungsstücke eingelegt sind, vor einem Einbacken über den erwähnten Sinterprozess zunächst verdichtet werden.
Schließlich wird die Aufgabe auch gelöst durch eine implantierbare Kiefergelenksprothese nach Anspruch 18.
Die obigen Ausgestaltungen und Weiterbildungen lassen sich, sofern sinnvoll, beliebig miteinander kombinieren. Weitere mögliche Ausgestaltungen, Weiterbildungen und Implementierungen der Erfindung umfassen auch nicht explizit genannte Kombinationen von zuvor oder im Folgenden bezüglich der Ausführungsbeispiele beschriebenen Merkmalen der Erfindung.
Die Erfindung wird nachstehend anhand von Ausführungsbeispielen in den Figuren der Zeichnung näher erläutert. In teilweise schematisierter Darstellung zeigen hierbei:

In sämtlichen Figuren sind gleiche bzw. funktionsgleiche Elemente und Vorrichtungen - sofern nichts anderes angegeben ist - mit denselben Bezugszeichen versehen worden.
Fig. 1 zeigt eine perspektivische explodierte Seitenansicht einer ersten erfindungsgemäßen Ausführungsform der im Ganzen mit 90 bezeichneten Kiefergelenksteilprothese mit einem ersten Implantatteil 10 mit einer ersten Komponente 20 und einer zweiten Komponente 30 sowie einem zwischen den Komponenten 20, 30 angeordneten Verbindungselement 70.
In der Darstellung der Fig.l ist die erste Komponente 20 schaftartig aus einem Titanmaterial ausgebildet und weist an ihrem freien Ende ein angeformtes erstes Verbindungsstück 22 auf, welches mit seinem Vorsprung 24 in der schematisierten Darstellung der Fig. 1 einen viereckigen Querschnitt hat. Die erste Komponente 20 wird mit ihrem anderen Ende mit der nicht gezeigten Mandibula verbunden. Dem ersten Verbindungsstück 20 gegenüberliegend ist eine zweite Komponente 30 mit einem zweiten Verbindungsstück 32 mit kugelförmiger Gestalt
gezeigt, die eine der ersten Komponente zugewandte Ausnehmung 34 aufweist, deren Querschnitt demjenigen des Vorsprungs 24 entspricht. Die zweite Komponente 30 ist dabei ein keramischer Kondylenkopf, bildet also einen Gelenkkopf 42. Durch den Vorsprung 24 der ersten Komponente 20 und die Ausnehmung 34 der zweiten Komponente 30 wird das erste Implantatteil 10 durch formschlüssiges Zusammenfügen der Komponenten 20, 30 lösbar miteinander verbunden.
Um die beiden Komponenten 20, 30 fest aneinander zu halten, sind diese durch ein zwischen den jeweiligen Verbindungsstücken 22, 32 angeordnetes Verbindungselement 70 aneinander festlegbar. Hierfür sind an den Verbindungsstücken 22, 32 der ersten Komponente 20 und der zweiten Komponente 30 miteinander fluchtende Durchgriffe 26, 36 angeordnet, in welche das stiftartige Verbindungselement 70 aus einem Polyethylen-Kunststoff im zusammengefügten Zustand der Komponenten 20, 30 eingeführt ist. Die Durchgriffe 26, 36 und das in diese eingeführte Verbindungselement 70 sind dabei im Wesentlichen quer zu der Erstreckungsrichtung des Gelenkkopfendes des Implantatteils 10 angeordnet. Außerdem zeigt die Fig.l, dass die beiden Komponenten 20, 30 an einander zugewandten Seiten jeweils mit einem umlaufenden Stützbereich 28, 38 versehen sind, an denen sie sich gegenseitig abstützen.
Die Fig. 2 zeigt eine perspektivische Seitenansicht der Ausführungsform gemäß Fig. 1 mit zusammengesetzter erster Komponente 20 und zweiter Komponente 30. Die Pfeildarstellungen der Fig.2 stellen hierbei die auf das erste Implantatteil wirkenden Kräfte dar. Die hierbei einander aufhebende Kräfte darstellenden Pfeile (A) zeigen, dass die durch das Verbindungselement 70 die beiden Komponenten 20, 30 an ihren Verbindungsstücken 22, 32 annähernd lastfrei fixiert sind, während bei Beaufschlagung mit einer Kraft durch den Formschluss der beiden Verbindungsstücke in Richtung des Pfeils (C) die hauptsächliche Aufnahme stattfinden und überdies die beiden Komponenten gegen Verdrehung gegeneinander gesichert sind (Pfeile (B)).
Die Fig. 3 zeigt eine weitere perspektivische Seitenansicht der Ausführungsform gemäß Fig. 1 und 2 mit zusammengesetzten Komponenten 20, 30. Der zwischen der ersten Komponente 20 und der zweiten Komponente 30 angeordnete Polyethylenstift als Verbindungselement 70 wurde dabei, nachdem die Komponenten 20, 30 zusammengefügt und in eine Sinterform (nicht dargestellt) eingelegt wurden, zunächst als Pulver vorliegend in die Durchgriffe 26, 36 der Komponenten 20, 30 eingebracht und verdichtet, um anschließend in einem Sinterprozess zwischen die Verbindungsstücke 22, 32 der Komponenten unter Beaufschlagung mit Temperatur und Druck eingebacken zu werden.
Die Fig. 4 zeigt eine explodierte perspektivische Seitenansicht gemäß einer zweiten Ausführungsform, der Kiefergelenksteilprothese 90‘ mit einem ersten Implantatteil 10‘ mit erster und zweiter Komponente 20‘, 30‘ sowie dem Verbindungselement 70‘. Die Darstellung
entspricht dabei Fig. 1, so dass auf die diesbezüglichen Ausführungen verwiesen wird. Gleiches gilt für die Fig. 5, in welcher eine perspektivische Seitenansicht der Ausführungsform aus der Fig. 4 mit zusammengesetzter erster Komponente 20‘ und zweiter Komponente 30‘ mit zwischen den Verbindungsstücken 22‘, 32‘ angeordnetem Verbindungselement 70‘ gezeigt ist, die der Darstellung in der Fig. 2 entspricht.
Der Unterschied der Darstellungen gemäß Fig. 4 und 5 zu den Darstellungen gemäß Fig. 1 und 2 besteht darin, dass es bei ersteren um die Darstellung eines patientenspezifischen ersten Implantatteils 10‘ handelt, diese also an die bei Patienten vorliegenden Gegebenheiten angepasst ist. Dies zeigt sich zum einen in der Gestaltung der zweiten Komponente 30 ‘ mit zweiten Verbindungsstück 32 ‘ als spezifischer Kondylenkopf mit entsprechender Formgebung. Zum anderen ist auch die Gestalt der ersten Komponente 20‘ mit dem ersten Verbindungsstück 22‘ an die (nicht gezeigte) Mandibula des Patienten angepasst, um dort problemfrei angeordnet werden zu können. Weitere spezifische Anpassungen bestehen darin, dass die Durchgriffe 26‘, 36‘ sowie entsprechend das Verbindungselement 70‘ kürzer ausgebildet sind, der Vorsprung 24 ‘ und die Ausnehmung 34‘ abgerundete Eckbereiche aufweisen und die Stützbereiche 28‘, 38‘ schmaler ausgebildet sind.
Die Fig. 6 zeigt eine perspektivische Seitenansicht der Ausführungsform aus den Fig. 4 und 5 im implantierten Zustand mit einer natürlichen Gelenkfläche als Widerlager. Dabei erkennt man den patientenspezifischen Kondylenkopf der zweiten Komponente 30‘ des ersten Implantatteils 10‘ ebenso wie dessen erste Komponente 20‘, die aus Gründen der Übersichtlichkeit ohne die Mandibula, an welcher sie angeordnet ist, gezeigt ist. Mittels der erfindungsgemäßen Kiefergelenksteilprothese 90 ‘ mit dem ersten Implantatteil kann an dem in Fig. 6 gezeigten Cranium 50 an dessen natürlicher Gelenkfläche 52 eine über den durch die zweite Komponente 30 ‘ gebildeten Kondylenkopf, dessen Bewegung durch die erste Komponente 20 ‘ abgestützt ist, eine sichere und definierte Abrollbewegung des Unterkiefers stattfinden.
Die Fig. 7 zeigt eine perspektivische Seitenansicht einer die Kiefergelenksteilprothese 90 umfassenden Kiefergelenksprothese 100 mit einem künstlichen zweiten Implantatteil 80, das mit einem ersten Implantatteil 10 einer vierten Ausführungsform mit verbundenen Komponenten 20, 30 im implantierten Zustand zusammenwirkt. Das erste Implantatteil 10‘ ist dabei an einem Unterkiefer 40 angeordnet, und an diesem mittels eines Befestigungsmittels 44, hier eine Lochanordnung mit den die zugehörigen Löcher durchgreifenden Stiften, festgelegt. Das freie, dem Cranium 50 zugeordnete Ende des ersten Implantatteils bildet einen Gelenkkopf 42‘, dessen Widerlager durch eine künstliche Gelenkfläche 82 gebildet ist. Diese ist Teil eines an dem Jochbein des Craniums 50 angeordneten zweiten Implantatteils 80.
Entsprechend werden auch mit der implantierbaren Kiefergelenksprothese 100, die die
Kiefergelenksteilprothese 90 aufweist, für die individuell vorliegende, das Widerlager bildende
Gelenkfläche 82, eine zuverlässige Gleitpaarung dadurch bereitgestellt, dass an dem ersten Implantatteil 10 mehrere Komponenten 20, 30 vorgesehen sind, die mit ihren Verbindungsstücken 22, 32 ineinandergreifen und mit diesen einen Formschluss bilden, wobei sie lösbar miteinander verbindbar sind.
Die Fig. 8 zeigt eine explodierte perspektivische Seitenansicht gemäß einer dritten Ausführungsform der Kiefergelenksteilprothese 90“ mit einem ersten Implantatteil 10“ mit erster und zweiter Komponente 20“, 30“ sowie dem Verbindungselement 70“, und die die Fig.
9 zeigt eine perspektivische Seitenansicht der Ausführungsform aus der Fig. 8 mit zusammengesetzter erster Komponente 20“ und zweiter Komponente 30“ mit zwischen den Verbindungsstücken 22“, 32“ angeordnetem Verbindungselement 70“ gezeigt ist. Obwohl dabei die dritte Ausführungsform eine größere Quererstreckung des Kondylenkopfs 42“ aufweist, kann doch hinsichtlich der Funktionalität der Elemente auch auf die obigen Ausführungen zu den Fig. 1, 2 4 und 5 verwiesen werden.
Mit Blick auf die Fig. 8 und 9 unterscheidet sich die dortige Ausführungsform der Kiefergelenksteilprothese 90“ mit einem ersten Implantatteil 10“ mit erster und zweiter Komponente 20“, 30“ sowie dem Verbindungselement 70“ von der in den Fig. 4 und 5 dargestellten zweiten Ausführungsform hinsichtlich der Ausgestaltung der ersten und zweiten Komponente 20“, 30“ sowie deren Festlegung aneinander. Die zweite Komponente 30“ ist dabei mit einem haubenartig bzw. schalenartig ausgebildeten zweiten Verbindungsstück versehen, das die Form eine halben Hohllinse mit der Ausnehmung 34“ aufweist, an deren Rand in etwa halbkreisförmig zwei einander gegenüberliegende Ansätze mit Durchgriffen 36“ im Richtung der ersten Komponente 20“ vorspringen.
Diese erste Komponente 20“ ist mit dem ersten Verbindungsstück 22“ versehen, das wiederum in Richtung der zweiten Komponente ragt, um bei Verbinden der beiden Komponenten 20“, 30“ mit seinem Vorsprung 24“ in die Ausnehmung 34“ zu greifen. Entsprechend den Durchgriffen 36“ der zweiten Komponente 30“, ist an der ersten Komponente der Durchgriff 26“ angeordnet. Durch die Durchgriffe 26“, 36“ greift das Verbindungselement 70“ dabei in einer zu der zweiten Ausführungsform im Wesentlichen senkrechten Richtung, so dass die Längsachse des Verbindungselements 70“ in den Fig. 8 und 9 im Wesentlichen in der Bewegungsebene der Kiefergelenksteilprothese 90“ liegt. Da die Durchgriffe 36“ einen Teil des Randes des Verbindungsstücks 32“ bilden, sind sie auch Teil des Stützbereichs 38“, der gegenüber einem entsprechend ausgebildeten Stützbereich 28“ der ersten Komponente 20“ zu liegen kommt.
Die Fig. 10a- lOe zeigt eine Seitenansicht von verschiedenen Schritten eines Fertigungsprozessen für ein erfindungsgemäßen Verfahrens zur Herstellung einer implantierbaren Kiefergelenksteilprothese 90, und zwar in zeitlicher Abfolge von Fig.10a nach Fig.lOe. Hierbei ist in der Fig.10a zunächst ein Unterteil 61 eines eine Sinterform bildenden Werkzeugs 60
gezeigt, das eine Ausnehmung 62 aufweist, in das ein erstes Implantatteil 10 eingelegt werden kann. Außerdem erkennt man von der Stirnfläche des Unterteils 61 vorspringende Stifte 66, die der korrekten Positionierung eines Mittelteils 63 (in den Fig. 10c- lOe gezeigt) des Werkzeugs 60 dienen.
Das in das Unterteil 61 des Werkzeugs 60 eingelegte erste Implantatteil 10 mit seiner durch einen Titangrundkörper gebildeten ersten Komponente 20 und seiner aus einem Kondylenkopf aus Zirkon gebildeten zweiten Komponente 30 ist in Fig. 10b gezeigt. Um das erste Implantatteil in dem Werkzeug sicher zu fixieren, wird an diesem ein Mittelteil 63 angeordnet, dessen Ausnehmung 69 die Bereiche des ersten Implantatteils 10 aufnimmt, die in der Fig. 10b über das Unterteil 61 heraus ragen. Die Position des Mittelteils ist, wie bereits erwähnt, durch die Stifte 64 des Unterteils 61, die in entsprechende Bohrungen 68 des Mittelteils 63 eingreifen, bestimmt. Hierdurch ist das erste Implantatteil 10 in seinem kondylenkopf seitigen Endbereich durch das Werkzeug 60 im Wesentlichen allseitig umschlossen.
Man erkennt jedoch in den Fig. 10c und lOd weiter, dass das Mittelteil 63 eine Bohrung 66 aufweist, die mit den Durchgriffen 26, 36 der ersten und zweiten Komponente 20, 30 des ersten Implantatteils 10 fluchtet. In der Fig. lOd ist hierzu gezeigt, dass durch diese Bohrung 66 eine dritte Komponente in Form eines Polyethylenpulvers 72 zwischen die erste und die zweite Komponente 20, 30 eingebracht wird und den durch die Durchgriffe 26, 36 gebildeten Raum ausfüllt.
Nach Einbringen des Polyethylenpulvers wird das Werkzeug durch Aufsetzen eines deckelartigen Oberteils 65 auf das Mittelteil 63 verschlossen, das Polyethylenpulver 72 anschließend verdichtet und dann ein Sinterprozess gestartet, durch den das Polyethylen als Verbindungselement 70 zwischen den beiden Verbindungsstücken 22, 32 des Gelenkkopfs 42, also der ersten Komponente 20 und der zweiten Komponenten 30 des ersten Implantatteils 10 dadurch angebracht wird, dass es zwischen den Komponenten 20, 30 quasi „eingebacken“ wird.
Obwohl die vorliegende Erfindung anhand bevorzugter Ausführungsbeispiele vorstehend beschrieben wurde, ist sie darauf nicht beschränkt, sondern auf vielfältige Art und Weise modifizierbar. Insbesondere lässt sich die Erfindung in mannigfaltiger Weise verändern oder modifizieren, ohne vom Kem der Erfindung abzuweichen.
Claims
1. Implantierbare Kiefergelenksteilprothese (90, 90’, 90”) mit: einem an einem Unterkiefer (40) befestigbaren ersten Implantatteil (10, 10’, 10”) mit einem Gelenkkopf (42, 42’, 42”), zu welchem eine an einem Cranium (50) angeordnete Gelenkfläche (52, 82) ein Widerlager bildet; wobei das erste Implantatteil (10, 10’, 10”) eine aus einem metallischen Werkstoff gebildete erste Komponente (20, 20’, 20”) aufweist, welche an dem Unterkiefer (40) befestigbar ist, und eine aus einer Keramik oder einem Kunststoff gebildete zweite Komponente (30, 30’, 30”) aufweist, welche den Gelenkkopf (42, 42’, 42”) aufweist; wobei die erste Komponente (20, 20’, 20”) mit einem ersten Verbindungsstück (22, 22’, 22”) versehen ist und die zweite Komponente (30, 30’, 30”) mit einem zweiten Verbindungsstück (32, 32’, 32”) versehen ist, und das erste Verbindungsstück (22, 22’, 22”) und das zweite Verbindungsstück (32, 32’, 32”) ineinandergreifend zumindest bereichsweise formschlüssig miteinander verbindbar oder verbunden sind.
2. Kiefergelenksteilprothese (90, 90’, 90”) nach Anspruch 1, wobei eines der Verbindungsstücke (22, 22’, 22”; 32, 32’, 32”) mit wenigstens einem Vorsprung (24, 24’, 24”) ausgebildet ist, welcher in wenigstens eine komplementäre Ausnehmung (34, 34’, 34”) an dem anderen Verbindungsstück (32, 32’, 32”, 22, 22’, 22”) greift.
3. Kiefergelenksteilprothese (90, 90’, 90”) nach Anspruch 2, wobei der wenigstens eine Vorsprung (24, 24’, 24”) des einen Verbindungsstücks (22, 22’, 22”; 32, 32’, 32”) und die wenigstens eine komplementäre Ausnehmung (34, 34’, 34”) des anderen Verbindungsstückes (32, 32’, 32”, 22, 22’, 22”) einen allseitigen Formschluss bilden.
4. Kiefergelenksteilprothese (90, 90’, 90”) nach einem der Ansprüche 1 bis 3, wobei die erste Komponente (20, 20’, 20”) und die zweite Komponente (30, 30’, 30”) durch wenigstens ein zwischen den jeweiligen Verbindungsstücken (22, 22’, 22”, 32, 32’, 32”) angeordnetes Verbindungselement (70, 70’, 70”) aneinander festgelegt sind.
5. Kiefergelenksteilprothese (90, 90’, 90”) nach Anspruch 4, wobei das wenigstens eine Verbindungselement (70, 70’, 70”) die erste Komponente (20, 20’, 20”) und die zweite Komponente (30, 30’, 30”) zumindest in einem Abschnitt (29, 39) des jeweils zugeordneten Verbindung Stücks (22, 22’, 22”; 32, 32’, 32”) durchgreift.
6. Kiefergelenksteilprothese (90, 90’, 90”) nach Anspruch 4 oder 5, wobei an den Verbindungsstücken (22, 22’, 22”; 32, 32’, 32”) der ersten Komponente (20, 20’, 20”) und der zweiten Komponente (30, 30’, 30”) miteinander fluchtende Durchgriffe (26, 36) angeordnet sind, in welche das wenigstens eine Verbindungselement (70, 70’, 70”) einführbar ist.
7. Kiefergelenksteilprothese (90, 90’, 90”) nach Anspruch 6, wobei die Durchgriffe (26, 36) und das in diese eingeführte wenigstens eine Verbindungselement (70, 70’, 70”) sich im Wesentlichen quer zu der Erstreckungsrichtung des Gelenkkopfendes des Implantatteils (10, 10’, 10”) angeordnet sind.
8. Kiefergelenksteilprothese (90, 90’, 90”) nach einem der vorhergehenden Ansprüche, wobei an dem ersten und dem zweiten Verbindungsstück (22, 22’, 22”; 32, 32’, 32”) jeweils ein dem anderen Verbindungsstück (32, 32’, 32”, 22, 22’, 22”) gegenüberliegend angeordneter Stützbereich (28, 28’, 38, 38’) vorgesehen ist.
9. Kiefergelenksteilprothese (90. 90‘, 90“) nach einem der vorhergehenden Ansprüche, wobei das erste Verbindungsstück (22, 22’, 22”) und das zweite Verbindungsstück (32, 32’, 32”) lösbar miteinander verbindbar oder verbunden sind.
10. Kiefergelenksteilprothese (90, 90’, 90”) nach einem der vorhergehenden Ansprüche, wobei die erste Komponente (20, 20’, 20”) mit dem ersten Verbindungsstück (22, 22’, 22”) aus einem Titanmaterial ausgebildet ist.
11. Kiefergelenksteilprothese (90, 90’ , 90”) nach einem der vorhergehenden Ansprüche, wobei die zweite Komponente (30, 30’, 30”) mit dem zweiten Verbindungsstück (32, 32’, 32”) aus einer Keramik oder einem thermoplastischen Kunststoff ausgebildet ist.
12. Kiefergelenksteilprothese (90, 90’, 90”) nach Anspruch 11, wobei der thermoplastische Kunststoff aus einem Polyolefin oder einem Polyaryletherketon gebildet ist.
13. Kiefergelenksteilprothese (90, 90’, 90”) nach einem der Ansprüche 4 bis 7, wobei das Verbindungselement (70, 70’, 70”) aus Polyethylen ausgebildet ist.
14. Verfahren zur Herstellung einer implantierbaren Kiefergelenksteilprothese (90, 90‘, 90“) mit: einem an einem Unterkiefer (40) befestigbaren ersten Implantatteil (10, 10’, 10”) mit einem Gelenkkopf (42, 42’, 42”), zu welchem eine an einem Cranium (50) angeordnete Gelenkfläche (52) ein Widerlager bildet; wobei das erste Implantatteil (10, 10’, 10”) eine aus einem metallischen Werkstoff gebildete erste Komponente (20, 20’, 20”) aufweist, welche an dem Unterkiefer (40) befestigbar
ist, und eine aus einem Kunststoff gebildete zweite Komponente (30, 30’, 30”) aufweist, welche den Gelenkkopf (42, 42’, 42”) ausbildet; wobei das Verfahren zumindest folgende Schritte aufweist: zur Verfügung stellen der ersten Komponente (20, 20’, 20”) mit einem ersten Verbindungsstück (22, 22’, 22”); zur Verfügung stellen einer zweiten Komponente (30, 30’, 30”) mit einem zweiten Verbindungsstück (32, 32’, 32”); formschlüssiges Zusammenfügen der Verbindungsstücke (22, 22’, 22”; 32, 32’, 32”) unter Beaufschlagen mit Temperatur und Vakuum.
15. Verfahren nach Anspruch 14, wobei zwischen den Verbindungsstücken (22, 22’, 22”, 32, 32’, 32”) ein Verbindungselement (70, 70’, 70”) angebracht wird.
16. Verfahren nach Anspruch 15, wobei das Verbindungselement (70, 70’, 70”) durch einen Sinterprozess zwischen den Verbindungsstücken (22, 22’, 22”; 32, 32’, 32”) angeordnet wird.
17. Verfahren nach Anspruch 15 oder 16, wobei das Verbindungselement (70, 70’, 70”) durch Verdichten eines Kunststoffpulvers hergestellt wird.
18. Implantierbare Kiefergelenksprothese (100), umfassend die Kiefergelenksteilprothese (90, 90’, 90”) mit dem ersten Implantatteil (10, 10’, 10”) nach einem Ansprüche 1 bis 12 und/oder hergestellt nach einem Verfahren gemäß der Ansprüche 13 bis 16 , welche ein zweites Implantatteil (80), welches eine an dem Cranium (50) anbringbare Gelenkfläche (52) aufweist, die das Wiederlager zu dem Gelenkkopf (42, 42’, 42”) bildet.
