Inkubator für früh und/oder neugeborene Patienten
Die vorliegende Erfindung betrifft einen Inkubator für früh- und/oder neugeborene Patienten.
Stand der Technik
Inkubatoren für früh/und neugeborene Kinder sind insbesondere als Bestandteil von Neugeborenenintensivstationen in Krankenhäusern bekannt. Inkubatoren sind typischerweise geschlossene und beheizbare Systeme, die wenigstens einen definierten Sauerstoffeintrag in das System aufweisen. Inkubatoren können zahlreiche, den Patienten überwachende Messsysteme aufweisen, die beispielsweise
verschiedene physiologische Parameter des Patienten kontrollieren.
Sensoren zur Bestimmung eines Sauerstoffgehalts in der Luft im Innern eines
Inkubators sind beispielsweise aus der DE 103 18 384 A1 bekannt. Die Druckschrifft offenbart einen von einer zentralen Gasversorgungsanlage und von Sauerstoff- Druckgasflaschen unabhängig zu betreibenden Inkubator. Der Inkubator weist einen elektrisch betriebenen Sauerstoffkonzentrator und einen Sauerstoffsensor für die Messung der Sauerstoffkonzentration im Inneren des Inkubators auf. Der
Sauerstoffkonzentrator ist hierbei als Druckwechselabsorber auf Zeolithbasis ausgebildet und der Sauerstoffsensor als elektrochemische Messzelle. Ein
Sauerstoffanteil in der dem Inkubator zugeführten Frischluft wird über einen
einstellbaren Sauerstoffdosierer geregelt.
Die Bestimmung von physiologischen Paramtern ist oftmals nicht ausreichend, um eine frühzeitige und umfassende Beurteilung des Zustands des Patienten zu ermöglichen.
Offenbarung der Erfindung
Erfindungsgemäß wird ein Inkubator mit den Merkmalen des unabhängigen
Patentanspruchs 1 vorgeschlagen. Vorteilhafte Ausgestaltungen sind Gegenstand der Unteransprüche sowie der nachfolgenden Beschreibung.
Die Erfindung schlägt einen Inkubator für früh- und/oder neugeborene Patienten vor, der wenigstens einen Sensor zur Erfassung von wenigstens einem Biomarker in dem Atem eines Patienten und/oder einem Inkubatorinnenvolumen aufweist.
Unter Biomarkern seien in diesem Zusammenhang insbesondere messbare Produkte von Organismen oder von einem Organismus abgegebene Stoffe verstanden, welche in dem Atem eines Patienten insbesondere in volatiler oder gasförmiger Form vorliegen können.
Die Erfassung von Biomarkern in dem Atem des Patienten und/oder dem
Inkubatorinnenvolumen ermöglicht eine zeitnahe und effektive Bestimmung von Krankheits- bzw. Zustandsparametern des Patienten. Hierdurch kann eine frühzeitige Erkennung von krankheitsbedingten Zustandsänderungen des Patienten gewährleistet und rechtzeitig präventive oder therapeutische Maßnahmen eingeleitet und
möglicherweise lebensbedrohliche und/oder mit hohen Zusatzkosten verbundene Komplikationen vermieden werden.
Vorteilhafterweise ist der wenigstens eine Sensor in oder an einer Gesichtsmaske, einer Nasenkanüle und/oder einem Vakuumabluftsystem ausgebildet. Somit können die Biomarker direkt aus der Atemluft des Patienten erfasst werden. Die
Gesichtsmaske oder Nasenkanüle weisen einen unmittelbaren Kontakt zu der Atemluft bzw. Gesicht des Patienten auf und zeichnen sich durch eine einfache Gestaltung und Austauschmöglichkeit sowie gute Bedienbarkeit aus. Das Vakuumabluftssytem ist beispielsweise als ein Absaugesystem oberhalb der Nase und/oder Mund des
Patienten ausgebildet und entnimmt somit kontinuierlich ein Probenvolumen direkt aus der Atemluft des Patienten. Diese Ausgestaltung bietet für den Patienten einen hohen Komfort, da das Vakuumabluftsystem keine unmittelbare Verbindung mit dem
Patienten aufweist.
Darüber hinaus kann der wenigstens eine Sensor auf der Innenseite einer Außenhülle des Inkubators, insbesondere an einer Austauschstelle zwischen Frischluft und Abluft
ausgebildet sein. Ein inkubatorinnenseitig angebrachter Sensor weist eine einfache Montage auf und ermöglicht die Messung von Biomarkern im gesamten Innern des Inkubators. Die bevorzugte Anbringung des Sensors an der Austauschstelle zwischen Frischluft und Abluft ermöglicht eine genaue und zeitlich exakt aufgelöste Erfassung von Biomarkern insbesondere bei Luftaustauschgeschwindigkeiten, die keine homogene Verteilung der Gase bzw. der volatilen Komponenten über den gesamten Inkubatorinnenraum erlauben.
Vorteilhafterweise umfasst der wenigstens eine Sensor einen chemischen Sensor und/oder einen Biosensor. Hierbei ist der chemische Sensor als Sensor zur Detektion gasförmiger Substanzen ausgeprägt und kann neben Biomolekülen auch Gase mit kleinerer Molmasse detektieren, die auch auf einen möglichen pathogenen Zustand des Patienten schließen lassen.
Bevorzugt umfasst der Inkubator wenigstens einen Sensor zur Erfassung von
Entzündungsmarkern, oxidativen-Stress-Markern, Nekrosemarkern,
Zuckerspiegelmarkern, Immunologiemarkern, Apoptosemarkern und/oder eines Gases, insbesondere NO, CO und/oder H2S. Die Erfassung insbesondere dieser Biomarker gibt Aufschluss über den Zustand von früh- und/oder neugeborenen Patienten.
Gemäß einer besonders vorteilhaften Ausgestaltung der Erfindung umfasst der Inkubator wenigstens einen weiteren Sensor zur Erfassung wenigstens eines physiologischen Parameters. Insbesondere transkutane Partialdrücke von 02 und C02, eine Blutgasanalyse, ein Säure/Base Wert, eine Körpertemperatur, ein Blutdruck, eine Atemfrequenz und/oder eine Herzschlagfrequenz werden hierbei von dem Sensor als Messsignal erfasst. Die Erfassung von physiologischen Parametern erlaubt einen Vergleich bzw. Einordnung der durch den wenigstens einen weiteren Sensor bestimmten Gasphasenwerte und dient somit einer überaus genauen Überwachung des Gesundheitszustands des Patienten.
Zweckmäßigerweise weist der Inkubator eine Auswerteeinheit zur Erzeugung und Auswertung von Messdaten auf der Grundlage der von dem wenigstens einen Sensor erfassten Biomarker auf.
Vorteilhafterweise erfolgt eine Aufzeichnung, Zuordnung und/oder Auswertung der von dem wenigstens einen Sensor erhaltenen Messwerte eines jeweiligen Patienten, wobei die durch die Auswerteeinheit erzeugten Messdaten mit Daten einer Datenbank verglichen und/oder einer fachlich kompetenten Person zur Verfügung gestellt werden. Somit lässt sich auf Grund der sensorbasierten Messsdatenerfassung und
Verarbeitung der Daten in einer Auswerteeinheit direkt eine mögliche Therapie des Patienten durch eine fachlich kompetente Person, beispielsweise einen Arzt oder medizinisch geschultes Personal, einleiten.
Weitere Vorteile und Ausgestaltungen der Erfindung ergeben sich aus der
Beschreibung und der beiliegenden Zeichnung.
Es versteht sich, dass die vorstehend genannten und die nachfolgend noch zu erläuternden Merkmale nicht nur in der jeweils angegebenen Kombination, sondern auch in anderen Kombinationen oder in Alleinstellung verwendbar sind, ohne den Rahmen der vorliegenden Erfindung zu verlassen.
Die Erfindung ist anhand zweier Ausführungsbeispiele in der Zeichnung schematisch dargestellt und wird im Folgenden unter Bezugnahme auf die Zeichnung ausführlich beschrieben.

Figur 1 zeigt schematisch eine bevorzugte Ausgestaltung eines
erfindungsgemäßen Inkubators in einer Seitansicht.
Figur 2 zeigt schematisch eine weitere bevorzugte Ausgestaltung eines
erfindungsgemäßen Inkubators in einer Seitansicht.
Figur 1 zeigt einen erfindungsgemäßen Inkubator in einer bevorzugten Ausgestaltung in einer Seitenansicht. Der früh- und/oder neugeborene Patient 4 befindet sich auf einer Liege 2 im Innern des Inkubators, der durch eine Außenhülle 1 von der äußeren Umgebung getrennt ist. Die Sauerstoffversorgung des Patienten wird durch das Abführen eines Abluftstroms und das Zuführen eines Zuluftstroms durch ein
Luftaustauschsystem 3 gewährleistet. Der früh- und/oder neugeborene Patient weist eine direkte Verbindung zu einem Sensor 20 zur Erfassung von physiologischen Parametern des Patienten, insbesondere transkutaner Partialdrücke von 02 und C02, einer Blutgasanalyse, eines Säure/Base Werts, einer Körpertemperatur, eines
Blutdrucks, einer Atemfrequenz und/oder einer Herzschlagsfrequenz, auf. Ein Sensor 10 zur Erfassung von Biomarkern, insbesondere Entzündungsmarkern, oxidative- Stress- Markern, Nekrosemarkern, Zuckerspiegelmarkern, Immunologiemarkern und/oder -, Apoptosemarkern, ist außerhalb der Außenhülle des Inkubators 1 angebracht und über einen Schlauch 6 mit einer Gesichtsmaske oder Nasenkanüle 5 verbunden. Der Sensor 10 könnte auch im Innern des Inkubators, z.B. in einem
Vakuumabluftsystem 7 oder auch im Innern der Gesichtsmaske oder Nasenkanüle 5, angeordnet sein. Die Gesichtsmarke oder Nasenkanüle 5 ist im Mund- und/oder Nasenbereich des Patienten 4 angeordnet und steht in direktem Kontakt mit dem Atem des Patienten. Die Messwerte beider Sensoren 10 und 20 werden an eine
Auswerteeinheit 100 weitergeleitet, welche entsprechende Messdaten erzeugt und diese auswertet.
Figur 2 zeigt einen erfindungsgemäßen Inkubator in einer weiteren bevorzugten Ausgestaltung in einer Seitenansicht. Gleiche Teile sind mit gleichen Bezugszeichen gekennzeichnet. Im Gegensatz zu Figur 2 weist der Inkubator nun einen Sensor 10' zur Erfassung von Biomarkern auf der Innenseite der Außenhülle 1 insbesondere in Umgebung des Luftaustauschsystems, auf.
Die Messwerte der Sensoren 10' und 20 werden wiederum zur Aufzeichnung,
Zuordnung und/oder Auswertung an die Auswerteeinheit 100 weitergeleitet. Die von der Auswerteeinheit erzeugten Messdaten können mit Daten einer Datenbank verglichen werden und/oder einer fachlich kompetenten Person zur Verfügung gestellt werden. Die Auswerteeinheit kann durch den Datenabgleich eine mögliche
Therapiemöglichkeit des Patienten der fachlich kompetenten Person zur Verfügung stellen. Insbesondere ist es möglich einen Inkubator mit zwei oder mehreren Sensoren, z.B. den Sensoren 10 und 10', auszubilden.
Claims
1 . Inkubator für früh- und/oder neugeborene Patienten, gekennzeichnet durch
wenigstens einen Sensor zur Erfassung von wenigstens einem Biomarker in dem Atem eines Patienten und/oder einem Inkubatorinnenvolumen.
2. Inkubator nach Anspruch 1 , dadurch gekennzeichnet, dass der wenigstens eine Sensor (10, 10') in oder an einer Gesichtsmaske oder Nasenkanüle (5) und/oder einem Vakuumabluftsystem (7) ausgebildet ist.
3. Inkubator nach Anspruch 1 , dadurch gekennzeichnet, dass der wenigstens eine Sensor (10, 10') auf der Innenseite einer Außenhülle (1 ) des Inkubators, insbesondere an einer Austauschstelle zwischen Frischluft und Abluft (3) ausgebildet ist.
4. Inkubator nach einem der Ansprüche 1 bis 3, dadurch gekennzeichnet, dass der wenigstens eine Sensor (10, 10') einen chemischen Sensor und/oder einen Biosensor umfasst.
5. Inkubator nach einem der vorstehenden Ansprüche, dadurch gekennzeichnet, dass der Inkubator wenigstens einen Sensor (10, 10') zur Erfassung von
Entzündungsmarkern, oxidativen-Stress-Markern, Nekrosemarkern,
Zuckerspiegelmarkern, Immunologiemarkern und/oder -, Apoptosemarkern und/oder eines Gases, insbesondere NO, CO und/oder H2S, aufweist.

Inkubator nach einem der vorstehenden Ansprüche, dadurch gekennzeichnet, dass der Inkubator wenigstens einen weiteren Sensor (20) zur Erfassung wenigstens eines physiologischen Parameters, insbesondere transkutaner Partialdrücke von 02 und C02, einer Blutgasanalyse, eines Säure/Base Werts, einer Körpertemperatur, eines Blutdrucks, einer Atemfrequenz und/oder einer Herzschlagfrequenz, aufweist.
7. Inkubator nach einem der vorstehenden Ansprüche, dadurch gekennzeichnet, dass der Inkubator eine Auswerteeinheit (100) zur Erzeugung und Auswertung von Messdaten auf der Grundlage der von dem wenigstens einen Sensor (10, 10")
erfassten Biomarker, weiterer gasförmiger Komponenten und/oder
physiologischen Parametern aufweist.
8. Verfahren zur Auswertung von Messdaten wenigstens eines Sensors (10) eines Inkubators insbesondere nach einem der Ansprüche 1 bis 7, wobei eine
Aufzeichnung, Zuordnung und/oder Auswertung der von dem wenigstens einen Sensor erfassten Biomarker eines jeweiligen Patienten erfolgt, wobei auf Grundlage der erfassten Biomarker erzeugten Messdaten mit Daten einer Datenbank verglichen und/oder einer fachlich kompetenten Person zur Verfügung gestellt werden.
