Abstract

L'invention concerne une méthode d'analyse in vitro permettant de prédire la résistance à un traitement à 1 ' azacitidine chez un patient, en utilisant la protéine BCL2L10 contenue dans un échantillon de fluide biologique prélevé sur ledit patient ainsi que des molécules biologiques fixant spécifiquement la protéine BCL2L10. Elle se caractérise en ce qu'on récupère un échantillon de fluide biologique chez un patient; on calcule le pourcentage de cellules totales dudit fluide biologique exprimant la protéine BCL2L10; on compare ledit pourcentage calculé à une valeur seuil de référence, cette valeur seuil étant comprise entre 20 et 60%; et on diagnostique la résistance à un traitement à 1' azacitidine chez un patient ayant un pourcentage de cellules exprimant la protéine BCL2L10 dans ledit fluide biologique supérieur à ladite valeur de référence.

Description

TEST DIAGNOSTIC DE LA RESISTANCE A L'AZACITIDINE
La présente invention a pour objet une méthode d'analyse permettant le diagnostic in vitro de la résistance à un traitement à l' azacitidine chez un patient. L'invention concerne également un kit d'analyse in vitro permettant de prédire la résistance d'un patient à un traitement à l' azacitidine, ainsi que l'utilisation d'un tel kit.
L' azacitidine, dont la formule est reprise ci- dessous :
est à ce jour le seul traitement autorisé pour les patients atteints de syndromes myélodysplasiques (SMD ou MDS en anglais pour MyeloDysplastic Syndrom) et de leucémies myéloïdes aiguës (LMA) , non éligibles pour l'allogreffe de cellules souches hématopoïétiques . L' azacitidine est en outre commercialisée dans le cadre du traitement de ces maladies sous le nom Vidaza®.
L' azacitidine (AZA) est un agent hypométhylant produisant de 40% à 60% de réponse dans ces deux maladies .
Les syndromes myélodysplasiques (SMD) et les leucémies myéloïdes aiguës (LMA) font partis des hémopathies myéloïdes qui se développent à partir des cellules souches de la moelle osseuse, comportant des précurseurs de la lignée granuleuse, correspondant aux globules blancs, de la lignée érythroblastique correspondant aux globules rouges, de la lignées mégacaryocytaire correspondant aux plaquettes et de la lignée histio-monocytaire . Les SMD se caractérisent par d'importants désordres de maturation d'une ou des trois lignées médullaires, granuleuse, érythroblastique et mégacaryocytaire, responsables de cytopénie. Les SMD peuvent également évoluer vers une leucémie aiguë (LA) . Le diagnostic classique repose sur l'étude cytologique du sang et de la moelle, sur la cytogénétique et sur la biologie moléculaire. Les SMD comprennent différentes anémies ou cytopénies réfractaires ainsi que le syndrome 5q- .
Les LMA se caractérisent, quant à elles, par la prolifération rapide de précurseurs médullaires des trois lignées, granuleuse, érythroblastique et mégacaryocytaire aboutissant à l'accumulation dans le sang et la moelle de cellules immatures, détruisant l' hématopoïèse normale. Leur diagnostic repose sur les mêmes techniques que pour les SMD. Elles comprennent des LMA indifférenciées, peu différenciées, myéloblastiques , monoblastiques, yélomonoblastiques ainsi que des leucémies aiguës érythroblastiques et des leucémies aiguës mégacaryoblastiques . Les LMA primaires ou secondaires peuvent être responsables de tumeurs dans divers organes ou tissus (peau, ganglions, sein, tractus digestif, rate, etc.) réalisant des sarcomes myéloïdes, autrefois appelés chlorome ou sarcome granulocytaire . Ils peuvent révéler la leucémie aiguë et posent de difficiles problèmes diagnostiques avec les lymphomes malins .
Les patients atteints de SMD ou de LMA traités à 1 ' azacitidine sont soit résistants à l' azacitidine (« AZA-résistants ») , soit sensibles à 1 ' azacitidine (« AZA-sensibles ») . Toutefois, même les patients « AZA- sensibles » semblent systématiquement subir un phénomène de rechute après un laps de temps plus ou moins long. Autrement dit, même si 40% des patients traités à 1' azacitidine y sont d'emblée résistants et environ 60% des patients sont sensibles au traitement pendant les premiers mois, tous devraient, à court ou moyen terme, développer une résistance à ce traitement. Ce phénomène est classiquement appelé la rechute et désigne l'acquisition d'une résistance à un traitement après avoir préalablement été sensible à ce traitement.
Il existe à ce jour des systèmes de notation pronostique permettant de prédire et de pronostiquer la survie globale des patients traités avec des agents hypométhylants . Ces systèmes sont basés sur un score pronostique évalué dans des sous-groupes de patients. Il s'agit de groupes de risque définis par l'étude du caryotype et de certaines caractéristiques cliniques des patients. Cependant, les résultats associés à ces systèmes de notation sont des prédicteurs de réponse peu fiables .
La moitié des patients atteints de SMD présentent un caryotype normal, et les patients présentant des anomalies chromosomiques identiques sont souvent cliniquement hétérogènes. Les mutations somatiques ponctuelles sont courantes dans les SMD. Des mutations des gènes TP53, EZH2 , ETV6, RUNX1 et ASXL1 sont des prédicteurs d'une faible survie globale chez les patients atteints de SMD indépendamment des autres facteurs de risque établis.
Toutefois, les systèmes actuels ne donnent pas de résultats convenables, permettant de diagnostiquer la sensibilité d'un patient à l' azacitidine sans lui administrer ce traitement.
A l'heure, actuelle, la seule méthode que l'on connaisse pour déterminer si un patient est résistant à un traitement à l' azacitidine est de lui administrer ce traitement pendant au moins 6 mois et de déterminer si ce traitement a un effet ou non.
Cette même méthode est utilisée pour identifier le phénomène de rechute chez un patient. En effet, à ce jour, le seul moyen que l'on connaisse pour identifier une rechute chez un patient est de déterminer le moment où le traitement à l' azacitidine n'est plus efficace sur ce patient. Il n'existe donc pas de méthode permettant de prédire le moment de cette rechute avant que les symptômes associés n'apparaissent.
Lorsqu'il est préconisé pour les patients atteints de SMD et/ou de LMA, le traitement à 1 ' azacitidine est injecté par voie sous-cutanée dans le haut du bras, dans la cuisse ou dans l'abdomen, quotidiennement pendant 7 jours, et est suivi d'une période de repos de 21 jours. Il peut entraîner de nombreux effets secondaires plus ou moins graves tels que saignement intracrânien, septicémie, modification de la pression artérielle, léthargie, sensation de malaise général, perte de cheveux. En outre, le coût d'un traitement à l' azacitidine est considérable. Il est évalué à environ 80 000 euros par année de traitement. De nos jours, aucune mesure ne permet donc de diagnostiquer de façon fiable et peu onéreuse si un patient est sensible ou résistant à un traitement à 1 ' azacitidine .
Aussi, il existe à ce jour un besoin de prédire la résistance d'un patient à un traitement azacitidine afin d'éviter l'administration d' azacitidine à un patient pour lequel ce traitement est inefficace, que ce soit dès le début de son administration ou bien quelques mois plus tard lorsque ledit patient aura développé le phénomène de résistance. En effet, administrer un tel traitement à un patient résistant est contraignant, peut s'avérer dangereux pour le patient et peut entraîner en outre des dépenses inutiles considérables. Ceci est valable aussi bien dans le cas d'un traitement à 1' azacitidine préconisé pour des patients atteints de SMD et/ou de LMA que pour tous traitements thérapeutiques à l' azacitidine . En effet la résistance à 1' azacitidine est liée à la molécule d' azacitidine, pas à la façon dont elle est administrée.
Pouvoir identifier au plus vite les patients qui sont d' emblée résistants ainsi que le moment de la rechute des patients qui sont dans un premier temps sensibles est également avantageux car cela permet de pouvoir proposer d'autres essais cliniques avant que les conditions cliniques desdits patients ne se dégradent.
Il est donc essentiel de pouvoir diagnostiquer au plus tôt si un patient sera sensible à un traitement à l' azacitidine et de pouvoir prédire le moment de sa rechute .
De façon surprenante, le demandeur a pu mettre en évidence un lien entre le niveau d'expression de la protéine BCL2L10 dans un échantillon de fluide biologique prélevé sur un patient et la sensibilité de ce patient à un traitement à 1 ' azacitidine .
Dans la suite de la description, le terme général BCL2L10 est défini comme pouvant correspondre au gène, au transcrit ARN ou encore à la protéine BCL2L10.
Le gène BCL2L10 est un membre de la famille Bcl-2, qui présente une activité anti-apoptotique in vitro. La protéine BCL2L10 partage avec la famille de protéines Bcl-2 les domaines BH1, BH4 et BH2. Le domaine BH3 qui est caractéristique des pro-apoptotiques de la famille Bcl-2 est, quant à lui, absent de la protéine BCL2L10. Cependant, il y a encore des résultats contradictoires dans la littérature en ce qui concerne les propriétés pro-apoptotiques ou anti-apoptotiques de BCL2L10, notamment parce que son orthologue supposé chez la souris a également été décrit comme ayant une activité pro-apoptotique .
BCL2L10 peut interagir avec des membres de la famille Bcl-2 notamment Bcl-2, Bcl-xL et Bax pour réguler l'apoptose dans différents contextes. Certaines publications comme par exemple l'article « Loss of BCL2L10 protein expression as prognostic predictor for poor clinical outcome in gastric carcinoma , Histopathology 2010, 57, 814-82 » présentent BCL2L10 comme un gène anti-apoptotique .
La surexpression de BCL2L10 a été décrite comme supprimant l'apoptose par inhibition de la libération de cytochrome C par la mitochondrie .
Récemment, il a été montré que l'agent hypométhylant decitabine déclenche l'apoptose et la régulation positive, également appelée « up régulation », de nombreux gènes dont BCL2L10. A ce jour, un lien entre la résistance de patients à certains traitements anticancéreux et l'expression du gène BCL2L10 a été mis en évidence, notamment dans la demande de brevet JP2010162031 (A) qui décrit le fait que l'amplification du gène BCL2L10 peut permettre de détecter les cellules cancéreuses qui sont résistantes à un traitement à base de camptothecines . De même, la demande de brevet US2009143236 (Al ) décrit une méthode de détection de l'acquisition de résistance à certains médicaments par l'étude de l'amplification de certains gènes dont notamment BCL2L10. Cependant, dans ces deux demandes de brevets les anticancéreux pour lesquels la résistance est évaluée ne concernent pas l' azacitidine .
La demande de brevet US2011/0129833 décrit quant à elle le fait qu'une augmentation de l'expression de gènes de la famille de Bcl-2 chez un patient est corrélée avec une diminution de la probabilité que ce patient réponde à un traitement de chimiothérapie. Cependant, rien dans cette demande de brevet ne suggère que de telles conclusions soient applicables à un traitement à base d' azacitidine .
L'article « Rôle of BCL2L10 méthylation and TET2 mutations in higher risk myelodysplastic, Leukemia . 2011 Dec;25 (12) » décrit le phénomène de résistance à 1' azacitidine . Ce document décrit l'existence d'un lien entre la méthylation du promoteur du gène BCL2L10 et la résistance à l' azacitidine . Spécifiquement, une hyper- méthylation du promoteur du gène BCL2L10 est corrélée à une faible survie des patients souffrant de cancers gastriques. Cette publication enseigne que les patients avec une importante méthylation du promoteur BCL2L10 sont à fort risque de S D et à faible chance de réponse aux traitements épigénétiques tel que les traitements à 1' azacitidine . Cependant, rien dans cette publication n'enseigne qu'un niveau d'expression élevé de la protéine BCL2L10 permet d'obtenir la même conclusion.
Au contraire, la publication suggère que c'est un niveau d'expression faible de BCL2L10 qui est corrélé à une faible chance de réponse à l' azacitidine . De plus, même si un fort niveau de méthylation du gène BCL2L10 était réellement lié à la résistance au traitement azacitidine, ces données n'auraient pas pu être corrélées avec le niveau d'expression de la protéine BCL2L10. En effet, le niveau de méthylation d'un gène n'est pas forcément lié à l'expression de la protéine issue de ce gène. C'est notamment le cas pour BCL2L10.
Aussi, au vu de l'art antérieur détaillé ci- dessus, rien ne suggère l'existence d'un lien entre le niveau d'expression de la protéine BCL2L10 et le phénomène de résistance à l' azacitidine . Et a fortiori, rien ne suggère l'existence d'un lien entre un niveau d'expression élevé de la protéine BCL2L10 et le phénomène de résistance à l' azacitidine . La solution au problème posé a pour objet une méthode d'analyse in vitro permettant de diagnostiquer la résistance à un traitement à 1 ' azacitidine chez un patient, en utilisant la protéine BCL2L10 contenue dans un échantillon de fluide biologique prélevé sur ledit patient ainsi que des molécules biologiques fixant spécifiquement la protéine BCL2L10, caractérisée en ce que :
on récupère un échantillon de fluide biologique chez un patient ;
on calcule le pourcentage de cellules totales dudit fluide biologique exprimant la protéine BCL2L10 ;
on compare ledit pourcentage calculé à une valeur seuil de référence, cette valeur seuil étant comprise entre 20 et 60% ; et
on diagnostique la résistance à un traitement à 1 ' azacitidine chez un patient ayant un pourcentage de cellules exprimant la protéine BCL2L10 dans ledit fluide biologique supérieur à ladite valeur seuil de référence.
De façon surprenante, le demandeur a mis en évidence l'existence d'un lien entre le pourcentage de cellules d'un fluide biologique d'un patient qui expriment la protéine BCL2L10 et le phénomène de résistance à 1 ' azacitidine .
La détermination d'une valeur seuil de référence de ce pourcentage au-delà de laquelle on peut conclure sur la résistance d'un patient à l' azacitidine n'avait jusque-là jamais été évoquée.
Cette méthode permet de diagnostiquer le phénomène de résistance à l' azacitidine chez un patient avant même d'avoir administré ladite molécule d' azacitidine au patient. Cette méthode permet également, de façon avantageuse, de prédire la rechute d'un patient qui a, auparavant, été sensible au traitement azacitidine.
La méthode d'analyse selon l'invention permet d'éviter tout traitement inutile d'un patient à 1' azacitidine . Ceci représente donc des avantages à la fois concernant la santé et le traitement adéquat des patients, ainsi que des avantages économiques. Un deuxième objet de l'invention concerne un kit d'analyse in vitro permettant la réalisation de la méthode d'analyse in vitro selon l'invention, ledit kit comprenant des molécules biologiques fixant spécifiquement la protéine BCL2L10 dans des cellules issues de fluides biologiques prélevés sur des patients.
Enfin, un troisième objet de l'invention concerne l'utilisation d'un kit d'analyse in vitro selon l'invention, pour la mise en œuvre d'une méthode de suivi d'un traitement à l' azacitidine afin d'en prédire la rechute.
Afin de mieux comprendre les mécanismes associés à la résistance à l' azacitidine in vitro, les inventeurs ont généré des cellules myéloïdes SKM1 résistantes à 1' azacitidine, appelées AZA-R ou SKMl-R. Par opposition, les cellules AZA-S ou SKM1-S sont des cellules sensibles à l' azacitidine .
L'invention sera mieux comprise à la lecture de la description non limitative qui suit, rédigée au regard des dessins annexés, dans lesquels :
La figure 1 représente les résultats d'un criblage, communément appelé « screening » des cellules issues de lignées de cellules SKM1, exprimant la protéine Bcl-2. Les cellules SKM1-S et SKMl-R sont traitées avec ΙμΜ d' azacitidine pendant 24h. Des expériences de buvardage de western (« western blot » en anglais), sont ensuite réalisées afin d'évaluer les quantités de protéines Bcl-2, Mcl-1, Bcl-xl et BCL2L10. Un anticorps anti-HSP60 a été utilisé comme contrôle de charge.
Les figures 2 à 6 représentent l'expression de la protéine BCL2L10 dans les lignées cellulaires SKMl-S et SKMl-R atteintes de LMA.
Dans les figures 2, 3 et 4 le niveau protéique de BCL2L10 est quantifié dans des cellules SKMl-S et SKMl-R par cytométrie de flux.
La figure 5 représente une analyse par réaction de polymérisation en chaîne à transcriptase reverse, que l'on note RT-PCR pour Reverse Transcription Polymerase Chain Reaction, de l'ARNm des cellules SKMl-S et SKMl-R.
La figure 6 montre les résultats d'un western blot permettant de visualiser le niveau protéique de BCL2L10 dans des cellules SKMl-S et SKMl-R.
Les figures 7 à 10 montrent la resensibilisation des cellules SKMl-R à l' azacitidine suivi de l'extinction de l'expression du gène BCL2L10. Le fait d'éteindre l'expression du gène BCL2L10 est communément désigné par le terme « knockdown », dans le cas présent knock-down de BCL2L10.
Les cellules SKMl-S et SKMl-R sont transfectées avec un ARN interférant siARN Luc, siARN BCL2L10 ou siARN Bcl-2. Après 72h de transfection les cellules sont stimulées avec de 1 ' azacitidine à ΙμΜ.
Dans la figure 7 le métabolisme cellulaire est mesuré 24 heures après stimulation à l'aide du test XTT (pour Xylose Tolérance Test). Les résultats représentés correspondent à la moyenne plus ou moins l'écart type à la moyenne (± SEM pour « Standard Error of the Mean ») de trois expériences indépendantes réalisées en quatre exemplaires .
La figure 8 montre les résultats d'un marquage de la caspase 3 visualisé par cytométrie de flux 24 heures après l'ajout d' azacitidine à ΙμΜ. La figure 9 montre les résultats d'un marquage du Propidium Iodide (PI) par cytométrie de flux, 24 heures après l'ajout d' azacitidine à ΙμΜ.
La figure 10 représente les résultats de Western Blots réalisés 24 heures après l'ajout d' azacitidine à ΙμΜ afin de déterminer l'inhibition de l'expression de BCL2L10 et de Bcl-2.
Dans les figures 11, 12 et 13, il est montré que l'expression protéique de BCL2L10 est spécifiquement augmentée chez les patients résistants à l' azacitidine .
La figure 11 représente l'expression des protéines BCL2L10, Bcl-2 et ERK visualisées par western blot sur des échantillons « frais » de moelle osseuse provenant de 7 patients sains, 7 patients sensibles à 1' azacitidine et 5 patients résistants à 1 ' azacitidine . Les résultats du western blot sont montrés pour deux patients de chaque sous-groupe.
La figure 12 représente l'expression des protéines BCL2L10 et ERK analysée à l'aide du logiciel Image J (Image J est un logiciel libre de traitement d'images écrit en Java par le National Institute of Health (NIH) ) et la quantification du ratio de l'expression de BCL2L10 par rapport à l'expression de ERK.
La figure 13 représente la quantification de l'expression des protéines BCL2L10 et ERK analysée à l'aide du logiciel Image J et la quantification du ratio de l'expression de BCL2L10 par rapport à l'expression de ERK.
Les figures 14 à 17 illustrent le fait que chez les patients résistants à 1 ' azacitidine atteints de SMD ou de LMA le pourcentage de cellules exprimant la protéine BCL2L10 dans la moelle osseuse est augmenté.
Dans la figure 14, le pourcentage de cellules exprimant la protéine BCL2L10 est quantifié par cytométrie de flux chez 32 patients atteints de SMD ou de LAM et sous traitement azacitidine et chez 8 patients sains, tous issus de la cohorte 1.
Dans la figure 15 le pourcentage de cellules exprimant la protéine BCL2L10 est quantifiée par cytométrie de flux dans des échantillons congelés dans du DMSO issus de 14 patients atteints de SMD à bas risque, de 31 patients atteints de SMD haut risque ou de LAM et traités à l' azacitidine, tous issus de la cohorte 2.
Le pourcentage de cellules exprimant la protéine
BCL2L10 est quantifié par cytométrie de flux dans des échantillons congelés dans du DMSO issus de 16 patients atteints de SMD à haut risque, ou de patients au diagnostic, comme illustré dans la figure 16.

BCL2L10 est quantifié par cytométrie de flux dans des échantillons congelés dans du DMSO issus de 15 patients atteints de SMD à haut risque ou de LAM, tous sous traitement azacitidine, comme illustré dans la figure 17.
Les figures 18a et 18b représentent la corrélation entre le pourcentage de cellules exprimant la protéine BCL2L10 et la survie globale des patients traités atteints de SMD ou de LAM.
La comparaison de la survie globale à l'allogreffe selon Kaplan-Meier a été censurée pour les patients atteints de SMD ou de LAM traités avec AZA ainsi que le pourcentage de cellules exprimant BCL2L10 dans leur moelle osseuse.
Les figures 19 à 22 permettent de valider la technique de quantification protéique de BCL2L10 par cytométrie de flux.
Dans la figure 19, des cellules issues d'une lignée HEK293 ont été transfectées soit avec des plasmides d'expression pcDNA3 intégrant la partie N- terminale du tag de l'épitope Myc de BCL2L10, soit avec des plasmides d'expression pcDNA3 intégrant la partie N- terminale du tag de l'épitope Myc seul. Le niveau d'expression protéique de BCL2L10 a été quantifié par cytométrie de flux. Les résultats de cette expérience sont représentés dans la figure 19.
Dans la figure 20, des cellules issues d'une lignée HEK293 ont été transfectées soit avec un ARN interférant si-Luc soit avec un ARN interférant si- BCL2L10. Le niveau d'expression protéique de BCL2L10 a été quantifié par cytométrie de flux. Les résultats de cette expérience sont représentés dans la figure 20.
Les figures 21 et 22 représentent le niveau protéique de BCL2L10 visualisé par western blot. Un anticorps anti-HSP60 est utilisé comme contrôle de charge .
La présente invention a pour objet une méthode d'analyse permettant le diagnostic in vitro de la résistance à un traitement à l' azacitidine chez des patients en effectuant notamment une quantification de l'expression de la protéine BCL2L10 par les cellules totales d'un fluide biologique.
Selon l'invention, les patients sont des êtres humains. De façon avantageuse, la méthode d'analyse est particulièrement adaptée aux patients atteints d' hémopathies malignes telles que les hémopathies myéloïdes. Plus particulièrement encore, les patients sont atteints de LMA ou SMD.
Selon l'invention, le fluide biologique est un fluide issu du corps humain. A titre non limitatif d'exemple de fluide biologique on peut citer la moelle osseuse, le sang, le liquide céphalo-rachidien, l'urine. De préférence, le fluide biologique selon l'invention est de la moelle osseuse. Selon l'invention, le terme « cellules totales » regroupe toutes les cellules présentes dans le fluide biologique prélevé. Dans le cas où le fluide biologique prélevé est de la moelle osseuse, les cellules totales comprennent notamment les cellules souches hématopoïétiques (CSH) et les cellules du stroma médullaire qui sont les cellules hématopoïétiques.
Selon l'invention, les molécules biologiques fixant spécifiquement la protéine BCL2L10 sont des molécules capables de se lier spécifiquement à la protéine BCL2L10. Avantageusement, ce sont des anticorps monoclonaux ou polyclonaux, des récepteurs solubles ou des aptamères, préférentiellement des anticorps monoclonaux ou polyclonaux. De préférence encore, les molécules biologiques fixant spécifiquement la protéine BCL2L10 sont des anticorps monoclonaux. A titre d'exemple non limitatif de molécules biologiques fixant spécifiquement la protéine BCL2L10 on peut citer l'anticorps monoclonal anti-BCL2LlO référencé « #3869 » par la société « Cell Signaling Technologies ».
Selon l'dnvention, la valeur seuil de référence, également appelée valeur « cut-off », correspond à un pourcentage de cellules BCL2L10 positives, c'est-à-dire un pourcentage de cellules exprimant la protéine BCL2L10, dans un fluide biologique.
Lorsque la valeur de pourcentage de cellules exprimant la protéine BCL2L10 dans les cellules totales d' un fluide biologique obtenue est supérieure à cette valeur « cut-off », les patients testés seront diagnostiqués comme étant résistants à l' azacitidine . A l'inverse, lorsque la valeur obtenue sera inférieure à cette valeur « cut-off », les patients testés seront diagnostiqués comme étant sensibles à l' azacitidine .
Selon l'invention, la valeur seuil de référence est comprise entre 20 et 60%, préférentiellement entre 30 et 55 %, de préférence encore, cette valeur seuil de référence est égale à 50%.
La méthode d'analyse selon l'invention permet de diagnostiquer la résistance à un traitement à 1 ' azacitidine chez un patient. Lesdits patients traités à l' azacitidine doivent généralement subir des ponctions de moelle osseuse tous les 1, 3, et 6 mois dans le cadre de leur traitement, puis tous les 3 mois par la suite. Ainsi, ce prélèvement de moelle osseuse réalisé dans le cadre d'un suivi classique de traitement à l' azacitidine peut également servir à la méthode d'analyse selon l'invention. Il n'est donc pas forcément nécessaire de réaliser des ponctions spécifiques de moelle osseuse chez les patients, en vue de ce diagnostic de la résistance à un traitement à l' azacitidine .
Autrement dit, ceci représente un avantage pour le patient, qui ne devra donc pas subir d'examen supplémentaire étant donné que la méthode d' analyse in vitro permettant le diagnostic de la résistance du patient à l' azacitidine se fera sur l'échantillon de moelle ponctionné dans le cadre du traitement classique.
Selon l'invention, la mesure du pourcentage de cellules du fluide biologique exprimant la protéine BCL2L10 est réalisée par cytométrie de flux ( Immunophénotypage ) , par chromatographie par interaction hydrophobe (en anglais HIC pour "hydrophobic interaction chromatography" ) , ou bien par réaction de polymérisation en chaîne quantitative (en anglais qPCR pour "quantitative Polymerase Chain Reaction"). De préférence, cette mesure est réalisée par cytométrie de flux (Immunophénotypage).
L'invention concerne également une méthode d'analyse in vitro permettant de diagnostiquer la résistance à un traitement à l' azacitidine chez un patient, par détection de la surexpression du gène BCL2L10 contenu dans un échantillon de fluide biologique prélevé sur ledit patient, caractérisée en ce que :
on récupère un échantillon de fluide biologique chez un patient ;
on calcule le pourcentage de cellules totales dudit fluide biologique exprimant BCL2L10 par détection de la surexpression du gène BCL2L10 ; on compare ledit pourcentage calculé à une valeur seuil de référence, cette valeur seuil étant comprise entre 20 et 60% ; et
on diagnostique la résistance à un traitement à l' azacitidine chez un patient ayant un pourcentage de cellules exprimant BCL2L10 dans ledit fluide biologique supérieur à ladite valeur seuil de référence.
Préférentiellement , la détection de la surexpression du gène BCL2L10 est effectuée par la méthode d'Hybridation génomique comparative CGH, la méthode de cytométrie en flux, la méthode ELISA, la méthode de puce à ADN, ou par réaction de polymérisation en chaîne quantitative (qPCR) . Préférentiellement encore, la détection de la surexpression du gène BCL2L10 est effectuée par la méthode d'Hybridation génomique comparative CGH, par la méthode de puce à ADN ou par réaction de polymérisation en chaîne quantitative (qPCR) . L'invention concerne également un kit d'analyse in vitro comprenant des molécules biologiques fixant spécifiquement la protéine BCL2L10 dans des cellules issues d'un échantillon de fluide biologique prélevé sur un patient, ledit kit permettant de prédire la résistance dudit patient à un traitement à l' azacitidine chez un patient ayant un pourcentage de cellules, dans ledit fluide biologique exprimant la protéine BCL2L10, supérieur à une valeur seuil de référence comprise entre 20 et 60%.
Elle concerne également un kit d'analyse in vitro comprenant au moins un réactif sélectionné dans le groupe constitué par:
une paire d'amorces apte à amplifier un fragment de BCL2L10, et
une sonde apte à détecter la présence de BCL2L10,
ledit kit permettant de prédire la résistance dudit patient à un traitement à l' azacitidine chez un patient ayant un pourcentage de cellules, dans ledit fluide biologique exprimant BCL2L10, supérieur à une valeur seuil de référence comprise entre 20 et 60%.
Un autre objet de l'invention concerne l'utilisation d'un kit ou de la méthode selon l'invention, pour la mise en œuvre d'une méthode de suivi d'un traitement à l' azacitidine afin d'en prédire la rechute.
Selon un mode particulier de réalisation de l'invention, l'utilisation du kit ou de la méthode selon l'invention permet également d'adapter le traitement en fonction de la réponse du patient.
Selon un mode particulier de réalisation de l'invention, lorsque l'on diagnostique la résistance à un traitement à l' azacitidine chez un patient, notamment atteint de syndrome myélodysplasique et/ou de leucémie myéloïde aiguë, on administre en outre audit patient un traitement alternatif comprenant au moins un agent antitumoral et/ou un agent anti-inflammatoire.
De préférence, on administre audit patient un composé antitumoral choisi parmi les agents alkylants, les anti-métabolites, les alcaloïdes végétaux, les inhibiteurs de la topoisomérase, et les antibiotiques antitumoraux .
A titre d'exemple non limitatif d'agent antitumoral utilisable selon l'invention, on peut citer notamment l'acadésine, appelée aussi AICAR pour 5-Aminoimidazole- 4-carboxamide-l- -D-ribofuranoside, les dérivés de l'acadésine, l' actinomycine D, l'amsacrine, les anthracyclines tels que la doxorubicine ou la daunorubicine, l'aracytine, l'ATRA (All-trans retinoic acid) , la bléomycine le bortezomib, le busulfan, les dérivés de la camptothécine, la cisplatine, la carboplatine, le chlorambucil, la décitabine, la dépakine, le docétaxel, les dérivés de
1 ' épipodophyllotoxine, l'erlotinib, l'etoposide, le 5- fluoro-uracile (5FU), la fludarabine, l'hydrea, 1' ifosfamide, les inhibiteurs d'histone déacétylase (HDAC),le lenalidomide, le méthotrexate, la mitomycine C, le paclitaxel, la plicamycine le purinethol, le thiotépa, la vincristine, la vinblastine et la vinorelbine. Des études ont été réalisées afin de mettre en évidence certains avantages de la présente invention. Les résultats de ces études sont reportés dans les exemples ci-après.
Exemple 1 : Validation de la technique de cytométrie pour la détection de BCL2L10.
Des cellules SKM1 résistantes à l' azacitidine (AZA) , notées « SKM1-R » défectueuses à la fois pour les processus d'apoptose et d'autophagie ont été générées. Comparativement à leurs homologues AZA-sensibles , notées « SK 1-S », les cellules SKM1-R montrent une expression accrue de la protéine BCL2L10 (Bcl-B) , un membre anti- apoptotique de la famille Bcl-2, mais les cellules SKMl- R et SKM1-S montrent des niveaux équivalents de protéines Bcl-2, Bcl-xL et Mcl-1, comme illustré par la figure 1.
Une augmentation de l'expression des protéines BCL2L10 a également été trouvée dans la masse de cellules SKM1-R avant dilution limitée, indiquant que la surexpression de BCL2L10 est liée à la résistance à 1 ' azacitidine (AZA) et n'est pas due à un effet clonal. Pour analyser l'expression protéique de BCL2L10, un test de cytométrie sur des cellules HEK293 a été développé.
Pour cela, des cellules HEK293 ont d'abord été transfectées avec une construction BCL2L10 taguée Myc « Myc-BCL2L10 » et l'efficacité de transfection a été évaluée en utilisant un anticorps anti-Myc, comme illustré dans la figure 19. L'expression des protéines BCL2L10 a été confirmée par Western Blot en utilisant un anticorps monoclonal anti-BCL2LlO comme illustré dans la figure 21.
Pour valider l'expérience de cytométrie de flux, un siARN spécifique a été utilisé, afin d'éteindre l'expression du gène BCL2L10 dans des cellules HE 293.
Dans cette condition, ni l'expression des protéines BCL2L10, ni le marquage de BCL2L10 n'ont été détectés respectivement par western blot et par cytométrie de flux, comme illustré, respectivement, par les figures 22 et 20. Ceci valide notre expérience de cytométrie basée sur la détection des protéines BCL2L10. Exemple 2 : La surexpression de BCL2L10 participe à la résistance des cellules SKM1 à l' azacitidine .
En utilisant le dosage décrit dans les figures 19 à 22, il a été établi que 73% de cellules SKM1-R expriment la protéine BCL2L10, contre seulement 39% de cellules SKM1-S, comme illustré aux figures 2 à 4. Une augmentation de l'expression de 1 'ARNm de BCL2L10 et des protéines BCL2L10 a également été détectée dans les cellules SKM1-R par RT-PCR et par western blot, comme illustré respectivement aux figures 5 et 6.
Pour déterminer si la surexpression BCL2L10 est une cause plutôt qu'une conséquence de la résistance à 1 ' azacitidine, les cellules SKM1-S et SKMl-R ont été transfectées avec un siARN contrôle ou avec des siRNA dirigés contre l'une ou l'autre des protéines BCL2L10 ou Bcl-2, puis traitées pendant 24h avec ou sans azacitidine, avant de déterminer la viabilité cellulaire et l'apoptose. La figure 7 montre que l' azacitidine a entraîné une perte du métabolisme cellulaire dans les cellules SKM1-S, mais pas dans les cellules SKMl-R, comme illustré dans la figure 5. L'extinction de l'expression du gène BCL2L10 permet la restauration de la sensibilité des cellules SKMl-R à 1 ' azacitidine, ce qui suggère un rôle important de BCL2L10 dans le phénomène de résistance à 1 ' azacitidine . De plus, l'apoptose a été le mécanisme principal par lequel l'extinction de l'expression du gène BCL2L10 a permis la sensibilisation à 11 azacitidine en augmentant la quantité de caspases 3 actives.
En outre, le marquage du Propidium Iodide (PI) a été détecté dans les cellules SKMl-R traitées avec un siARN BCL2L10, comme illustré dans les figures 8 et 9. Cet effet était spécifique pour BCL2L10 car un siARN dirigé contre la protéine Bcl-2 a échoué à le faire dans des conditions identiques, comme le montrent les figures 8 et 9. Enfin dans la figure 10 il a été vérifié par western blot que les deux siARN sont très efficaces pour bloquer l'expression de leurs cibles respectives. Une fois regroupées, nos données ont permis d'établir que la surexpression de la protéine BCL2L10 est responsable de la résistance à l' azacitidine dans les cellules SKMl-R. Exemple 3 :. La surexpression de BCL2L10 permet de prédire la résistance à l' azacitidine chez les patients atteints de SMP
L'expression de BCL2L10 a également été analysée par western blot sur des échantillons provenant de patients lorsque la quantité de matériel à analyser était suffisante. Les résultats présentés dans les figures 11 à 13 montrent que le niveau de BCL2L10 par rapport au niveau de BCL-2 est variable selon les patients. La protéine ERK a été utilisée comme contrôle interne pour chaque échantillon de patient. Ceci a permis de montrer que l'expression protéique de BCL2L10 versus ERK est très faible chez les patients sains, comme le montre la figure 12. A l'inverse, l'expression de la protéine Bcl-2 n'est pas significativement différente dans les 3 groupes de patients comme illustré par la figure 13. Les résultats suggèrent que l'expression de BCL2L10 permet de prédire la résistance à l' azacitidine chez les patients atteints de SMD.
Exemple 4 : l'expression de la protéine BCL2L10 est un biomarqueur de la résistance à l' azacitidine chez les patients atteints de SMD.
Le pourcentage de cellules exprimant la protéine BCL2L10 dans la moelle osseuse de 8 patients sains, de 24 patients sensibles à 1 ' azacitidine et de 8 patients résistants à 1 ' azacitidine a été déterminé en utilisant l'expérience de cytométrie de flux sur la cohorte 1. Les caractéristiques cliniques de chaque patient sont données dans les tableaux 1, 2A et 2B ci-dessous :

Comme cela est montré dans la figure 14, la valeur moyenne pour les échantillons de moelle osseuse fraîchement isolée de patients sains et de patients sensibles à l' azacitidine est respectivement de 0%, avec des valeurs allant de 0 à 18%, et 8%, avec des valeurs allant de 0 à 40%, de cellules exprimant la protéine BCL2L10, alors que la valeur moyenne pour les cellules de moelle osseuse issues de patients résistants à l' azacitidine est de 85%, avec des valeurs allant de 57 à 99%, de cellules exprimant la protéine BCL2L10 avec une valeur p inférieure à 0,0001, comme illustré par la figure 11. Lorsque l'on compare rétrospectivement les échantillons issus de 14 patients atteints de SMD à bas risque aux échantillons de 21 patients sensibles à l' azacitidine et de 10 patients résistants à l' azacitidine, tous issus de la cohorte 2, il s'avère que les patients atteints de SMD à bas risque ont une médiane de 0% de cellules exprimant BCL2L10, les extrêmes étant de 0 et 11%. La figure 15 illustre également que les patients résistants à l' azacitidine ont un pourcentage de cellules exprimant la protéine BCL2L10 beaucoup plus important, égal à 33% (p<0.0001), contre 10% pour les patients sensibles. De plus, à partir du groupe de patients décrit dans la figure 8, des sous-groupes d'analyse sont réalisés. Les 10 patients « premièrement » réfractaires à 1 ' azacitidine testés ont un pourcentage de cellules exprimant BCL2L10 égal à 29% ce qui est supérieur à celui des 6 patients sensibles à l' azacitidine au diagnostic testés qui est de 10%. Ces résultats sont représentés figure 16 (p=0,023). Au moment de la rechute, les 4 patients « premièrement » sensibles à l' azacitidine testés montrent un pourcentage de cellules exprimant la protéine BCL2L10 égal à 23% donc fort comparé aux 11 patients sensibles à 1 ' azacitidine et sous traitement testés, pour lesquels le pourcentage de cellules exprimant la protéine BCL2L10 est égal à 14%, comme illustré dans la figure 17 (p=0,0002).
Exemple 5 : le pourcentage de cellules exprimant la protéine BCL2L10 prédit la survie globale des patients atteints de SMD et de LAM.
Avec une valeur seuil de référence, également appelée « cut-off », égale à 50% de cellules exprimant la protéine BCL2L10, sur les cellules totales du fluide biologique, le test a permis d'obtenir d'excellentes prédictions positives et négatives. D'une façon générale, la sensibilité et la spécificité du test étaient respectivement de 80% et 85%.
Avec un suivi médian de 4 mois, les extrêmes étant 0,1 et 7,5 mois, à partir de la date de la quantification de BCL2L10, la survie globale (OS) pour la cohorte 1 a été significativement meilleure dans les sous-groupes exprimant faiblement BCL2L10 que dans les sous-groupes exprimant fortement BCL2L10 (p = 0,0016) comme illustré dans la figure 18a. Le graphe représenté à la figure 18b représente, sur une durée plus longue (environ 15 mois versus environ 6 mois) , la corrélation entre le pourcentage de cellules exprimant BCL2L10 et la survie globale (OS) chez des patients souffrant de SMD ou de LAM traités par l' azacitidine . Cette figure 18b montre les courbes Kaplan- eier de survie globale des deux groupes de patients SMD ou LAM traités par l'AZA présentant plus ou moins de 50% de cellules exprimant BCL2L10 dans leur moelle osseuse.
Une survie globale d'une durée de 3 mois a été estimée à 95% pour les sous-groupes exprimant peut BCL2L10 contre 51% pour les sous-groupes exprimant fortement BCL2L10. Pour tous les patients dans le sous- groupe exprimant fortement BCL2L10 la maladie a progressée .

Claims

REVENDICATIONS
1. Méthode d'analyse in vitro permettant de diagnostiquer la résistance à un traitement à l' azacitidine chez un patient, en utilisant la protéine BCL2L10 contenue dans un échantillon de fluide biologique prélevé sur ledit patient ainsi que des molécules biologiques fixant spécifiquement la protéine BCL2L10, caractérisée en ce que :
on récupère un échantillon de fluide biologique chez un patient ;
on calcule le pourcentage de cellules totales dudit fluide biologique exprimant la protéine BCL2L10 ;
on compare ledit pourcentage calculé à une valeur seuil de référence, cette valeur seuil étant comprise entre 20 et 60% ; et
on diagnostique la résistance à un traitement à l' azacitidine chez un patient ayant un pourcentage de cellules exprimant la protéine BCL2L10 dans ledit fluide biologique supérieur à ladite valeur seuil de référence.
2. Méthode selon la revendication 1, caractérisée en ce que le fluide biologique est de la moelle osseuse.
3. Méthode selon l'une des revendications 1 ou
caractérisée en ce que ladite valeur seuil référence est égale à 50%.
4. Méthode selon l'une quelconque des revendications 1 à 3, caractérisée en ce que la mesure du pourcentage de cellules du fluide biologique exprimant la protéine BCL2L10 est réalisée par cytométrie de flux, par chromatographie par interaction hydrophobe (HIC) ou par réaction de polymérisation en chaîne quantitative (qPCR), préférentiellement par cytométrie de flux.
Méthode selon l'une quelconque des revendications 1 à 5 caractérisée en ce que les molécules biologiques fixant spécifiquement la protéine BCL2L10 sont des anticorps spécifiques à la protéine BCL2L10.
Méthode d'analyse in vitro permettant de diagnostiquer la résistance à un traitement à l' azacitidine chez un patient, par détection de la surexpression du gène BCL2L10 contenu dans un échantillon de fluide biologique prélevé sur ledit patient, caractérisée en ce que :
on récupère un échantillon de fluide biologique chez un patient ;
on calcule le pourcentage de cellules totales dudit fluide biologique exprimant BCL2L10 par détection de la surexpression du gène BCL2L10 ; on compare ledit pourcentage calculé à une valeur seuil de référence, cette valeur seuil étant comprise entre 20 et 60% ; et
- on diagnostique la résistance à un traitement à l' azacitidine chez un patient ayant un pourcentage de cellules exprimant BCL2L10 dans ledit fluide biologique supérieur à ladite valeur seuil de référence.
Méthode selon la revendication 6, caractérisé en ce que la détection de la surexpression du gène BCL2L10 est effectuée par la méthode d'Hybridation génomique comparative CGH, la méthode de cytométrie en flux, la méthode ELISA, la méthode de puce à ADN, ou par réaction de polymérisation en chaîne quantitative (qPCR) .
Méthode selon la revendication 7, caractérisée en ce que la détection de la surexpression du gène BCL2L10 est effectuée par la méthode d'Hybridation génomique comparative CGH, par la méthode de puce à ADN ou par réaction de polymérisation en chaîne quantitative (qPCR) .
Méthode selon la revendication 8, caractérisée en ce que la détection de la surexpression du gène BCL2L10 est effectuée par la méthode de puce à ADN ou par réaction de polymérisation en chaîne quantitative (qPCR) .
Kit d'analyse in vitro comprenant des molécules biologiques fixant spécifiquement la protéine BCL2L10 dans des cellules issues d'un échantillon de fluide biologique prélevé sur un patient, ledit kit permettant de prédire la résistance dudit patient à un traitement à l' azacitidine chez un patient ayant un pourcentage de cellules, dans ledit fluide biologique exprimant la protéine BCL2L10, supérieur à une valeur seuil de référence comprise entre 20 et 60%.
Kit d'analyse selon la revendication 6, caractérisé en ce que ledit fluide biologique est de la moelle osseuse.
Kit d'analyse in vitro comprenant au moins un réactif sélectionné dans le groupe constitué par : une paire d'amorces apte à amplifier un fragment de BCL2L10, et
une sonde apte à détecter la présence de BCL2L10, ledit kit permettant de prédire la résistance dudit patient à un traitement à l' azacitidine chez un patient ayant un pourcentage de cellules, dans ledit fluide biologique exprimant BCL2L10, supérieur à une valeur seuil de référence comprise entre 20 et 60%.
Utilisation d'un kit selon l'une des revendications 10, 11 ou 12, pour la mise en œuvre d'une méthode de suivi d'un traitement à l' azacitidine afin d'en prédire la rechute.
14. Utilisation d'un kit selon la revendication 13, caractérisée en ce qu'elle permet l'adaptation dudit traitement selon la réponse dudit patient.
15. Utilisation selon l'une des revendications 13 ou 14, caractérisée en ce que lesdits patients sont atteints de syndrome myélodysplasique et/ou de leucémie myéloïde aigiie.
