BESCHREIBUNG DESCRIPTION
Perfundierbarer Bioreaktor zur Herstellung von menschlichen oder tierischen GewebenPerfusable bioreactor for the production of human or animal tissues
The invention relates to a perfusable bioreactor for the production of human or animal tissues or tissue equivalents, the preparation of which is based on a construct cultured in the interior, the interior is enclosed by a shell and has at least one inlet and an outlet for a liquid nutrient medium, the bioreactor is connectable to a unit for generating a perfusion pressure of the nutrient medium. This tissue replacement is used in particular for clinical therapeutic application.
For the purposes of the invention, "perfusable bioreactors" are bioreactors which allow the construct introduced into them to be flowed through primarily by a liquid medium and can be flowed around secondarily.
Im Sinne der Erfindung werden die Bezeichnungen Blutgefäßäquivalente und Blutgefäßwandäquivalente analog dieser Definition verwendet."Constructs" within the meaning of the invention are artificially produced three-dimensional tissue equivalents which contain living cells in a three-dimensional matrix, in particular combinations of scaffolds and living cells (scaffold-cell combinations), if appropriate also combined with matrix factors Designations Blood vessel equivalents and blood vessel wall equivalents used analogously to this definition.
For the production of tissue in vitro, various types of perfusion bioreactors have been developed to date. However, until now, the main focus has been on the production of bioreactors with rigid walls whose shape is not adapted to the tissue to be cultivated. Therefore, stresses and influences on the tissue growing in vitro do not correspond to those of a natural tissue in vivo. However, it is precisely mechanical stresses which have a not insignificant influence on tissue growth in vivo and also in vitro should be modeled. In addition, there is the disadvantage that even if a perfusion of the tissue or tissue equivalent is intended, it is usually not a flow ie S., but more is a flow around, and thus there is no optimal supply inside such a construct. This problem plays a role in particular for soft tissue and blood vessels. The provision or production of nutrition and blood flow through blood vessels is a major unresolved problem in tissue engineering (cell and tissue engineering). Even at low tissue volumes, it is important to implement a vasculature or equivalent because, for distances greater than about 100-300 microns to the next blood capillary, diffusion to nutrition is no longer sufficient. Such a tissue therefore also requires its own blood vessel supply, which naturally has to be adapted to the shape of the implant. There is therefore a need for bioreactors in which supplying blood vessels can be cultured in combination with any desired tissue and which, in addition, meet the physical / mechanical requirements of soft-tissue and / or vascular / microvascular engineering.
In previous bioreactors, although already pulsatory perfusion have been realized, which should simulate the blood pressure, in particular to condition artificial vascular constructs to the blood pressure forces in vivo. In the environment of a rigid bioreactor wall, however, these are often not physiological or are unphysiologically reflected, which can also lead to the destruction of the cells in the reactor. Thus, it is necessary to provide physiological tissue compliance (extensibility of the tissue) in the three-dimensional environment, which is not possible with previous systems. Another problem is the design of the shape of tissue constructs, for example, to compensate for defects in the subcutaneous fatty tissue near the surface. Depending on the particular application, in particular as tissue replacement, the tissue to be produced requires an individual shape, ie a defined spatial configuration. It is of particular importance that the tissue construct after implantation the Fills the defect as precisely as possible to achieve the desired aesthetic result. For the engineering of soft tissues, especially fatty tissue for surface contouring or defect compensation, but also for bone, which is used in contour effective localizations, it is desirable to achieve a targeted specific shape.
Also in terms of blood supply, it is important to consider the shape as the tissue is being made, so that the blood supply will be properly sized and not later destroyed by shape corrections.
In previous approaches, the preforming and definition of the outer shape of a tissue engineered tissue construct is typically achieved via the shape of a scaffold on which the cells grow and multiply, forming the outer shape of the scaffold in which the artificial tissue is formed with a specific differentiation.
However, it would be an ideal approach to be able to work scaffold-free or with rapidly absorbable framework materials. At the same time, however, the outer shape must be predetermined and ideally reflect the individual defect in which the construct should fit later. Even with the use of scaffolds, cultivation in perfusion bioreactors, which are not adapted to the scaffold in shape, poses a potential disadvantage. It is often achieved in these bioreactors only a flow around the nutrient medium and not a perfusion, so that the central skeleton parts u. May not be sufficiently fed.
The object of the invention is to provide a bioreactor which overcomes the disadvantages of the prior art.
The object of the invention is solved by the features of claim 1.
Material besteht.Invention is essential that at least a sub-segment of this shell made of elastic Material exists.
Importantly, the shell of the bioreactor is largely elastic and its elasticity along with that of the tissue or vascular equivalent inside has a mechanical "compliance" corresponding to that of the target tissue.
The elastic sub-segments of the sheath ensure the exercise of physiological mechanical stresses (pressures and forces), e.g. by a pulsatory perfusion from the inside with pressures targeted in the physiological or pathological area (blood pressure). For example, a pulsatile perfusion via the tissue and the hydrostatic pressure of the nutrient medium against the elastic sheath (wall) is forwarded and it can act on the tissue mechanical or hydrodynamic stresses. The essential difference from previous solutions lies in the fact that this perfusion dynamics takes place in an elastic environment and, by adjusting the elasticity of the chamber wall, the compliance of natural blood vessels and tissue in a physiological and pathological situation can be modeled.
Connected thereto may also be a tight-fitting, form-fitting design of the reactor wall to the fabric to be produced, which has the advantage that the fabric elongation is adjustable by the elasticity of the reactor wall. Another advantage is that the form-fitting enclosure a perfusion of the scaffold is supported and a simple rinsing with medium is avoided.
Then, the contour of the interior of the bioreactor substantially corresponds to the outer contour of the fabric to be produced.
It is furthermore preferred that the inner contour of the interior corresponds in at least more than 50% to the surface of the outer contour of the human or animal tissue or tissue equivalent to be produced.
In contrast to other bioreactors, the construct fills the bioreactor according to the invention to a large extent, the supply is not carried out by rinsing the Constructs with the medium, but primarily by perfusion of a hollow fiber or hollow conduit system, a preformed or growing, artificial vasculature, a porous framework or a combination of at least two of these principles (shown in Fig. 1 A to C).
The solution according to the invention thus also includes the individual, dimensionally accurate covering of a framework 4 (scaffold) in individual form, so that the framework narrows closely from the elastic shell 1, d. H. the elastic chamber wall, surrounded, and is perfused by the perfusion medium. By perfusion, the regulation of pressures and forces and tissue compliance in physiological limits, a microvascular vascular network should be able to develop that will ultimately take care of the supply. This will receive its central inflow 2 and outflow 3 at the preformed channels or resorbable hollow fiber tubes, so that these connections can be connected as artificial supply blood vessels microsurgically in the recipient storage to local blood vessels and thus a blood flow to the tissue is guaranteed immediately after implantation. The preformation, d. H. defined spatial configuration of the tissue to be implanted (tissue implant), which has the spatial configuration of the tissue produced with the bioreactor according to the invention, also has the advantage that the fabric produced fits exactly into the defect to be supplied, so that an optimal functional and aesthetic result is reached.
The preparation of these bioreactors can be carried out in a known manner via CAD / CAM techniques from three-dimensional image data sets of the defect to be supplied or by taking individual, dimensionally accurate defect models that have been produced by means of CAD / CAM techniques. The erfϊndungsgemäße solution also includes the individual, dimensionally accurate wrapping (eg by coating, deep drawing) of a scaffold (scaffolds, which has also been prepared for example via imaging and CAD / CAM accurately) in individual form, so that the framework closely from the shell, ie the chamber wall, is surrounded, and perfusion medium is flowed through. Ports and tributaries are incorporated into this enclosure, piping systems may be incorporated in the framework in this case.
A particular advantage of the bioreactor according to the invention is, individually to the desired shape of the tissue to be implanted, d. H. of the tissue to produce tailored disposable bioreactors.
The dependent claims 2 to 14 give further advantageous embodiments of the invention according to claim 1 again, without limiting these.
Flowed through skeleton or hollow conduit system and thus has supplied the tissue located in the chamber with oxygen and nutrients, it flows through the outflow from the interior (cavity) of the bioreactor out.
The resorbable or permanent waveguide system integrated in the bioreactor or the distribution over the porous framework initially takes care of the tissue, if necessary, until it can supply itself due to the development of its own vascular system or until it is implanted. The growth of a Gefaßsystems is possibly supported by the hydrodynamic stress, which acts due to the pulsatile perfusion in the vicinity of the hollow conduit system. Depending on the flexibility of the selected material composition of the waveguide system, the transmission of the mechanical impulses and their intensity can be varied. The production of an artificial vascular system can be done by means of a predefined two-dimensional matrix or a three-dimensional braid done.
As an alternative to a waveguide system that is produced artificially, that is to say with framework materials, or simply a porous, perfusable scaffold, a blood vessel system produced without tissue by means of tissue engineering and its developing vessel protrusions can take over the distribution of the perfusion medium and thus the supply of the surrounding tissue, or a combination of synthetic resorbable scaffolds and tissue engineering vessels.
Furthermore, devices for monitoring (monitoring and control) can be integrated into the elastic wall of the cavity of the bioreactor. These include, for example, viewing windows for direct optical observation (eg by microscopy, fluorescence microscopy, laser scanning microscopy, etc.). The functional monitoring is carried out via a probe system which monitors substance concentrations and physical or chemical parameters such as O 2 and CO 2 concentration, pressure in the chamber and in the remaining bioreactor, oxygen partial pressure, pH, flow rate and temperature. Elongation of the elastic walls can be monitored with strain gauges. The monitoring also actively contributes to the regulation of growth conditions in the bioreactor system, since it is integrated into a control loop as sensor technology.
A particular advantage of this approach is to tailor to the desired shape of the tissue to be implanted, i. H. of the tissue to produce tailored disposable bioreactors.
All of the above-mentioned advantages of the invention thus contribute significantly to the improvement of previous bioreactor systems, the growth conditions in bioreactors and the quality of cultured tissue constructs and have positive effects on tissue engineering (cell and tissue culture) in general and special. The invention is explained in more detail below with reference to exemplary embodiments without having conclusively illustrated all possible uses of the invention.
Beispiel 1 - Herstellung eines individuellen, elastischen präformierten BioreaktorsExample 1 - Preparation of an Individual Elastic Preformed Bioreactor
After three-dimensional representation of a human or animal tissue defect by means of known imaging methods, a three-dimensional data set is calculated with which the planning of the shape of the bioreactor is carried out in order to be able to achieve the shape of the tissue to be produced according to the invention. These raw data used for this purpose can come from various known imaging modalities (CT, MRI (magnetic resonance imaging), ultrasound, etc.) and are preprocessed with suitable image processing methods.
Finally, the three-dimensional wireframe model exported to a CAD / CAM system can be translated into a 3D model of the bioreactor with high precision from a 3D printer, a CNC miller or other three-dimensional shaping device.
Here, the bioreactor is made directly from elastic biocompatible material (elastomers, e.g., silicones) or the 3D model serves as a mold for the casting. Starting from three-dimensional patient data (CT, MRT, further modalities), after appropriate preprocessing of the raw data, a three-dimensional wireframe model with corresponding spatial resolution in the respective shape of the required tissue is produced by means of a CAD / CAM system. The geometry data of the wireframe model are then loaded into an adequate three-dimensional shaping system (3D printer, CNC milling machine, etc.) and the bioreactor is thus manufactured with very high precision.
Eine weitere alternative Möglichkeit ist die formgenaue Umhüllung 1 von GerüstenEither the production of the bioreactor is done directly, or it is first made the mold for the casting of the bioreactor with a corresponding material. It can be produced both once, as well as several times, ie reusable bioreactors. Fig. 1 A to C show variants of the elastic bioreactor for the production of tissues with an elastic sheath 1 in defect form. Another alternative possibility is the dimensionally accurate wrapping 1 of scaffolding
The shape is given by the scaffold 4, i. this has possibly been produced by means of CAD / CAM according to the image data of the defect. Connections 2 and inflows 3 are incorporated into this enclosure 1, line systems can be incorporated in this case in the framework. After wrapping and attaching the connectors, the bioreactor, including the wrapped scaffold, is operational for one-time use. The arrows in FIGS. 1A to C at the inlet opening 2 and the outlet opening 3 show the direction of medium perfusion.
Corresponding connections for monitoring and perfusion systems, as well as force transmission points for mechanical loads, are already taken into consideration in the planning phase of the bioreactor and these are incorporated in the CAD / CAM system in the three-dimensional wireframe model.
In the same form, piping systems, hollow fiber systems, or negative molds for piping systems which leave behind channels after removal can be incorporated into the cavity or the walls.
Beispiel 2 - Implementierung von versorgenden Gefäßen (Fig. 1 A bis C)Example 2 - Implementation of Serving Vessels (Figs. 1A to C)
Possibilities of incorporating a vascular or conduit system into the tissue include incorporation of preformed hollow fiber or conduit systems, tissue engineering of vessels, or a combination of both. For example, it would be possible to produce a piping system by a casting process. Wires 6 of a suitable, smooth material are laid in the bioreactor and connect the inlet opening 2 with the outlet opening 3. The filling of the bio-factor takes place with particles of a carrier material which has been colonized with the cells of the target tissue (overgrown microcarriers). These are initially cultivated separately until the cells (stem cells, pre-differentiated or differentiated cells) have a certain density achieved. They are then added together with fibrin into the bioreactor, which polymerizes by adding thrombin, so that the overgrown microcarriers / particles of a carrier material may be present in a fibrin matrix. Possibly. Endothelial cells can also be added, so that a capillary-like system can form. The wires 6 are removed and remain conduits, tubes or channels from the inlet 2 to the outlet 3, via which the perfusion can be done with medium. (Figure 1A shows a bioreactor for the production of tissue with removable wires as placeholders for channels and conduits). Possibly. is an additional colonization of the channels with vascular wall cells (smooth muscle cells, endothelial cells) possible sequentially. The growth of a vascular system is possibly promoted by the hydrodynamic stress, which acts on the close environment of the artificial vessel walls due to the pulsatile perfusion. This tissue-engineered, artificial, blood vessel system and its vascular sprouts that develop during the cultivation period take over the distribution of the perfusion medium and thus the supply of the surrounding tissue.
Alternatively, it is also possible to lay a preformed, possibly resorbable hollow fiber system 5, which then serves as a pipe system for the supply (FIG. 1B, elastic bioreactor for the production of tissue using a hollow fiber system). The resorbable or permanent waveguide system 5 integrated into the bioreactor or the distribution via the porous framework initially takes care of the supply of the tissue, if necessary, until it can supply itself due to the development of its own vascular system or until it is implanted. It is later resorbed and replaced with vessels or functionally resorbed if blood flow through collateral blood supply after transplantation is sufficient.
Fig. 1 C zeigt einen elastischenIt is further possible to use only a porous scaffold 4, whose pores are flowed through by the nutrient medium. Fig. 1 C shows an elastic
Die in Figur 1 C im Inneren ersichtlichen Pfeile zeigen die Fließrichtung des Mediums in dem porösen Gerüst an.Bioreactor for the production of tissue using a porous framework for medium distribution. Possibly. can channels and lines 7 (with a larger pore or Channel diameter), if necessary, be incorporated from resorbable material, so that even with proliferation of the cells, a flow is maintained, and a settlement with the vessel wall cells is possible, so that vessels can form. The arrows which can be seen in the interior in FIG. 1C indicate the direction of flow of the medium in the porous framework.
Beispiel 3 - Bioreaktor für Weichgewebeersatz (Fig. 1 C)Example 3 - Bioreactor for Soft Tissue Replacement (Fig. 1C)
Von dem zu versorgenden Weichgewebedefekt wird ein virtuelles 3 D-Modell erzeugt, auf dessen Grundlage ein formgenaues Gerüst (Scaffold) 4 (weichbleibend) mittels CAD/CAMThe soft tissue defect to be treated is used to create a virtual 3-D model, on the basis of which a shapely framework (scaffold) 4 (soft) with CAD / CAM
Durch die Porosität desTechniques is made. The framework is porous and contains channels 7 for perfusion which open at the pre-calculated locations for inlet 2 and outlet 3. Due to the porosity of the
(Die in Figur 1 C im Innern ersichtlichen Pfeile zeigen die Fließrichtung des Mediums in dem porösen Gerüst an.)The framework ensures that the medium can be distributed sufficiently throughout the framework from the lines. (The arrows visible in FIG. 1C in the interior indicate the direction of flow of the medium in the porous framework.)
The framework is then sheet-like covered with an elastic plastic, e.g. by deep drawing or by coating (preferably silicones). At the predetermined inflows and inlets of probes prefabricated fittings are polymerized. This creates an individual reactor for an individual defect. The colonization can then be carried out by inoculation with suspended cells (possibly several times), possibly sequentially (first mesenchymal cells of the mesenchyme, then vessel wall and endothelial cells for the vessels). The method can in principle be applied to any vascularized tissue.
Beispiel 4 - Zusätzliche Integration von Vorrichtungen in die elastische Hülle (Kammerwandung) des BioreaktorsExample 4 - Additional integration of devices into the elastic sheath (chamber wall) of the bioreactor
Depending on which material is used in the production of the bioreactor, this affects the transparency of the chamber wall. Therefore, it may be necessary, especially in the case of materials that are not or not sufficiently transparent To integrate viewing windows for optical monitoring (monitoring and control) in the wall (see example transparent film as a monitoring window 26 in the experimental bioreactor according to Fig. 4a and 4b). Furthermore, additional device for regulating the local or total resilience of the bioreactor system based on eg highly elastic, biocompatible membranes can be integrated into the wall.
The monitoring (monitoring and control) of the growth parameters inside the bioreactor can be done via a responsive probe system which is installed via predefined ports in the chamber. In this case, substance concentrations and physical or chemical parameters, such as O 2 and CO 2 concentration, pressure, oxygen partial pressure, pH, viscosity, flow rates and temperature are measured. The monitoring also actively contributes to the regulation of growth conditions in the bioreactor system, since it is integrated into the control loop as sensor technology.
Beispiel 5 - Herstellung eines Blutgefäßsystems mittels Tissue EngineeringExample 5 - Preparation of a blood vessel system by means of tissue engineering
An artificial, supplying blood vessel system produced by means of tissue engineering and its vessel sprouts developing during the cultivation period take over the distribution of the perfusion medium and thus the supply of the surrounding tissue.
Beispiel 6 - Herstellung von Blutgefäßen oder Blutgefäßnetzen, anderen Geweben (Fig.2a und b sowie 3a und b)Example 6 - Preparation of blood vessels or blood vessel nets, other tissues (FIGS. 2a and b and 3a and b)
The simplest geometry is in the production of a single blood vessel. Here, the bioreactor for the production of a blood vessel according to Fig. 2a and Fig. 2b consists only of a cylindrical elastic body 8, which corresponds to the outer diameter of the blood vessel. At the ends (FIG. 2) there are in each case couplings / connections 9 at which the vessel / vessel equivalent 10 (eg an elastic, resorbable Framework material with a tubular shape) can be clamped (Fig.2a). It is pushed onto Schlaucholiven 11 and this in turn placed in a Luer-lock holder on the connection 9 of the bioreactor, whereby the seal is achieved (on both sides). Then the construct can be perfused with medium and colonized with cells, if not already done before clamping (smooth muscle cells and / or progenitor cells and / or endothelial cells, possibly sequentially). The arrows in Fig. 2a and Fig. 3a correspond to the direction of the medium flow.
If possible, perfusion is performed using a pulsatile perfusion mode, which simulates the blood pressure conditions in natural vessels, or slowly increases from low pressures to physiological pressures. This slowly forms a natural, resistant (against pressure) Gefaßwand, with physiological compliance, etc. (In Figure 2a, two dotted lines are shown, which represent the elastic sheath 1 when deflected by perfusion (overdrawn).) In the same way, a blood vessel network instead of the tubular construct and bioreactor, a more complicated geometry of a branched network 12 according to FIG. 3 is used. (Figure 3a shows in plan view and Figure 3b in cross section the elastic bioreactor for the preparation of a blood vessel network or a blood vessel network equivalent.) The basic procedure is identical.
Beispiel 7 - Herstellung eines vaskularisierten Gewebes mit kompartimentiertem BioreaktorExample 7 - Preparation of a vascularized tissue with compartmented bioreactor
Beispiel 8 - Anschluss und Betrieb des selbstregulierenden pulsatorischen PerfusionssystemsFor some applications it makes sense not to make the whole construct in one step. This is possible with the compartmentalized version of the bioreactor. To produce a vascularized soft tissue, for example, the blood vessel network, as described in Example 6, is first prepared in a compartment and then a dividing wall is opened to the second, sterile, still unused compartment. There, the actual graft tissue or equivalent is then deposited (as a scaffold with cells, settable scaffold or particles, or scaffold-free with cells), so that it is fed via the existing vascular network. Example 8 - Connection and operation of the self-regulating pulsatile perfusion system
A self-regulating pulsatory perfusion system is connected to the bioreactor or to the hollow conduit system established in it and serves to simulate physiological or experimental pressure conditions.
Beispiel 9 - AnwendungenExample 9 - Applications
Applications for the bioreactor of the invention arise wherever the interactions between vessel and stroma or mesenchymal or other tissues play a role. These are many areas besides the already outlined applications in regenerative medicine and tissue engineering. As outlined in the previous examples, the system can be operated with natural, explanted tissues and vessels in the same way as tissue equivalents or artificial tissues. This results in a broad scope. These may be, for example, basic studies, in particular in the study of circulatory diseases, but also of many metabolic disorders, such as e.g. Obesity, in which the interplay of vessels and fat cells plays an essential role. Furthermore, it may be useful as a metastatic model in oncological research. Questions about wound healing can be answered, and it can also be used as an angiogenesis model in basic research. An important branch is also the application in the testing of pharmaceuticals, e.g. the testing of the transfer of drugs to the interstitium or other issues. Here, and in other applications, it can also be used as a substitute for animal testing.
Beispiel 10 - Variante für experimentelle Zwecke (Fig. 4a und 4b)Example 10 - Variant for Experimental Purposes (Figures 4a and 4b)
A miniaturized variant for experimental applications, in which a Vascular equivalent or blood vessel 10 together with a tissue section (Kontrukt / tissue piece) 21 is cultivated so that it can be subjected to comprehensive monitoring (monitoring and control) is described below (Figure 4a and b). It consists in that a slim holder 14 is integrated into the chamber wall and connects the two end faces, at the ends (of the holder) are fastened in each case connections 16.1 and 16.2, which serve to connect a blood vessel or blood vessel equivalent. The port 16.2 at the inlet 2 must be designed so that the vessel / vessel equivalent 10 through the large opening for loading and assembly 17, with screw cap 25, introduced sterile and can be coupled to the end face 18.2. For this purpose, the end face 18.2 is provided with a smaller opening with a flange through which a coupling 19 can be introduced from the outside with a Schlaucholive on which the vessel / Gefaßäquivalent 10 is fixed. This coupling 19 is for example attached to the flange in a liquid-tight manner with a Luer-Lock principle and fixes the vessel / vessel equivalent. Thereafter, the vessel / vessel equivalent is attached to the port 16.1 (eg Schlaucholive). The conduit for the drain 3 is guided in the holder 8 or along it (14.1). Thus, a tubular construct is perfused, all other features of the chamber are retained. In this way, a blood vessel can be produced or simulated by means of tissue engineering that is in direct contact with a supplied tissue section (construct / tissue piece) 21. As a result, it is possible, for example, to investigate the conditions under which sprouts (small blood vessels) from the central vessel enter the adhering tissue. If one uses human or animal blood vessels or tissue instead of tissue engineered constructs, one can also study physiological or pathological processes in vitro that were previously reserved for animal experiments. This preferably applies to pathological and physiological processes on vessels or to the circulatory system, for obesity research and for the testing of pharmacological substances in which the interactions between blood vessels and tissue play a role.
