  • the neck is palpated and anatomically assessed, and the site of surgery is indicated with a marker 78 as shown in fig. 17 .
  • the slide ring 70 of the first engagement means of the inner branch 52 is attached to the endotracheal tube 72 as shown in fig. 18 .
  • the slide ring 70 is conveyed slidingly to reach the position shown in fig. 19 past the vocal cords, i.e. so far that the transition at the second end of the first branch 52 into the first coupling part 57 reaches a tube marker of e.g. 28 cm for an adult patient, or hits the inflated cuff 73, thereby locating the receiving means 55 of the first end 54 of the first branch 52 distanced from the vocal cords and between tracheal cartilage rings.
  • the first coupling part 57 of the first branch 52 now protrudes from the mouth 76 of the patient 75 ready for mating with the second coupling means 62 of the second branch 53.
  • the penetration means 60 is placed in the guide means 59 of the second branch 53, which is then moved in the direction of the arrow A3 shown in fig. 19 to reach the joined position of the first and second coupling parts 57,62 as shown in fig. 20 .

  • the penetration means shaft 82 is removed from the guide means 57 in the direction of the arrow A5 seen in fig. 21b leaving the tubular penetration guide tube 81 behind for insertion of the second engagement means 69 as indicated with the arrow A6 in fig. 21c .
  • the second engagement means 69 includes a barb 71 for coupling together, with the first engagement means 55, e.g. a track or groove in the first branch 52 as shown in fig. 21d and 21e .
  • the engagement of the first 55 and second 69 engagement means may elicit a click noise informing the operator of the engagement.
  • the second branch 53 is then detached from the tubular penetration guide tube 81 and removed in the direction of the arrow A7 in fig. 21e .
  • the dilator assembly 1 is secured to the line 63 and the dilator assembly 1 is now retracted inside the trachea, as indicated by arrow A8 in fig. 21d , together with the tubular penetration guide tube 81 through which the line 63 passes by gently pulling the first branch 52 out of the mouth, again by sliding along the endotracheal tube 72.
  • the U-shaped dilator element 2 with its stabilizer element 4 and connected retractor element 3 thereby dilates the tracheostoma 84 to desired dimension as shown in fig. 23 when said dilator assembly components are moved into the trachea as indicated by the bend arrow A9 in fig. 23 .
  • the stabiliser element 4 is removed in the direction of the arrow A10 as shown in fig. 24 .
  • the retractor element 3 is then pivoted caudally until it is locked in the tracheostoma 84 and the first end part 25 of retractor element 3 is arranged substantially perpendicular to the neck.
  • the endotracheal tube 72 can be inspected or alternatively palpated with a finger.
  • the endotracheal tube 72 is un-cuffed and gently pulled out until its tip is positioned just above the first end part 25 of retractor, that is below the retractor opening and as illustrated in fig. 26 an appropriate tracheostomy tube 85 is introduced in the tracheostoma 84, as indicated by arrow A11.
  • Ventilation through the tracheostomy tube 85 is established and the retractor element 3 is disconnected from the dilator element 2, which is fully removed via the mouth as indicated with the arrow A112in fig. 27 .
  • retractor element 3 is moved out of the tracheostoma 84 as indicated by the arrow A12 in fig. 28 to leave the tracheostomy tube 85 in place in the artificial ventilation opening 84 in the neck, the tracheostoma 84, as seen in fig. 29 .
  • the procedure can also be done in the reverse order, in which case the retractor element is removed first and the dilator element and endotracheal tube removed in a second step.
  • the dilator assembly is pulled inside the trachea via the mouth and out through the incision in the anterior neck wall to thereby dilate the penetrated neck wall and creating a tracheostoma.
  • the proximal end part serves in this embodiment as the retractor, in which case the proximal end part may be configured as a temporary insertion guide for the tracheotomy tube.
  • the first branch may expediently serve for positioning the first engagement means inside the trachea at the location intended for the stoma.
  • the penetration means may include a third engagement means for engaging the first engagement means at the end of the line so that when the penetration means are withdrawn from the guide means the line will be tighten by the engaged first and third engagement means.
  • the dilator element can be retracted inside the mouth along the endotracheal tube and out through the incision for dilating said incision and subsequent installation of the tracheotomy tube.

Claims (14)

  1. Eine Dilatationsvorrichtung (1), die für eine perkutane Dilatationstracheostomie bei einem Patienten mit einem bereits zur Beatmung eingesetzten Endotrachealtubus (72), wobei die Dilatationsvorrichtung ein Dilatationselement (2) mit einer Wand (11) und einem proximalen Endteil (5), das sich in ein distales Spitzenteil (6) über ein Zwischenteil (7) erstreckt, dadurch gekennzeichnet, dass wenigstens die Wand (11) des proximalen Endteils (5) des Dilatationselements (2) einen im Wesentlichen U-förmigen Querschnitt hat, der über wenigstens die Länge des proximalen Endteils (5), eine Dilatationsöffnung (20) definiert, wobei die Dilatationsöffnung (20) den bereits eingesetzen Endotrachealtubus ergreift und aufnimmt.
  2. Eine Dilatationsvorrichtung (1) nach Anspruch 1, dadurch gekennzeichnet, dass die Wand (11) des Zwischenteils (7) des Dilatationselements (2) einen im Wesentlichen U-förmigen Querschnitt hat.
  3. Eine Dilatationsvorrichtung (1) nach Anspruch 1 oder 2, dadurch gekennzeichnet, dass wenigstens das proximale Endteil (5) und/oder das Zwischenteil (7) des Dilatationselements (2) über ihre Länge mit demselben oder einem unterschiedlichen Krümmungsradius gekrümmt ist.
  4. Eine Dilatationsvorrichtung (1) nach einem der vorangehenden Ansprüche 1, 2 oder 3, dadurch gekennzeichnet, dass wenigstens ein Teil der Länge des Dilatationselements (2) sich von dem proximalen Endteil (5) in Richtung auf den distalen Spitzenteil (6) verjüngt.
  5. Eine Dilatationsvorrichtung (1) nach einem der vorangehenden Ansprüche 1-4, dadurch gekennzeichnet, dass die Dilatationsvorrichtung (1) weiter ein Retraktionselement (3) hat, das mit dem Dilatationselement (2) zusammenwirkt.
  6. Eine Dilatationsvorrichtung (1) nach Anspruch 5, dadurch gekennzeichnet, dass der proximale Endteil (5) des Dilatationselements (1) eine erste Kupplung (8) zum Koppeln mit einer zweiten Kupplung (37a, 27b, 38) an dem Retraktionselement (3) hat.
  7. Eine Dilatationsvorrichtung (1) nach einem der Ansprüche 6 oder 6, dadurch gekennzeichnet, dass
    - ein erstes Endteil (25) des Retraktionselements (3) zum Ergreifen des proximalen Endteils (5) des Dialtationselements (2) eingerichtet ist,
    - ein gegenüberliegendes zweites Endteil (26) des Retraktionselements (3) in Richtung auf ein freies Ende (27) vorragt, und
    - die zweite Kupplung (37a, 37b, 38) des Retraktionselements (3) zum Koppeln mit der ersten Kupplung (8) des Dilatationselements an dem ersten Endteil (25) oder dem zweiten Endteil (26) des Retraktionselements (3) angeordnet ist.
  8. Eine Dilatationsvorrichtung (1) nach einem der vorangehenden Ansprüche 5, 6 oder 7, dadurch gekennzeichnet, dass wenigstens ein längsverlaufender Teil der Wand (24) des Retraktionselements (3) einen im Wesentlichen U-förmigen Querschnitt hat, der eine Retraktionsöffnung (34) entlang wenigstens eines Teiles der Länge des Retraktionselements (3) begrenzt.
  9. Eine Dilatationsvorrichtung (1) nach einem der vorangehenden Ansprüche 5-8, dadurch gekennzeichnet, dass wenigstens ein Teil der Länge des Retraktionselements (3) gekrümmt ist.
  10. Eine Dilatationsvorrichtung (1) nach einem der vorangehenden Ansprüche 5-9, dadurch gekennzeichnet, dass die Dilatationsvorrichtung (1) weiter ein Stabilisierungselement (4) zum Stabilisieren des Dilatationselements und des Retraktionselements (3), wenn das Dilatationselement (2) und das Retraktionselement (3) zusammengekoppelt sind, aufweist.
  11. Eine Vorrichtung zum Erleichtern der Tracheostomie von der Art mit
    - einem ersten Zweig (52) für eine Anordnung im Inneren der Trachea (74),
    - einem zweiten Zweig (53), der mit dem ersten Zweig (52) zum Erzeugen eines Tracheostomas (84) zusammenwirkt,
    - wobei der erste Zweig (52) ein erstes Ende (54) hat, das in die Trachea (74) einzuführen ist, wobei das erste Ende (54) eine Aufnahme (55) und ein gegenüberliegendes Ende (56), das mit einem ersten Kupplungsteil (57) versehen ist, hat,
    - der zweite Zeig (53) ein erstes Ende (58) mit einer Führung (58) für ein den Hals durchdringendes Mittel (60) und ein gegenüberliegendes zweites Ende (61) mit einem zweiten Kupplungsteil (62) zum Koppeln mit dem ersten Kupplungsteil (57) des ersten Zweiges (52) hat, und
    - der erste Zweig (52) und der zweite Zweig (53) lösbar an den jeweiligen zweiten Enden (56, 61) aneinandergekoppelt sind,
    dadurch gekennzeichnet, dass
    - die Vorrichtung weiter die Dilatationsvorrichtung (1) nach einem der vorangehenden Ansprüche 1-10 aufweist.
  12. Eine Vorrichtung nach Anspruch 11, dadurch gekennzeichnet, dass die Dilatationsvorrichtung (1) weiter derart ausgebildet ist, dass das distale Spitzenteil (6) des Dilatationselements (19) mit einer Leine (63) versehen ist, wobei die Leine (63) ein freies Ende (64) mit einem ersten Eingriffsmittel (69) hat und das erste Ende (58) des ersten Zweiges (52) ein zweites Eingriffsmittel (55) zum Ergreifen des ersten Eingriffsmittels (69) hat, das in die Trachea (74) durch die Führung (59) des zweiten Zweiges (53) eingesetzt ist.
  13. Eine Vorrichtung nach Anspruch 12, dadurch gekennzeichnet, dass das erste Eingriffsmittel (69) ein Steckerteil und das zweite Eingriffsmittel (55) ein Buchsenteil ist oder umgekehrt, wobei das Steckerteil und das Buchsenteil miteinander zur Verriegelung ausgebildet sind.
  14. Die Vorrichtung nach einem der Ansprüche 11, 12 oder 13, dadurch gekennzeichnet, dass das Dilatationselement (2) zum Lösen des Retraktionselement (3) angeordnet ist zum Verlassen des Retraktionselements (3) in situ in den Tracheostoma (84) in Antwort auf die Aufbringung einer Zugkraft in die Leine (63).
