Definitions

  • the present invention relates to the field of orthopaedic surgery and, more specifically, to the technical sector relating to devices used in a hip prosthesis.
  • the invention relates to a universal acetabular cup for a hip prosthesis which is improved by comparison with prior art acetabular cups, as described in the preamble of Claim 1, together with a use of the acetabular cup for the construction of a hip prosthesis as described in Claim 5.
  • the dependent Claims 2-4 represent further advantageous embodiments of the invention.
  • the acetabular cup forms part of the hip prosthesis and is a recessed component with a characteristic bowl shape in which the head of the stem rotates.
  • the stem is the other part of the prosthesis, which is inserted into the patient's femur.
  • the acetabular cup is housed in the acetabulum of the patient's pelvis, and forms the hip prosthesis in combination with the stem and its head.
  • acetabular cups there are many existing designs and geometrical shapes of acetabular cups at the present time, enabling the patient's requirements to be met according to the physiological conditions present in the patient's acetabulum. If the inner structure of the acetabulum is not degraded, the typical acetabular cup design is oversized with respect to the dimensions of the housing in the acetabulum, thus providing a "press-fit" connection between the acetabular cup and the said housing.
  • extension is simply an appendage which is provided on the acetabular cup and which is inserted into the iliac bone to fix the acetabular cup and so prevent it from moving with respect to the acetabulum: in other words, the extension enables the acetabular cup to be secured to the acetabulum.
  • a person skilled in the art namely an orthopaedic surgeon, uses the term "modified acetabular cup".
  • the extension is fixed by pressure, or by equivalent connecting means, to the acetabular cup to form a permanent connection; it is also possible for the extension to be integrated into the acetabular cup.
  • the stability of the implant is dependent on the connections between the acetabular cup and the appendage, and between the appendage and the patient's bone structure.
  • the appendage is fixed to the bone by the forcible penetration of one end of the extension into a substantially complementary cavity formed in the bone.
  • the object of the present invention is to provide an acetabular cup which can be used in any physiological condition of the acetabulum which is encountered in the patient, in other words an acetabular cup of a modular type which is adaptable to multiple configurations and fixing methods.
  • a further object of the invention is to provide an acetabular cup comprising a fixing with greater mechanical stability than that of the known designs.
  • the acetabular cup with an appendage, particularly an extension, of the known type has the disadvantage that it is not easy to insert the appendage into the acetabular region; the physiology of the acetabulum is different in each patient to be fitted with a prosthesis, and existing modified acetabular cups are not universal, but are designed for a specific range of fixing conditions, as described in GB 2 366 733 in the name of Corin.
  • the acetabular cup proposed by the invention has a plurality of through holes formed in the hemispherical body of the acetabular cup, with multiple inclinations with respect to the equator (i.e. the holes are on multiple parallels), by means of which the following can be connected, according to the physiological conditions inside the acetabulum:
  • an iliac stem 2 the term “iliac stem 2” denotes an appendage which can be positioned in a cavity of the iliac bone.
  • an iliac stem 2 and at least one extension 2a the term “extension” denotes an appendage whose dimension (i.e. cross section) is smaller than that of the iliac stem 2 and which is introduced from the inside of the acetabular cup so as to penetrate (and be fixed in) the bone; the latter element (the extension 2a), when used in combination with the iliac stem 2, has a supplementary securing function.
  • one or more extensions 2a also having a smaller cross section than an iliac stem 2 as mentioned above, all introduced into the bone through the holes 4 provided in the acetabular cup body Ia, these extensions being housed in the acetabular cavity after passing through the said holes 4.
  • the iliac stem 2 which is an element of the invention, is fixed to the acetabular cup, and specifically to the substantially hemispheric body of the acetabular cup Ia, by means of a screw V or any other equivalent fixing means.
  • the iliac stem 2 has a (female) thread 3 at one end for receiving a screw V.
  • the screw V is the means of connection between the appendage and the acetabular cup body Ia.
  • the extension 2a which is also an element of the invention, is characterized by a smaller cross section than that of the iliac stem 2, and is fixed to the acetabular cup body Ia by means of an extension fixing plug Vl.
  • the principal advantage of the invention is the modular nature of the acetabular cup, due to the multiple holes present on the parallels of the hemispheric body (of the acetabular cup), which enables the orthopaedic surgeon with a wide range of possible geometrical configurations for fixing the appendage, particularly the iliac stem, in such a way as to ensure good contact of the outer surface of the acetabular cup with the hollow bone region (i.e. the acetabulum) of the patient.
  • the invention therefore proposes a universal acetabular cup, also known as a modular acetabular cup for the reasons mentioned in the preceding paragraph, for a hip prosthesis, as described in the appended Claim 1.
  • Figure 1 is a lateral view of two appendages 2 and 2a, namely an iliac stem 2 provided with a thread at one end which serves to lock the acetabular cup, and an extension 2a, where the thread is used solely for extraction, since the locking is carried out by means of a suitable plug Vl .
  • Figure 2 shows a complete lateral and cross-sectional view of one embodiment of the implant proposed by the invention, fitted in the acetabulum.
  • the implant is fixed by means of two appendages, namely an iliac stem 2 and a supporting extension
  • the acetabular cup body Ia is substantially hemispheric, and is provided with through holes 4 which have different inclinations with respect to the equator
  • FIG. 3 shows the hemispheric body Ia of the acetabular cup, viewed from above, illustrating the through holes which are positioned with different inclinations with respect to the equator E (i.e. on different parallels).
  • the acetabular cup for a hip prosthesis proposed by the invention comprises a body of substantially hemispheric shape Ia 5 and an extension 2, for fixing the prosthesis into the pelvis, particularly by fixing it to the iliac bone or to other surrounding bony regions or to both, so as to prevent any movement of the acetabular cup with respect to the acetabulum.
  • the acetabular cup 1 is provided with a plurality of through holes 4, formed in the substantially hemispheric body Ia of the cup, these holes being characterized in that their axes converge on the centre of the acetabular cup body C.
  • the said holes are also at different inclinations (i.e. on different parallels) from each other with respect to the equator of the acetabular cup; in particular, the said inclinations vary over a range from about 30° to 70° .
  • the plurality of holes allows the orthopaedic surgeon to choose from the multiple possible configurations of the acetabular cup, the appendage and the acetabulum, thus adapting the implant to the specific morphological conditions (of the acetabulum) of the patient.
  • the fact that the aforementioned axes coincide allows an optimal fixing of the acetabular cup and the extension.
  • the appendage is substantially of two types:
  • An iliac stem provided with a conical or cylindrical end which can be introduced into the iliac bone to secure the acetabular cup to the said bone, the other end being in contact with the outer surface of the acetabular cup body Ia and having a female thread 3, or an equivalent fixing device, having an axis coinciding with the axis of a through hole 4.
  • the stem is connected permanently to the acetabular cup body Ia by a fixing means, in particular a screw V.
  • the orthopaedic surgeon initially fixes the stem 2 in the appropriate cavity prepared in the iliac bone, and then introduces the acetabular cup into the acetabular cavity, after which he fixes the cup to the stem by means of a screw V or any equivalent element.
  • the end of the stem nearer the body of the acetabular cup bears on the latter at a point coinciding with a through hole 4; more specifically, the axis of the stem coincides with the axis of the through hole 4.
  • An extension having a smaller cross section than that of the iliac stem and provided with a conical or cylindrical end which can be introduced into the bony region of the acetabulum.
  • the said extension has a cross section such that practically the whole of its length can pass through the through holes 4, and its outer shell is provided with a stop element, for example a shoulder S, having a shape such that it prevents the axial movement of the extension with respect to the acetabular cup body when the extension has been introduced into the hole.
  • the extension is locked and/or fixed permanently to the hemispheric body of the acetabular cup by fixing means, particularly a threaded plug Vl.
  • the smaller geometrical dimensions (i.e. cross section) of the extension with respect to the iliac stem are due to the difference in function between the two appendages.
  • the two aforementioned types of appendage can be used in a separate or complementary way.
  • the axes of the holes 4 used for fixing coincide with the axes of the appendages used, regardless of whether they are stems or extensions, thus enabling an optimal mechanical connection to be made.
  • the fact that the aforementioned axes coincide allows an optimal fixing of the acetabular cup and the extension.
