Zenner et al. ("Totally implantable hearing device for sensorineural hearing loss", The Lancet, Vol. 352, Nov. 1998, No. 9142, Seite 1751) sowie in zahlreichen Patentschriften, so unter anderem in US-A-5 360 388, US-A-5 772 575, US-A-5 814 095 und US-A-5 984 859. The The principle in the predominant embodiments is an ossicle of the middle ear or the inner ear directly via a mechanical or hydromechanical stimulus stimulate and not via the amplified acoustic signal of a conventional hearing aid, in which the amplified sound signal is fed to the external auditory canal. The actor These electromechanical systems are stimulated with various physical Transducer principles implemented, such as by electromagnetic and piezoelectric Systems. The advantage of this method is mainly that of conventional Hearing aids improved sound quality and in the case of fully implanted systems in invisibility seen the hearing prosthesis. Such partially and fully implantable electromechanical Hearing aids have been described by H.P. Zenner et al (ENT 1998, 46: 844-852), H. Leysieffer et al. ("A fully implantable hearing aid for the hearing impaired: TICA LZ 3001 ", ENT 46: 853-863, 1998) and H.P. Zenner et al. (" Totally implantable hearing device for sensorineural hearing loss ", The Lancet, Vol. 352, Nov. 1998, No. 9142, Page 1751) and in numerous patents, including US-A-5 360 388, US-A-5 772 575, US-A-5 814 095 and US-A-5 984 859.
Recently, such partially and fully implantable hearing systems for rehabilitation have become one Inner ear damage in clinical use. This shows the need to Implantation of upcoming patients the expected hearing improvement or sound quality to demonstrate preoperatively. This is with well-known audiological procedures that are still used today only work with airborne sound or structure-borne sound stimulation of the hearing, without operative Intervention not possible.

With the device proposed in DE 196 18 961 A1 and DE 196 18 945 A1 In the meantime, a clinical trial was carried out on normal hearing test subjects who statistically significant showed that this method is reproducible and valid as well can be used without restrictions on the safety of the test subjects.
Basically, however, there is the problem that when using the device and the There is an individually different hearing in patients with hearing loss. These differences relate in particular to spectrally very different courses of the resting hearing threshold as well as a possible positive recruitment for transient signals (Increase in loudness perception) and a reduced frequency resolution. For the preoperative demonstration of an implantable hearing system with the specified devices and methods can hardly be achieved because an individual Compensation of the hearing loss in question is not possible, i.e. an adjustment of the Signal processing electronics in the sense of an electromechanical converter A hearing aid cannot be fitted. This necessarily results in the serious disadvantage of the proposed devices and methods that the patient is on under no circumstances experienced the auditory impression from the preoperative demonstration, which he later described Implantation and individual adaptation of the hearing implant to its individual Has hearing loss.
It is therefore an object of the present invention, an apparatus and a method for preoperative Demonstration of at least partially implantable hearing aids to create a Allow non-invasive hearing testing, as is the case after implantation and adjustment of an individual hearing implant.
According to the present invention, this object is achieved in a preoperative device Demonstration of at least partially implantable hearing systems for the rehabilitation of Hearing disorders with an electromechanical transducer from the side of the external ear canal forth with at least approximately the center of the eardrum and thus the End point of the hammer handle is non-invasively connectable to the eardrum in mechanical To vibrate, and an electronic audio signal generating unit, solved by a between the audio signal generating unit and the electromechanical converter Connected electronic audio signal processing unit for controlling the electromechanical Converter, which the audio signal processing electronics of each corresponds to implanting implant or the latter, in particular by software and / or Hardware and software, simulated.
In this way, the hearing impaired patient can expect the expected effect and the Sound impression of the implantable hearing system in question conveyed as realistically as possible become.
Preferred embodiments of the invention result from the subclaims.
There are preferably means for adapting the audio signal processing unit to the individual Hearing loss of the patient concerned is provided.
Halbleiterspeichern, analogen Tonträgern (Magnetband), Audio-CD's, CD-Rom's etc. Furthermore, the audio signal generating unit is preferably provided with means for playing back a Data or sound carrier assigned. Coming as test and demonstration audio signals all types of signals that are commonly and familiarly used in audiology like pure sine tones, narrow and broadband noise, speech and music etc. As Data carriers or generation of these test signals also come with all known possibilities considered as analog and / or digital generation or synthesis, analog and / or digital filing in all known types of non-rewritable or rewritable analog and / or digital storage media such as Semiconductor memories, analog sound carriers (Magnetic tape), audio CD's, CD-Rom's etc.
In a further embodiment of the invention, means for storing parameter sets for setting the audio signal processing unit and means for selection and transmission any parameter set provided to the audio signal processing unit his. In this embodiment of the demonstration device according to the invention, different "Standard" parameter sets for setting the audio signal processing unit, in which the individual parameters are coordinated with one another in an advantageous manner Be determined in advance and saved. The operator of the demonstration device can then select any of the saved parameter sets without it being an individual Setting individual parameters is also required to convey more or less optimal parameter settings no in-depth knowledge of the effects individual parameters or the interaction of several parameters with each other, so that the demonstration device is also less trained in this regard Staff can be served.
The audio signal processing unit preferably has a programmable processor unit on, in particular a personal computer (PC) or a digital signal processor (DSP). The term "personal computer" or "PC" used here is to be understood such that he also notebooks, laptops, etc. as well as any other "external" computer, i.e. of the Converter drivers include independent computers.
Here, the programmable processor unit can perform the functions of both the audio signal generation unit as well as the audio signal processing unit.
According to a particularly preferred embodiment, the audio signal processing unit has a driver electronics for driving the electromechanical transducer, wherein then a digital / analog converter between the programmable processor unit and the driver electronics can be connected. Especially when as programmable Processor unit a personal computer is provided, which functions both Audio signal generating unit and the audio signal processing unit executes the driver electronics and the digital / analog converter are integrated in one hardware interface be, which is then connected between the personal computer and the converter.
In contrast, the programmable processor unit is a digital one Signal processor (DSP), so a particularly compact demonstration device can be implemented, if the driver electronics, the digital / analog converter and the digital signal processor are integrated in a hardware interface. To operate this hardware interface too can further facilitate a display for displaying audio signal generation and audio signal processing data be provided, which is also integrated into the hardware interface or that can be connected to it.
If the audio signal processing unit has audio signal processing electronics as well has driver electronics for controlling the converter, as in the respective implanting implant are included, so these two assemblies can be advantageous Be integrated into an interface.
In addition, it is independent of the individual fluctuations in the biological load impedance To achieve the impression of the output-side deflection of the transducer is the electromechanical one Conveniently designed so that its mechanical source impedance in the entire spectral transmission range is significantly larger than the mechanical load impedance, which are formed by the biological system of the eardrum, ossicular chain and inner ear becomes.
The examination can be made even more comfortable for the patient if the electromechanical Transducers through the constructive design of the converter housing acoustically so is encapsulated that the sound signal emitted by the vibrating transducer structures is minimized so that at high stimulation levels an acoustic masking of the contralateral, not examined ear can be dispensed with.
The electromechanical converter can be based on the principle of electrodynamic, electromagnetic, magnetostrictive, capacitive or piezoelectric conversion based, where a electromechanical transducers operating according to the piezoelectric principle are preferred, because magnetic stray fields can be completely avoided in this way.
In a further embodiment of the invention, one with the electromechanical transducer couplable coupling element can be provided, which is from the outside through the outer auditory canal to at least approximately the center of the eardrum and thus to the end point the hammer handle can be placed non-invasively. Here, the coupling element is preferably as Rod-shaped component that is rigid in the axial direction, the component of the converter facing away from the injury-free, mechanical contact to the center of the Drumhead guaranteed, it proves to be particularly advantageous if the coupling element is designed to be easily bendable manually, so that it matches the individual geometric shapes of the outer ear canal can be adjusted.
It is easier for the examining person to insert the device according to the invention and at the same time make it safer for the patient if the electromechanical converter is in a transducer housing to be inserted into the entrance area of the outer auditory canal whose geometric dimensions are selected so that the examining person also using a microscope a clear view of the center of the eardrum retains mechanically contacting end of the coupling element.
Furthermore, the coupling element is not mechanically fixed, but via a mechanical plug connection connected to the converter, so z. B. different coupling elements realize that, for example for hygienic reasons, easily replaceable and as a disposable item are executable.
Preferably, the electromechanical transducer is optionally connected to the mechanical coupling element designed so that the first mechanical resonance frequency on upper end of the spectral transmission range of ≥ 10 kHz. Let it this way short settling times can be achieved due to the broadband nature.
In a further embodiment of the invention, a positioning device for positioning the Transducer provided with respect to the umbo, which allows the transducer, or if this is coupled to the coupling point via a coupling element, the effective end of the mechanical coupling element, for example one with the electromechanical transducer connected coupling rod, precisely to the destination.
To the spatial position between the positioned transducer or the coupling element and to fix the umbo is preferably also a fixing device for the fixed, backlash-free Connection of the positioning device to the human skull is provided.
According to a further preferred embodiment of the invention, there is between the positioning device and the electromechanical transducer an intermediate member arranged so is designed and dimensioned so that there are quasi-stationary positioning settings from the positioning device transmits to the electromechanical transducer, but a transmission at least dynamic forces from the positioning device to the transducer and if necessary, the coupling element at least so far that the risk of a Middle or inner ear damage is significantly reduced.
In the demonstration device according to the invention, the converter including the coupling element follows the relatively slow, in the present case called quasi-stationary changes in position caused by Actuation of the positioning device can be effected. The doctor can use it Coupling element precise and free of relative movements to structures in the human Bring the body, especially the umbo, as the target point. In contrast, one unintended external influences, usually jerky or intermittent, for example by bumping the positioning device by hand, an instrument or the like, dynamic forces acting on the positioning device Transducer and the coupling element kept at least to a considerable extent.
A structurally particularly simple solution is obtained if the intermediate link as Spring member is executed. It has the spring link and the electromechanical Converter and the coupling element including spring / mass system preferably one Natural frequency in the range from 0.5 to 5 Hz.
The method according to the invention is thus carried out in two phases. So in one first phase on a signal memory for different sets of audiological adjustment parameters Output signals filed as an audio signal processing unit of the implanting hearing system. In a second phase, the actual demonstration phase, is then given to a hearing impaired patient who has the auditory impression of the implanting hearing system is to be conveyed, a transducer from the outside through the outside Ear canal non-invasively coupled to approximately the center of the eardrum, whereupon output signals stored on the signal memory are applied to the converter to vibrate the eardrum mechanically. In this way the implant functions can be non-invasive to a possible implant carrier and still be conveyed realistically.
Different settings of the implant can be simulated or demonstrate when output signals are applied to the converter as they are for different Sets of audiological fitting parameters were obtained.
The electromechanical transducer can be used directly or by means of a coupling element through the outer ear canal to the eardrum, at least approximately the center of the eardrum.

In addition, the
Instead of the mode of operation and the signal behavior of the audio signal processing electronics of the The preoperative demonstration system can be used to simulate the hearing aid to be implanted also be designed so that the audio signal processing unit for controlling the electromechanical Converter has audio signal processing electronics, as in the implanting hearing aid is included. This implementation variant of the one proposed here Demonstration system is shown in FIG. 3 illustrates.
Varianten.Here, the
The in FIG. 4 schematically shown further implementation variant largely corresponds to that

In addition to the implant system (IMP) 34, the
In FIG. 5 shows a
Gewichtskraft selbsttätig bewegt.A
Ende der Linearführung 48.The travel path of the
By rotating the
The
The
When the ball joint 44 is released, the entire

einem beliebigen, körperfesten Zielpunkt 14.The connecting element can be released by loosening the clamping
In FIG. 6 is a preferred combination of the
The
At the
When using a
After inserting the conical holding
In FIG. 7 is a further embodiment of the device explained here for preoperative
Demonstration illustrates in which the
Similar to the embodiments according to FIGN. 5 and 6 is the
An
When the ball joint 44 is released, the
In the illustrated embodiment, the
It is understood that the
