Description
Ascorbic acid as an adjuvant in the chemotherapy and radiation therapy of malignancies.
The invention relates to the use of ascorbic acid in combination with chemotherapeutic agents to increase the inhibitory effects of the chemotherapeutic agent and to reduce its serious side effects, and the use of ascorbic acid for radio protection in radiation therapy.
It is known that some cancers, e.g. Lymphoblastic anemia, Hodgkin's disease or testicular tumors respond very well to the constantly improved chemotherapy. However, there are numerous, sometimes very widespread cancers that do not respond adequately to chemotherapy. The administration of chemotherapeutic drugs is often associated with considerable side effects such as hair loss, nausea, vomiting, cardiomyopathy, bone marrow suppression, immune suppression, haematopoietic dysfunction, leukopenia etc. There is therefore a great need both for medicaments which increase the sensitivity of cancer cells to chemotherapeutic agents and for medicaments which mitigate or completely suppress the side effects observed when using chemotherapeutic agents, but also in radiation therapy.
Weiterhin ist aus epidemiologischen Studien bekannt, daß engeIt is also known from epidemiological studies that close
There are correlations between the vitamin C status and the frequency of occurrence of certain types of tumor. Low Vitamin C plasma levels correlate closely with the risk of developing esophageal, laryngeal, oral cavity, pancreatic, stomach, rectal, breast and cervical cancer. By reducing carcino. Generic and mutagenic substances in the organism vitamin C has an essential protective function. The preventive effect of vitamin C in relation to tumor types of the digestive tract can be partly explained by the properties of the vitamin to prevent the formation of nitrosamines. It is also known that vitamin C is one of the most effective antioxidants in the human body. Radicals arise from endogenous metabolic products and are induced by exogenously added substances or radiation. They damage proteins, enzymes, lipids and the genetic material. Studies on human blood plasma have shown that vitamin C is particularly effective in protecting against peroxide radicals. The water-soluble vitamin C regenerates oxidized vitamin E and thereby provides additional protection for the lipid membranes.
One of the first studies on the use of vitamin C in cancer treatment was carried out by Cameron and Pauling.
The group of subjects included 100 terminal cancer patients who received 10 g of vitamin C orally daily. The control group consisted of 1,000 patients who received comparable therapy without vitamin C. An increase in the survival time of the cancer patients treated with vitamin C compared to the control group was observed (1).
Weiterhin ist von Cameron ein Behandlungskonzept zur Therapie von Krebspatienten beschrieben worden (2), das auf einer adjuvanten, hoch dosierten, intravenösen Vitamin C TherapieCameron has also described a treatment concept for the therapy of cancer patients (2) based on adjuvant, high-dose, intravenous vitamin C therapy
1. Verbesserung des Allgemeinbefindens und des Karnofsky- Indexes,(0.5 to 10 g / day), which is followed by continuous oral administration of vitamin C (10 to 30 g). The following therapeutic effects were described: 1. improvement of general well-being and the Karnofsky index,
2. Erleichterung der Schmerzen bei skeletalen Metastasen,2. Relief of pain in skeletal metastases,
3. Absinken der Proteintumormarker im Serum,3. sinking of the protein tumor markers in the serum,
In favorable cases, resorption of malignant pleural effusions and reduction of the pulmonary metastasis size (2).
There are also case studies in the literature which describe an improved general condition and spontaneous remissions in cancer patients under high-dose vitamin C therapy (3) and (4).
The positive results of Cameron and Pauling (1) could later not be partially confirmed, but in vitro tests on cell lines have given indications that vitamin C is cytotoxic to tumor cell lines in certain concentrations. The results obtained from cell cultures cannot, however, be easily transferred to in vivo conditions. However, it is considered certain that vitamin C has a cytotoxic effect on tumor cell lines in certain concentrations. The growth of normal cell lines is therefore not affected in these concentrations. Vitamin C has a differentiated effect on the growth of neoplastic and normal cell lines. Based on the examinations carried out to date, it has not yet been possible to decide whether vitamin C is a tumor-cytotoxic therapeutic.
Wichtiger als das tumor-zytotoxische Potential des VitaminsMore important than the tumor cytotoxic potential of the vitamin
Darüberhinaus werden die Schmerzen, insbesondere bei skeletalen Metastasen, vermindert und Nebenwirkungen wie Übelkeit und Erbrechen reduziert.C are the effects of this vitamin on the entire organism weakened by cancer. Vitamin C increases the body's defense mechanisms in a variety of ways. This affects both the cellular and the humoral immune response of the body. Vitamin C stimulates lymphocyte blastogenesis, stimulates the mobility and chemotaxis of neutrophils, eosinophils and monocytes and leads to a significant improvement in the blood count by stabilizing the number of leukocytes and platelets despite simultaneous chemotherapy. In the area of the humoral immune response, higher vitamin C administration leads to an increase in antibody production and stimulation of phagocytosis. Even in the presence of carcinogens such as dibutylamine and sodium nitrite as nitrosamine formers, which normally lead to a suppression of the cellular and humoral immune response, vitamin C increases the immune system. In addition to strengthening the immune system, vitamin C is also essential for the integrity of the membrane and the stability of the connective tissue. In addition, the pain, especially in the case of skeletal metastases, is reduced and side effects such as nausea and vomiting are reduced.
It has also been observed that enteral nutrition is often disturbed in terms of quantity and absorption during chemotherapy and radiation therapy. The antioxidant vitamins α-toxopherol, carotene and ascorbic acid are particularly affected. At the same time, however, the intracellular antioxidative potential is often reduced by cytostatics. Especially with the anthracyclines, mitomycin and etoposide it can be assumed that the cytotoxic effect is largely due to the formation of aggressive radicals. The consequences of these oxidative processes induced by chemotherapeutic agents are damage to membranes, protein, enzymes and the genetic material.
To check whether a loss-compensating substitution of antioxidant vitamins is possible, an investigation was previously carried out on 22 patients. The patients received an oral dose of 825 mg DL-α tocopherol, 45 mg beta-carotene and 450 mg ascorbic acid daily 3 weeks before the start of chemotherapy and radiation therapy. In addition to significantly reduced lipid peroxide concentrations in the blood, supplementary patients also found evidence of reduced hepatotoxicity in chemotherapy (5). However, it was not possible to determine from this the influence of the antioxidant vitamins taken orally on the antineoplastic effects of chemotherapy and radiation therapy and whether their side effects are alleviated. Since the absorption after oral administration can also be impaired during chemotherapy or radiation therapy, nothing could be inferred from these experiments about the effects of a high-dose vitamin C preparation administered parenterally over a period of several weeks.
It has now been found that ascorbic acid or its salts significantly improve the success of chemotherapy and radiation therapy and reduce its side effects when used as an adjuvant in an amount of more than 5 g in chemotherapy or radiation therapy. In general, the ascorbic acid or its salts are used in an amount of 5.0 to 30.0 g, preferably in an amount of 5.0 to 10.0 g. The parenteral preparation should have a pH between 6.0 and 8.0. An increase in the inhibitory effects of the chemotherapeutic agent and a tissue-protective effect during radiation treatment as well as a reduction in the sometimes very serious side effects could be observed. This surprising synergism occurs in vitro when vitamin C therapy is combined with the chemotherapy drugs adriamycin, bleomycin sulfate, mitomycin C, methotrexate, vincristine sulfate, 5-fluorouracil, paclitaxel, doxorubicin and cisplatin. In animal experiments, an extended survival time was demonstrated when combined with adriamycin. The effects of ascorbic acid as an adjuvant in tumor therapy are underlined by the following case studies:
Fall 1case 1
Vitamin C infusions of 7.5 g vitamin C per infusion were administered to a patient with a right ablatio due to carcinoma and subsequent multiple local recurrences and skin metastases. The vitamin C infusions led to a decrease in skin metastases. Vitamin C therapy was superior to the previously administered locally acting cytostatic.
Fall 2Case 2
jedoch keine derartigen Interaktionen. In one patient with metastatic breast cancer and massive bone metastasis, 10 vitamin C infusions, each with 7.5 g vitamin C, were administered. There was a significant improvement in the blood count (stabilization of the leukocytes and platelets) despite simultaneous chemotherapy. Vitamin C therapy significantly alleviated the pain caused by bone metastasis. It is noteworthy that the effects of the cytostatics examined are not affected by the administration of high amounts of ascorbic acid or its salts. It is now certain that many cytostatics have a radical-generating mechanism that has a cytotoxic effect on the tumor tissue. The side effects induced by chemotherapy are also attributed to an increase in the radical level in many tissues. In theory, the additional administration of antioxidants such as ascorbic acid or its salts should lead to a reduction in the radical concentrations and thus to a weakening of the effects of the chemotherapeutic agents. The results available so far show however, no such interactions. None of the cytostatics examined showed a loss of activity when combined with vitamin C. In order to reduce the theoretically not entirely excluded effects in chemotherapy, it is recommended not to administer the ascorbic acid at the same time and together with the chemotherapeutic agent, but the ascorbic acid during the breaks in the chemotherapy to restore the vitamin C, which is reduced by the disease and the chemotherapy. To administer mirror. It is advisable to keep a gap of at least 24 hours between the administration of the chemotherapeutic agent of vitamin C.
It has also been shown that the adjuvant injection of vitamin C increases the radiation tolerance of normal tissue (skin and bone marrow) without having to change the radiation dose required to reduce the tumor tissue. An intravenous infusion of 7.5 g of vitamin C, both before and after radiation, shows clearly positive results. The skin reactions accompanying the radiation can be avoided and the radioprotection of the bone marrow is significantly improved by vitamin C. The radiation tolerance of normal tissue increases by about 20% without the radiation dose required to remove the tumors due to the vitamin C infusions having to be increased. This is also an indication that vitamin C has a differentiated effect on normal and tumor tissue.
