Claims (17)
- Appareil formant agrafeuse chirurgiale (10), comprenant :
un logement (13);
un arbre (18) monté de façon non-tournante dans ledit logement pour un mouvement longitudinal relatif à l'intérieur;
une enclume (20) montée sur une extrémité distale dudit arbre; un poussoir d'agrafes annulaire (30) en saillie axialement à partir d'une extrémité distale dudit logement vers ladite enclume;
des moyens (19) pour déplacer ledit arbre dans ledit logement; et
des moyens annulaires pour monter sur ledit logement, en alignement avec ledit poussoir, une cartouche d'agrafage annulaire (33) contenant une pluralité d'agrafes pour agrafer du tissu disposé entre ladite enclume et ladite cartouche, ledit moyen comprenant un manchon (36) monté de façon coulissante sur ledit logement en vue d'un mouvement entre une position d'extension et une position rétractée;
l'appareil étant caractérisé par des moyens de came (40) entre ledit logement et ledit manchon pour fixer ledit manchon relativement audit logement pendant le mouvement proximal de ladite enclume vers un point prédéterminé relativement audit logement, ledit moyen de came étant positionné relativement audit manchon pour un mouvement relativement audit logement en réponse à la continuation du mouvement proximal de ladite enclume depuis ledit point, par quoi ladite enclume pousse ledit moyen annulaire relativement audit poussoir pour expulser les agrafes d'une cartouche d'agrafes sur celui-ci pour agrafer du tissu entre ladite enclume et une cartouche sur ledit logement. - Appareil formant agrafage chirurgical selon la revendication 1 , dans lequel le moyen de came (40) est actionné par une surface de came (25) sur l'arbre à une localisation intermédiaire à l'intérieur dudit logement.
- Appareil formant agrafeuse chirurgicale selon la revendication 2, dans lequel la surface de came est conique et annulaire.
- Appareil formant agrafeuse chirurgicale selon l'une des revendications précédentes, comprenant un couteau annulaire (29) faisant saillie axialement à partir d'une extrémité distale dudit logement vers ladite enclume; et dans lequel le poussoir d'agrafe annulaire (30) est situé radialement à l'extérieur dudit couteau; et ledit moyen (19) pour déplacer ledit arbre dans ledit logement déplace également ladite enclume vers ledit couteau.
- Appareil formant agrafeuse chirurgicale selon l'une des revendications précédentes, dans lequel le moyen annulaire pour monter la cartouche comprend une bague de montage concentrique à et autour dudit poussoir, pour le montage de ladite cartouche sur celui-ci.
- Appareil d'agrafage chirurgical selon la revendication 5, dans lequel le moyen de came comprend des moyens pour fixer ladite bague relativement audit logement pendant le déplacement de ladite enclume à un point déterminé espacé dudit couteau et pour relâcher ladite bague de sa fixation avec ledit logement en réponse à la continuation du mouvement de ladite enclume vers ledit couteau, par quoi ladite enclume pousse le tissu et ladite cartouche vers ledit poussoir pour expulser lesdites agrafes de ladite cartouche tout en poussant lesdites agrafes à travers le tissu et contre ladite enclume pour le rabattement de celles-ci.
- Dispositif selon l'une des revendications précédentes, dans lequel le moyen de came comprend une pluralité de billes espacées circonférentiellement montées dans ledit logement pour un mouvement radial en réponse à la continuation dudit mouvement proximal de ladite enclume.
- Appareil selon l'une des revendications précédentes, dans lequel ledit logement comprend une paire de sections coaxiales (14, 15) ajustées de façon télescopique l'une dans l'autre.
- Appareil selon l'une des revendications précédentes et qui comprend, en outre, un ressort entre l'une des sections de logement et ledit manchon pour solliciter ledit manchon dans ladite portion en extension.
- Appareil selon la revendication 9, dans lequel l'autre desdites sections de logement (14, 15) comprend une surface extérieure conique (17), et ledit manchon présente une surface intérieure conique (37) pour buter contre ladite surface extérieure dans ladite position en extension.
- Appareil selon l'une des revendications précédentes, dans lequel ledit poussoir est une bague annulaire possédant au moins une rangée annulaire de doigts (31) pour pousser une rangée d'agrafes (31) d'une cartouche (33) montée sur ledit moyen annulaire.
- Appareil selon l'une des revendications précédentes, dans lequel ledit moyen pour déplacer ledit arbre est un câble flexible (19) fixé à une extrémité proximale dudit arbre et un tube flexible (55) autour du câble, son extrémité distale étant fixée audit logement.
- Appareil selon l'une des revendications précédentes, qui comprend, en outre, un moyen de verrouillage amovible (45) pour empêcher un mouvement proximal dudit arbre au-delà d'un certain point.
- Appareil selon la revendication 13, dans lequel ledit moyen de verrouillage comprend un loquet (46) monté pivotant dans ledit logement possédant une extrémité proximale pour buter contre ledit arbre, un moyen de came (51) pour faire pivoter ledit loquet dudit arbre et un câble (53) s'étendant depuis ledit moyen de came pour faire déplacer ledit moyen de came afin de relâcher ledit loquet.
- Appareil selon l'une des revendications précédentes, qui comprend, en outre, au moins une rainure (23) dans ledit arbre (18) et au moins une cannelure (24) sur ledit logement recevant ladite cannelure pour immobiliser ledit arbre en rotation dans ledit logement.
- Appareil selon l'une des revendications précédentes, dans lequel le logement comprend un épaulement annulaire (16) contre lequel le manchon peut buter dans sa position rétractée.
- Appareil selon l'une des revendications précédentes, dans lequel le manchon (36) possède une pluralité d'ouvertures radiales (39) pour décharger ou éventer l'espace entre le manchon et le logement.
