Claims (5)

1. Ensemble d'imprimés principalement destiné à être utilisé par exemple dans les hôpitaux pour enregistrer des détails d'une analyse médicale, comprenant une feuille de support (1) avec un première partie destinée à recevoir des informations concernant un échantillon et une seconde partie comprenant un conteneur (15) pour l'échantillon, dans lequel la première partie de la feuille de support (1) peut recevoir sur elle des informations écrites, l'ensemble comprend une feuille transparente (5) fixée sur trois côtés (6, 7, 8) sur la seconde partie de la feuille de support pour former un sac avec un embouchure ouverte pour contenir l'échantillon et l'ensemble comporte une ligne de pliage (9) autour de laquelle il peut être plié, une ligne d'adhésif (10) étant prévue au voisinage de l'embouchure du sac (5a), caractérisé en ce que la feuille de support (5) est une partie d'une bande continue (2), et la ligne d'adhésif (10) est prévue sur le côté opposé de la ligne de pliage (9) pour venir en contact avec l'embouchure du sac (5a) lorsque l'ensemble est plié, l'embouchure du sac (5a) étant dirigée vers la ligne de pliage (9).

Ensemble d'imprimés suivant la revendication 1, caractérisé en ce que deux trous (13) de suspension sont prévus au voisinage de la ligne de pliage (9) et de part et d'autre de celle-ci pour se recouvrir lorsque le sac est plié et permettre à l'ensemble d'être suspendu sur une barre transversale (23).
3. Ensemble d'imprimés suivant la revendication 1, caractérisé en ce que la ligne de pliage est constituée par une ligne de perforations de pliage (9) qui est située à mi-distance entre le bord interne de la feuille transparente (5) et le milieu de la ligne d'adhésif (10).
4. Ensemble d'imprimés suivant l'une quelconque des revendications précédentes, comportant plusieurs feuilles copiantes (13a) associées à la partie de la feuille de support (1) destinée à recevoir des informations.
5. Ensemble d'imprimés suivant l'une quelconque des revendications précédentes, dans lequel la feuille de support (1) et la feuille transparente (5) comportent des ouvertures marginales d'entraînement pour faire coïncider les deux feuilles.
