Abstract

Изобретение относится к области медицины, а именно к сосудистой хирургии, и может быть использовано при лечении окклюзионно-стенотических заболеваний аорто-подвздошно-бедренного сегмента. Сущность изобретения заключается в осуществлении трансфеморального доступа в верхней трети бедра к общей бедренной артерии, удалении атеросклеротических бляшек, проверке восстановления центрального кровотока и закрытии артериотомного отверстия, при этом отличие состоит в том, что дополнительно выполняют внебрюшинный доступ к подвздошным артериям и терминальному отделу брюшной аорты на стороне поражения, под визуальным и тактильным контролем выполняют продольные мультифокальные короткие артериотомии длиной 2-2,5 см в количестве 3-4 на наружной и общей подвздошной артерии, а при необходимости и на бифуркации аорты, из которых выполняют удаление тромбов, интимы и атеросклеротических бляшек до неповреждённых наружных слоёв стенки артерии, после чего осуществляют дополнительную проверку восстановления центрального кровотока гидродинамической имитацией пульсации струи гепаринизированного раствора в сегменте дезоблитерации в анте- и ретроградном направлениях.The invention relates to medicine, namely to vascular surgery, and can be used in the treatment of occlusive-stenotic diseases of the aorto-ilio-femoral segment. The essence of the invention lies in the implementation of transfemoral access in the upper third of the thigh to the common femoral artery, removal of atherosclerotic plaques, verification of the restoration of central blood flow and closure of the arteriotomy opening, while the difference is that in addition, extraperitoneal access to the iliac arteries and the terminal abdominal aorta is performed on to the side of the lesion, under visual and tactile control, longitudinal multifocal short arteriotomies 2-2.5 cm long are performed in the amount of 3-4 on the external and common iliac artery, and, if necessary, on the aortic bifurcation, from which blood clots, intima and atherosclerotic plaques are removed to the intact outer layers of the artery wall, after which an additional check of the restoration of the central blood flow is carried out by hydrodynamic imitation of the pulsation of the stream of heparinized solution in the deobliteration segment in the ante- and retrograde directions.
