Abstract

Изобретение относится к медицине, а именно к трансплантационной хирургии, и представляет методику трансплантации почки из мини-доступа без использования хирургического дренажа. Задачей изобретения является профилактика начальных дисфункций почечного аллографта и снижение частоты ранних хирургических осложнений у реципиентов почки. Сущность изобретения заключается в следующем. Доступ в забрюшинное пространство обеспечивается из мини-разреза (длина 5-6 см) в подвздошной области. Выполняют мобилизацию участка подвздошных сосудов и верхушки мочевого пузыря, готовят ложе для аллографта почки между m. iliopsoas и передней брюшной стенкой. Почечный трансплантат укладывается в подготовленное ложе так, чтобы он был полностью скрыт в подвздошной ямке, а в зоне операционной раны располагались только почечная вена, артерия, которые из этого мини-доступа анастомозируются с мобилизованным на протяжении 3-4 см участком подвздошных сосудов. Изменяется последовательность наложения анастомозов: в первую очередь формируется артериальный, а затем венозный анастомоз. Следующий этап - выполнение уретеронеоцистоанастомоза с антирефлюксной защитой, который также выполняется из мини-доступа. Конечный этап операции - ушивание раны без хирургического дренажа, в котором нет необходимости из-за малотравматичности вмешательства.The invention relates to medicine, namely to transplant surgery, and presents a method of kidney transplantation from mini-access without the use of surgical drainage. The objective of the invention is the prevention of initial renal allograft dysfunctions and reducing the incidence of early surgical complications in kidney recipients. The invention consists in the following. Access to the retroperitoneal space is provided from a mini-incision (length 5-6 cm) in the iliac region. Mobilize the site of the iliac vessels and the top of the bladder, prepare a bed for an allograft of the kidney between m. iliopsoas and anterior abdominal wall. The renal transplant is placed in the prepared bed so that it is completely hidden in the iliac fossa, and in the area of the surgical wound there was only the renal vein, an artery, which from this mini-access was anastomosed with a portion of the iliac vessels mobilized for 3-4 cm. The sequence of application of anastomoses changes: first, an arterial and then venous anastomosis is formed. The next step is the implementation of ureteroneocystoanastomosis with antireflux protection, which is also performed from mini-access. The final stage of the operation is suturing the wound without surgical drainage, which is not necessary due to the low-invasiveness of the intervention.
