Заключение: эритроциты, коллоид, обилие лимфоцитов, единичные гистиоциты, плазматические клетки, клетки фолликулярного эпителия с явлениями пролиферации, а также встречаются полиморфные клетки с широкой цитоплазмой типа Тюртла, ядра увеличены, видны кое-где ядрышки. Данная цитокартина возможна при хроническом тиреотоксикозе.Cytological examination dated December 15, 2018. Под интубационным наркозом передним воротникообразным доступом по Кохеру на шее обнажена щитовидная железа. Правая доля - 7,0x5,0x3,5 см, плотно-эластической консистенции, бугристая, имеется киста размерами 0,5x0,8 см. Левая доля - 6,0x3,5x2,5 см,, плотная, бугристая. Учитывая длительный прием препарата ’’Тирозол”, капсула щитовидной железы плотно спаена с окружающими тканями и кровоточивая, железа рыхлая. Выполнена субтотальная субфасциальная резекция щитовидной железы с иссечением перешейка, с оставлением тканей с обеих сторон в виде пластинки по задней стенке (3-5 гр). После тщательного гемостаза под париетальный листок 4-й фасции шеи в зоне разветвления возвратного гортанного нерва устанавливают гемостатическую губку, на которую затем вливаем интерферон 4 мл, введение в остаточную ткань дополнительно интерферона. Полость дренирована резиновыми полосками и марлевыми турундами. Послойные швы на рану. Асептическая повязка. Послеоперационный период протекал без осложнений, клипсы сняты на 7-е сутки, рана без признаков воспаления, заживление первичным натяжением.The postoperative period proceeded without complications, the clips were removed on the 7th day, the wound had no signs of inflammation, healing by primary intention.
На момент выписки состояние удовлетворительное. Жалоб нет. Температура тела 36,5°С. Кожные покровы и видимые слизистые чистые, обычной окраски. Лимфоузлы не увеличены. Дыхание через нос, свободное. ЧДД - 16 в минуту. В легких аускультативно везикулярное дыхание по всем полям, хрипов нет. Тоны сердца приглушены, ритмичные. АД 130/90 мм рт. ст. Пульс 76 уд/мин.
