Melanoma is a tumor that develops from melanocytes, cells that originate from the neural crest. Although most melanomas arise in the skin, they can also occur on mucosal surfaces or other sites where neural crest cells migrate. Melanoma occurs primarily in adults, and more than half of cases occur in normal-looking areas of the skin. Early signs in a nevus that may be suggestive of malignant transformation include darkening or uneven color change, itching, enlargement, or development of satellites. Ulceration or bleeding are later signs. Melanoma most often occurs on the extremities in women and on the trunk or head and neck in men, but it can occur anywhere on the surface of the skin.
The prognosis is influenced by clinical and histological factors, as well as the anatomical location of the lesion. The thickness and/or level of melanoma invasion, mitotic index, tumor infiltration by lymphocytes, and ulceration or bleeding at the primary tumor site influence prognosis.
81(24): 18931904, 1989; Slinguff et al., Cancer 70(7): 1917-1927, 1992; Koh, NEJM 325(3): 171-182, 1991; Shumate et al., Am J Surgery 162(4): 315-319, 1991). Microscopic satellites in stage I melanoma may be an unfavorable prognostic histologic factor, but this is controversial (Leon et al., Archives of Surgery 126(2): 14611468, 1991). Melanomas that occur on the extremities or in women appear to have a better prognosis (Blois et al., Cancer 52(7): 1330-1341, 1983; Clark et al., J. National Cancer Inst. 81(24): 18931904, 1989; Slinguff et al., Cancer 70(7): 1917-1927, 1992; Koh, NEJM 325(3): 171-182, 1991; Shumate et al., Am J Surgery 162(4): 315- 319, 1991). Clinical staging is based on whether the tumor has spread to regional lymph nodes or to distant organs and tissues. For disease clinically limited to the primary site, greater thickness and depth of local melanoma invasion means a higher chance of lymph node metastasis and a worse prognosis. Melanoma can spread through local expansion (via the lymphatic system) and/or hematological routes to distant organs and tissues. Any organ can be affected by metastases, but the lungs and liver are the most common sites. The risk of relapse may decrease significantly over time, although late relapses are not uncommon.
(Overett and Shiu, Cancer 56:1222-1230,1985).Melanoma that has not spread beyond the primary site is highly curable. Most of these cases are tumors that have not penetrated below the papillary dermis (Stage II; thickness 1.0 mm or less). Melanoma that has spread to regional lymph nodes (Stage III) can be cured by extensive (2-4 cm) excision of the primary tumor and removal of affected regional lymph nodes (Karakousis et al, Ann Surg Oncol 13: 533-541, 2006; Batch et al, J Clin Oncol 27(36): 6199-6206, 2009). Melanoma that has spread to distant organs and tissues (Stage IV) is rarely curable with standard therapy, although ipilimumab and vemurafenib provide prolonged survival, and significant survival gains are sometimes achieved by resection of the metastasis (Overett and Shiu, Cancer 56:1222-1230, 1985).
56: 106-130, 2006; Ries et al., Cancer 88: 2398-2424, 2000).Melanoma is the fifth most common cancer in men and the sixth most common cancer in women in the United States of America (USA), with an estimated 76,250 new cases and 9,180 deaths expected in 2012 (Siegel et al., CA Cancer J Clin 62(1): 10-29, 2102). In Europe, the annual incidence rate of melanoma is slightly lower than in the USA, being approximately 7 per 100,000 compared with 18 per 100,000 in the USA (Ries et al, 2000). In Europe, approximately 83,729 new cases were diagnosed in 2008, and approximately 85,086 new cases were expected in 2010 (GLOBOCAN 2008, 2010). The incidence of melanoma is rapidly increasing worldwide; in the US the increase was 270% between 1973 and 2002. This rate is the highest among various types of cancer, with the exception of lung cancer in women (Jemal et al., CA Cancer J Clin. 56: 106-130, 2006; Ries et al., Cancer 88: 2398-2424, 2000).
Traditional nonsurgical treatments for unresectable or advanced melanoma in adults include chemotherapy (dacarbazine, temozolomide, or other agents, alone or in combination) or interleukin-2. Although some schemes provided an objective answer, it was usually short-lived. Newer treatments such as BRAF inhibition (vemurafenib) and immune-stimulating agents (ipilimumab) have demonstrated significant improvements in overall survival for a limited percentage of treated patients compared with control treatment, but toxicity is an issue.
Despite these efforts, the incidence of melanoma is rapidly increasing worldwide. There is a need for additional treatments for melanoma. The present invention addresses this and other needs.

Detailed description of the invention
The invention provides a method of treating melanoma, comprising administering to a patient with stage IIIb to IV melanoma an effective amount of an immune checkpoint inhibitor and a herpes simplex virus, wherein the herpes simplex virus does not contain functional ICP34.5 genes, does not contain a functional ICP47 gene, and contains a gene encoding human granulocyte-macrophage colony-stimulating factor (GM-CSF). Tumor cells are transfected by direct injection of the virus into accessible tumor lesions, in which the virus replicates in the tumor cells, causing tumor cell necrosis and release of tumor antigens. Local expression of human GM-CSF induces local and distant immunological responses to tumor antigens at the injection site and in distant tumor metastases.
CTLA-4.CTLA-4 is an immune checkpoint molecule that negatively regulates the T cell activation pathway. CTLA-4 is a negative regulator of T cell activation. CTLA-4 blockade demonstrated an increase in T cell activation and proliferation. The combination of herpes simplex virus and anti-CTLA-4 antibody is designed to increase T cell activation through two distinct mechanisms to enhance the antitumor immune response to tumor antigen released following lytic replication of the virus in the tumor. Thus, the combination of herpes simplex virus and anti-CTLA-4 antibody may enhance the destruction of injected and uninjected tumors/distant tumors, improve overall tumor response, and prolong overall survival, particularly when comparing overall survival benefit with that achieved in the case of monotherapy with antibody to CTLA-4.
As used herein, the term immune checkpoint inhibitor refers to molecules that completely or partially reduce, inhibit, interfere with, or modulate one or more checkpoint proteins. Checkpoint proteins regulate T cell activation or function. Numerous checkpoint proteins are known, such as CTLA-4 and its ligands CD80 and CD86; as well as PD1 and its ligands PDL1 and PDL2 (Pardoll, Nature Reviews Cancer 12: 252-264, 2012). These proteins are responsible for co-stimulatory or inhibitory interactions of T cell responses. Immune checkpoint proteins regulate and maintain self-tolerance and the duration and amplitude of physiological immune responses. Immune checkpoint inhibitors contain antibodies or are derived from antibodies.
As used herein, the term antibody includes glycosylated and non-glycosylated immunoglobulins of any isotype or subclass or antigen-binding region thereof that competes with the intact antibody for specific binding, unless otherwise specified, including monoclonal antibodies, bispecific antibodies, minibodies, domain antibodies, synthetic antibodies, antibody mimetics, chimeric antibodies, humanized antibodies, human antibodies, hybrid antibodies, antibody conjugates, single chain antibodies, antibody derivatives, antibody analogues and fragments of the foregoing, respectively. Also included are immunological fragments of antibodies (e.g. Fab, Fab', F(ab')2 or scFv), whether such antibodies were produced, in whole or in part, by immunization, recombinant technology, by synthetic methods in vitro, or otherwise. Thus, the term antibody includes objects obtained, expressed, created or isolated by recombinant methods, for example: (a) antibodies isolated from an animal (for example, a mouse) transgenic for human immunoglobulin genes, or a hybridoma obtained from it, (b) antibodies isolated from a host cell transfected to express an antibody, such as from a transfectome, (c) antibodies isolated from a recombinant, combinatorial antibody library, and (d) antibodies produced, expressed, generated or otherwise isolated , involving splicing of human immunoglobulin gene sequences with other DNA sequences. Such antibodies contain variable and constant regions derived from germline immunoglobulin sequences of two different animal species. However, in some embodiments, such antibodies may be subject to in vitro mutagenesis (or, in the case of an animal transgenic for human immunoglobulin sequences, somatic mutagenesis in vivo), and thus the amino acid sequences of the VH and VL regions of the antibodies are sequences that , although derived from and related to the germline VH and VL sequences of specific species (eg, humans), may not exist naturally within the germline antibody repertoire of the species in vivo. Unless otherwise specified, the term antibody includes, in addition to antibodies containing two full-length heavy chains and two full-length light chains, their derivatives, variants, fragments and muteins. In some cases, the antibody may contain fewer chains, such as antibodies naturally found in camelids, which may only contain heavy chains.

ГМ- 5 044256The invention provides a herpes simplex virus (HSV) that does not contain functional ICP34.5 genes, does not contain a functional ICP47 gene, and contains a gene encoding human GM-CSF. In one embodiment, the herpes simplex virus is HSV-1. The ICP34.5 gene is located in the terminal repeats of the long region of HSV and, thus, there are two copies in the genome. Previous studies have shown that functional deletion of the neurovirulence gene, ICP34.5, renders the virus nonvirulent. The ICP47 gene is located in a unique short region of HSV. The ICP47 gene product interacts with hosts that are associated with antigen processing (TAP1 and TAP2) and blocks antigen processing via major histocompatibility complex (MHC) class I molecules. Deletion of the ICP47 gene allows more extensive antigen processing within infected cells and aims to concomitantly enhance immune response. response against infected cells. GM- 5 044256
CSF is a cytokine known to be involved in stimulating immune responses. Using homologous recombination with plasmid DNA, heterologous genes, such as those encoding human GM-CSF, can be inserted into the HSV viral genome, and viral genes such as ICP34.5 and ICP47 can be functionally deleted. In one embodiment, the herpes simplex virus is talimogene laherparepvec.
Talimogene laherparepvec, HSV-1 [strain JS1] ICP34.5-/ICP47-/hGM-CSF (formerly known as OncoVEX GM CSF ), is a tumor-injected oncolytic immunotherapy containing an immune-enhanced HSV-1 that selectively replicates in solid tumors (Lui et al., Gene Therapy, 10:292-303, 2003; US Pat. No. 7,223,593 and US Pat. No. 7,537,924). HSV-1 was obtained from strain JS1 deposited in the European Cell Culture Collection (ECCC), accession number 01010209. In talimogene laherparepvec, the HSV-1 virus genes encoding ICP34.5 are functionally deleted. Functional deletion of ICP34.5, which acts as a virulence factor during HSV infection, limits replication in non-dividing cells and makes the virus non-pathogenic. The safety of HSV with functionally deleted ICP34.5 has been demonstrated in numerous clinical studies (MacKie et al, Lancet 357: 525-526, 2001; Markert et al, Gene Ther 7: 867-874, 2000; Rampling et al, Gene Ther 7:859 -866, 2000; Sundaresan et al, J Virol 74: 3822-3841, 2000; Hunter et al, J Virol Aug; 73(8): 6319-6326, 1999). In addition, ICP47 (which blocks the presentation of viral antigen to MHC class I and II molecules) is functionally deleted in talimogene laherparepvec. Additionally, functional deletion of ICP47 results in earlier expression of US11, a gene that promotes viral propagation in tumor cells without reducing tumor selectivity. The coding sequence for human GM-CSF, a cytokine involved in stimulating immune responses, is inserted into the viral genome in the talimogene laherparepvec. The insertion of the gene encoding human GM-CSF is made in such a way that it almost completely replaces the ICP34.5 gene, as a guarantee that any potential recombination event between talimogene laherparepvec and a wild-type virus can only produce an inferior, non-pathogenic virus, and cannot lead to the generation of a wild-type virus carrying the human GM-CSF gene. The HSV thymidine kinase (TK) gene remains intact in the talimogene laherparepvec, making the virus susceptible to antiviral agents such as acyclovir. Thus, acyclovir can be used to block the replication of talimogene laherparepvec, if necessary.
Talimogene laherparepvec exerts a direct oncolytic effect through viral replication in the tumor and also induces an immune antitumor response enhanced by local expression of GM-CSF. Since melanoma is a common disease, this dual activity is beneficial for therapeutic treatment. Anticipated clinical benefits include killing injected tumors, killing local, locoregional and distant tumors not injected, reducing the development of new metastases, reducing the rate of overall progression and relapse rate after treatment of the primary disease, and increasing overall survival.
В целом, вирус хорошо переносился, и дозы до 1x108 БОЕ/дозу не приводили к появлению признаков, указывающих на проблемы с безопасностью (см., например, Liu et al., Gene Ther 10: 292-303, 2003).The efficacy of talimogene laherparepvec has been tested in a variety of in vitro (cell line) and in vivo murine tumor models and has demonstrated tumor eradication or significant tumor growth inhibition at doses comparable to those used in clinical studies. Non-clinical evaluation also confirmed that GM-CSF enhances the generated immune response, enhancing the response in injection-treated and non-injection-treated tumors, and also increases surface levels of MHC class I molecules as a result of ICP47 deletion. Talimogene laherparepvec was injected into normal and tumor-bearing mice to evaluate its safety. Overall, the virus was well tolerated, and doses up to 1x108 PFU/dose did not result in evidence of safety concerns (see, eg, Liu et al., Gene Ther 10: 292-303, 2003).
27(15a): реферат 6θ18, 2009; Kaufman et al., Ann Surgic Oncol. 17: 718-730, 2010; Kaufman and Bines, Future Oncol. 6(6): 941-949, 2010). Oncol. 271 (12):907-913, 2009). Clinical studies have been or are ongoing in several types of advanced tumors (advanced solid tumors, melanoma, squamous cell carcinoma of the head and neck, and pancreatic cancer), with over 400 subjects treated with talimogene laherparepvec (see, e.g., Hu et al., Clin Can Res. 12: 6737-6747, 2006; Harrington et al., J Clin Oncol. 27(15a): abstract 6θ18, 2009; Kaufman et al., Ann Surgic Oncol. 17: 718-730, 2010; Kaufman and Bines, Future Oncol. 6(6): 941-949, 2010). Clinical data indicate that talimogene laherparepvec has the potential to provide complete clinical response in patients with advanced melanoma. In particular, high complete response rates were achieved in stage IIIc to IV melanoma (Scenzer et al., J. Clin. Oncol. 271(12):907-913, 2009). In addition, a response was observed in injection-treated and non-injection-treated locations, including visceral locations.
The immune system includes many inhibitory biochemical pathways that are critical for maintaining autotolerance and modulating immune responses. In T cells, the amplitude and quality of the response is initiated through antigen recognition by the T cell receptor and is regulated by immune checkpoint proteins that balance costimulatory and inhibitory signals.
Cytotoxic T lymphocyte antigen 4 (CTLA-4) is an immune checkpoint protein that negatively regulates the biochemical pathway of T cell activation (Fong et al., Cancer Res. 69(2):609-615, 2009 Weber Cancer Immunol. Immunother, 58:823-830, 2009). CTLA-4 blockade demonstrated increased T cell activation and proliferation. CTLA-4 inhibitors include antibodies to CTLA-4. Antibodies to CTLA-4 bind to CTLA-4 and block the interaction of CTLA-4 with its ligands CD80/CD86 expressed on antigen presenting cells, and thus block the negative regulation of immune responses caused by the interaction of these molecules. Examples of antibodies to CTLA-4 are described in US patent No. 5811097; 5811097; 5855887; 6051227; 6207157; 6682736; 6984720; and 7605238. One of the antibodies to CTLA-4 is tremelimumab (ticilimumab, CP-675206). In one embodiment, the anti-CTLA-4 antibody is ipilimumab (also known as 10D1, MDX-D010), a fully human IgG monoclonal antibody that binds to CTLA-4. Ipilimumab is marketed under the brand name Yervoy™ and is approved for the treatment of unresectable or metastatic melanoma.
WO03042402, WO2008156712, WO2010089411, WO2010036959, WO2011066342,Another immune checkpoint protein is programmed cell death protein 1 (PD-1). PD1 limits T cell activity in peripheral tissues during the inflammatory response to infection and limits the autoimmune response. In vitro blockade of PD1 enhances T cell proliferation and cytokine production in response to challenge with specific target antigens or allogeneic cells in mixed lymphocytic responses. A strong correlation between PD1 expression and response has been demonstrated for PD1 blockade (Pardoll, Nature Reviews Cancer, 12: 252-264, 2012). Blockade of PD1 can be achieved through various mechanisms, including antibodies that bind to PD1 or its ligand, PDL1. Examples of PD1 and PDL1 blockers are disclosed in US Pat. Nos. 7,488,802; 7943743; 8008449; 8168757; 8217149, and published PCT Patent Applications WO03042402, WO2008156712, WO2010089411, WO2010036959, WO2011066342,
WO2011159877, WO2011082400 и WO2011161699. In some embodiments, the PD1 blockers include anti-PD-L1 antibodies. In some other embodiments, PD1 blockers include anti-PD1 antibodies and similar binding proteins, for example, nivolumab (MDX 1106, BMS 936558, ONO 4538), a fully human IgG4 antibody that binds to and blocks activation of PD-1 by its ligands PD-L1 and PD-L2; lambrolizumab (MK-3475 or SCH 900475), a humanized anti-PD-1 monoclonal IgG4 antibody; CT-011, a humanized antibody that binds to PD1; AMP-224 is a B7-DC fusion protein; Fc portion of antibody; BMS-936559 (MDX-1105-01) for PD-L1 (B7-H1) blockade.
Cancer Res. July 15(18) 3834). Med. 207:2175-86 и Sakuishi et al., 2010, J. Exp. Med. 207:218794).Other immune checkpoint inhibitors include lymphocyte activation gene-3 (LAG-3) inhibitors, such as IMP321, a soluble Ig fusion protein (Brignone et al., 2007, J. Immunol. 179:4202-4211). Other immune checkpoint inhibitors include B7 inhibitors, such as B7-H3 and B7-H4 inhibitors. In particular, the antibody to B7-H3 MGA271 (Loo et al., 2012, Clin. Cancer Res. July 15(18) 3834). In addition, TIM3 (T cell immunoglobulin domain and mucin domain 3) inhibitors are included (Fourcade et al., 2010, J. Exp. Med. 207:2175-86 and Sakuishi et al., 2010, J. Exp. Med. 207:218794).
Without being bound by any theory of the invention, in one embodiment, a combination of a herpes simplex virus lacking functional ICP34.5 genes, lacking a functional ICP47 gene, and containing a gene encoding human GM-CSF, and an immune checkpoint inhibitor can scale up specific antitumor T cell responses compared with ipilimumab monotherapy in patients, and this effect may be particularly observed in previously untreated, inoperable, stable IIIb-IV melanoma. The combination is designed to enhance the systemic antitumor response to tumor antigens following lytic replication of talimogene laherparepvec in tumors. Thus, combination therapy may result in greater killing of injected tumors as well as uninjected/distant tumors, including micrometastatic disease, with increased tumor complete response rates and duration of response. Overall, these effects may contribute to improved overall survival, especially compared with ipilimumab monotherapy.
(Kapadia and Fong, J Clin Oncol., 23:8926-8928, 2005).The goal of using talimogene laherparepvec in combination with an immune checkpoint inhibitor is to enhance T cell activation through various mechanisms, and thereby increase dendritic cell-mediated tumor antigen presentation (Kaufman et al., Ann Surg Oncol., 17(3):718 -730, 2010), following the release of tumor antigens by lytic viral replication, enhanced by local expression of GM-CSF, and counteracting immune tolerance by blocking immune checkpoint inhibitor-mediated inhibitory signals, such as CTLA-4, on T cells (Kapadia and Fong , J Clin Oncol., 23:8926-8928, 2005).
Talimogene monotherapy laherparepvec demonstrated a significant response rate and durable responses in a Phase 2 trial and a clinically meaningful increase in overall survival is anticipated (Senzer et al, J Clin Oncol. 27(34):5763–5771, 2009). Ipilimumab monotherapy is known to provide a clinically significant but relatively small increase in overall survival (approximately 2-3 months, compared with dacarbazine or peptide vaccine), and overall and partial responses occurred in only a small subset of subjects (approximately 10.9%). Additionally,
7,2.- 7 044256 responses with ipilimumab often occurred late, with up to 10% of patients demonstrating responses after initial progression during therapy (Hodi et al, N Eng J Med., 363:711-723, 2010; Robert et al N Eng J Med. , 364:2517-2526, 2011). This limits the effectiveness of the agent in patients with relatively aggressive disease. Combination with talimogene laherparepvec may shorten the time to response compared with ipilimumab monotherapy. A combination of a herpes simplex virus lacking the functional ICP34.5 gene, lacking the functional ICP47 gene and containing the gene encoding human GM-CSF, and an immune checkpoint inhibitor is administered to patients with stage IIIb to IV melanoma. Melanoma staging is based on the TNM cancer staging system developed by the American Joint Commission on Cancer (AJCC Cancer Staging Manual, Eds. Edge et al., Springer, 7th edition, 2010), which is included here in its entirety. The herpes simplex virus and immune checkpoint inhibitor are formulated, dosed and administered to the patient in a manner consistent with Good Medical Practice. In one embodiment, talimogen laherparepvec is administered in a buffer solution containing 2% sorbitol, 4% inositol in phosphate-buffered saline, pH 7.2.
Talimogene laherparepvec is administered by intratumoral injection into injectable cutaneous, subcutaneous and lymph node tumors at a dose of up to 4.0 ml of 10 6 plaque forming units/ml (PFU/ml) on the 1st day of the 1st week, after which a dose of up to 4 .0 ml 10 8 PFU/ml on the 1st day of the 4th week, and then every 2 weeks (± 3 days). The recommended volume of talimogene laherparepvec for injection into the tumor(s) depends on the size of the tumor(s) and should be determined according to the injection volume guidelines in the table.
Guidance regarding the volume of talimogene laherparepvec injection based on tumor size
All injectable tumor sites (cutaneous, subcutaneous, and lymph node disease, which may be injectable with or without ultrasound guidance), within reason, should be injected with the maximum dose volume available according to individual dose determination. On each day of treatment, the recommended injection priority is as follows: any new tumor available for injection that has appeared since the last injection; by tumor size, starting with the largest tumor; any tumor(s) previously inaccessible for injection that becomes available for injection.
The immune checkpoint inhibitor is administered in the form of a composition containing one or more additional components, for example, a physiologically acceptable carrier, excipient or diluent. The compositions may contain one or more substances selected from the group consisting of a buffer, an antioxidant, for example, ascorbic acid, a low molecular weight polypeptide (for example, containing less than 10 amino acids), a protein, an amino acid, a carbohydrate, for example, glucose, sucrose or dextrins, a chelating agent , for example, EDTA, glutathione, stabilizer and excipient. Neutral buffered saline or saline mixed with specific serum albumin are examples of suitable diluents. In addition, preservatives such as benzyl alcohol may be added in accordance with appropriate industry standards. The composition can be prepared in the form of a lyophilisate, using solutions of suitable excipients (eg sucrose) as diluents. Suitable components are non-toxic to recipients at the doses and concentrations used. In one embodiment, the immune checkpoint inhibitor is an anti-CTLA-4 antibody. In another embodiment, the anti-CTLA-4 antibody is ipilimumab formulated at a concentration of 5 mg/ml in 20 mM Tris HCl, 0.1 M sodium chloride, 0.1% w/v. mannitol solution, 0.1 mM diethylenetriamine-pentaacetic acid solution, 0.01% w/v. polysorbate 80 solution, pH 7.0. Additional examples of components that may be used in pharmaceutical preparations are provided in any publication of Remington's Pharmaceutical Sciences, including the 21st edition (2005), Mack Publishing Company, Easton, Pennsylvania.
Ipilimumab is administered intravenously over (90±15) minutes at a dose of 3 mg/kg every 3 weeks (±3 days) for up to 4 infusions, starting after the third dose of talimogene laherparepvec. If talimogene laherparepvec and ipilimumab are administered on the same day, talimogene laherparepvec should be administered first.
Therapy will be continued as medically indicated or until the desired temperature is achieved.
Однако курс лечения для любого индивидуального пациента может быть модифицирован в клинической практике.- 8 044256 chanting effect (for example, those disclosed herein). For example, patients may receive talimogene laherparepvec until complete response, resolution of all injectable tumors, and disease progression according to modified criteria for immune-related response (MSRI). Due to the mechanism of action, patients may experience growth of existing tumors or the development of new tumors before the maximum clinical effect of talimogene laherparepvec occurs. Therefore, it is contemplated that administration should be continued for at least 6 months from the date of the first dose, provided that the subject has no evidence of clinically significant deterioration in medical condition requiring discontinuation of treatment and the patient can tolerate treatment. However, the course of treatment for any individual patient can be modified in clinical practice.

IIIb-IV.An open-label, Phase Ib/2 evaluation of the safety and efficacy of talimogene laherparepvec and ipilimumab compared with ipilimumab monotherapy in subjects with previously untreated, unresectable stage IIIb-IV melanoma.
Ib.Phase Ib.
The objective of this study was to determine the safety and tolerability of talimogene laherparepvec in combination with ipilimumab based on the incidence of dose-limiting toxicity (DLT) in subjects with previously untreated, unresectable stage IIIb to IV melanoma. The study was an open-label, multicenter, non-comparative study. Talimogene laherparepvec in combination with ipilimumab will be administered to up to 18 study subjects.
Talimogene laherparepvec is administered by intratumoral injection into injectable skin, subcutaneous and lymph node tumors, initially at a dose of up to 4.0 ml of 10 6 plaque forming units/ml (PFU/ml) on the 1st day of the 1st week, after which the dose is administered up to 4.0 ml 10 8 PFU/ml on the 1st day of the 4th week, and then every 2 weeks (±3 days). Ipilimumab is administered intravenously at a dose of 3 mg/kg every 3 weeks (±3 days) for up to 4 infusions, starting on day 1 of week 6 (i.e., the time of the third dose of talimogene laherparepvec), then on day 1 9th week, 1st day of 12th week and 1st day of 15th week. If talimogene laherparepvec and ipilimumab are administered on the same day, then talimogene laherparepvec is administered first.
Subjects are treated with talimogene laherparepvec until complete response, resolution of all injectable tumors, disease progression according to modified criteria for immune-related response (MSRI), or intolerance to study treatment, whichever occurs first.
Subjects will be assessed for safety approximately 30 (+7) days after the last dose of talimogene laherparepvec or 60 (+7) days after the last dose of ipilimumab, whichever occurs later, and monitored for survival for approximately 24 months after after the end of the testing period.
IV.The outcome of this study was to demonstrate the safety and tolerability of talimogene lagerparepvec in combination with ipilimumab based on the incidence of dose-limiting toxicity (DLT) in subjects with previously untreated, unresectable stage IIIb to IV melanoma.
Dose-limiting toxicity was defined as any grade (grade) >3 immune-related adverse effect (AE) or grade. >4 AEs of any etiology occurring between the first dose of ipilimumab and a time point 6 weeks later. The occurrence of ODT had to be <1 in the first 6 evaluable patients or <2 in the first 9 evaluable patients (if 2 ODT occurrences were observed in the first 6 patients). Key inclusion criteria for the study were the presence of previously untreated, unresectable stage IIIBIV melanoma, ECOG status 0-1, measurable disease, and >1 injectable cutaneous, subcutaneous, or lymph node tumor.
Currently, 19 patients are enrolled (13 patients received >1 dose of talimogene laherparepvec or ipilimumab); 9 patients completed the CCT period, 1 patient withdrew consent prior to ipilimumab administration, and 9 patients had not yet completed the CCT period. All patients available for the CCT assessment had received at least 4 doses of talimogene laherparepvec and 2 doses of ipilimumab at the end of the CCT assessment (6 weeks after the 1st dose of ipilimumab). No dose-limiting toxicities were observed in patients available for evaluation. To date, serious side effects have been reported in 1 of 19 patients (grade 3 nausea and bloating at week 11 of treatment). Two partial responses were reported at week 12 among 9 available for

To evaluate the safety of talimogene lagerparepvec in combination with ipilimumab based on the incidence of all adverse events (AEs), grade 3 AEs, serious adverse events (SAEs), and events requiring discontinuation of the study drug, local effects on the tumor (i.e. , pain, inflammation and ulceration), clinically significant changes in laboratory parameters and clinically significant changes in vital signs not attributed to ODT.
2.Phase 2.
The primary objective of this study is to evaluate the safety and efficacy of talimogene laherparepvec in combination with ipilimumab versus ipilimumab monotherapy for overall survival (OS) in subjects with previously untreated, unresectable stage IIIb to IV melanoma. The study is an open-label, multicenter, randomized trial. Approximately 140 subjects are randomized 1:1 to receive the following:
Рандомизация стратифицируется стадией заболевания (стадия IIIb/с, стадия IVM1a и стадия IVM1b против стадии IVM1c) и BRAF V600E (мутация против отсутствия мутации).Randomization is stratified by disease stage (stage IIIb/c, stage IVM1a, and stage IVM1b vs stage IVM1c) and BRAF V600E (mutated vs no mutation).
Talimogene laherparepvec is administered by intratumoral injection into skin, subcutaneous tumors and lymph node tumors accessible for injection, initially at a dose of up to 4.0 ml 106 PFU/ml on the 1st day of the 1st week, after which a dose of up to 4.0 ml is administered 10 8 PFU/ml on the 1st day of the 4th week, and then every 2 weeks (±3 days). Ipilimumab is administered intravenously at a dose of 3 mg/kg every 3 weeks (±3 days) for up to 4 infusions. Subjects randomized to arm 1 receive ipilimumab starting on day 1 of week 6 (i.e., the time of the third dose of talimogene laherparepvec), then on day 1 of week 9, day 1 of week 12. th week and 1st day of the 15th week. If talimogene laherparepvec and ipilimumab are administered on the same day, then talimogene laherparepvec is administered first. Subjects randomized to arm 2 receive ipilimumab starting on day 1 of week 1, followed by day 1 of week 4, day 1 of week 7, and day 1 of week 10.
Subjects are treated with talimogene laherparepvec until complete response, resolution of all injectable tumors, disease progression according to modified criteria for immune-related response (MSRI), or intolerance to study treatment, whichever occurs first. Due to the mechanism of action, subjects may experience growth of existing tumors or the development of new tumors before the maximum clinical effect of talimogene laherparepvec occurs. Therefore, it is contemplated that administration should be continued for at least 6 months from the date of the first dose, provided that the subject has no evidence of clinically significant deterioration in health condition requiring discontinuation of treatment and the patient can tolerate treatment.
Subjects are assessed for safety approximately 30 (+7) days after the last dose of talimogene laherparepvec or 60 (+7) days after the last dose of ipilimumab, whichever occurs later, and survival is monitored for approximately 24 months after the end of randomization.
This study evaluates the overall survival efficacy of talimogene laherparepvec in combination with ipilimumab versus ipilimumab monotherapy in subjects with previously untreated, unresectable stage IIIb to IV melanoma. The outcome of the study is to measure the increase in the magnitude of tumor-specific T-cell responses and the duration of time to response compared with ipilimumab monotherapy.
Secondary objectives of this study include:
