Description
Field of technology
The invention relates to a pharmaceutical combination intended for use in a method of preventing or treating ophthalmic diseases or disorders.
State of the art
Ophthalmic diseases or disorders are often debilitating diseases leading to total or partial blindness. For example, in developed countries, age-related macular degeneration (AMD) is the leading cause of blindness in people over 60 years of age. A recent systematic review and meta-analysis found that 8.7% of the world's population has AMD, and with increasing life expectancy, the number of people with the disease is projected to be approximately 196 million in 2020, reaching 288 million in 2040. Approximately 1.75 million Americans have AMD, a number expected to rise to nearly 3 million by 2020. There is currently no therapy available to treat the dry, slowly progressive form of AMD. Thus, there is a significant unmet need in the medical field for the treatment of ophthalmic diseases or disorders such as AMD.
Brief description of the invention
In the present invention, it has been unexpectedly discovered that a pharmaceutical combination containing a PPAR agonist such as pioglitazone and a p38 inhibitor, for example a compound of formula III as defined below, such as pamapimod, is suitable for the prevention or treatment of ophthalmic diseases or disorders, in particular macular degeneration , such as AMD.
Р38, для предотвращения хориоидальной неоваскуляризации в мышиной модели возрастной макулярной дегенерации (ВМД).P38, to prevent choroidal neovascularization in a mouse model of age-related macular degeneration (AMD).
Short description
CNV (choroidal neovascularization) in mice or rats is an accepted model to mimic the wet form of age-related macular degeneration (AMD). The model was induced by laser photocoagulation, which creates breaks in Bruch's membrane. The subsequent pathological cascade includes inflammation, angiogenesis and proteolysis.
A CNV development study was performed in a mouse model to evaluate the efficacy of pioglitazone and pamapimod, alone or in combination. Three laser burns were used to induce CNV in the eyes of C57BI/6J mice. Pioglitazone or pamapimod or the pioglitazone/pamapimod combination was administered to groups of animals orally once daily, starting one day before laser treatment. After 14 days of treatment, the pioglitazone/pamapimod combination reduced new vessel area and reduced retinal thickness at the CNV region more than either agent alone. These data support the possibility of increasing the effectiveness of the combination of a PPAR agonist (pioglitazone) and a P38 inhibitor (pamapimod) in the treatment of AMD and related eye diseases.
СпособыMethods
Manipulations with animals.
ESAVI/219/04.10.07/2014).Animal handling was in accordance with the ARVO Regulation on the Use of Animals in Ophthalmic and Vision Research and EU Directive 86/609/EEC for animal experiments, using protocols approved and supervised by the Finnish Animal Experimentation Council (Experimentica Ltd. animal license number ESAVI/219 /04.10.07/2014).
Male C57BL/6J mice were obtained at 7 weeks of age (Janvier Labs, France) and housed at a constant temperature (22±1°C) in a light-controlled environment with unlimited access to food and water.
To induce CNV, animals were anesthetized with an intraperitoneal injection of ketamine (37.5 mg/kg; Ketalar, Pfizer Oy Animal Health, Helsinki, Finland) and medetomidine (0.45 mg/kg; Domitor, Orion Oy, Espoo, Finland). A drop of 0.5% tropicamide (Santen) was applied to the cornea to dilate the pupils. Laser photocoagulation was performed once using a 532 nm Lumenis Novus Spectra diode laser (Lumenis Ltd., Israel) attached to a slit lamp. Viscotears® liquid coating and gel (Novartis) were used to applanate the cornea. Three laser lesions were performed in each eye. After laser treatment, anesthesia was reversed with the alpha-2 antagonist medetomidine, atipamezole (0.5 mg/kg intraperitoneal, Antisedan, Orion Pharma, Espoo, Finland).
Drug treatment.
Тестируемые соединения [пиоглитазон HCl (2,5 мг/мл), памапимод (10 мг/мл) и комбинацию пиоглитазон HCl (2,5 мг/мл) + памапимод (10 мг/мл)] готовили в этом носителе и обрабатывали ультразвуком перед введением (приблизительно 3 мин).Oral dosage forms were prepared in vehicle and adjusted to 4.8 ± 0.05 using buffer. Test compounds [pioglitazone HCl (2.5 mg/ml), pamapimod (10 mg/ml) and the combination of pioglitazone HCl (2.5 mg/ml) + pamapimod (10 mg/ml)] were prepared in this vehicle and sonicated before injection (approximately 3 minutes).
Aflibercept (Eylea®, Bayer Pharma AG) was purchased as a ready-made solution for intravitreal injection at a concentration of 40 mg of aflibercept per 1 ml of solution. In the present study, a dose of 80 μg (injection volume 2 μl) of aflibercept per mouse eye was used.
Compounds were administered orally by gastric irrigation (pioglitazone, pamapimod) at a dose of 0.2 ml per 20 g mouse weight or by IVT (aflibercept) using a 5 μl glass microsyringe (Hamilton Bonaduz AG, Bonaduz, Switzerland). Aflibercept was administered only to the laser-treated right eye. The injection volume was 2 μl per eye.
In vivo визуализация.In vivo imaging.
Общую толщину сетчатки рассматривали как толщину всех слоев от слоя нервных волокон до RPE (здоровые участки измерения) или до подразумеваемой линии, соединяющей RPE вокруг участка повреждения (участки, обработанные лазером).CNV lesions were monitored using Envisu R2200 SD-OCT (spectral domain optical coherence tomography system; Bioptigen Inc./Leica) in anesthetized mice as described above. The thickness of each retina was measured at 24 random grid sites and at all three laser-treated sites. Total retinal thickness was considered as the thickness of all layers from the nerve fiber layer to the RPE (healthy measurement sites) or to the implied line connecting the RPE around the lesion site (laser-treated sites).
Collection and dyeing of fabrics.
At the end of the study, mice were killed by transcardial perfusion, first with 0.9% NaCl, then with 4% paraformaldehyde in 0.1 M phosphate buffer, pH 7.4. Eyes were collected, and plane-mounted choroids were prepared from each treated eye and fifteen (15) control eyes (three samples from each treatment group).
Image J (v.1.51f, National Institutes of Health, USA).Plane-mounted choroids were stained with fluorescein-labeled isolectin B4 (Vector Laboratories) to detect CNV lesions and visualized using a Leica DM IRBE microscope (Leica Microsystems, Germany) with an epifluorescence attachment and analyzed using Image J software (v.1.51f, National Institutes of Health, USA).
