Medizinische Vorrichtungzum Einführen vonMedical device for introducing
medizinischen Anordnungen ect. durchmedical orders etc. by
Gewebeschichten in den KörperTissue layers in the body
The invention relates to a medical device for introducing medical devices etc. through tissue layers into the body according to the preamble of claim 1.
In addition to the conventional surgical incision and the layer-by-layer approach through body walls, minimal access routes from the outside of the body through layers of tissue to body cavities or intracorporeal structures or fluid collections are becoming increasingly important for therapeutic and diagnostic interventions in medicine. Drains, tubular sleeves, guide rods, instruments, optical devices, liquids, air, CO2/tissue samples, organs or organ parts can then be introduced or removed from the body through these access routes.
The indications for such access routes according to techniques of minimally invasive therapy or diagnostics can be seen, for example, in the context of laparoscopic or thoracoscopic interventions.
But access routes are also required for placing drains. For example, in the chest area, an access route through the chest wall is required to drain fluid accumulations in the chest cavity (pleural effusion) to the outside or to suck out air accumulations in the chest cavity (pneumothorax) using suction in order to achieve a negative pressure that allows the lungs to expand.
Wie für die Brusthöhle werde'n auch für die Bauchhöhle mit Einschluß des kleinen Beckens Zugangswege benötigt, um Operationsinstrumente, optische Vorrichtungen, Flüssigkeiten, Luft,As for the chest cavity, access routes are also required for the abdominal cavity , including the small pelvis, in order to transport surgical instruments, optical devices, fluids, air,
However, drains are also often inserted after operations in the abdominal cavity for the targeted external drainage of fluids (blood, secretions, etc.), which are led out through the abdominal wall.
The known indications for access through the abdominal wall layers to hollow organs located intra-abdominally include the placement of a gastric or feeding tube, which creates a connection from the stomach to the outside of the body for inflow and outflow.
If renal insufficiency is diagnosed, the treatment choice of CAPD (Continuous Ambulatory Peritoneal Dialysis) also requires appropriate access through the abdominal wall for special drainage. Other indications for placing a drainage through the layers of the abdominal wall are abscess formations or cysts that need to be drained to the outside.
Various trocars are known for this purpose, which are pierced through the body wall from the outside. In order to largely avoid injury to internal organs or structures, the abdomen is inflated with CO 2 using a thin needle during laparoscopic procedures, so that the abdominal wall is lifted away from the organs and a certain safety distance is created for the first trocar that follows.
Problems and risks occur when inserting the needle for gas insufflation and the first trocar. Both are blind maneuvers, in contrast to the second and subsequent trocar insertions, since the insertion can be directly controlled visually using the optics that can be inserted into the first trocar that has already been placed. Complications described when inserting the needle for gas insufflation and the first trocar as blind maneuvers include injuries to internal organs and structures, including injury to the abdominal aorta, and incorrect insufflations into the wrong tissue layers.
The invention is intended to solve the problem of designing a device for introducing medical devices into the body in such a way that minimally invasive therapy and diagnostics can be carried out safely and with greater precision. In particular, the function of a gas insufflation needle and a subsequent first trocar should be combined and at the same time be able to be carried out in an optically controlled manner. In addition, the integrated application device for a local anesthetic makes it possible to infiltrate a certain amount of local anesthetic into the tissue when carrying out the above functions, precisely where the device has a traumatic effect and causes pain - namely at the cutting tip.
The problem is solved by the features specified in the characterising part of claim 1.
It is particularly advantageous that the previously blind maneuvers - insertion of the gas insufflation needle and insertion of the subsequent first trocar - can now be combined and carried out with direct optical control.
Another advantage is that the control of the gas insufflation can be operated by the surgeon himself using the handle of the device and thus in a sterile area.
The guide rail, which is preferably designed as a semi-tubular channel, also makes it possible to remove it from the side of the already placed drainage, tube sleeve, optic, instrument, etc. and to reattach it in the same way without having to be guided over the end of the drainage tube, tube sleeve, optic, instrument, etc.
It is also possible to use the device for placing drains, for example for placing a thoracic drain. The drain to be placed intracorporeally is placed with the inserted optics on the concave side of the guide rail and under direct optical control.
trolle durch den ausgewählten Interkostalraum geführt, wobei der Durchtritt in die Brusthöhle optisch kontrollierbar wird und das Lungengewebe, die Thoraxinnenwandung oder ein Pleuraerguß hinsichtlich pathologischer Strukturen oder Läsionen einsehbar und dokumentierbar wird.trolls through the selected intercostal space, whereby the passage into the chest cavity can be visually controlled and the lung tissue, the inner wall of the chest or a pleural effusion can be viewed and documented with regard to pathological structures or lesions.
The guide rail is positioned in such a way that the traumatizing, e.g. semi-tubular device with the attached drainage can be pushed over the upper edge of the lower rib into the chest, whereby the upper half of the drainage, which is not enclosed by the guide rail and is therefore soft, slides past the lower edge of the upper rib. The intercostal vascular and nerve bundle runs along the lower edge of the upper rib and is thus protected. In addition, the upper half of the drainage, which is not enclosed by the device and is therefore soft, can be compressed when pushed through a narrow intercostal space.
It is also within the scope of the invention that the device according to the invention consisting of a guide rail with a handle can be used in different embodiments depending on the indication, for example only as a device with a handle, in an extended embodiment in combination with an application device for a local anesthetic - consisting of a fillable container, a supply line and an injection needle - or in the complete equipment additionally with another hollow needle for the supply or discharge of gases or liquids.
Further advantageous embodiments of the invention are protected in the subclaims. In the following, embodiments of the device according to the invention with conceivable variants are described in more detail with reference to drawings.

Ist die Spritze 5 beim Einschieben in Pfeilrichtung A am Boden 17 (Fig. 2) des Handgriffes 2 angeschlagen, und ist damit die Kanüle 6 über das Ende 10 der Führungsschiene 1 vorgeschoben, gemäß Pfeil- Figure 1 shows a side view of the medical device according to the invention with a channel-shaped, in particular half-tubular guide rail 1, which is expediently connected, or can be connected, to a handle 2, including a certain angle G. A drainage 3 is provided on the concave side of the guide rail 1, into which an optical device - shown here as a rigid optical tube 4a, attached camera 4b, and camera cable 4c - is inserted. The drainage 3, which is provided on the concave side of the guide rail 1, can be fixed with the inserted optics 4a using a bracket 9. It is within the scope of the invention that the bracket 9 is designed to be pivotable, so that when the bracket 9 is pivoted away to the side, for example, a drainage 3 can be placed laterally in the guide rail 1 and can also be removed again. The handle 2 can be designed as a hollow body into which, for example, a syringe 5, preferably in a commercially available embodiment, can be inserted. The inserted part or a part of it, e.g. the scale on the syringe body 5a, e.g. for a drawn-up local anesthetic, is visible through a correspondingly positioned recess 2a in the handle wall. When the syringe plunger 5c is pressed in the direction of arrow A, the syringe 5 slides forward in the handle 2 and at the same time downwards on an inclined plane 20 provided in the handle 2 (see Fig. 2). At the same time, the cannula 6 connected to the syringe 5 can be pushed out along the guide rail 1 over the cutting end 10 of the guide rail 1. If the syringe 5 hits the base 17 (Fig. 2) of the handle 2 when being pushed in in the direction of arrow A, and the cannula 6 is pushed forward over the end 10 of the guide rail 1, according to the arrow
In the direction B (Fig. 1b), further pushing of the syringe plunger 5c against the stop 17a that has been reached causes an injection of the medium drawn up in the syringe 5, e.g. a local anesthetic. At least in the end region la of the guide rail 1, or over its entire or almost entire length, a guide is provided, e.g. designed as a tube or as a dome 19, in which at least one cannula, e.g. the cannula 6 - e.g. for the injection of a local anesthetic and e.g. another hollow needle 8 lying next to it - e.g. for the supply or discharge of gases or liquids - is or are slidably guided.
Along the guide rail 1, next to the cannula 6, the further hollow needle 8 can be guided, which can be advanced from the handle 2 beyond the end 10 of the guide rail 1. To advance the hollow needle 8, a pressure surface 16 that can be operated from the outside and is connected to the hollow needle 8 is formed in the handle area. Furthermore, the hollow needle 8 can be connected via a connecting piece 22 to a line system 7 (see Figure 2) that either continues directly outward from the handle 2 or that at least reaches to an attachment piece 13 to which an external line system 12 can be connected. A lever 14 can be pressed into a corresponding recess 15 in the handle 2 (arrow direction D), via which a control of the CO2 that is supplied from the outside (supply line 12) can be effected.
Mit einer Schraube 9c ist bei über die Führungsschiene 1 geschwenkten Figure la shows a cross section through the guide rail 1 at the height of line II (Fig. 1). The drainage 3 is placed on the concave side of the guide rail 1, into which the optical tube 4a is inserted. The cannula 6 is guided along the guide rail 1 next to the further hollow needle 8. The bracket 9 is pivotally mounted on the guide rail 1 by means of a joint 9a. It can be pivoted away from the guide rail 1 after a locking mechanism 9b on the opposite side has been released. The locking mechanism consists, for example, of two interlocking hooks, each on the guide rail 1 and in the corresponding position on the bracket 9. With a screw 9c, when the cannula 6 is pivoted over the guide rail 1, the
5and locked bracket 9, the drainage 3 applied can be clamped against the concave side of the device 1. In the same way, instead of a drainage 3, an instrument, a tubular sleeve or the like can be applied and/or fixed. 5
Figure 1b shows an enlarged detail of the cutting end 10 of the guide rail 1 with the cannula 6 pushed forward, e.g. for the injection of a local anesthetic (arrow direction B), and with the hollow needle 8 also pushed forward, e.g. for the insufflation of liquid or gaseous media, for example CO2 (arrow direction C). The hollow needle 8 has a closed end 8a and preferably two outflow openings 8b for the medium, e.g. CO2, which can be introduced from the outside via the handle. In order to protect the cannula and hollow needle tips during rotating and cutting movements of the guide rail 1, and to avoid a traumatizing effect of the cannula and hollow needle tips on the tissue, there is a guide at the end of the guide rail 1, e.g. in the form of a cap 19, through which the cannula 6 and the hollow needle 8 are guided and into which the cannula 6 and the hollow needle 8 disappear when retracted. Both the cannula 6 and the hollow needle 8 can be pushed back and forth over the end of the guide rail 1 using suitable actuating means, starting from the handle 2.
Nach Gebrauch ist die Kanüle 6 am Kanülenansatz 6a von der Spritze 5 diskonnektierbar und durch eine Figure 2 shows a longitudinal section through the housing of the handle 2, which has a hollow body 21 into which a commercially available syringe 5 can be inserted. The syringe 5 can be connected to an application device, e.g. a cannula attachment 6a of the cannula 6, and when pushed forward (arrow direction A) slides down an inclined plane 20 to the bottom 17 of the hollow body 21. In this injection position, the cannula 6 is pushed forward axially in the arrow direction B over the cutting edge 10 of the device 1 - further pushing of the syringe plunger 5a against the stop 17a causes the injection of the local anesthetic drawn up in the syringe 5. After use, the cannula 6 can be disconnected from the syringe 5 at the cannula attachment 6a and replaced by a
A spring mechanism, preferably in the form of a leaf spring 17b, pushes the syringe 5 back into the starting position automatically after the injection process or when the syringe 5 is pulled back in the opposite direction to the arrow A, whereby the needle tip retreats and disappears into the cap 19.
Fig. 3 nebeneinander dargestellt.
- einrastet, die derart angeordnet sind, daß durchAlong the indicated guide rail 1, next to the cannula 6, another hollow needle 8 is guided, which can be pushed axially downwards over the cutting end 10 of the guide rail 1 by pressure on the pressure surface 16 in the direction of arrow C with rods 23. For a better overview, the cannula 6 and the hollow needle 8 are shown one below the other in Fig. 2, and next to each other in Fig. 3. The hollow needle 8 is connected via an application device, e.g. a connector 22, preferably in the form of a T-piece, to a supply or discharge hose 7 for gases or liquids, which continues outwards via a coupling 13. The use of a flexible hose 7 makes it possible to follow the axial movement (direction of arrow C) of the hollow needle 8. In a suitable position, a trigger 14 can be pressed into a recess 15 on the handle housing 2 (arrow direction D), which pushes up a leaf spring 24 when pressure is applied by means of a fork-like rod 14a. The hose 7 is held between the fork legs of the fork-like rod 14a so that it can follow the axial movement of the hollow needle 8. A plunger 25 connected to the leaf spring 24 thus releases the hose 7, which is squeezed in the starting position by the spring force of the leaf spring 24 transmitted to the plunger 25. The flow rate can be controlled depending on the pressure on the trigger 14 and the resulting release of the hose 7.
An embodiment is also conceivable in which, when the hollow needle 8 is pushed forward in direction C, a suitable projection 26 formed on the rod 23 engages in one of several notches 27 lying one below the other - depending on the desired depth of impression - which are arranged in such a way that
5A rotary movement (arrow direction F) enables unlocking. The hollow needle 8 can then be returned to the starting position, preferably supported by the spring force of a compression spring 31.

Figure 3 shows, in a top view of the top of the handle 2, that the guide rail 1 can be separated from the handle 2 and, for example, a guide rail 1 with a used, blunt end 10 can be exchanged for a new guide rail 1 with a sharp end 10. In the preferred embodiment, the guide rail 1 can be pushed from below into the correspondingly shaped bearing on the handle 2. Two webs 28, 28a on the guide rail 1 in the corresponding milled recesses 29, 29a guide the pushing-in movement and also provide a force-locking connection between the guide rail 1 and the handle 2 for cutting rotational movements. For the injection, it is expedient that when pressure is applied on the plunger 5c of the syringe 5, the handle 2 can be grasped securely and without slipping, for example with the index and middle fingers in the correspondingly shaped notches 30, 30a on the handle 2. Furthermore, Fig. 3 shows the opening 18 on the upper side of the handle 2 for inserting or removing the cannula 6, the pressure surface 16 for pushing down the hollow needle 8 and the viewing window for the scale 5b of the syringe 5.
