Beschreibung
Die Erfindung betrifft eine künstliche Hüftgelenkspfanne aus Kunststoff, insbesondere zur Reoperation in dem Acetabulum des menschlichen Beckenknochens.
Eine derartige künstliche Hüftgelenkspfanne ist zum Beispiel aus dem Gebrauchsmuster G 83 26 328 bekannt, die ebenfalls für Reoperation^.n bestimmt ist, also für Fälle, in denen eine bereits implantierte künstliche Hüftgelenkspfanne aufgrund aufgetretener Lockerungen, Infektionen oder dergleichen durch eine neue künstliche Hiiiftgelenkspfanne ersetzt werden muß. Da die Entfernung einer künstlichen Hüftgelenkspfanne aus dem Acetabulum und die nachfolgende Vorbereitung der Acetabulum-Höhlung zur Aufnahme der neuen Pfanne in aller Regel mit einer Vergrößerung der Acetabulum-Höhlung verbunden ist, besitzt diese bekannte Hüftgelenkspfanne einen vergrößerten Außendurchmesser und darüber hinaus einen flanschartigen Stützkragen, an dem die Pfanne - zusätzlich zu der Einzementierung - mit Befestigungsmitteln am Beckenrand fixiert werden kann. Diese bekannte Pfanne besitzt außerdem dem Stützkragen gegenüber eine Anfasung, die im einoperierten Zustand unten liegt und eine ausreichende Bewegungsfreiheit auch dann ermöglichen soll, wenn - aufgrund der anatomischen Gegebenheiten - die Pfanne so in den Beckenknochen einzementiert ist, daß der Pfannenrand gegen die Horizontale stark geneigt ist. Obwohl durch die Anfasung Bewegungsbehinderungen des Patienten und das Einklemmen von Nervensträngen
zwischen den prothetischen Oberschenkelteilen der Pfanne vermieden werden können, stützt sich der prothetische Hüftkopf bei steil einzementierten Pfannen Oftmals nur in einem kleinen oberen Bereich der Pfannen-Innenfläche sb, wodurch die sichere und zuverlässige Führung und Lagerung des Hüftkopfes in der Pfanne nicht immer gewährleistet ist.
Aufgabe der Erfindung ist es daher, die künstliche Huftgelenkspfanne der eingangs genanten Art derart weiterzubilden, daß auch stark geneigt einzementierte Pfannen den Hüftkopf des entsprechenden Oberschenkelteils des Hüftgelenks ausreichend umfassen und sicher aufnehmen.
Diese Aufgabe wird bei der Hüftgelenkspfanne der eingangs genannten Art erfindungsgemäß dadurch gelöst, daß daß die Pfanne unterhalb des Stützkragens einen Etützabschnitt aufweist.
Die Vorteile der Erfindung liegen insbesondere darin, daß die Hüftpfanne auch bei stark geneigtem Einbau, daß heißt bei einer spitzwinklig zu einer horizontalen Richtung verlaufenden Pfannen-Hauptachse, dem Hüftkopf ein ausreichend großes Gegenlager in der Pfannen-Innenfläche bietet. Dadurch wird sichergestellt, daß der Hüftkopf auch bei steil eingesetzten Pfannen einen ausreichenden Halt in der Pfanne erfährt, und daß die Krafteinleitung über eine ausreichend große Auflagefläche erfolgt, wodurch die unvermeidliche Verformung der Pfanne im einoperiexten Zustand bei Langzeitgebrauch verringert wird.

tn den Figuren 1 und 2 ist eine künstliche Hüftgelenkspfanne 1 aus Kunststoff, insbesondere aus einem fewebeverträglichen Polyethylen dargestellt. Die pfanne besitzt die Form eines Kugelschalen-Ab-Schnitts, der in einer ersten Meridianebene 7 über feinen Winkelbereich von etwa 180° einen auf der Pfanften-Außenflache 2 angeformten flanschartigen Stlitzkragen 6 aufweist, dessen freie Kante 8 unter Bildung feiner Hohlkehle 9 zur Außenseite hin umbördelt ist. tn die Außenfläche 2 der Pfanne 1 sind - in zu der fersten Meridianebene 7 parallelen Ebenen - umlaufende Muten oder Rillen 24 eingeformt, die beim Einoperiefcen in dem umgebenden Zementbett einen guten Fortnfcchluß, und damit einen festen Halt erzeugen sollen, feur Verbesserung der Haftung im Zementbett sind außerdem noch Quernuten 26 in die Außenfläche 2 eingearbeitet, die im Bereich des flanschatt.lgen Stützkragens 6 in Bohrungen in 28 enden, durch welche die Pfanne 1 mittels zusätzlicher Fizationsmittel mechanisch am Beckenrand befestigt oder angeschraubt werden kann.

einer Randausnehmung versehen, derart, daß der Rand 16 im Bereich der Anfasung 14 gegenüber der ersten Meridianebene 7 und einem vorgegebenen Winkel O^ verläuft.
In der dargestellten Ausführungsform der Pfanne 1 ist der Rand 12 des Stützabschnitts 10 und der Rand 16 im angefasten Bereich der Pfanne näherungsweise in einer Ebene, die gegenüber der ersten Meridianebene 7 um einen vorgegebenen Winkel oC* geneigt ist, der zwischen 20° und etwa 45° liegt. Die durch den Rand 12 und den Rand 16 aufgespannte Ebene verläuft näherungsweise durch den Mittelpunkt der Innenfläche 4 und wird daher als zweite Meridianebene 18 bezeichnet.

Künstliche Hüftgelenkspfanne aus Kunststoff, insbesondere zur Reoperation in dem Acetabulum des menschlichen Beckenknochens, wobei die Pfanne als Abschnitt einer Kugelschale ausgebildet ist, die in einer ersten Meridianebene über einen Bereich von etwa 180 ° einen nach außen vorstehenden, flanschartigen Stützkragen enthält, dessen freie Kante unter Bildung einer Hohlkehle zur Außenseite hin umbördelt ist, mit einer Anfasung der Kugel&chale im stützkragenfreien Bereich der Pfanne,
dadurch gekennzeichnet, daß die Pfanne (1) unterhalb des Stützkragens (6) einen Stützabschnitt (10) aufweist.

2. Künstliche Hüftgelenkspfanne nach Anspruch 1, dadurch gekennzeichnet, daß der Rand (12) des Stützabschnitts (10) und der Rand (16) des angefasten Pfannenbereiches näherungsweise in einer Ebene liegen, die gegenüber der ersten Meridianebene (7) um einen vorgegebenen Winkel O^ geneigt ist.
3. Künstliche Hüftgelenkspfanne nach Anspruch 2, dadurch gekennzeichnet, de3 der Winkel O^ zwischen 20° und 45° liegt.
4. Künstliche Hüftgelenkspfanne nach Anspruch 2 oder 3,
dadurch gekennzeichnet, daß der Rand (12) des Stützabschnitts (10) und der Rand (16) des angefasten IPfannenbereiches in einer zweiten Meridianebene (18) liegen.
