Description

  • GEBIET DER ERFINDUNG
  • Die vorliegende Erfindung bezieht sich auf eine Vorrichtung zum Unterstützen des Einsetzens eines prothetischen Implantats in einen Knochen in einer geeigneten Ausrichtung.
  • HINTERGRUND DER ERFINDUNG
  • Schäfte werden in prothetischen Gelenkimplantaten verwendet; um das Implantat in einem Knochenhohlraum zu verankern. Der Knochen wird typischerweise zum Aufnehmen des Schafts durch Bohren eines Loches in dem Knochen und durch Erzeugen einer Öffnung, die bemessen und geformt ist, um den Schaft des Implantats aufzunehmen, präpariert. Der Schaft wird in einen präparierten Hohlraum eines Knochens eingesetzt, und eine Gelenklagerfläche, die befestigt ist an oder verbunden ist mit dem Schaft, erstreckt sich außerhalb des Hohlraums. Viele Schäfte sind gerippt, d. h. sie weisen sich distal in den Schäften erstreckende longitudinale Nuten auf, die eine größere Schaftilität und Verankerung des Schaftes gewährleisten, wenn er in den präparierten Hohlraum eingesetzt ist. Ein sich distal erstreckender longitudinaler Abschnitt zwischen zwei solchen Nuten ist eine Rippe.
  • Wenn ein gerippter Schaft in einen präparierten Hohlraum eingesetzt worden ist, ist es typischerweise extrem schwierig, den Schaft zu drehen, um ihn geeignet umzuorientieren. Um dies zu tun, muss der Schaft entfernt und wieder eingesetzt werden, was den Knochen beschädigen und die Operationszeit erhöhen kann. Typischerweise wird der Schaft ausgerichtet, indem der Knochen und der Schaft markiert werden und die Markierungen verwendet werden, um den Schaft relativ zum Knochen zu orientieren. Der Nachteil dieser Methode ist die mögliche Ungenauigkeit in der Ausrichtung. Da die Markierung auf dem Schaft und dem Kno chen nicht nahe beieinander liegen, können eine Paralaxe und andere Probleme, die mit der Ausrichtung durch das Auge zusammenhängen, resultieren. Zudem kann sich der Schaft von seiner ausgerichteten Position weg bewegen, während er eingesetzt wird.
  • Dementsprechend ist eine Aufgabe der vorliegenden Erfindung, ein Mittel zum Ausrichten eines gerippten Schaftes eines orthopädischen Implantats, bevor und während es in einen präparierten Knochenhohlraum eingesetzt wird, bereitzustellen.
  • In EP-A-0342014 und WO88/01854 wird eine Hülse gezeigt, die in einen präparierten Knochenhohlraum zum führenden Einsetzen eines femoralen Implantats eingesetzt ist. Die Hülse erlaubt ein axiales Einsetzen, während eine Rotationsbewegung eingeschränkt wird. Wenn das Implantat eingesetzt ist, verbleibt die Hülse innerhalb des Knochenhohlraums.
  • ZUSAMMENFASSUNG DER ERFINDUNG
  • Erfindungsgemäß wird eine entfernbare Ausrichtungsführung, die zum führenden Einsetzen eines gerippten Teiles einer orthopädischen Prothese in einen präparierten Knochenhohlraum ausgebildet ist, wie in Anspruch 1 der beigefügten Ansprüche dargelegt, bereitgestellt.
  • Weitere Aspekte der Erfindung sind in den Ansprüchen 2 bis 13 der beigefügten Ansprüche dargelegt.
  • Der Knochen kann präpariert werden, indem Knochenmaterial aus dem Knochenkanal entfernt und eine Öffnung mit einer vorbestimmten Form in den Knochenhohlraum gespant wird. Bevorzugt entspricht das Rillenelement der Ausrichtungsführung dem Rippenteil auf eine Art, die zu einer Berührung mit einem kleinen Bereich des Schaftes führt.
  • Die Ausrichtungsführung kann im Wesentlichen in Form eines unterbrochenen Kreises oder Umfangs ausgebildet sein, um eine Flexibilität beim Entfernen der Ausrichtungsführung zu erreichen. Das Mittel zum Entfernen der Ausrichtungsführung von dem Schaft kann z. B. eine Lasche, ein Vorsprung oder eine Öffnung zum Einführen eines Werkzeuges zum Herausstemmen der Führung sein.
  • DETAILLIERTE BESCHREIBUNG DER ZEICHNUNGEN
  • 1 zeigt eine perspektivische Explosionsdarstellung eines Knochens, bei dem ein Teil eines präparierten Hohlraumes weggebrochen ist, einer Ausrichtungsführung und eines Implantats, das durch die Ausrichtungsführung hindurch eingesetzt ist;
  • 2 zeigt eine Perspektivansicht des Knochens, der Ausrichtungsführung und des Implantats aus 1 mit der Ausrichtungsführung an seiner Position und mit dem durch die Ausrichtungsführung hindurch zum Teil in den Knochen eingesetzten Implantat; und
  • 3 zeigt eine Perspektivansicht der erfindungsgemäßen Ausrichtungsführung.
  • DETAILLIERTE BESCHREIBUNG DER ERFINDUNG
  • Es folgt eine Beschreibung einer bevorzugten Ausführungsform der vorliegenden Erfindung in Verwendung mit einem Hüftschaftimplantat. Im Folgenden wird unter Bezugnahme auf die 1 bis 4 eine Ausrichtungsführung 10 beschrieben. Die Ausrichtungsführung 10 weist einen Grundkörper, einen Führungsabschnitt 12 und eine Öffnung 14 auf. Der Grundkörper und der Führungsabschnitt 12 bilden eine äußere Form, die zumindest einem Abschnitt eines bearbeiteten Hohlraumes 21 in dem Knochen 20 entspricht. Der Führungsabschnitt 12 hat einen inneren Umfang 15 mit Nuten oder Rillen 16 zum Aufnehmen von Rippen 31 des Schaftes 30 eines Implantats 50. Vorsprünge 13 in dem Führungsabschnitt 12 können den Schaft 30 des Implantats 50 berühren. In dieser Ausführungsform bilden die Nuten 16 und die Vorsprünge 13 ein Rillenelement zum führenden Einsetzen des Schaftes 30 in den Knochenhohlraum. In Varianten dieser Ausführungsform können Nuten 16, Vorsprünge 13 oder ähnliches allein oder zusammen verwendet werden, um den gerippten Schaft 30 rillengeführt einzusetzen.
  • 1 zeigt einen präparierten Knochen mit einem bearbeiteten Hohlraum 21 und einem longitudinalen Hohlraum 22, der in dem Kanal des Knochens ausgebildet ist. Zumindest ein Abschnitt des bearbeiteten Hohlraums 21 hat eine Form, die der Form der Ausrichtungsführung 10 entspricht. 1 zeigt weiterhin ein Implantat 50 mit einem Schaft 30, der sich distal entlang der Längenabmessung des Schaftes 30 erstreckende Rippen 31 aufweist; mit einem Vorsprung oder einer proximalen Geometrie 35, die geformt ist, um in den bearbeiteten Hohlraum 21 des Knochens zu passen; und mit einem Hals 33, der sich von dem proximalen Ende des Implantats erstreckt, zum Aufnehmen eines Kugelabschnittes einer Kugel- und Muffenverbindung.
  • Die Rillen 16 in dem inneren Umfang 15 der Führung sind so platziert, dass, wenn der Schaft 30 in die Ausrichtungsführung 10 eingesetzt ist, die Rippen 31 des Schaftes 30 in die Rillen 16 in der Ausrichtungsführung 10 eingefügt sind, um den Schaft 30 in einer geeigneten oder ausgerichteten Position innerhalb des Knochenhohlraums zu halten oder zu führen.
  • In dieser Ausführungsform ist die Form des bearbeiteten Knochenhohlraumes 21 im wesentlichen konisch, um einen elliptischen Endabschnitt 23 zu bilden. Die Ausrichtungsführung 10 ist ähnlich geformt wie der Grundkörper und bildet einen elliptischen Endabschnitt 18 an dem proximalen Ende des Grundkörpers. Auch das Implantat 50 weist einen ähnlich geformten proximalen Abschnitt mit einem elliptischen Endabschnitt 34 auf. Der Knochenhohlraum 21 weist weiterhin einen Einschnitt 24 auf, der den konischen Erweiterungen 19, 35 auf der Ausrichtungsführung 10 bzw. auf dem Implantat 50 entspricht. Der Einschnitt 24 des Knochenhohlraumes 21 und die konische Erweiterung der Ausrichtungsführung 10 verhindern, dass die Ausrichtungsführung 10 zu weit in die Knochenhohlräume 21, 22 eingesetzt wird. Der Einschnitt 24 des Knochenhohlraumes 21 und die kegelförmige Erweiterung 35 des Implantats 50 verhindern auf ähnliche Weise, dass das Implantat 50 zu weit in die Knochenhohlräume 21, 22 eingesetzt wird, und sie verhindern somit, dass der Hals 33 ungeeignet positioniert ist.
  • Im Gebrauch ist die Ausrichtungsführung 10 in den bearbeiteten Knochenhohlraum 21 eingesetzt, wobei der elliptische Endabschnitt 18 der Ausrichtungsführung 10 bezügl. des elliptischen Endabschnittes 23 des bearbeiteten Hohlraums 21 ausgerichtet ist. Der Schaft 30 des Implantats 50 ist in den inneren Umfang 15 des Führungsabschnittes 12 der Ausrichtungsführung 10 eingesetzt, wobei der elliptische Endabschnitt 34 des Implantats 50 im Wesentlichen in einer Linie mit einer Nut 9 entlang des elliptischen Endabschnittes 18 der Ausrichtungsführung 10 ausgerichtet ist. Um das Implantat 50 geeignet ausgerichtet einzusetzen, ist die proximale Geometrie 32 im Wesentlichen so ausgerichtet, dass sie in den bearbeiteten Knochenhohlraum 21 passt. Die Rippen 31 sind in den Rillen 16 des Führungsabschnittes 12 platziert, und das Implantat 50 wird geführt durch die Ausrichtungsführung 10 in den Knochenhohlraum 22 geschoben. Wenn der Schaft 30 in die Führung 10 eingesetzt ist, wird die Position oder Orientierung beibehalten, bis der Schaft 30 in dem Knochenhohlraum 22 eingesetzt ist. Während der Schaft 30 eingesetzt wird, bilden die Rippen 31 einen Pfad innerhalb des Knochenhohlraums 22, dem der Schaft während der Einführung des Abschlusssegments des Schaftes 30 im Wesentlichen folgt. Wenn das Implantat im Wesentlichen in den Knochenhohlräumen 21, 22 eingesetzt ist, aber bevor es vollständig eingesetzt ist, wie in 2 dargestellt, wird die Ausrichtungsführung 10 entfernt, z. B. durch Einsetzen einer Vorrichtung in die Öffnung 14 in der Führung 10 oder durch Ziehen einer Lasche oder eines Vorsprunges, um die Führung von dem Hohlraum 21 und von dem Schaft 30 des Implantats 50 zu entfernen. Das Implantat 50 wird dann weiter zu seiner Stelle geschoben, wobei der elliptische Endabschnitt 34 in den elliptischen Endabschnitt 23 des Knochens eingefügt wird und wobei die konische Erweiterung 35 benachbart zu einem Einschnitt 24 des Knochenhohlraums 21 an diesen angrenzt.
  • Obwohl die Ausrichtungsführung der vorliegenden Erfindung unter Bezugnahme auf eine besondere Ausführungsform, d. h. in Verwendung mit einem gerippten Hüftschaft, wobei die Rippen des Schaftes als Führungsrippe oder Referenz verwendet werden, beschrieben wird, ist es für einen Fachmann offensichtlich, dass verschiedene Änderungen gemacht werden können, ohne sich von dem Charakter und dem Umfang der Erfindung, wie sie durch die beigefügten Ansprüche definiert sind, zu entfernen. Zum Beispiel kann die Einsetzungsführung verwendet werden, um das Einsetzen von jeglicher schaftähnlicher Komponente zu unterstützen, die ein geripptes Merkmal auf der äußeren Oberfläche des Schaftes aufweist, um den Schaft in einer geeignet ausgewählten, ausgerichteten Position rillengeführt einzusetzen. Die Ausrichtungsführung kann jede beliebige Anzahl von Einkerbungen oder Vorsprüngen aufweisen, um das geeignete Einsetzen des Schaftes des Implantats durch Rillen zu führen.

Claims (13)

  1. Entfernbare Ausrichtungsführung (10), die zum führenden Einsetzen eines gerippten Teiles (30) einer orthopädischen Prothese (50) in einen präparierten Knochenhohlraum (21, 22) ausgebildet ist, wobei die Ausrichtungsführung entfernbar ist, wenn die Prothese (50) in den Knochenhohlraum (21, 22) eingesetzt ist, umfassend: einen Körper, der eine äußere Form aufweist, die mit zumindest einem Abschnitt (21) des präparierten Hohlraums übereinstimmt; einen inneren Abschnitt (12) des Körpers zum Aufnehmen des gerippten Teiles (30), wobei der innere Abschnitt (12) ein Rillenelement (13; 16) aufweist, das darin zum ineinandergreifenden Aufnehmen des gerippten Teiles (30) ausgebildet ist.
  2. Ausrichtungsführung nach Anspruch 1, dadurch gekennzeichnet, dass die Ausrichtungsführung zum Aufnehmen eines gerippten Schafts (30) eines prothetischen Verbindungsteiles (50) ausgebildet ist.
  3. Ausrichtungsführung nach Anspruch 2, dadurch gekennzeichnet, dass das Rillenelement zumindest eine Nut (16) aufweist.
  4. Ausrichtungsführung nach einem der Ansprüche 2 oder 3, dadurch gekennzeichnet, dass das Rillenelement zumindest einen Vorsprung (13) aufweist.
  5. Ausrichtungsführung nach Anspruch 3, dadurch gekennzeichnet, dass das gerippte Teil (30) zumindest eine Rippe (31) aufweist, die sich longitudinal auf dem Schaft (30) erstreckt.
  6. Ausrichtungsführung nach einem der Ansprüche 2 bis 5, dadurch gekennzeichnet, dass der Schaft (301 gerippt ist und dass das Rillenelement (13; 16) des inneren Abschnittes (12) eine innere Wand (15) aufweist, die zum ineinandergreifenden Aufnehmen zumindest einer Rippe (31) des gerippten Schaftteiles (30) ausgebildet ist.
  7. Ausrichtungsführung nach Anspruch 6, dadurch gekennzeichnet, dass das Rillenelement eine Vielzahl von Nuten (16) aufweist, die in der inneren Wand (15) zum Aufnehmen einer Vielzahl von Rippen (31) des gerippten Schaftteils (30) ausgebildet sind.
  8. Ausrichtungsführung nach Anspruch 6, dadurch gekennzeichnet, dass das Rillenelement einen Vorsprung zum Zusammenwirken mit dem Schaftteil (30) in einer vorbestimmten Position aufweist.
  9. Ausrichtungsführung nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass die Ausrichtungsführung weiterhin Mittel zum Entfernen der Führung (10) von dem Schaft (30) aufweist.
  10. Ausrichtungsführung nach einem der vorhergehenden Ansprüche, dadurch gekennzeichnet, dass die Ausrichtungsführung weiterhin Mittel (14) zum Entfernen der Führung (10) von dem proximalen Hohlraum (21) des Knochens (20) aufweist.
  11. Ausrichtbares, prothetisches, orthopädisches Implantatsystem mit: einer Prothese (50) mit: einem Schaft (30) mit einem proximalen Abschnitt (33, 35); einem distalen Abschnitt (30) und zumindest einer Rippe (31), die sich entlang des Schafts (30) erstreckt; wobei der proximale Abschnitt (33, 35) eine proximale Geometrie (35) aufweist, die so geformt ist, dass sie einem proximalen Hohlraum (21), der in einem Knochen (20) zum Aufnehmen der Prothese (50) ausgebildet ist, entspricht; und einer Ausrichtungsführung (10), die entfernbar ist, wenn die Prothese (50) in den Knochenhohlraum (21) eingesetzt ist, mit: einem Körper mit einer äußeren Form, die mit dem proximalen Hohlraum (21) des Knochens (20) übereinstimmt, einer Öffnung im Körper zum Aufnehmen des gerippten Schafts (30) der Prothese, wobei die Öffnung eine innere Wand (15) mit zumindest einer Nut (16) darin zum Aufnehmen zumindest einer Rippe (31) des Schafts (30) ausbildet.
  12. Implantatsystem nach Anspruch 11, dadurch gekennzeichnet, dass die Ausrichtungsführung (10) weiterhin Mittel (14) zum Entfernen der Führung von dem Schaft (30) aufweist.
  13. Implantatsystem nach einem der Ansprüche 11 oder 12, dadurch gekennzeichnet, dass die Ausrichtungsführung (10) weiterhin Mittel (14) zum Entfernen der Führung von dem proximalen Hohlraum (21) des Knochens (20) aufweist.
