Abstract
Description
The invention relates to a bipolar RF coagulation device Cutting device for minimally invasive surgery.
An instrument of the type is the German patent application P 43 03 882 refer to. This instrument has two of them Shaft axis of vertically protruding jaw parts between two End positions parallel to and away from each other can be moved away and form a pair of pliers. In the contact areas of the merged jaw parts is a coagulation area embedded, which are mirror images of each other. The mouth form and enclose parts in the folded state a simple cavity in which a surgical knife be moved through the contact plane of the jaw parts can. Tissue absorbed between the jaw parts can be so be regulated and cut immediately afterwards. The Opera tion movements are made by one at the near end of the steerer attached handle part over drive device running therein carried out. By being perpendicular from the shaft axis protruding jaws, the instrument is relatively sparse rig.
From the publication DE 42 04 051 A1 an anvil shears for (high frequency) surgical purposes, in particular for lapa known roscopic operations. These scissors have a firm standing jaw part (anvil) and a movable jaw part (Blade) on. The blade can be swiveled on a Ge at the anvil steering pin and is operated by a pull rod. From a given position, the Blade perform a pure stroke movement, but in Longitudinal direction of the blade takes place.
In another publication, EP 0 572 131 A1, a bipolar surgical scissors with a swiveling and egg nem fixed cutting blade described, also here actuation by means of a rod over a handle follows. This instrument is also for the minimally inva sive surgery provided.
The invention has for its object an RF coagulation with integrated cutting device and one-handed Serve handle part to provide that in the minimally inva sive surgery can be used.
This object is achieved by the Merk listed in claim 1 times solved. In subclaims 2 to 6 are additional Refinements characterized, the gripper-like In affect the distal end. The further subclaims che 7 to 10 characterize designs of the handle part on proximal end of the device.
The advantages of the RF coagulation device with integrated Cutting edges consist in the design of the pliers. your movable jaw has two ranges of movement. The touch surfaces of both folded jaw parts are removed when opening, first parallel to a predetermined one Distance from each other, then the movable jaw part white ter about its axis of rotation up to a maximum foldable Position can be rotated. This is between the coagulati Tissue collected on the surfaces can be coagulated evenly and then separable between the coagulation zones. By opening the jaws wide, tissue can easily be picked up.
The design of the handle part separates them clearly and safely Gripping movement from the cutting movement through a pressure point, which is made noticeable via the spring mechanism and with the an operating hand can be overcome.
The axially movable and about the shaft axis via a pre Giving angle rotatable steering wheel also allows rotating the forceps attached to the distal end as well limited swiveling of the entire forceps, no matter which one Position the jaw parts straight to each other.
The invention is described below with reference to the drawing illustrated embodiments explained.

Figs. 1a to 1c show the forceps at the distal end of the Koagulationsgeräts with the three winning positions of the movable jaw part 1. In Fig. 1a, the movable jaw part 1 is in its initial position and most open, ie rotated about the axis of rotation 2 away from the fixed jaw part 3 . The resilient tongues 4 , which are an extension of the agulation surfaces to the rear, hold the movable jaw part 1 in position and press it into the opened position when the overtube 5 is retracted. The contact surfaces of the two jaw parts 1 , 3 are corrugated over their length, so that collected tissue can be held more securely. The movable jaw part 1 is rounded at its free end, the fixed jaw part 3, on the other hand, is somewhat tapered and completely rounded with a smaller radius. The latter serves to make it easier to collect the tissue. The rounding prevents tissue punctures.
Die Zungen 4 beider Maulteile bilden vorzugsweise auch die Strom zuführung zu den Koagulationsflächen 31.Both jaw parts 1 , 3 have similar coagulation surfaces 31 inserted, which move backwards into the open as resilient tongues 4 ra. If the jaw parts 1 , 3 are placed on the shaft, they serve as a resilient clamping element in the movable jaw part 1 and as a position stabilization in the fixed jaw part 3 , which is anchored by the locking finger 32 at the shaft end. The tongues 4 of both jaw parts preferably also form the current supply to the coagulation surfaces 31 .
By moving the overtube 5 from its initial position according to FIG. 1a, the movable jaw part 3 is rotated about the axis of rotation 2 and swings toward the fixed jaw part 3 , namely up to the intermediate position in which the two contact surfaces of the jaw parts 1 , 3 are parallel to one another ( Fig. 1b). From this intermediate position, the rotary movement of the movable jaw part 1 is inhibited. With further advancement of the overtube 5 , an elongated hole 6 in the bearing for the movable jaw part 1 only allows a displacement of the axis of rotation 2 perpendicular to the pliers axis 7 , so that the contact surfaces of the two jaw parts 1 , 3 while maintaining the parallel setting up to completely closed end position, which are further moved towards one another in FIG. 1c. When retracting the overtube 5 , the movable jaw part 3 automatically goes back into one of the positions of FIGS . 1c to 1a through the holding and retracting tongues 4 .
FIGS. 2a to 2c show the lationsgeräts patch at the proximal end of the Koagu gripping device with which the pliers on di stalen end via coaxial, run into one another tubes 5, 20 Be is actuated and in the cylindrical cavity of the RF Verbin dung on the coagulation the jaw contact surfaces.
First of all, the fixed handle part 8 and the handle part 10 , which is rotatably mounted about the axis 9, are widely spaced apart by the tension of the installed spring 11 ( FIG. 2a). The handle plate 12 is rigidly connected to the movable handle part 10 and rotates about the axis 9 . The radially point of rotation 9 pointing, rounded at its free end bolt 13 , which protrudes beyond the curvilinear part of the engagement plate 12 and engages in the blind hole of the guide piece 14 radially attached to the overtube 5 , moves when tightening the movable handle part 10 Over tube 5 to the front and thereby folds the movable jaw part 1 to the fixed until the jaw parts 1 , 3 are completely folded.
During the change from the position according to FIG. 2a to the position according to FIG. 2b, the lever mechanism 15 , 16 , 17 does nothing. The lever 15 is guided on the one hand with its one end in the elongated hole of the disc 18 , which is fixedly attached to the handle plate 12 , and on the other hand rotatably attached to one end of the angled lever 16 which is rotatably mounted on its kink area. At the other end of the angled lever 16 , in turn, the rotatably mounted lever 17 attaches, which acts with its other end on the drive tube 20 for the surgical knife 19 embedded in the pliers.
The use of the knife actuation is inhibited by the radially acting spring mechanism 21 placed at the end of the near pipe (see magnifying glass in FIGS . 2a, b, c) and must be released by overcoming the pressure point of the spring mechanism 21 generated by the ball 22 . The Federmechanis mus 21 consists of at least one radially pressed onto the tubular drive 5 by spring force into the recess 22 , which inhibits the ser ser while collapsing the jaw parts 1 , 3 . Depending on the need for stability, several, such as three such locking mechanisms can be evenly distributed around the circumference of the pipe.
Fig. 2c shows the situation of the fully contracted handle parts 8 , 10 and in the magnifying glass in this position ineffective spring mechanism 21st The surgical knife ser 19 is now pushed all the way forward in the guide of the pliers and has cut through the collected tissue between the two coagulation points. When the movable handle part 10 is released, the restoring force is supported by the cylinder spring which is tensed during the cutting process. It is housed in the cylinder housing 26 .
The between the handle and the spring mechanism coaxial lying steering wheel at the near end of the tube can be rotated about the axis 24 in both directions by up to about 120 ° and axially shift so far that the pliers attached at the far end by up to 40 ° against Axis 24 can be pivoted (see Fig. 3).
The socket 27 has access to the cavity of the shaft 28 . It is designed for flushing, suction and lighting purposes and is flanged to the appropriate device.
Fig. 4 zeigt die Realisierung einer solchen Antriebsvariante. The lever mechanism 15 , 16 , 17 can be replaced by a rack 29 and two spur gears 28 , 27 if the engagement disc 12 still has a corresponding toothing on its curved edge GE. Fig. 4 shows the realization of such a drive variant. The movement of the jaw parts 1 , 3 occurs in the manner described up to a recess 27 in the engagement disc 12 , up to which the two spur gears 27 , 28 experience no drive when the handle parts 8 and 10 contract . Only when pulling through further does the teeth interlock and move the knife via the drive tube 20 .
Fig. 5 shows a further variant for the lever mechanism 15 , 16 , 17 , which is realized with a cam plate 28 and a spur gear 29 . For this purpose, the engagement disk 12 on its curve edge, the spur gear 29 and the cam disk 28 also have teeth. During the closing of the jaw parts 1 , 3 , the cam disc 28 only moves with it, without acting on any part. From a position that can be given before, it strikes the drive tube 20 for the surgical knife 19 and moves it.
In both variants, the spring in the cylinder housing 26 drives the return drive if the movable handle part 10 is released.
The surgical knife 19 in the pharynx 30 of the forceps can have different shapes, depending on which shape is appropriate for severing the collected tissue between the two coagulation marks. In FIGS. 6a-e, the variants are shown successively.
Fig. 7a zeigt die einfachste Be rührflächenform der Ebene. Three design variants of the coagulation surfaces 31 are shown in FIGS . 7a to c in a section through the jaw parts 1 , 3 with parallel contact surfaces, and in pairs in the closed state of the jaw parts 1 , 3 and there besides in the slightly opened. Fig. 7a shows the simplest loading surface shape of the plane. FIG. 7b, the two intersect the planes at a predetermined angle α, the line of intersection in the throat 30 extends. In a cylinder surface segment, the coagulation surfaces are finally in FIG. 7c, the axis of the pliers running parallel to the lateral surface axis.
