Description

Die Erfindung betrifft eine in einer natürlichen Beckenknochenpfanne fixierbare, im wesentlichen halbkugelförmige Gelenkpfanne nach dem Duo- Kopf-Prinzip für ein künstliches Hüftgelenk.
Duo-Kopf-Gelenkpfannen sind seit langem bekannt. Sie weisen in der Regel eine hochglanzpolierte Außenpfanne, eine darin gelagerte Zwischenschale und einen daraus nicht luxierbar gelagerten künstlichen Gelenkkopf auf, der in der Zwischenschale über seinen Äquator hinaus in die Zwischenschale hineingedrückt ist. Bei der Implantation einer derartigen Duo-Kopf- Gelenkpfanne wird das Acetabulum, welches sich im weitgehend intakten Zustand befindet, nicht ausgefräst, wie dies bei Einsatz der üblicherweise verwendeten zementierbaren oder zementlos zu fixierenden Gelenkpfannen vorgenommen wird. Allerdings sind die Indikationsfälle der letztgenannten Gelenkpfannen ganz andere als jene, bei denen der Einsatz einer Duo-Kopf- Gelenkpfanne in Betracht gezogen wird. Die meist hochglanzpolierte Außenschale wird einfach in das natürliche Acetabulum mit dem darin belassenen Knorpel gesetzt. Die künstliche Gelenkpfanne wird sodann mit einem Hüftstiel verbunden, beispielsweise durch eine konische Klemmung zwischen dem Kugelkopf und dem Hüftstiel. Es wird davon ausgegangen, daß sich die Gelenkpfanne selbständig die richtige Lage sucht und darin verharren wird.
Problematisch sind allerdings die Fälle, in denen Mikrobewegungen für eine Verlagerung der Außenschale im Acetabulum sorgen.
Aufgabe der vorliegenden Erfindung ist es vor diesem Hintergrund, eine künstliche Gelenkpfanne nach dem Duo-Kopf-Prinzip für ein Hüftgelenk so auszubilden, daß sie sicher in ihrer Einbaulage verharrt.
Gelöst wird diese Aufgabe durch die Gelenkpfanne mit den Merkmalen des Anspruchs 1. Vorteilhafte Weiterbildungen ergeben sich aus den Unteransprüchen.
Demnach wird vorgeschlagen, die eingangs erwähnte Gelenkpfanne so auszubilden, daß sie eine Anzahl radial von ihrer Außenkalotte abstehender Fixationselemente aufweist und mit einer die Außenkalotte an den übrigen Stellen bedeckenden Schicht von Hinterschnitten und/oder offene Maschen bildenden Partikeln versehen ist.
Knorpel und das natürliche Bindegewebe im und am Acetabulum bleibt - wie oben ausgeführt - erhalten. Die erfindungsgemäße Duo-Kopf-Gelenkpfanne wird in das an sich intakte Acetabulum gedrückt, wobei lediglich die radial von der Außenkalotte abstehenden Fixationselemente in den Knorpel hineingreifen. Die Fixationselemente sorgen mithin für eine Primärfixation und eine Rotationsarretierung der Gelenkpfanne im Acetabulum. In die an den übrigen Stellen die Außenkalotte bedeckenden Schicht von Hinterschnitten und/oder offenen Maschen wächst hier kein Knochenmaterial ein, da sie ja auch nicht unmittelbar an der Knochenwand des Acetabulums anliegt. Vielmehr dient diese Schicht im vorliegenden Fall dazu, daß Bindegewebe einwächst und so für ein Auffangen von Mikrobewegungen sorgt.
Gemäß einer bevorzugten Ausführungsform sind die Fixationselemente als Domen ausgebildet, so daß diese beim Eindrücken der Duo-Kopf- Gelenkpfanne in das Acetabulum Gelegenheit haben, die Knorpelschicht des Acetabulums zu durchdringen.
Die von den Partikeln gebildeten Maschen sind vorteilhaft so dimensioniert, daß sie eine Maschenweite im Bereich von 0,5 bis 2,5 mm aufweisen.
Bevorzugt sind wenigstens vier Fixationselemente vorgesehen, die radial von der Außenkalotte abragen. Dies verleiht der Gelenkpfanne eine hinreichend hohe Rotationsarretierung und eine ausreichende Primärfixation.
Vorteilhafterweise sind die Fixationselemente mit der Gelenkpfanne nach dem Duo-Kopf-Prinzip verschraubt, und zwar in darin vorgesehenen Durchbohrungen mit Innengewinde. Es können eine ganze Anzahl von solchen Durchbohrungen mit Innengewinde auf der Kalotte der Gelenkpfanne vorgesehen sein, beispielsweise sechs oder acht. Diese Ausführung gestattet es, die Fixationselemente individuell den Verhältnissen im jeweiligen Acetabulum auf der Außenkalotte verteilt anzubringen.
Die Erfindung wird anhand eines Ausführungsbeispiels gemäß der Zeichnungsfiguren näher erläutert. Hierbei zeigt:
Fig. 1 einen Äquatorialschnitt durch die erfindungsgemäße Gelenkpfanne, und
Fig. 2 eine Aufsicht auf die Gelenkpfanne gemäß Fig. 1.
Wie aus Fig. 1 ersichtlich, weist die Gelenkpfanne 1 eine in etwa halbkugelförmige Kalotte auf, in die in an sich bekannter Weise eine Zwischenschale 6, beispielsweise aus hochverdichtetem Polyethylen, eingesetzt ist, welches den Gleitpartner für eine künstliche Gelenkkugel 5 bildet.
Von der Außenkalotte ragen radial drei Fixationselemente 3 nach außen ab. Beim Eindrücken dieser Gelenkpfanne 1 in das natürliche Acetabulum durchgreifen die im vorliegenden Falle als Domen ausgebildeten Fixationselemente die Knorpelschicht und die Wandung des Acetabulum. Um eine Rotationsarretierung zu erreichen, müssen wenigstens zwei Fixationselemente 3 vorgesehen sein.
Die Außenkalotte ist an den übrigen Stellen mit einer Schicht 4 von Partikeln versehen, die Hinterschnitte und/oder offene Maschen ausbilden, in welche Bindegewebe einwachsen soll.
In einer anderen Ausführungsform sind die Dornen 3 kein einstückiger Bestandteil der Kalotte der Gelenkpfanne 1, sondern lassen sich vielmehr mittels eines Schraubgewindes in - hier nicht dargestellten - Durchbohrungen in der Außenkalotte der Gelenkpfanne 1 schrauben. Damit lassen sich individuelle Implantate schnell und problemlos zusammenstellen.

Claims (5)

1. In einer Beckenknochenpfanne fixierbare, im wesentlichen halbkugelförmige Duo-Kopf-Gelenkpfanne (1) für ein künstliches Hüftgelenk mit einer Anzahl radial von der Außenkalotte abstehender Fixationselemente (3) und mit einer die Außenkalotte an den übrigen Stellen bedeckenden Schicht (4) von Hinterschnitten und/oder offene Maschen bildenden Partikeln.
2. Gelenkpfanne nach Anspruch 1, bei der die Fixationselemente (3) als Dornen ausgebildet sind.
3. Gelenkpfanne nach Anspruch 1 oder 2, bei der die von den Partikeln gebildeten Maschen eine Maschenweite im Bereich von 0,5 mm bis 2,5 mm aufweisen.
4. Gelenkpfanne nach einem der Ansprüche 1 bis 3, bei der wenigstens vier Fixationselemente (3) radial von der Außenkalotte abragen.
5. Gelenkpfanne nach einem der Ansprüche 1 bis 4, bei der die Fixationselemente (3) mit der Gelenkpfanne (1) verschraubt sind.
