Description
Bei einer Kataraktoperation wird die natürliche Augenlinse,
welche ihre Durchsichtigkeit verloren hat, gegen eine künst
liche Intraokularlinse ersetzt. Hierbei werden Operations
mikroskope verwendet, deren Lichtquelle in aller Regel
Halogenlampen sind, die auch im Ultraviolettbereich Strahlen
aussenden. Bekanntlich ist die Netzhaut empfindlich gegen
über Ultraviolettstrahlen. Kurzwellige UVC(100-280nm) -
Strahlung im Sonnenlicht wird durch die Ozonschicht ausge
filtert und kommt daher auf der Erde nicht vor. Der
UVB(280-320 nm)-Strahlungsanteil, welcher etwa 3% der
Sonnenlicht-Ultraviolettstrahlung ausmacht, wird am natür
lichen Auge von der Hornhaut und Bindehaut ausgefiltert.
Der UVA(320-400 nm)-Anteil wird durch die natürliche
Augenlinse absorbiert. Wenn während der Kataraktoperation
die natürliche Augenlinse entfernt ist, besteht die Gefahr
der Netzhautschädigung durch den entsprechenden Ultraviolett
strahlungsanteil der Lichtquelle des Operationsmikroskops.
Dieser Strahlungsanteil muß daher aus dem Licht des Opera
tionsmikroskops herausgefiltert werden. Hierzu ist es be
kannt, UV-Schutzfilter in entsprechende Filterhalter des
Operationsmikroskops einzusetzen. Dieses UV-Schutzfilter
bzw. mehrere derartige verschiedene UV-Bereiche abdeckende
Schutzfilter sind nur eine Art von den mehreren Filterarten,
welche bei einem Operationsmikroskop zum Einsatz kommt.
Beispielsweise gehören zum Filtersatz eines solchen Mikro
skops noch Grauglasfilter, Tageslichtfilter, Grünfilter,
Wärmeschutzfilter und dgl. mehr, die je nach Verwendung des
Operationsmikroskops insbesondere in der Mikrochirurgie zum
Einsatz kommen.
Die Bereitstellung dieser Filter und der jeweilige Einsatz
der Filter im Filterhalter erfordert eine ungewöhnlich hohe
Sorgfalt, um Risiken für die Gesundheit des zu behandelnden
Patienten auszuschließen. Die Handhabung der Filter ist daher
zwangsläufig mit Fehlerquellen verbunden.
Aufgabe der Erfindung ist es daher, das Risiko der Netzhaut
schädigung bei einer Kataraktoperation, bei welcher die na
türliche getrübte Augenlinse gegen eine künstliche Augen
linse ausgewechselt wird, zu minimieren.
Diese Aufgabe wird erfindungsgemäß durch ein Filter gelöst,
welches in Form einer präcorneal anwendbaren Lösung vor
liegt, in welcher eine UV-, insbesondere UVA(400-320 nm)-
Absorbersubstanz, enthalten ist.
Diese in bevorzugter Weise viskoelastisch ausgebildete Lö
sung wird vor dem Entfernen der natürlichen Augenlinse
auf die Außenseite der Hornhaut aufgebracht. Durch die in
der Lösung enthaltene UV-Absorbersubstanz werden dann vor
allem die UVA-Spektralanteile der Lichtquelle, insbesondere
einer Halogenlampe des Operationsmikroskops, abgedeckt bzw.
ausgefiltert, so daß keine Beschädigung der Netzhaut durch
diesen Strahlungsanteil während der Kataraktoperation mehr
entsteht. Selbst bei einer Fehlbedienung beim Einsetzen des
Filters im Operationsmikroskop wird durch die Absorber
substanz, welche in der auf die Hornhaut aufgebrachten Lösung
enthalten ist, die Netzhaut des Auges bei entfernter natür
licher Augenlinse gegen UV-Strahlen geschützt.
In bevorzugter Weise kommt als viskoelastische Lösung eine
Hydroxypropylmethylcellulose(HPMC)-Lösung zum Einsatz. Ins
besondere handelt es sich hier um eine 2%ige HPMC-Lösung.
Die zum Einsatz kommende HPMC besitzt bevorzugt ein mittleres
Molekulargewicht von 80 000 bis 90 000. Der Polymerisations
grad kann zwischen 400 und 450 liegen. Der Anteil an Methoxy-
Gruppen kann bei 25 bis 35% und der Anteil an Hydroxygruppen
bei 8 bis 9% liegen. Die 2%ige HPMC-Lösung besitzt bei
20°C bevorzugt eine Viskosität von 4 bis 5 Pa×s. Die
Osmolarität beträgt etwa 180 bis 320, und der pH-Wert liegt
bei 5,5 bis 6,7. Die Bestandteile der Lösung können bevor
zugt so ausgewählt werden, daß 1 ml Lösung enthält:
Als UV-Absorbersubstanz, insbesondere für den
UVA(400-320 nm)-Spektralanteil, kommen bevorzugt Benzo
triazol-Derivate in Frage. Insbesondere handelt es sich
hier um
2-(3′-tert-butyl-2′-hydroxy-5′-methylphenyl)-5-chlorobenzo
triazol;
2(2′-Hydroxy-3′,5′-di-tert-amylphenyl)benzotriazol;
2(2′-Hydroxy-5′-methylphenyl)benzotriazol;
2-tert-Butyl-6-(5-chlor-2H-benzotriazol-2-yl)-4-cresol;
und
2-tert-Butyl-6-(5-chloro-2H-benzotriazol-2-yl)-p-cresol.
2(2′-Hydroxy-3′,5′-di-tert-amylphenyl)benzotriazol;
2(2′-Hydroxy-5′-methylphenyl)benzotriazol;
2-tert-Butyl-6-(5-chlor-2H-benzotriazol-2-yl)-4-cresol;
und
2-tert-Butyl-6-(5-chloro-2H-benzotriazol-2-yl)-p-cresol.
Als UV-Absorbersubstanzen sind jedoch auch 2-Hydroxybenzo
phenon-Derivate verwendbar.
In bevorzugter Weise ist die UV-Absorbersubstanz in die
HPMC einpolymerisiert. Die UV-Substanz kann jedoch in der
viskoelastischen Lösung auch dispergiert sein.
Claims (7)
1. Filter zum Schutz der Netzhaut eines Auges gegen Ultra
violettstrahlen, insbesondere UVA(400-320 nm) -Strahlen,
bei einer Kataraktoperation, gekennzeichnet durch eine
präcorneal anwendbare Lösung, in welcher eine UV-, insbeson
dere UVA(400-320 nm)-Absorbersubstanz enthalten ist.
2. Filter nach Anspruch 1, dadurch gekennzeichnet, daß die
Lösung eine viskoelastische Lösung ist.
3. Filter nach Anspruch 2, dadurch gekennzeichnet, daß die
viskoelastische Lösung eine Hydroxypropylmethylcellulose-
(HPMC)-Lösung ist.
4. Filter nach Anspruch 1, dadurch gekennzeichnet, daß der
UV-Absorber ein Benzotriazol-Derivat ist.
5. Filter nach einem der Ansprüche 1 bis 4, dadurch gekenn
zeichnet, daß der UV-Absorber in die Lösung einpolymerisiert
ist.
6. Filter nach einem der Ansprüche 1 bis 5, dadurch gekenn
zeichnet, daß der UV-Absorber in der Lösung dispergiert ist.
7. Filter nach einem der Ansprüche 1 bis 6, dadurch gekenn
zeichnet, daß die Lösung eine Viskosität von 4 bis 5 Pa×s bei
20°C aufweist.
