Abstract
Description
Composites are novel materials with their own physical and material properties, which are particularly suitable for replacing the body's own tissue. They consist of a plastic matrix, in which up to 80% inorganic fillers in z. Part of the microscopic grain size is embedded as a so-called microfiller. Composite materials have recently been used for the production of dental fillings in a direct process, in that the material is introduced plastically into the tooth cavity and brought to polymerization there. Another application is found in the indirect process, in which a so-called die model is made from the tooth preparation and a composite inlay or onlay is produced on it in the laboratory process by modeling and polymerization. So far, composites have been widely used as prefabricated replacement teeth for partial and full dentures. In crown and bridge prosthetics, composites have so far only been used for veneering a labial facet, ie the outer surface of a crown cast in dental metals in the predominant part of the anatomical shape. The crown according to the invention and its manufacturing method relate to a completely new conception of the crown technique, in which the model tooth is blunt only with a very thin metal layer of metal foil or consists of very thinly cast and reworked casting alloy ( 2 ) or is completely omitted as a limit case ( 3 ) Fig. 1. On this film cap there are micro and / or macro retentions ( 4 ) and their surface is optionally provided with a chemical composite system so that the composite ( 1 ) enters into a flat connection with the metal structure, free of capillary gaps. In a preferred embodiment, the framework is formed from a 60 to 120 μm thick metal foil, the surfaces of which are provided with fine gold for diffusion fusion and to which an adhesive has been applied, a gold mesh ( 4 ) has been burned on or chemical composite systems, preferably silanes, have been applied .
Previous experience with composite materials has shown that laboratory mixing the material properties too impaired. That's why income is used today Component composites in plastic form, which after application and modeling only under temperature and pressure polymeri be settled. One-component composites cannot more in the previous modeling technique, e.g. B. means Brushes are piled up. Freehand modeling has a spatial shape on a thin scaffold cap tough plastic material then unsuitable as soon as ver different layers are layered on top of each other on a round Object must be arranged.
The manufacturing method according to the invention consists in that the composite ( 1 ) is industrially pre-layered ( 7 ) in a hollow mold ( 6 ) analogous to a tooth crown in accordance with the later tooth color and light-optical depth effect. In this plastic semi-finished product, the crown cap ( 2 ) provided with chemical and mechanical retentions ( 4 ) only needs to be pressed in on both sides and pulled out again as a composite from the elastic hollow form in order to be polymerized after individual, light shape modeling. Compared to the previous production of veneer crowns of all types, this process saves an enormous amount of time and almost eliminates the known modeling errors with regard to color and light-optical effect. The composite tooth crown manufacturing method according to the invention is used in four preferred variations:
Preforming an anatomical crown according to the topographical arrangement in the dental arch in an elastic hollow form with the individual layer parts ( 10 ) made of opaquer, neck mass, dentine core and enamel mass as well as the central cavity for pressing in the crown framework ( Fig. 2).
Preforming of the labial ( 9 ) and palatal ( 9 a) facets or half-shells of a tooth crown and taking up the composite materials one after the other by repeatedly pressing in and then modeling the separation points ( Fig. 3).
Preforming the individual layer portions ( 10 ) accordingly the color range for the optional inclusion of individual layer portions according to the color scale ( Fig. 4).
Preforming of the layer materials by the dental technician itself by the elastic negative forms of a Tooth crown as a modeling or transfer aid serves. Separate forms are available here, the dimensions of the gradual stratification related corresponds to the order of the stratification masses.
The manufacturing method according to the invention, via a separate, pre-layered half-shell labially and palatally, is rationalized and error-free in that the elastic shapes ( 6 ) are introduced into a device which bring the half-shells together on or around the crown framework with a precise fit.
In another preferred embodiment, the tooth crown made of composites without a metal inner frame is provided directly on the tooth model die over a distance or insulating layer ( 5 ), namely when a composite is selected whose carrier matrix consists of tensile material.
Another variant of the production according to the invention The procedure of the composite tooth crown consists of the anatomical crown shape made of one material (e.g. wax) model that burns without residue. After manufacture This primary material is removed from a mold and the composite injected into the cavity or pressed in under pressure. There Color-optical layering is not possible in this process is preferably transparent or translucent Composites used, which is then painted.
A further embodiment of the method according to the invention is that thin, transparent plastic films ( 12 ) are used for the prefabrication of unpolymerized composite half-shells, which reproduce the labial tooth facet and sepa rat the palatal or lingual facet or half of the tooth like a formwork and with each other press around a tooth stump or a crown framework to give a full crown. These half formworks are provided individually and / or with a handle on the outside or are connected to each other in such a way that they are pressed against each other in the same form, or they are individually in a large range, aligned with color and shape, filled with color-layered, unpolymerized composite . In the case of various composites, the color-covering base layer, the so-called opaquer, cannot be produced as a one-component layer or layer shell, but is mixed with powder and liquid. In these cases, the process according to the invention for producing a composite crown is modified such that the opaquer is applied and polymerized in a conventional manner. Only then is the crown framework provided with the polymerized colored top layer introduced into the course of the process according to the invention by pressing into the preformed composite molds.
The tooth crown made of composite material and its manufacture moving is a crucial innovation in the direction on the cost reduction of restorative measures in relation on the rationalization or easier work for the Laboratory for flawless tooth shade reproduction and optical effect - but especially with regard to tissue compatibility thanks to a precisely fitting crown margin by the preferred use of the film frame or the framework-free composite crown the problems of casting contraction and ceramic firing are completely eliminated.
