Description
Extrakorporale Schockwellen eignen sich zur Lithotripsie von Gallen oder Harnsteinen.
Die Schockwellen können z. B. durch eine Funkenentladung, einen
laserinduzierten Durchbruch oder auf piezoelektrischem Wege erzeugt werden.
Mit Hilfe einer "Kallotte" werden diese Schockwellen dann im zweiten
Brennpunkt fokussiert, der im Zielvolumen liegen sollte (z. B. Nierenstein, Gallenstein).
Die Schockwellen durchwandern dabei das Körpergewebe noch unfokussiert
und werden erst im Brennpunkt maximal fokussiert ihre Wirkung entfalten.
Knochen erweist sich dabei als "schalldicht", während alle anderen Organ- und Bindegewebe
im Körper den Schall ohne Schaden für das Gewebe weiterleiten.
Lediglich im Lungengewebe kann es zu Zerreißungen von Alveolen bei
Durchwanderung von Schockwellen kommen.
Bisher wurden auf Grund des sehr großen Wellenwiderstandes zwischen
Steinen und Gewebe ausschließlich sehr harte Strukturen im Körper mit
Stoßwellen disintegriert (Nieren- und Gallensteine).
Beschießt man fokussiert mit hoher Energie Gewebe, so kommt es zu Zerreißungen
von Geweben und Zellen, z. B. mit nachfolgender Hämolyse und Blutabgang
im Urogenitaltrakt.
Erste Vorversuche mit einem Lithotriptor am Gewebe haben erkennen lassen,
daß bei Einwirkung von Stoßwellen auf das Gewebe eine zunächst kurzzeitige
Ischämie, d. h. Minderdurchblutung auftreten kann. Wird nun innerhalb
dieser Ischämiezeit erneut das Gewebe mit Stoßwellen exponiert, so tritt anschließend
eine wesentlich längere Zeit der Minderdurchblutung auf, die
auch stärker als bei der ersten Stoßwellenapplikation ausfällt.
Besonders im kleinen Becken finden sich Organe oder pathologische Wucherungen,
die sehr stark durchblutet sind. Bei der Entfernung dieser Gewebe kann
intraoperative eine sehr starke Blutung auftreten. So sind z. B. transurethrale
oder suprapubische Operationen der Prostata bei gutartiger Wucherung von
Prostatagewebe häufig mit einer stärkeren Blutung verbunden. Auch transurethrale
Blasenoperationen oder selbst die Entfernung der gesamten Blase
kann zu einer heftigen Blutung führen.
Mit Hilfe einer fokussierten Stoßwellenapplikation auf das zu operierende
Gewebe oder auch nur die zuführenden Gefäße kann die konsekutive Ischämie
ausgenutzt werden, um die Operation blutarm später durchzuführen.
So kann z. B. Prostatagewebe oder eventuell nur die zuführenden Arterien
zur Prostata (bei 5 und 7 Uhr) mit Schockwellen "beschossen" werden, und
dann unmittelbar nach der ersten oder zweiten Stoßwellenapplikation die
transurethrale Resektion des Gewebes erfolgen. Hierbei kann dann der Blutverlust
während der Operation minimiert werden oder auch bei gleichem Blutverlust
eine wesentlich größere Resektionsmenge pro Zeit entfernt werden.
Denkbar wären auch noch andere Applikationsformen wie Stoßwellenapplikation
auf Tumorgewebe oder die zuführenden Gefäße zu Tumoren vor der
chirurgischen Entfernung (z. B. Blasentumor vor Resektion, z. B. vor Blasenentfernung,
z. B. vor Tumorentfernung in oder in der Nähe eines anderen
Hohlorgans).
Denkbar wäre auch noch eine andere Applikationsform der Stoßwellen:
vor einer Chemotherapie oder Radiotherapie wird das Gewebe beschossen
und durch die Ischämie vorgeschädigt oder bei kleinen Geweben oder
Tumoren diese selbst durch die Schockwellen "denaturiert".
Mit Hilfe eines intraluminalen Schalles (z. B. transrektaler Schall für Prostataadenom
oder Karzinom oder Rektumtumor) wird das Zielvolumen der Stoßwellenapplikation
bestimmt. So kann z. B. das gesamte Prostatavolumen in kleinere
Zielvolumina aufgeteilt werden, die dann nacheinander der Stoßwellenapplikation
unterzogen werden oder eventuell nur die zuführenden Gefäße zur Prostata
in den Stoßwellenfokus gebracht werden. Mit Hilfe des transrektalen
Ultraschalls ist die exakte Darstellung der Prostata und auch ihrer zuführenden
Gefäße (indirekt durch Darstellung der Kapsel) möglich. Auf einem Monitor
kann das Zielvolumen dann betrachtet werden und so mit der Stoßwellenquelle
verbunden werden, daß eine Schockwellenapplikation exakt auf das
Zielvolumen erfolgen kann.
Nachfolgende Zeichnung soll das Prinzip nochmals graphisch erläutern:
Die Zeichnung soll nur exemplarisch das Prinzip der intraluminalen Ortung
das Gewebezielvolumens und die Ankoppelung an eine Stoßwellenquelle
am Beispiel der Prostata erläutern. Denkbar ist jedoch eine universelle
Anwendung des Prinzips für andere Organe oder Tumore.
Die Ortung von Gewebestrukturen mit Hilfe des intraluminalen Ultraschalls
und einer externen Stoßwellenquelle ist die am einfachsten realisierbare Möglichkeit
der Gewebeidentifikation und nachfolgender Stoßwellenapplikation.
Grundsätzlich wäre auch das Einbringen der Stoßwellenquelle in das Hohlorgan
selbst möglich bei gleichzeitiger Ortung intraluminal mit Ultraschall oder
Röntgendarstellung oder die Kombination einer intraluminalen Stoßwellenapplikation
zusammen mit einer externen Ortung des Gewebes mit Ultraschall
oder Röntgen oder andere bildgebende Verfahren (CT, NM . . .).
Claims (3)
1. Kombination einer ultraschallgesteuerten oder auch radiologisch gesteuerten
Ortung mit einer externen Stoßwellenquelle, dadurch gekennzeichnet, daß
z. B. ein transrektaler Ultraschallkopf so an eine Stoßwellenquelle angekoppelt
wird, daß damit ein bestimmtes Zielvolumen im kleinen Becken genau geortet
und "beschossen" werden kann.
2. Anspruch nach 1 dadurch gekennzeichnet, daß die Ortung des Zielvolumens
auch mit Hilfe einer anderen intraluminären Sonde erfolgen kann - intravaginal,
intraurethral, intravesikal oder im Magen-Darm-Trakt, so daß auch
hier eine Ankoppelung an die Stoßwellenquelle erfolgt.
3. Anspruch nach 1 dadurch gekennzeichnet, daß auch die Stoßwellenquelle
selbst intraluminal eingebracht werden kann (z. B. intravesikal, transrektal . . .)
und die Ankoppelung mit einer entweder zusätzlichen intraluminalen Ortung
oder auch externen Ortung (z. B. externer Ultraschall oder radiologisch)
erfolgen kann.
