Description

Die Erfindung betrifft ein Implantat für die biegeweiche, zugfeste Verbindung zweier Knochen- bzw. Gelenkteile, wobei zwei Ankerteile vorgesehen sind, von denen jeweils eines am Knochen- bzw. Gelenkteil befestigt ist und beide Ankerteile miteinander gelenkig verbunden sind.
Am Beispiel des Schultereckgelenkes kann diese Problematik anschaulich erläutert werden. Es bildet die Verbindungsstel­ le zwischen dem Schlüsselbein und einem kleineren weiter außen liegenden Knochen. Ober- und unterhalb des Knochenkon­ taktes werden die beiden sich berührenden Knochen durch Bänder zusammengehalten. Zerreißen diese, können sich die Knochen voneinander entfernen. Dies wurde bisher unterbunden durch Hakenplatten oder durch eine Kugelgelenkplatte. Erstere schränken die Drehbeweglichkeit ein und es besteht die Ge­ fahr des ungewollten "Aushakens", letztere ist zwar auf Zugbelastung ausgelegt, wird bei Anschlagen der zueinander beweglichen Teile ungewollt doch auf Biegung (vermutlich auch noch durch relativ hohe Drehträgheitskräfte des Armes) belastet, was zum Implantatsbruch führt.
Aus der FR-PS 00 64 546 ist zwar eine Vorrichtung zum Zusam­ menhalten der Teile des menschlichen Schulter-Eck-Gelenkes bekannt, bei dem am Schlüsselbein und auf dem Acromium jeweils eine Platte befestigt ist und ein Gelenk die beiden Platten miteinander verbindet. Das Gelenk besteht jedoch aus einem Stiel, der an der einen Platte befestigt ist, und einer Kugel am Stielende, die in einer kugelförmigen Ausneh­ mung der anderen Platte ruht. Dieses Gelenk weist den bedeu­ tungsvollen Nachteil auf (weil mit seinem Versagen eine neuerliche Operation verbunden ist), daß es im Anschlagsbe­ reich des Kugelstiel-Elementes spontan biegesteif wird und diese Biegespannungen im Stiel zum Ermüdungsbruch führen. Während bei der Fixierung von Knochenbrüchen vom Implantat verlangt wird, daß es möglichst gegen alle Standardlastfälle hinreichend steif dimensioniert ist, wird bei Bänderrissen im Gelenkbereich eine Beweglichkeit des Gelenkes verlangt, um dessen Versteifung während der Heilung des Bandschadens auszuschließen. Von einem solchen Inplantat wird also we­ sentlich erwartet, daß es die Kontaktflächen der beiden sich im Gelenk berührenden Knochen zwar nahe beieinander hält, d. h. es muß zugfest sein, ansonsten aber die Beweglichkeit des Gelenkes möglichst wenig beeinträchtigt.
Die der Erfindung zugrundeliegende Aufgabe besteht darin, eine zugfeste Verbindung der Ankerteile zu bieten, die eine nahezu unbegrenzte Ermüdungsfestigkeit aufweist, bei Biege­ belastung nicht bricht und auch nicht nur eine durch starre Teile allzu begrenzte Drehbeweglichkeit aufweist.
Die Lösung ist im kennzeichnenden Teil des Anspruches 1 beschrieben.
Die übrigen Ansprüche geben vorteilhafte Weiterbildungen und Ausführungsformen der Erfindung an.
Der besondere Vorteil der Erfindung ist darin zu sehen, daß keine lokale Flexibilität durch ein Gelenk mit notwendig begrenzter Beweglichkeit vorherrscht, sondern eine praktisch unbegrenzte Beweglichkeit durch eine verteilte Flexibilität über die gesamte Länge des Stahlseils zwischen den Ankerele­ menten gewährleistet ist.
Die Erfindung wird im folgenden anhand dreier Ausführungs­ beispiele mittels der Fig. 1 bis 3 näher erläutert.
Das im Prinzip in Fig. 1 und als Clavicula-Platte in Fig. 2 vorgeschlagene Implantat besteht aus zwei Verankerungs- oder Befestigungselementen 1 und 2, die an den beiden gelenkig zu verbindenen Knochen (nicht dargestellt) angebracht, z. B. angeschraubt werden. Als biegeweiche Verbindung dient nun ein Stahlseil 3, das einfach oder mehrfach zwischen den Befestigungselementen 1, 2 verläuft. Da jeder Einzeldraht des Seiles 3 aus z. B. Stahl seine eigene neutrale Faser bezüglich Biegebelastung hat, verträgt dieses Verbindungs­ element 3 fast beliebige Verdrehungen der verbundenen Kno­ chenteile zueinander. Ein Bruch ist damit völlig ausge­ schlossen und ein guter Ersatz für reale Bänder geschaffen.
Als Führungselemente für die beiden Schlingen 4 und 5, die von dem Seil 3 gebildet werden, dienen durchgehende Bohrun­ gen 1′-1′′′ bzw. 2′ bis 2 IV oder 2′-2′′′ auf den beiden Verankerungs- bzw. Befestigungselementen 1 und 2. Die Boh­ rungen 1′-1′′′ und 2′-2′′′ bilden ein Dreieck und die Bohrungen 2′-2IV ein Doppeldreieck mit jeweils abgerunde­ ten Ecken. Das Seil 3 ist in den Bohrungen verlegt. Im eigentlichen Gelenkbereich 6 zwischen den Verankerungs- bzw. Befestigungselementen 1 und 2 verlaufen die Stahlseilberei­ che parallel zueinander (wie dargestellt) oder sie überkreu­ zen sich, wozu allerdings zumindest die aufeinanderzulaufen­ den Bohrungen 1′′-2′′ und 1′′′-2′ in Richtung und Höhe entsprechend ausgerichtet sein müssen.
Ein besonderer Vorteil beim Ausführungsbeispiel einer Clavi­ culaplatte nach Fig. 2 besteht in der Möglichkeit des suk­ zessiven Anbringens der Elemente 1 und 2, was das operative Vorgehen erleichtert.
In Fig. 3 ist eine weitere Möglichkeit der Aneinanderhalte­ rung der beiden Befestigungselemente 1 und 2 (nicht darge­ stellt) anstelle des Seils 3 beschrieben. Es handelt sich um die Kombination Spannschraube 6 - Kortikalisschraube 7, wobei jeweils eines der beiden Teile an einer der Befesti­ gungselemente 1 bzw. 2 angeordnet ist. Beide Teile 6 und 7 werden mittels der Zugschraube 8 zusammengehalten, die in ihrem mittleren, meist freiliegenden Bereich zwischen den Befestigungselementen 1 und 2 zu einem flexiblen Seilstück 9 ausgebildet ist.

Claims (5)

1. Implantat für die biegeweiche, zugfeste Verbindung zweier Knochen- bzw. Gelenkteile, wobei zwei Ankerteile vorge­ sehen sind, von denen jeweils eines am Knochen- bzw. Ge­ lenkteil befestigt ist und beide Ankerteile miteinander gelenkig verbunden sind, dadurch gekennzeichnet, daß die Verbindung von mindestens einem Seil (3; 8, 9) gebildet ist.
2. Implantat nach Anspruch 1, dadurch gekennzeichnet, daß das Seil (3) mit jeweils einem Ende (4, 5) an einem Ankerteil (1, 2) gehaltert ist und einfach oder mehrfach verlegt sein kann.
3. Implantat nach Anspruch 1, dadurch gekennzeichnet, daß die Verbindung von einer Zugschraube (8) und einem Seil­ stück (9) gebildet ist.
4. Implantat nach Anspruch 1 oder einem der folgenden, da­ durch gekennzeichnet, daß die Seilbereiche (6) des Seils (3) im Übergangsbereich von einer Schlinge (4) zur ande­ ren (5) entweder parallel zueinander verlaufen oder sich kreuzen.
5. Implantat nach Anspruch 1 oder einem der folgenden, da­ durch gekennzeichnet, daß das Seil (3, 8) ein Stahlseil, bestehend aus Einzelfasern, ist.
