Description
The invention relates to pharmaceutical compositions for the treatment and inhibition of bone resorption and demineralization, caused by excessive secretion of parathyroid hormone ( PTH ). Such bone resorption and demineralization cause hypocalcaemia, which triggers toxic effects in soft tissue.
Bone resorption or demineralization can be done directly or indirectly as a result of changes in calcium and Phosphate content in the body fluids more warm-blooded Animals occur.
PTH is usually secreted in the required physiological amounts, which vary at different times depending on the body's needs. It participates in the regulation of the metabolic activities of bones, kidneys and the intestinal absorption of calcium in warm-blooded animals like humans. However, excessive synthesis of PTH can occur, for example in primary hyperparathyroidism, with pathological bone resorption and demineralization being induced. The resulting hypocalcaemia has toxic effects in the soft tissue.
35, Seiten 708-711 (1983)).A particularly important condition in which PTH triggers accelerated bone demineralization is osteoporosis, which usually occurs in postmenopausal women (Calcium Tissue Intl. 35, pages 708-711 (1983)).
It is an object of the present invention to provide a therapeutic agent by means of which bone resorption induced by PTH can be inhibited.
Further tasks result from the description below.
Diese Aufgaben werden erfindungsgemäß gelöst.These tasks are solved according to the invention.
The present invention accordingly relates to a pharmaceutical Composition for inhibiting parathyroid Hormone-triggered bone resorption, which causes is characterized in that it is an active ingredient for oral Treatment of a warm-blooded mammal with parathyroid hormone Over-secretion a dose of about 0.1-6 g / kg body weight and a dose amount for systemic treatment 0.1-2 g / kg body weight of a non-toxic, water-soluble, pharmaceutically acceptable peroxydiphosphate compound contains.
PTH and its effects are described in "The Role of Calcium in Biological Systems", Volume II, Anghileri, CRC Press, Boca Raton, Florida, 1982, Chapter II, pages 204-212. PTH is a single chain protein that contains 84 amino acids. It can be characterized as PTH (1-84). PTH is synthesized in the body from a precursor called Pro-PTH that contains 90 amino acids. Pro-PTH is itself an intermediate product that is synthesized from a protein with 115 amino acids and is referred to as pre-Pro-PTH. Typically in warm-blooded animals, PTH is formed enzymatically from its precursors on a synthetic route, and the PTH is then secreted into the circulation in the amounts required for the body. PTH protects the organism from severe hypocalcaemia. Accordingly, when the circulating concentration of ionized calcium in the circulation is low, the secretion rate of PTH is stimulated.
The metabolic activity of bones, kidneys and intestinal organs is influenced directly or indirectly by PTH to increase calcium absorption and phosphate excretion. Bone in particular is affected by PTH in a manner that is both time and dose dependent. High doses of PTH initially increase the rate of bone resorption followed by an increase in the rate of bone formation. In the early stages, there is also an increase in net calcium delivery, which is independent of bone resorption.
If PTH is secreted excessively and not in accordance with the normal dose / time relationship in response to metabolic requirements, excessive bone resorption and demineralization occurs, leading to hypocalcaemia and toxic effects in the soft tissues.
Hydrogen peroxide is known to oxidize methionine groups. One might suspect that such a reaction reduces PTH activity. Surprisingly, however, the reduction in PTH activity which is brought about by hydrogen peroxide is considerably lower than that which is achieved according to the invention when using a peroxydiphosphate compound. In addition, peroxydiphosphate acts safely, compatible and efficiently in warm-blooded animals, since it is broken down enzymatically by alkaline and acidic phosphatases according to the following equation: in which X is a non-toxic, pharmaceutically acceptable cation or a residue of an organic ester group. Peroxydiphosphate-degrading phosphatases are present in both saliva and plasma, intestinal fluid and white blood cells.
Apparently, the slow release of oxygen is due to Particularly effective in inhibiting excessive PTH secretion and thus reduces the undesirable and toxic effects the excessive PTH secretion.
The peroxydiphosphate compound ( PDP ) is a non-toxic, pharmaceutically acceptable compound that goes beyond the salts described in US Pat. No. 4,041,149.

The compounds include alkali metal salts, such as, for example, lithium, sodium and potassium salts, alkaline earth metal salts, such as, for example, magnesium, calcium and strontium salts, as well as zinc, tin salts and quaternary ammonium salts and also organic C 1-12 -alkyl, adenylyl, guanylyl, Cytosylyl and thymylyl esters of peroxydiphosphate and the like. Alkali metals, especially the potassium salt, are preferred among the inorganic cations. Tetra potassium peroxydiphosphate is a stable, odorless, finely divided, free-flowing, white and non-hygroscopic crystalline solid with a molecular weight of 346.35 and an active oxygen content of 4.6%.
Eine 10%-ige Lösung in Wasser bei 25°C zeigte nach 4 Monaten keinen Verlust an aktivem Sauerstoff; bei 50°C zeigte eine 10%-ige Lösung nach 6 Monaten einen Verlust an aktivem Sauerstoff von 3%.Tetra potassium peroxydiphosphate is 47-51% water soluble at 0-61 ° C, however insoluble in common solvents such as Acetonitrile, alcohols, ethers, ketones, dimethylformamide, Dimethyl sulfoxide and the like. A 2% aqueous solution has a pH of around 9.6 and a saturated solution has a pH of around 10.9. A 10% solution in Water at 25 ° C showed no loss after 4 months active oxygen; at 50 ° C showed a 10% solution 6 months of 3% loss of active oxygen.
Preferred organic compounds are those which have hydrophobic properties, such as, for example, the C 1-12 -alkyl radicals, and those which facilitate the rapid uptake of the peroxydiphosphate residue by the cell, such as adenylyl, guanylyl, cytosylyl and thymylyl esters.
The peroxydiphosphate compound can be oral or systemic to be administered to the excessive PTH secretion inhibit.
Suitable pharmaceutical carriers for oral ingestion are coated tablets formed from one material are resistant to stomach acids in the stomach prevailing pH is (about 1-3) which is the peroxydiphosphate would be inactivated by these stomach acids. The carriers with the solid material enclosed in them from tableted granules of the peroxydiphosphoric acid salt in contrast, are dissolved by the intestinal fluid, which has a higher pH of about 5.5 to 10 and which does not inactivate the peroxydiphosphate and therefore the leaves enzymatic action of the phosphatase, which in Humans or other warm-blooded animals is present. A suitable solution for coating tablets is from typically about 40-50 and preferably about 45 parts by weight, a fatty acid ester such as N-butyl stearate, typically about 15-25 and preferably about 20 parts by weight, a wax such as Carnuba wax, typically about 20-30 and preferably 25 parts by weight of a fatty acid such as Stearic acid, and typically about 5-15 and preferably about 10 parts by weight of a cellulose ester such as cellulose acetate phthalate, and typically about 400-900 parts by weight an organic solvent. Further suitable coating materials include shellac and Copolymers of maleic anhydride and ethylenic compounds such as polyvinyl methyl ether. Such coatings are different from tablets that are broken down in the oral cavity and where the tablet material is typically Trap about 80-90 parts by weight of mannitol and about 30-40 parts by weight Contains magnesium stearate.

Tableted granules of the peroxydiphosphate salt are made by Mix about 30-50 parts by weight of the peroxydiphosphate salt with about 45-65 parts by weight of a solid polyhydroxy sugar like mannitol and moistening with about 20-35 parts by weight a liquid polyhydroxy sugar solution such as Sorbitol, sieves for dimensioning, mixing with about 20-35 parts by weight of a binder such as magnesium stearate and compressing the granules into tablets on a tablet machine produced. The tableted granules will be by spraying a foam of a solution of the coating material coated and to remove the solvent dried. Such tablets are different from Dental tablets, usually made from tableted granules without a special protective cover.
An effective administration dose of peroxydiphosphate with The prescribed schedule for oral administration about 0.1-6 g / kg body weight daily; with systematic Application such as intramuscular, intraperitoneal or intravenous injection, the dose is about 0.1-2 g / kg Body weight daily.
Physiologically acceptable, pyrogen-free solvents are suitable carriers for systemic administration in itself known way. Saline solution, with phosphate on a physiological Buffered pH of about 7-7.4 is the preferred Systemic administration vehicle. Such Solvents are different from water wetting vehicles typically used in dentifrices will. Such a solution is typically made by using deionized, distilled water sterilized it using Limulus Amebocyte Lysate (LAL) - Tests described by Tsuji et al in "Pharmaceutical Manufacturing ", pages 35-41 (1984) on lack of pyrogenicity examined, then a phosphate buffer with a pH of, for example, about 8.5-10 from pyrogen-free sterile water, 1-100 mg peroxydiphosphate Derivative and sodium chloride to a concentration of about 0.5-1.5% by weight. The solution can be to Use in ampoules after being filled through Passage was resterilized through a micropore filter.
Alternativ können Lösungen wie Ringer-Lösung mit einem Gehalt an 0,86 Gew.% Natriumchlorid, 0,03 Gew.% Kaliumchlorid und 0,033 Gew.% Calciumchlorid verwendet werden.Alternatively, solutions like Ringer's solution with a Content of 0.86% by weight sodium chloride, 0.03% by weight potassium chloride and 0.033% by weight calcium chloride can be used.
The following example shows that peroxydiphosphate compounds ( PDP ) can inhibit the release of calcium from bone due to PTH secretion.
The test in which PTH isolated from bovine parathyroid glands (obtained from Böhringer-Mannheim) induces bone resorption in a bone culture system was used to determine whether PDP deactivates PTH activity in bone resorption.
Bone cultures of rat fetuses were as described by Raisz in J. Clin. Invest. 44, pages 103-116 (1965), was prepared by injecting rats with 45 CaCl 2 on day 18 of gestation. The rats were sacrificed on day 19 and the radii and ulnae of the embryos with cartilage ends removed and brought to cultivation in BGJ medium (Gibco, Buffalo, NY) at 37 ° C. with 5% CO 2 . The medium was supplemented with 5% warmed fetal calf serum (3 hours at 57 ° C). Four bones each were placed in one well of a 24-well tray (Nunc, Gibco), each well containing 0.5 ml of medium. The release of 45 Ca into the culture medium from bones incubated in the presence of a test substance was compared to the release from bones incubated in control medium; the results of bone resorption were expressed as a relation.
PTH was diluted with sterile bovine serum albumin solution. PTH was treated at 37 ° C with various concentrations of PDP as the tetrapotassium salt. The excess PDP was removed by membrane dialysis (maximum molecular weight 3500). The unreacted PDP can diffuse outwards while the PTH with a molecular weight of more than 3500 is retained within the bag. The data obtained are shown in the table below.
Nr. 3 gegenüber Nr. 4.The data presented in the table show that PTH induces bone resorption compared to the control (bovine serum albumin), cf. No. 1 compared to No. 2. If PTH is treated with PDP , bone resorption is significantly reduced, cf. No. 2 versus No. 3. In addition, hydrogen peroxide in an amount that provides an equivalent amount of oxygen as PDP is less effective than PDP in lowering bone resorption caused by PTH , see FIG. No. 3 versus No. 4.
The above results are representative of the activity of the PDP tetrapotassium salt and other non-toxic water soluble, pharmaceutically acceptable PDP salts such as other alkali metal salts, alkaline earth metal salts, the zinc and tin salts as well as the C 1-12 alkyl PDP salts and other organic PDP -Compounds, particularly including the adenylyl, guanylyl, cytosylyl and thymylyl esters and quaternary ammonium PDP salts to inhibit PTH secretion in rats and mammals in general.
