Abstract
Description
The invention relates to an instrument oil Mineral oil and / or vegetable oil based for use for surgical and dental instruments and devices, especially in their handpieces.
Care measures for surgical and dental Instruments and devices usually close cleaning a treatment of the instrument or Device with an instrument oil or lubricant a. Only then is sterilization carried out preferably in a steam sterilization consists.
Lately has been in the specialist literature quite often the problem has been raised that instrument oils or lubricant a nutrient medium or a protection for potentially pathogenic germs.
It is known that in steam sterilization also thin layers of oil, for example Oil films 10 µm thick, under normal sterilization conditions becoming sterile, a particular problem place the frequently existing columns in the Instruments and devices that are in the Fill the lubrication with oil. These volumes are at steam sterilization for water vapor inaccessible and therefore do not hire calculable risk of infection.
To also use these lubricant parts sterilize safely from conventional instrument oils, would need much higher sterilization temperatures and longer sterilization times can be selected. Instead of the usual for steam sterilization Temperature of about 130 ° C at 2 bar pressure to heat to temperatures of 180 to 200 ° C. However, these are much higher temperatures because of the plastic parts often found in instruments problematic and also require one higher expenditure of energy and time.
bezeichnete Wasser/Öl-Emulsion ergeben. Meist
In addition to the instrument oils, concentrates are commercially available which, when diluted with water, give a water / oil emulsion called instrument milk. Most of the water used is not germ-free, and the emulsions act as a broth for germ proliferation. An application of the usual preservatives to kill the germs, such as. B. formaldehyde, phenol and their derivatives, is eliminated because these substances are physiologically questionable and the lubricants come into contact with open wounds.
The object of the invention is to provide an instrument oil create that even in thicker layers and inaccessible places with the help of conventional Steam sterilization under normal conditions can be safely sterilized.
According to the invention, this object is achieved by that the instrument oil cationic emulsifiers are clogged.
The cationic emulsifiers provide during the Steam sterilization for a uniform Water absorption and distribution in the instrument oil. The usual temperatures for steam sterilization and residence times in the autoclave are sufficient for safe germ killing. The cation active Emulsifiers are also thermally stable, so that by the sterilization process does not decompose it Connections occur.
Die Anteile der Emulgatoren im Instrumentenöl betragen vorzugsweise 1 bis 10 Vol.%.The proportions of the emulsifiers in the instrument oil are preferably 1 to 10% by volume.
Special thermal stability, good emulsification and Dispersing effect and good biodegradability and excellent skin tolerance Emulsifiers, the derivatives of fatty amines or Are polyalkylamines. Emulsifiers in particular have proven by tallow fat propylene diamine. These are much more effective than ethoxylates of fatty alcohols or linear aliphatic alcohols (non-ionic surfactants). This depends on her polar character (cationic surfactants) together. Amine ethoxylates are preferably used which Contain 3 moles of ethylene oxide.
Pure paraffin oil in accordance with the specifications of the German Medicinal Products Book (DAB 8) for viscous paraffin oil is preferably used in the instrument oil, with physiologically harmless additives for increasing the lubricating effect and / or for preserving the instrument oil being added in special cases. The lubricating effect can be achieved in particular by adding triglycerides of fatty acids with C 8 to C 10 chains in a proportion of 5 to 25% by volume. With these substances, conventional anti-abrasion agents can be completely replaced in the past, so that the instrument oil is only composed of physiologically practically harmless components.
The following test series show the improvement the sterilizability of a commercially available Instrument oil by the amine Ethoxylates.
The investigations were carried out on an instrument model performed, which consists of two circular metal plates with a diameter of 10 mm, which consists of a steel wire clamp are pressed together and by three protrusions at a distance of 0.1 mm are kept.
The lower plate was loaded with a suspension of test spores, then oiled and sterilized in the autoclave after placing the second plate. In test series A, native soil spores served as test spores; the inoculum contained approx. 2.7 × 10 5 germs per plate. In the test series B, C and D, Bacillus subtilis was used, about 5.3 × 10 6 spores were applied to each plate. The sterilization conditions are shown in Table 1. In experiment D, a commercial sterilizer with a fractionated vacuum was used. The microbial growth test following the sterilization was carried out by transferring the platelets into a nutrient medium (trypticase soy broth (TSB)). In control experiments, the platelets were only provided with the spores, but not coated with oil and then sterilized in an autoclave.
A commercially available instrument oil was initially used as the oil, to which 10% of the instrument oil additive according to the invention (ethoxylate with 3 moles of ethylene oxide of tallow fat propylene diamine) was added in a further test series. A series of experiments with a corresponding addition of a nonionic emulsifier (ethoxylate with 5 mol ethylene oxide of linear aliphatic secondary alcohols with C 12 to C 15 chains) served as a comparison for the intensity of action.
The results of these test series are in Table 2 listed. Non-sterile are given in each case Samples for the number of samples examined. In one further control experiments were carried out with germs and oil-treated platelets that are not have been sterilized. These samples always showed one bacterial growth (results are not in Table 2 included).
The control experiment in Table 2 shows that Platelets that were not coated with oil sterilization is sometimes not sufficiently sterile were, which means the sterilization conditions overall were just barely sufficient and each Obstruction by the oil as an increased occurrence of non-sterile samples.
From table 2, 3rd line, the improvement can clearly be seen the sterilization ability of the instrument oil by the addition of cation-active according to the invention Amine ethoxylates can be read, because at same sterilization conditions no non-sterile Samples have occurred, in contrast to the commercially available Oil without this amine ethoxylate addition and in Contrary to the commercial oil with another non-ionic ethoxylate additive.
The lubricating effect was finally tested in a practical test an oil with the amine Ethoxylate addition checked. It was found that even with the addition of 10 vol.% amine Ethoxylate the warming of the tested handpiece was only slightly higher than when using the commercially available instrument oil. The differences in abrasion with a lubrication with commercially available and instrument oil according to the invention were also low.
Overall, it can be said that the Amine ethoxylate additives according to the invention for Instrument oil on the one hand the lubricating properties of the instrument oil and on the other hand however the sterilizability of the instrument oil so improve that under the usual conditions steam sterilization with safe germ killing also in crevices and other water vapor inaccessible volumes of instruments and handpieces can be expected. This could be clearly demonstrated be that the amine according to the invention Adding ethoxylate the negative effects of oil coating can pick up for steam access.
