Abstract
Description
Device for Removing Ureteral Stones The invention relates to a device for removing ureter stones, with a catheter or the like. Guide tube and at least one loop to grip behind the ureteral stone, the snare catheter combined with a drainage catheter and the snare and the like. Is set back in relation to the drainage catheter end according to patent (patent application P33 13 895.8) The so-called Zeiss loop is already known, in which an im Inner wire as an aid to form a loop at the inner end of the catheter is used. The wire is at a distance from the inner tip of the catheter led out laterally movable and fixed to the outer end of the Catheter connected. By pulling on the wire, the end of the catheter is bent over and becomes a Loop shaped. The disadvantage here is that the minimal, clear loop cross-section is determined by the distance of the exit point from the end of the catheter. aside from that To remove a ureteral stone, the catheter must be attached to the Stone pushed past with its inner end to approximately the area of the renal pelvis and only then can it be bent into a noose. Then the catheter withdrawn, trying to grasp the ureteral stone. Disadvantageous is there that on the one hand the loop formation only in a corresponding manner large cavity is possible and practically not in the area of the stone thus results In the area of the stone there is also practically no manipulation with this Zeiss loop. In addition, it is disadvantageous that practically no drainage of the kidney is possible, see above that the catheter can only remain in place for a limited time.
The main application P 33 13 895.8 is therefore already a A combination of a loop catheter and a drainage catheter has been proposed been. It is also provided that to form an active loop opposite Set back from the inner end of the catheter is an exit point for the snare wire and an attachment point for the Wire is provided. This advantageously already offers the possibility of essentially forming the loop solely through the wire while the catheter remains roughly straight. As a result, the loop formation can take place directly when it is to be grasped Stone made. It has also been shown that the deflection point of the wire at his A slightly raised point emerges from the guide tube when the loop is drawn in or bulge forms, which can be a problem in cramped conditions. In particular This deflection protrusion is made in the area of the ureter orifice and also at the Passing past the ureteral stone disturbingly noticeable.
The object of the present invention is to provide a device of the initially mentioned to create the type mentioned, in which the catheter also in the area of the to be removed Stone is easy to handle and the loop can also be extended there as an active loop and can be manipulated and with the insertion of the catheter also under cramped conditions Conditions, especially when the ureter orifice into the bladder or when passing it is quite possible on the stone to be removed.
To solve this problem, it is proposed according to the invention in particular that that a lateral outlet opening on the catheter provided for the snare wire a greater distance from the inner end of the drainage catheter than Attachment point of the snare wire to the catheter. This will while maintaining the advantages avoided that a deflection point of the wire in the form of a bulge or the like.
bildet. When the noose wire is drawn in, the outside can move The wire cut off of the catheter is essentially smooth to the outside of the catheter and thus does not form an annoying, protruding bulge. This also makes it possible in particular to pass the catheter with the loop area Passing the ureteral stone is much easier, even in confined spaces possible.
The fastening end of the noose wire is expedient articulated with the catheter or the like. tied together. Through this connection joint is a largely bend-free setting of the wire in the connection area to the given loop size, especially when completely drawn in Wire where it rests on the outside of the catheter guide tube, no bend or eastward There is deformation in the fastening area. Rather, the wire can be flat and place it tightly against the outer jacket of the catheter and is therefore particularly annoying not when inserting the catheter.
One embodiment provides that the fastening end of the noose wire has a bend for hanging in a fastening opening. This hanging connection forms a detachable connection joint, which on the one hand facilitates the assembly of the noose wire simplified and this can also be removed in a simple manner if necessary.
Additional refinements of the invention are set out in the further subclaims listed. The invention and its essential details are based on the following the drawing explained in more detail.
It shows: FIG. 1 a partial side view of a device with a Drainge catheter with noose wire in different positions, 2 shows a partial longitudinal section of a catheter with a catheter forming a loop Wire, Fig. 3 is a representation corresponding approximately to FIG. 2, but here with the location of the Noose wire in the retracted state, FIG. 4 shows a modified embodiment of the fastening end of a noose wire and FIG. 5 shows a modified version Embodiment of the fastening end of a noose wire.
A device 1, which in the further course of the description briefly Also known as a catheter, it is used to remove ureter stones. Included the catheter consists of a combination of a drainage catheter and a Sling catheter. This combination gives the possibility that the Urinary drainage is possible during the ureteral stone removal. So far it has been tried to achieve drainage after inserting the loop catheter and grasping the ureteral stone, an additional ureteral catheter was placed became. However, it is usually difficult and often not even possible to get one slide the second catheter past a stone. With the combined catheter 1, on the other hand, is at the same time the possibility of draining the kidney and grasping it given by a ureteral stone.
In particular in Fig. 1 it can be clearly seen that the loop 9 opposite set back from the inner end of the catheter 16 is arranged. at this end 16 a curve 17 is provided through which the passing of the entire catheter on a stone, in particular by rotating the catheter, is relieved. In this catheter 1 according to the invention it is now provided that a lateral outlet opening 8 on the catheter provided for the snare wire 5 is at a greater distance from the end 16 of the catheter than the attachment point 10 of the noose wire 5 on the guide tube 4 of the catheter. Through this training can the loop wire 5 in the retracted state (cf. dashed lines Fig. 1 and 3) lay smoothly on the outside of the guide tube 4. One else practically unavoidable deflection bulge that more or less strongly over the circumference of the guide tube 4 would protrude, is avoided. Both previous catheters resulted in a deflection of the noose wire the lateral exit from the guide tube 4 by around 1800 (in the retracted position) this bending point is a protruding over the circumference of the guide tube 4, corresponding bulge formed.
In particular in Fig. 3 it can be seen that by the invention Arrangement and design of the exit point and attachment point of the wire such a deflection with a corresponding bulge is avoided.
At the fastening end 10 of the wire 5 there is a joint-like one Connection to the catheter is in place, through which the wire runs in this area can adapt largely deformation-free to the respective loop position.
The attachment of the end of the wire 5 can, as in FIGS to 5 recognizable, preferably by bending the end of the wire and hanging it in this bend takes place in a fastening opening 21. In addition to a simple This also gives the possibility to assemble the wire simply by unhooking it refer to. In the exemplary embodiments shown in FIGS. 2 and 3, the bend is 22 hook-shaped or optionally L-shaped with the hook end bent back. Such a hook end can be shaped particularly easily and already results in one good connection of the wire 5 with the guide tube 4 of the Catheter 1.
A modified embodiment of a bend 22a is shown in FIG. 4. Here the bend is approximately L-shaped with the hook end pointing forward. Here, too, hanging in the wire end is particularly easy, and it is here too increased protection against jumping out of the wire end in the event of a loop formation given. Another possible embodiment of a bend 22b is shown in FIG. 5. This is approximately Z-shaped with the hook end pointing forward.
Aushaken erfolgen. This embodiment forms practically a combination of the in Fig. 2 and 4, both with tensile load on the wire 5 as well as with thrust to form a loop a jumping out of the Noose wire end is practically impossible. Still it can be done by hand on simple Way to hang or unhook.
In the embodiment of FIG. 5, it is also advantageous if the Hook end 24 in the fastening position approximately at the one opposite the fastening opening 21 Catheter inner wall 23 rests. This results in a defined position of the passage area of the wire through the fastening opening 21 under both tensile and compressive loads of the wire.
It should also be mentioned that the fastening end of the noose wire 5 can also be attached to the outside of the catheter, e.g. by means of an external Wrapping of the catheter forming shrink tube or the like.
The drainage catheter 1 can in addition to that provided at the inner end Drainage opening 18 also have transverse bores in the further course. Possibly could these then also be used as fastening openings 21 at the same time, one has the advantage of adapting the hanging point to a desired loop size to be able to vary.
As already mentioned, the noose wire 5 can be drawn in Location practically smooth on the outside of the Position the guide tube 4. Since this wire, which is preferably made of steel, is comparatively thin, it stands over the catheter outer sheath, especially when inserting the catheter or Leading past the stone disturbing projection.
According to a further development of the invention, in the area between the fastening point 10 of the wire 5 and the outlet opening provided for it 8, on the catheter a longitudinal one, preferably approximately the cross-section of the wire corresponding longitudinal groove or the like. Be provided recess.
When the wire 5 is drawn in, it would be outside the guide tube area lying part of the wire can then put in the recess, so that even the Wire 5 is located within the circumferential cross-section of the catheter. Through this would then also be the use of a noose wire made of a somewhat thicker material, if necessary also made of plastic possible.
An embodiment of a catheter 1 is shown in FIGS each shown with a noose wire 5. However, there can also be several, preferably independently displaceable noose wires can be provided. these can either be attached distributed around the circumference of the catheter, or according to a preferred one Embodiment is provided that, for example, two to five, preferably three loop wires Arranged distributed over a circumferential area between preferably 450 and 1800 are. In a further embodiment of the invention, the fastening ends of the Loop wires are connected to one another, the connection point preferably is located essentially within the guide tube 4. Through this connection the distance between the individual loops remains - in the circumferential direction of the catheter - about the same, so that practically a basket can be formed with these loops.
In addition to loop wires with a roughly circular cross-section, there are also loop wires those with a cross-section deviating from the circular shape are in question. The cross section can be approximately rectangular or oval, elliptical or the like. be trained. Man thereby achieves a preferred bending level through which certain Positioning options for the sling can be improved. If necessary, the outlet opening can also be used 8 have a corresponding guide profile for the wire 5.
This results in one through the guide and the fastening point predetermined level when pushing out the wire, a corresponding loop formation.
In Fig. 1 one can still see one pushed into the drainage cavity 19 Stiffening wire 20, by means of which the insertion of the catheter is simplified. To this stiffening wire 20 can be pulled out after insertion. Im shown In the exemplary embodiment, the drainage cavity 19 also forms the guide cavity for the noose wire 5. It should also be mentioned that the external operator of the or the loop wires od one or more, preferably removable handles.
Like. Can be provided for manipulating these loop wires. The guide tube 4 of the catheter can be either a metal spiral hose or a plastic hose, the catheter being designed to be dimensionally stable, at least in the hooking area, to hold the noose wire end Combination with one another be essential to the invention.
