OperationsmikroskopSurgical microscope
The invention relates to a surgical microscope for dental procedures, especially for surgical procedures on teeth and jaws.
The use of surgical microscopes for surgical interventions on fine biological structures has been known for a long time. Techniques known as microsurgery have become standard in ear, nose and throat surgery, eye and brain surgery and numerous other fields of application. Surgical microscopes are also frequently used in oral and maxillofacial surgery, especially in operations in the back of the jaw or in plastic surgery on the face and near the jaw, in order to keep the risk of injury to facial nerves and scarring in the facial area as low as possible.
For a few years now, surgical microscopes have also been used in the field of dentistry, especially for root canal, gum and tooth root treatments.
Da trotz Liegeposition des Patienten viele Bereiche desSurgical microscopes used in dentistry to date are similar to corresponding devices used in ENT surgery. It is assumed that the dentist sits to the side and behind the patient's head and looks into the patient's mouth at an angle of 0-60° from the vertical. Since, despite the patient's lying position, many areas of the

If the jaw and teeth cannot be seen from the angle mentioned, the dentist uses mirrors to view the inaccessible areas.
For simple procedures where only one hand is needed for other instruments, the dental mirror can be held with the other hand. However, for longer treatments at one point under the microscope, bimanual treatment would be preferred. In particular, the need to track the microscope and the shaking movement of the hand-held mirror represent disadvantages for microsurgical surgical techniques in the dental field.
During longer operations, the dentist often has to move the microscope with one hand and put down the dental mirror or the treatment instrument. If he works with a foot switch for the movement, he is tied to a specific location and needs a motor-controlled and therefore more complex surgical microscope.
All surgical microscopes available on the market today have these disadvantages.
The object of the invention is to create a surgical microscope with which the dentist can work better, especially with both hands.
The task is solved by the fact that the surgical microscope has an adjustable reflecting element attached to a holder that can be brought into the observation beam path in front of the lens. The reflecting element no longer needs to be held by hand, but is attached to the surgical microscope and can be adjusted accordingly so that the surgical field can be easily observed through the microscope. The dentist has both hands free for his actual work. He also no longer needs to ensure that he constantly adjusts the reflecting element, as is necessary when he is holding it in his hand and the hand position slowly changes.
Dabei wird vorteilhafterweise eine Arretierung für die Schwenkbewegung vorgesehen, die bei einer vorgege-Advantageously, the holder and the reflecting element can be swivelled away from the area in front of the lens in order to be able to carry out observations and operations in which the holder and the reflecting element would only be a hindrance. In this case, a locking mechanism for the swivel movement is advantageously provided, which at a given
Dadurch wird Verletzungsgefahr für den Patienten vermieden.the force acting on the holder and/or the reflective element is overcome. The holder and reflective element therefore give way when the patient hits them with a corresponding head movement. This avoids the risk of injury to the patient.
The distance between the reflective element and the lens can be changed. In order to avoid the risk of injury to the patient, it is advantageous to provide that the length of the holder can be changed and that a locking mechanism is provided for the length change, which is overcome by a predetermined force acting on the holder and/or reflective element. The locking mechanisms can be, for example, detachable snap-lock connections or a friction brake, each of which enables one or a few fixed settings or continuous adjustment.
The adjustable-length mount can have a telescopic rod, a scissor rod, a sliding swivel arm, a parallelogram rod, a double arm, a gooseneck or a ball chain.
In order to create appropriate adjustment options, the reflecting element can advantageously be pivoted about two axes that are essentially perpendicular to one another. The holder and reflecting element can be adjusted using drive means, as can the holder and reflecting element be locked in place. Mechanical, electric motor, magnetic, pneumatic or hydraulic drives are suitable as drive means. On the other hand, adjustment and/or orientation can also be carried out manually. The reflecting element can, for example, be adjusted manually using a hand lever arranged on it.
In an advantageous embodiment, the reflective element is a mirror. In another advantageous embodiment, it is a prism, although for certain special applications, a combination of a mirror with a prism is also conceivable.
The invention is described below with reference to the accompanying drawing, which shows an embodiment of the invention in perspective view.
The surgical microscope 11 is essentially directed downwards so that the dentist can look into the jaw area through the eyepieces 13 and the front lens 12 in an ergonomically relaxed position. The central axis of the two visual rays is indicated by 20a and 20b. The object is usually illuminated by the front lens 12, with the light being supplied by a light guide 19 or a light source being built into the microscope. Deflecting mirrors and optical elements are arranged in the surgical microscope so that the light emerges almost parallel to the two visual rays 20a and 20b.
According to the present invention, an element 14 is fixedly attached to the surgical microscope, for example with a pivoting device 15, via which the element can be pivoted into two defined positions, preferably provided with locking devices. In the rest position (not shown in the picture), the element 14 is pivoted backwards and therefore away from the patient's face. In the position shown, it is positioned under the microscope.
The element 14 consists of a length-adjustable holder 16, at the end of which a reflective element 17 is attached with a suspension 18. The holder 16 is preferably a telescopic rod that can be extended to the desired length by hand. However, other embodiments are conceivable, e.g. a pair of scissors, a swivel arm, a gooseneck, a parallelogram arrangement, a double arm or a ball chain. The natural friction of the holder 16 is either sufficient to maintain the length, or it can be supported, e.g. by a clamping mechanism, so that positioning can be carried out without great effort, but no change in length occurs during the operation. For the safety of the patient, the holder 16 should always be able to move upwards if it bumps into the patient.
In jedem Fall soll das spiegelnde Element 17 so gelagert sein, daß esA reflective element 17 is attached to the end of the holder 16, preferably via a cardanic bracket 18. However, any other movable suspension is also possible, which allows rotation of the reflective element 17 on two axes. The reflective element 17 is a mirror or a prism that can be brought into a certain position by the dentist. Depending on the type of holder 16 or suspension 18, the reflective element is either forced to be positioned in the visual rays 20a, 20b, or the doctor aligns the mirror before or during the dental operation according to the direction of view. In any case, the reflective element 17 should be mounted in such a way that it
nach der Ausrichtung seine Lage beibehält und nicht durch die Schwerkraft verändert werden, bei Aufstoßen auf Patientengewebe jedoch nachgeben kann.maintains its position after alignment and is not displaced by gravity, but can give way when it hits patient tissue.
The alignment of the reflecting element 17 can preferably be done with a hand lever similar to a joystick. The hand lever can be attached directly to the reflecting element 17, which gives the doctor a similar handling as with a dental mirror. However, it is also possible to control the reflecting element 17 indirectly via a mechanism or drives that are controlled by the hand lever.
All parts of element 14 can be operated either manually or by motor. All parts can be secured by friction or by mechanical, motor, hydraulic, pneumatic or magnetic clamping.
