Description
Die Erfindung betrifft ein Oberschenkelteil einer Hüftgelenk- Endoprothese. The invention relates to a thigh part of a hip joint Endoprosthesis.
Known thigh parts of this type (DE-OS 25 51 013) are so designed that on the one hand during their implantation by means of cement good adhesion in the bone bed is achieved without using it Cracks or breaks occur in the cement layer, and that on the other hand there is the possibility of also the thigh parts to be removed after implantation and possibly by others to replace. The latter is particularly because of the well known and the much discussed problem of loosening the prosthesis and breaks often desired or required (Munich Medical Weekly Journal 118, 1976, No. 22, pp. 693-700; Archives Orthopedic Trauma Surgery 77, 1973, pp. 154-164). Here however, there are still problems with the design of the thigh parts of the genus for the case that neck sections of different lengths are desired there is a mere extension or shortening of the neck section considerable changes in the load conditions in use and the shaft of the thigh part would have to be realigned in the thigh bone.
The invention has for its object a thigh part to create a hip joint endophesis in different sizes, taking into account the anatomical conditions achieved similar tension in the thigh will.
Zur Lösung dieser Aufgabe dienen die kennzeichnenden Merkmale des Anspruchs 1.The characteristic features serve to solve this task of claim 1.
The invention has the advantage that the doctor between Thigh pieces can choose different sizes the resulting different burdens wear without the thigh for the implantation of a thigh part different size, shape or curvature to have to prepare. Should it turn out that in individual cases selected thigh part does not meet individual needs of the patient, the doctor can use a thigh part choose with a different neck length without the shaft of having to move the thigh part outwards or inwards, because the thigh is always the same regardless of the length of the neck Maintains orientation relative to the pool or for the purpose of better Resilience held in a straight position below the hip is.
Es zeigtThe invention is hereinafter described in connection with the accompanying Drawing explained in more detail using an exemplary embodiment. It shows

Fig. 9 to 11 are front views of three different sizes, according to the invention thigh parts, wherein the relationship between different lengths, distances and angles can be seen.
According to FIG. 8, a prosthetic hip joint 11 contains a thigh part 13 , which is inserted into the medullary canal M of the thigh F by means of cement C , and a hip section 15 for implantation in the acetabular cup A using cement C.
According to FIG. 1, the thigh part 13 contains a head 17 , a neck 19 , a shaft 21 and a shoulder 23 adjoining the connection point between the neck 19 and the shaft 21 . The thigh part 13 is preferably made in one piece and is made of a suitably rigid, biocompatible material such as chrome cobalt or stainless steel, the head 17 being rigidly connected to one end of the neck 19 and the other end rigidly connected to the proximal end 25 of the shaft 21 is connected. The shoulder 23 is provided at that end of the neck 19 which is in contact with the shaft 21 .
The head 17 has an outer surface, the larger part 29 of which is spherical, so that the head fits into the socket 31 of the hip section 15 ( FIG. 8). The head 17 is flat along a bottom 33 , the bottom 33 surrounding the junction of the head 17 with the neck 19 , and a radius 33 'is provided at the junction.
The cross section of the neck 19 is elliptical, the narrower part or the shorter diameter of the ellipse in the line extending through the line AP in the anterior / posterior direction, while the wider part or the longer diameter of the ellipse in the line extending through the Line LM indicated lateral / medial direction runs. It should be noted here that the expressions "anterior / posterior" and "laterial / medial" directions are determined by the orientation of the implanted thigh part 13 relative to the patient's body, ie the anterior / posterior direction runs from the body front to back and the lateral / medial direction sideways. It follows that the shaft 21 has more metal in the lateral / medial direction or plane where the stresses are greatest, while at the same time the neck is thinner in the anterior / posterior direction or plane, so that less Disturbances occur with the hip section 15 , which results in a greater scope for the forward and backward movement of the thigh part 13 and the thigh F. It should also be noted that the neck 19 is narrower in the region 37 lying just below its point of contact with the head 17 and widens outwards from there to its end, where the shoulder 23 is provided at the point of connection with the shaft 21 . The shoulder 23 overhangs the proximal end 25 of the shaft 21 on the anterior, posterior and medial side, while the lateral edge of the neck 19 is essentially flush with the lameric side of the proximal end 25 of the shaft 21 .
The shaft 21 is curved in the front view shown in FIG. 1 and is thicker in its proximal section 41 than in the straight, distal section 43 . In the plan view of FIG. 2, the shaft 21 is straight.
The shaft has a smooth, uninterrupted outer surface 45 without depressions and is continuously beveled from the proximal end 25 towards the distal end 47 in both the anterior / posterior and in the lateral / medial dimensions or planes. The cross section of the shaft 21 is thus oval, the oval becoming increasingly smaller from the proximal end 25 in the direction of the distal end 47 , as can be seen from the sections of the shaft 21 shown in FIGS. 4 to 7. The larger diameters of the ovals are arranged in the lateral / medial direction (LM) , while the shorter diameters of the ovals are arranged along the anterior / posterior direction. Furthermore, the corners are rounded and there are no sharp edges that could cut into the cement and cause breaks or cuts in the cement. With the special configuration of the shaft 21 described , synergistic results are also achieved, which can be understood from the following description of the use of the hip joint prosthesis 11 .
At least three sizes of the thigh part 13 are preferably provided, which are designed so that on the one hand they are anatomically compatible, but on the other hand a minimal concentration of tension occurs in the thigh parts, combined with an improved distribution of the weight load and an increased freedom of movement. FIGS. 9 to 11 show how the different sizes with each other preferably are related. Here, FIG 9 10 11 shows. A thigh part with small neck, Fig. A thigh portion with a medium-sized neck and Fig. A thigh portion with a longer neck. For the sake of clarity, the reference numerals of the various parts of the thigh part with the short neck are provided with a prime index (e.g. 13 ' ), while the reference numerals of the medium-sized neck or long neck with a double line (e.g. B. 13 '' ) or with a three-dash (z. B. 13 ''' ) are provided. From Fig. 9 to 11 it follows that the distances D ', D'' and D''' between the lines L ', L'' and L''' through the centers of the heads 17 ', 17'' and 17th ''' And parallel to the lateral edges 44', 44 '' and 44 '''of the straight sections 43', 43 '' and 43 ''' are the same in all three sizes, and that to create different neck sizes, the angles S ', S'' and S''' between the neck axes N ', N'' and N''' and the lateral edges 44 ', 44'' and 44''' of the straight sections 43 ', 43'' and 43 '''are different sizes. The specific dimensions of the three sizes are as follows when using a 25mm head:
Other head sizes can also be provided, for example a head size of 32 mm, to which different neck lengths would belong.
The hip section 15 is preferably made of plastic, e.g. B. Ultra high molecular weight polyethylene, made in one piece.
When implanting the artificial hip joint 11 , the usual surgical methods for implanting the hip section 15 in the acetabular cup A and the thigh part 13 in the thigh F by means of cement C are used. Various synergistic results are obtained using the invention, in particular in connection with the structure and configuration of the two sections. This includes, among other things, the wedge action or the outward movement of the cement into the cavities of the bone when the shaft 21 is inserted into the medullary canal M of the thigh F due to the continuous beveling of the shaft 21 from the proximal end to the distal end. Because of this wedge effect, the shaft 21 is also securely secured against wobbling after the cement has hardened. In addition, a uniform distribution of the load along the entire length of the shaft is ensured.
Nevertheless, the continuous bevel enables the shaft 22 to be removed easily at a later point in time, for example if a correction is required. The above-mentioned angular relationship between the thigh parts with increasingly long necks finally results in anatomical compatibility with improved thigh support.
